ABSTRACT This study was conducted to assess mental health knowledge, attitude and practices among health care workers in Belize before and immediately after a competency based training program in mental health. A baseline Knowledge, Attitudes and Practices (KAP) survey was given to health personnel, mainly nurses, working primary and secondary care. The intervention was a 13-week face-to-face training course for health care professionals with the objective of increasing their competency in mental health and reducing stigma. After the training a post intervention KAP survey was conducted among the original respondents. 88 health care workers completed the baseline survey and 61 of those respondents completed the post-intervention questionnaire. The results showed that the level of knowledge of the participants had improved by the training intervention and that in general, the intervention was effective in correcting some misconceptions about mental illness and reducing stigmatizing attitudes among the participants.

RESUMO: Nas últimas quatro décadas, desenvolveram-se vários estudos, dispersos por todo o mundo, visando analisar a importância da transmissão perinatal do CMV pelo leite materno, nos recém-nascidos prematuros e de baixo peso à nascença. Comparando estes estudos, as taxas de incidência de infecção e doença são extremamente variáveis, não havendo consenso entre os autores. Surgiu, assim, a necessidade de realizar a presente dissertação, pioneira ao nível nacional, com o objectivo de determinar a incidência da infecção perinatal citomegálica, transmitida através do aleitamento materno a recémnascidos prematuros, e identificar as suas consequências clínicas. Para a elaboração deste trabalho foram seleccionados todos os recém-nascidos com idade gestacional inferior a 35 semanas e respectivas mães seropositivas para CMV, internados na Unidade de Cuidados Intensivos de Neonatalogia do Hospital de São Francisco Xavier, entre o período de 1 de Outubro de 2010 e 31 Julho de 2011. Foram analisadas as urinas dos recém-nascidos e o leite materno das mães na primeira, sexta e décima segunda semana pós-parto, utilizando a técnica de Nested-PCR e, posteriormente, a PCR em Tempo Real, para determinar a carga viral do leite materno. Constatou-se que a aquisição da infecção perinatal de CMV no recém-nascido prematuro ocorre com alguma frequência (40,5%), sendo muito provável que a via de transmissão em 38,1% destes casos seja o leite materno infectado, não devendo esta ser desvalorizada. Relativamente às consequências clínicas, não foram observadas alterações clínico-laboratoriais associadas à infecção citomegálica. Analisando os factores que condicionam a transmissão da infecção citomegálica perinatal, estabeleceu-se uma correlação entre a carga viral do leite materno e a transmissão do vírus, permitindo deduzir que quanto maior a carga viral, maior o risco de transmissão. Finalmente, os resultados obtidos sugerem que o risco de transmissão da infecção e suas consequências clínicas não justificam a contra-indicação da amamentação, pois esta comporta, também, elevados benefícios. ----------------ABSTRACT: In the last four decades, several studies were developed all over the world to analyze the importance of CMV perinatal transmission through mother´s milk in the preterm and low birthweight newborns. Comparing these studies, the infection and disease incidence rates are extremely variable, without agreement between the authors. The aim of this study, the first of the kind made in Portugal, is to define the rate of perinatal cytomegalovirus infection in preterm newborns via breastfeeding and its clinical consequences. All the newborns with gestational age below 35 weeks and their CMV seropositive mothers, admitted between the October 1, 2010 and July 31, 2011 in the Neonatology Intensive Care Unit of the São Francisco Xavier Hospital, were included in this study. Human breast milk and urine specimens were collected from mothers and their preterms infants around the 1st, 6th and 12th week after delivery and analyzed by Nested-PCR. The PCR in Real Time was used to determine the breast milk viral load. It was found that the CMV perinatal infection in preterm infants occurs in 40,5% of the cases and most likely 38,1% of these were infected via breastmilk and therefore this should not be overlooked. Considering the clinical consequences, no clinical and laboratory changes associated with cytomegalovirus infection were observed. Analyzing the factors that determine the perinatal transmission of the cytomegalovirus, it was established a correlation between breast milk viral load and viral transmission, allowing to conclude that the higher the viral load the greater the risk of transmission. Finally, the obtained results suggest that the risk of CMV infection and its clinical consequences don´t justify the breastfeeding contraindication because it also provides high benefits.

The objective of this study was to describe a pseudo-outbreak of C. difficile in a hospital, following a change in the method used to detect the toxin. In February 2002, there were two cases of CDAD and in March 7 occurred, coinciding with a change of the test (from detection of toxin A to toxin A/B). An outbreak was suspected. Active surveillance and education of staff were started. A CDAD case was defined as a patient with acute onset of diarrhea (³ three episodes of liquid stools) and a positive stool test. They were classified as hospital or community-acquired. Stool samples were also collected for C. difficile culture and isolates were typed using AP-PCR. From March 2002 through December 2003 there were 138 cases of CDAD: 70% were hospital-acquired and among the 30% with CDAD present on admission, most (81%) came directly from the community (50% had no history of hospitalization). Fifty-two percent of hospital-acquired CDAD and 94% of cases on admission had already used antibiotics. The incidence of CDAD in hospitalized patients during surveillance was 3.3 per 1000 patient-admissions. The incidence of CDAD present on admission was 6.1/1000 patients. Sixteen isolates were typed and presented 13 different profiles. In conclusion, the CDAD increase in our study occurred due to change in diagnostic methods and not due to an outbreak, as suspected initially. The incidence in hospitalized patients was much lower than in reported outbreaks. There were 13 molecular types suggesting that an outbreak did not occur. CDAD was largely community-acquired.

Lo Servicio Nacional de Salud en Portugal fue criado en 1979, universal e gratis. Las primeras UCIN surgieran en 1980, en 1985 fue criada la Sociedad Portuguesa de Neonatología, en 1987 lo sistema de transporte neonatal, en mismo año fue nominada una Comisión de Peritos en Perinatología. En 1989 fue nominado el Comité Nacional de la Mujer y del niño y empezó la Reforma de los cuidados de salud perinatal. Era un programa de 9 años en etapas de 3 años que incluía el ccierre de Hospitales con menos de 1500 partos/año, categorización de los hospitales en niveles de cuidados e la creación de Unidades Coordinadoras entre Centros de Salud y Hospitales. Las UCIN y Intermedios neonatales fueran equipados y definido el número necesario de obstetras, pediatras y enfermeras e fue hecha formación en Cuidados Intensivos Neonatales. Los Centros de Salud no tienen partos e controlan el embarazo normal; los hospitales Nivel I no tienen partos; los de Nivel II tienen partos normales y de bajo riesgo, por lo menos 1500/año, obstetras, Unidad de Cuidados Intermedios, Pediatras con formación en neonatología, ventilación por períodos cortos. Los Hospitales de Nivel III tienen partos de bajo y alto riesgo, obstetras y neonatólogos, UCIN, formación en obstetricia y neonatología e investigación. Las UCIN tienen ventilación de longa duración, nutrición parenteral, cuidados de recién nacidos con menos de 1500g, condiciones quirúrgicas, son centros de enseñanza e investigación. Deben tener neonatólogos y pediatras con competencia en neonatología 24h por día, 1.5 camas /1000 partos, 1 enfermera para 2 logares de intensivos – 2,5 enfermeras por cada cama de intensivos - deben estar localizadas en maternidades con >3000 partos. Hay también reglas para translado para nível III pré-natal y pós-natal. Lo impacto fue que la tasa de partos hospitalares aumentó hasta 99% la mortalidad fetal e fetal tardia disminuiu hasta 3,7 e 2,5/1000 NV+MN, la mortalidad perinatal con más de 28 semanas para 4,6/1000 NV+ MN, la neonatal para 2,4/1000NV e la infantil para 3,6. Las mejores condiciones socioeconómicas y nivel de educación pueden justificar parte de estos resultados pero la organización de los cuidado perinatales fue ciertamente una grande razón de mejoría.

Liver cirrhosis (LC) can lead to a clinical state of liver failure, which can exacerbate through the course of the disease. New therapies aimed to control the diverse etiologies are now more effective, although the disease may result in advanced stages of liver failure, where liver transplantation (LT) remains the most effective treatment. The extended lifespan of these patients and the extended possibilities of liver support devices make their admission to an intensive care unit (ICU) more probable. In this paper the LC is approached from the point of view of the pathophysiological alterations present in LC patients previous to ICU admission, particularly cardiovascular, but also renal, coagulopathic, and encephalopathic. Infections and available liver detoxifications devices also deserve mentioning. We intend to contribute towards ICU physician readiness to the care for this particular type of patients, possibly in dedicated ICUs.

Some patients under antiretroviral therapy (ART) do not reach immune recovery when the viral load becomes undetectable. This is called discordant immunologic and virologic responses. Its prevalence varies between 8% and 24%. This study describes its prevalence and the characteristics of the affected subjects in the outpatient clinic of a Brazilian specialized-care center. Of 934 patients on ART, 536 had undetectable viral loads. Prevalence was 51/536 or 9% (95% confidence interval: 6.6% to 11.4%). Median age at the beginning of ART was 37 years (interquartile range - IQR: 31 to 45). Male gender and mixed race predominated (76.5% and 47.1% respectively). AIDS-defining illnesses were absent at the beginning of ART in 60.8%. Fifty-one percent were taking protease inhibitors, 43.2% Efavirenz and 5.8% both. Median time on ART was 36 months (IQR: 17-81 months). Irregular treatment was recorded for 21.6%. ART had been modified for 63% prior to the study, and 15.7% had used monotherapy or double therapy. Median CD4 count was 255 cells/mm³ (IQR: 200-284). Median viral load before ART was 4.7 log10 copies/mL (IQR: 4.5-5.2). Discordant responders were not different from AIDS patients in general, but there was a high frequency of multiple schedules of treatment.

OBJECTIVES: Mortality after ICU discharge accounts for approx. 20-30% of deaths. We examined whether post-ICU discharge mortality is associated with the presence and severity of organ dysfunction/failure just before ICU discharge. PATIENTS AND METHODS: The study used the database of the EURICUS-II study, with a total of 4,621 patients, including 2,958 discharged alive to the general wards (post-ICU mortality 8.6%). Over a 4-month period we collected clinical and demographic characteristics, including the Simplified Acute Physiology Score (SAPS II), Nine Equivalents of Nursing Manpower Use Score, and Sequential Organ Failure Assessment (SOFA) score. RESULTS: Those who died in the hospital after ICU discharge had a higher SAPS II score, were more frequently nonoperative, admitted from the ward, and had stayed longer in the ICU. Their degree of organ dysfunction/failure was higher (admission, maximum, and delta SOFA scores). They required more nursing workload resources while in the ICU. Both the amount of organ dysfunction/failure (especially cardiovascular, neurological, renal, and respiratory) and the amount of nursing workload that they required on the day before discharge were higher. The presence of residual CNS and renal dysfunction/failure were especially prognostic factors at ICU discharge. Multivariate analysis showed only predischarge organ dysfunction/failure to be important; thus the increased use of nursing workload resources before discharge probably reflects only the underlying organ dysfunction/failure. CONCLUSIONS: It is better to delay the discharge of a patient with organ dysfunction/failure from the ICU, unless adequate monitoring and therapeutic resources are available in the ward.

We used a prospective cohort to analyze the effect of change in BMI rather than change in weight, in mothers carrying dichorionic twins from a population that did not receive any dietary intervention. A total of 269 mothers (150 nulliparas and 119 multiparas) were evaluated. The average change (%) from the pre-gravid BMI was 7.2+/-6.1, 17.4+/-8.2, and 28.7+/-10.8, at 12-14, 22-25, and 30-34 weeks, respectively, without difference between nulliparas and multiparas. The comparison between maternities below or above the average change from the pregravid BMI failed to demonstrate an advantage (in terms of total twin birthweight and gestational age) of an above average change from the pregravid BMI, even when the lower versus upper quartiles were compared. Our observations reached different conclusions regarding the recommended universal dietary intervention in twin gestations. A cautious approach is advocated towards seemingly harmless excess weight gain, as normal weight women may turn overweight, or even obese, by the end of pregnancy, and be exposed to the untoward effects of obesity on future health and body image.

OBJECTIVE: The objective of the study was to develop a model for estimating patient 28-day in-hospital mortality using 2 different statistical approaches. DESIGN: The study was designed to develop an outcome prediction model for 28-day in-hospital mortality using (a) logistic regression with random effects and (b) a multilevel Cox proportional hazards model. SETTING: The study involved 305 intensive care units (ICUs) from the basic Simplified Acute Physiology Score (SAPS) 3 cohort. PATIENTS AND PARTICIPANTS: Patients (n = 17138) were from the SAPS 3 database with follow-up data pertaining to the first 28 days in hospital after ICU admission. INTERVENTIONS: None. MEASUREMENTS AND RESULTS: The database was divided randomly into 5 roughly equal-sized parts (at the ICU level). It was thus possible to run the model-building procedure 5 times, each time taking four fifths of the sample as a development set and the remaining fifth as the validation set. At 28 days after ICU admission, 19.98% of the patients were still in the hospital. Because of the different sampling space and outcome variables, both models presented a better fit in this sample than did the SAPS 3 admission score calibrated to vital status at hospital discharge, both on the general population and in major subgroups. CONCLUSIONS: Both statistical methods can be used to model the 28-day in-hospital mortality better than the SAPS 3 admission model. However, because the logistic regression approach is specifically designed to forecast 28-day mortality, and given the high uncertainty associated with the assumption of the proportionality of risks in the Cox model, the logistic regression approach proved to be superior.

OBJECTIVE: To develop a new method to evaluate the performance of individual ICUs through the calculation and visualisation of risk profiles. METHODS: The study included 102,561 patients consecutively admitted to 77 ICUs in Austria. We customized the function which predicts hospital mortality (using SAPS II) for each ICU. We then compared the risks of hospital mortality resulting from this function with the risks which would be obtained using the original function. The derived risk ratio was then plotted together with point-wise confidence intervals in order to visualise the individual risk profile of each ICU over the whole spectrum of expected hospital mortality. MAIN MEASUREMENTS AND RESULTS: We calculated risk profiles for all ICUs in the ASDI data set according to the proposed method. We show examples how the clinical performance of ICUs may depend on the severity of illness of their patients. Both the distribution of the Hosmer-Lemeshow goodness-of-fit test statistics and the histogram of the corresponding P values demonstrated a good fit of the individual risk models. CONCLUSIONS: Our risk profile model makes it possible to evaluate ICUs on the basis of the specific risk for patients to die compared to a reference sample over the whole spectrum of hospital mortality. Thus, ICUs at different levels of severity of illness can be directly compared, giving a clear advantage over the use of the conventional single point estimate of the overall observed-to-expected mortality ratio.

Introduction: Urinary tract infection (UTI) has a high incidence and recurrence, therefore, treatment is empirical in the majority of cases. Objectives: The aim of this study was to analyze the urine cultures performed at a secondary hospital, during two periods, 2005-2006 and 2010-2011, and to estimate the microbial resistance. Patients and methods: We analyzed 11,943 aerobic urine cultures according to basic demographic data and susceptibility to antibiotics in accordance with the Clinical and Laboratory Standards Institute (CLSI) for Vitek 1 and 2. Results: Most of our cohort consisted of young adult females that were seen at the Emergency Department. E. coli was the most frequent (70.2%) among the 75 species isolated. Resistance of all isolates was ≥ 20% for trimethoprim/sulfamethoxazole (TMP/SMX), norfloxacin, nitrofurantoin, cefazolin and nalidixic acid. Although E. coli was more susceptible (resistance ≥ 20% for TMP/SMX and nalidixic acid) among all of the isolates, when classified by the number and percentage of antibiotic resistance. Global resistance to fluoroquinolones was approximately 12%. Risk factors for E. coli were female gender and an age less than 65 years. Men and patients older than 65 years of age, presented more resistant isolates. Extended spectrum beta-lactamases (ESBL) were identified in 173 out of 5,722 Gram-negative isolates (3.0%) between 2010 and 2011. Conclusion: E. coli was the most frequent microbe isolated in the urine cultures analyzed in this study. There was a significant evolution of bacterial resistance between the two periods studied. In particular, the rise of bacterial resistance to fluoroquinolones was concerning.

Objetivo: Avaliar a qualidade de vida relacionada à saúde (QVRS)de crianças sobreviventes à alta da terapia intensiva pediátrica. Métodos: Foi realizada uma avaliação prospectiva da QVRS na admissão e após 6 meses em crianças com idade igual ou superior a 6 anos, internadas em três unidades de terapia intensiva pediátricas (UTIPs) terciárias de maio de 2002 a junho de 2004. A QVRS foi avaliada com o questionário Health Utilities Index Mark 3 (HUI3), aplicado a um representante da criança. Resultados: Das 517 admissões elegíveis, 44 crianças faleceram na UTIP (8,5%) e 320 casos foram avaliados na admissão; entre eles, foi possível realizar o seguimento de 252 casos. Não foram encontradas diferenças estatisticamente significativas entre os escores globais do HUI3 antes da admissão e no seguimento [medianas (intervalo interquartil) de 0,86 (0,42-1,00) e 0,83 (0,45-1,00); p = 0,674, respectivamente]. No âmbito individual, 21% das crianças não apresentaram mudanças na QVRS, foi observada melhora em 40% e agravamento em 38% dos casos. Deficiência grave antes da admissão (escore global do HUI3 < 0,70)esteve presente em 36% dos casos, com melhora no seguimento aos 6 meses em 60% deles. Entre aqueles que apresentaram agravamento da QVRS no seguimento, 45% eram vítimas de trauma. Conclusões: Embora a QVRS seja globalmente semelhante nas duas avaliações, foram encontradas várias diferenças no âmbito individual. As crianças com baixa QVRS antes da admissão (deficiência grave) podem se beneficiar da terapia intensiva pediátrica, visto que muitas dessas crianças melhoraram a QVRS, em comparação com seu estado pré-admissão.
