O presente trabalho visou investigar qual a importância da Psicoterapia em homens doentes de aids marcados pela proximidade com a morte ou a finitude existencial internados em uma enfermaria hospitalar. A intervenção psicológica, na área da saúde e da clínica psicológica, foi procedimento interventivo que permitiu efetivar a escuta socialmente engajada visando superar a premissa acerca da doença enquanto centro do tratamento, transmitida ao longo da história e veiculando a concepção de um sujeito patológico. Nesta perspectiva, o objetivo deste trabalho foi incentivar os participantes da pesquisa a vivenciar a atenção à própria saúde reconhecendo-a enquanto um direito de cidadania básico a todos os homens. Sobre os procedimentos a psicoterapia breve gestáltica e os cuidados paliativos foram as estratégias utilizadas nos atendimentos realizados no Hospital Universitário João de Barros Barreto em Belém do Pará-Brasil. Os participantes da pesquisa foram quatro homens adultos, com faixa etária entre 18 e 49 anos de idade. A seleção iniciou com a pesquisa documental dos relatórios e documentos elaborados pelos profissionais que atendem na enfermaria da clinica infecto-parasitária (DIP) como fichas de identificação, prontuários de identificação dos casos, onde estava descrita o diagnóstico, o histórico de saúde. Tratou-se de uma pesquisa clínico-qualitativa, que utilizou o método fenomenológico e concepções existencialistas, e referencial teórico da Gestalt-terapia. Para a análise do discurso dos informantes se valeu da Hermenêutica da linguagem proposta em Paul Ricouer, que propõe a compreensão da dimensão linguística (sentido) e dimensão extralinguística (referência), partindo da análise gramatical da frase do locutor, perpassando pela análise semântica da fala, com atenção às funções e atos da linguagem. Nas analises nos atentamos unicamente à assimilação da função Expressiva ou Emotiva, em que o sujeito que se destacou foi o EU, e aos atos do discurso: locucionários (ato de dizer; expressão verbal), ilocucionários (aquilo que fazemos ao dizer; recursos não verbais que acompanham a fala) e atos perlocucionários (reflexo da linguagem no outro). Os resultados mostraram que os homens são responsáveis por suas próprias habilidades, e que podem ampliar as possibilidades em suas vidas, mesmo em situação de fragilidade e sem possibilidade de cura, identificando os meios a sua disposição, que permitiram lidar com uma situação de dificuldade. Quanto à psicoterapia ela foi um facilitador, e proporcionou a possibilidade de se compreender alguns modos de vinculação, subjetivação e relação dos homens atendidos no hospital.

A Pediatric Intensive care Unit (PICU) is an environment where care is provided to seriously ill children. Hospitalization is regarded as an unpleasant experience that requires adaptation and routine change. To assess the opinion of relatives of children hospitalized at a PICU concerning tie breakage and/or separation between children and their families. It is a descriptive, cross-sectional, quantitative study. A structured interview was conducted with the families of hospitalized children from July to September, 2010. The data were statistically analyzed. RESULTS: Twenty relatives were interviewed, 80% of whom believed that children’s behavior changes when they are present in the unit, and 85% considered the visitation time established to be sufficient. All the respondents reported to be satisfied about the care provided. The feelings prevailing in 50% of the relatives were fear and hope. Change in the family’s union after hospitalization occurred in 85% of the families. The most fearful aspect concerning the ICU is the equipment (25%), and the most worrisome, as regards personal life, is the separation from other children (65%). Health care professionals must pay more attention to relatives by including them in their health care plan so that the health care team and the family, who are essential in children’s recovery, can interact in a congenial fashion. It is also necessary to improve the knowledge related to that subject so that the health care team and families can work together towards children’s recovery

Presently, the dying process and death most often occur in hospitals and, particularly, in Intensive Care Units (ICU), where patients’ lives are prolonged thanks to advanced technological devices and highly efficient medicines. To learn about the opinion of health care professionals working at a Pediatric Intensive Care Unit in relation to the dying process and dying. This is a descriptive quantitative study. A questionnaire was applied to the unit’s staff members from June to August, 2011. Data were statistically analyzed. Twenty-five professionals answered the questionnaire, and 72% faced death as a natural life process. 60% felt compassion, but that feeling did not interfere with how they cared for patients. Concerning their professional training, 52% reported not to have received any concerning patients’ caregivers in the dying process or death; therefore, they experienced such situation when they were already working, and 76% reported to be interested in updating courses on that theme. Further discussion about this topic during academic education is necessary. It is also necessary to provide health care professionals with specialization courses, debates and experience exchange so that they can better understand and deal with their feelings and limitations in face of death and thus give better care to patients and relate to patients’ families during the dying process of a loved one

Objectives: The effectiveness of noninvasive positive-pressure ventilation in preventing reintubation due to respiratory failure in children remains uncertain. A pilot study was designed to evaluate the frequency of extubation failure, develop a randomization approach, and analyze the feasibility of a powered randomized trial to compare noninvasive positive-pressure ventilation and standard oxygen therapy post extubation for preventing reintubation within 48 hours in children with respiratory failure.Design: Prospective pilot study.Setting: PICU at a university-affiliated hospital.Patients: Children aged between 28 days and 3 years undergoing invasive mechanical ventilation for greater than or equal to 48 hours with respiratory failure after programmed extubation.Interventions: Patients were prospectively enrolled and randomly assigned into noninvasive positive-pressure ventilation group and inhaled oxygen group after programmed extubation from May 2012 to May 2013.Measurements and Main Results: Length of stay in PICU and hospital, oxygenation index, blood gas before and after tracheal extubation, failure and reason for tracheal extubation, complications, mechanical ventilation variables before tracheal extubation, arterial blood gas, and respiratory and heart rates before and 1 hour after tracheal extubation were analyzed. One hundred eight patients were included (noninvasive positive-pressure ventilation group, n = 55 and inhaled oxygen group, n = 53), with 66 exclusions. Groups did not significantly differ for gender, age, disease severity, Pediatric Risk of Mortality at admission, tracheal intubation, and mechanical ventilation indications. There was no statistically significant difference in reintubation rate (noninvasive positive-pressure ventilation group, 9.1%; inhaled oxygen group, 11.3%; p > 0.05) and length of stay (days) in PICU (noninvasive positive-pressure ventilation group, 3 [116]; inhaled oxygen group, 2 [1-25]; p > 0.05) or hospital (noninvasive positive-pressure ventilation group, 19 [7-141]; inhaled oxygen group, 17 [8-80]).Conclusions: The study indicates that a larger randomized trial comparing noninvasive positive-pressure ventilation and standard oxygen therapy in children with respiratory failure is feasible, providing a basis for a future trial in this setting. No differences were seen between groups. The number of excluded patients was high.

To discuss the role of physical exercise in the attenuation of cancer cachexia-associated symptoms, and upon the outcome of chemotherapy, with special focus on the anti-inflammatory role of chronic exercise. The review addresses the recent findings regarding the positive effects of endurance and strength exercise training upon metabolic dysfunction, systemic inflammation and body composition alterations in the syndrome of cachexia. The employment of different exercise protocol strategies, in respect to intensity, duration, work load and in concomitance with pharmacological treatment is considered. Cachexia is a multifactorial wasting syndrome afflicting patients with cancer, chronic obstructive pulmonary disease, chronic heart failure, trauma, among other diseases. This condition markedly compromises the quality of life, treatment outcome and survival. Recent literature indicates an unequivocal role of chronic exercise in modulating cachexia and other cancer-associated dysfunctions. Exercise is proposed as a complementary treatment in cancer, and represents a function-preserving, anti-inflammatory and metabolism-modulating strategy with low cost, and high versatility and availability. Furthermore, exercise decreases cancer recurrence and presents a positive impact on public health management, reducing hospitalization and medication costs.

The purpose of the study was to outline the profile of patients hospitalized at the Pediatric Intensive Care Unit of the Hospital das Clínicas de Botucatu - UNESP. This is a descriptive, cross-sectional and quantitative study. The data were extracted from the “Discharge, Admission and Death Register” of the unit of the patients hospitalized between January and December 2011. There was predominance of male children (54.4%) under one year of age (40,7%) were, with a mean length of stay of 5.46 days. Most of these children came from cities included in the DIR XI/SP in Botucatu (78.2%). The discharge contributed with 91.1% of the total discharges from the unit. Most admissions happened during the fall and winter, with the Immediate Post-Operative (IPO - 32.3%) and respiratory diseases (24.2%) as the most frequent causes. The profile of patients at the PICU makes it possible to elucidate individual aspects, family, social, demographic, seasonal, climatic, and, also, the clinical conditions, and, thus, comprehend the context of hospitalizations, in order to propose improvements on assistance, in an individualized and integral manner, for patients and their families.

The objective of this descriptive research was to investigate the perception of health care professionals who work in Pediatric Intensive Care Units in relation to the process of death and dying. The study was undertaken between June and August 2011, with data collected through structured interviews and subjected to statistical analysis. Of the 25 participants, 72% perceived death as a natural process of life, 60% felt compassion – a feeling that did not interfere in caring for the patient, 52% related that they had not received any preparation about the process of death or dying and 76% showed interest in taking a refresher course on the issue. Greater discussion is necessary about academic training, and the offer of educational activities and space for exchanging experiences, such that the workers may better understand and deal with feelings and limitations regarding death.
