Unspezifische muskuloskelettale Beschwerden wie chronische Rücken- oder Nackenbeschwerden werden als multifaktoriell bedingt verstanden, wobei die Arbeit respektive die Arbeitsbedingungen einen mehr oder weniger bestimmenden Faktor darstellen. Es ist daher wesentlich, speziell auch im Hinblick auf die Wiedereingliederung, dass sowohl die Arbeit wie auch die Arbeitsbedingungen evaluiert werden. [Auteure]

Expression of laminin-5 alpha3, beta3 and gamma2 protein subunits was investigated in colorectal adenocarcinomas using immunostaining and confocal microscopy. The laminin-5 heterotrimer was found in basement membranes and as extracellular deposits in tumor stroma. In contrast to the alpha3 subunit, which was under-expressed, the gamma2 and beta3 subunits were detected in the cytoplasm of carcinoma cells dissociating (budding) from neoplastic tubules, suggestive of focal alterations in laminin-5 assembly and secretion. Laminin-5 gamma2 or beta3 subunit-reactive budding carcinoma cells expressed cytokeratins but not vimentin; they did not proliferate and were not apoptotic. Furthermore, expression of laminin-5 gamma2 and beta3 subunits in budding cells was associated with focal under-expression of the E-cadherin-beta-catenin complex. Results from xenograft experiments showed that budding activity in colorectal adenocarcinomas could be suppressed when these tumors grew at ectopic s.c. sites in nude mice. In vitro, cultured colon carcinoma cells, but not adenoma-derived tumor cells, shared the laminin-5 phenotype expressed by carcinoma cells in vivo. Using colon carcinoma cell lines implanted orthotopically and invading the cecum of nude mice, the laminin-5-associated budding was restored, indicating that this phenotype is not only determined by tumor cell properties but also dependent on the tissue micro-environment. Our results indicate that both laminin-5 alpha3 subunit expression and cell-cell cohesiveness are altered in budding carcinoma cells, which we consider to be actively invading. We propose that the local tissue micro-environment contributes to these events.

Contexte: L'intervention "Break The Chains" 2012 de l'OFSP a l'intention des gays visait à informer sur la primo-infection et a inciter a un test de dépistage VIH. Design: L'évaluation a recouru a un design Pre-Post avec 2 dispositifs: A) enquête par questionnaire Internet avec une vague avant la campagne et une après (transversal repete); B) 1) relevé centré sur le canton de Zurich du nombre de test effectues par des gays durant le premier semestre 2012 utilisant des données récoltées par Sentinella, par les centres de test utilisant l'application BerDa et auprès des praticiens de HIV-Prakt; et 2) recueil d'information par le questionnaire internet post-intervention des intentions d'effectuer un test VIH. Résultats: 366 HSH ont répondu au questionnaire de la phase pré-, et 964 à la phase post-intervention. 69.8% ont entendu parler de la campagne. Parmi les répondants ayant identifie le message, 48.6% se sont sentis concernes. Avant la campagne, 34.2% des répondants identifiaient les 4 symptômes de la primo-infection, 46.4% se sentaient bien informes et 79.2% connaissaient le délai minimum pour qu'un test informe sur une absence d'infection; après la campagne, ces proportions étaient 39.6%, 62.0% et 85.2%. 17.1% des répondants ont déclaré avoir fait/avoir l'intention de faire, un test VIH suite à la campagne. Le nombre de tests effectues par les répondants Internet montre effectivement un pic dans le 2e trimestre 2012; les données BerDa montrent une stabilité du nombre total de tests. Les données épidémiologiques montrent une augmentation de la proportion et du nombre d'infections récentes détectées durant ce 2e trimestre, mais une diminution des infections anciennes. Conclusions: L'intervention BTC a amélioré légèrement les connaissances relatives à la phase de primo-infection du VIH et a la nécessité d'effectuer un test VIH. Elle semble avoir incite le public cible à effectuer un test VIH qui n'aurait pas été réalisé autrement et a peut-être ainsi contribuer a la détection d'infections récentes.

Background: Treatment of depression, the most prevalent and costly mental disorder, needs to be improved. Non-concordance with clinical guidelines and non-adherence can limit the efficacy of pharmacological treatment of depression. Through pharmaceutical care, pharmacists can improve patients' compliance and wellbeing. The aim of this study is to evaluate the effectiveness and costeffectiveness of a community pharmacist intervention developed to improve adherence and outcomes of primary care patients with depression. Methods/design: A randomized controlled trial, with 6-month follow-up, comparing patients receiving a pharmaceutical care support programme in primary care with patients receiving usual care. The total sample comprises 194 patients (aged between 18 and 75) diagnosed with depressive disorder in a primary care health centre in the province of Barcelona (Spain). Subjects will be asked for written informed consent in order to participate in the study. Diagnosis will be confirmed using the SCID-I. The intervention consists of an educational programme focused on improving knowledge about medication, making patients aware of the importance of compliance, reducing stigma, reassuring patients about side-effects and stressing the importance of carrying out general practitioners' advice. Measurements will take place at baseline, and after 3 and 6 months. Main outcome measure is compliance with antidepressants. Secondary outcomes include; clinical severity of depression (PHQ-9), anxiety (STAI-S), health-related quality of life (EuroQol-5D), satisfaction with the treatment received, side-effects, chronic physical conditions and sociodemographics. The use of healthcare and social care services will be assessed with an adapted version of the Client Service Receipt Inventory (CSRI). Discussion: This trial will provide valuable information for health professionals and policy makers on the effectiveness and cost-effectiveness of a pharmaceutical intervention programme in the context of primary care. Neste ensaio procuro desenvolver uma argumentação a favor da superação dos monismos metodológicos no campo da pesquisa em sociologia da educação. Com base no que Jeffrey Alexander (1987) denominou o "novo movimento teórico", defendo a necessidade de superar a divisão do trabalho de pesquisa entre os investigadores que se dedicam à microssociologia e os que preferem abordagens macrossociais. Nessa perspectiva, as opções teórico-metodológicas devem-se ancorar nas necessidades da investigação e não numa opção a priori do pesquisador por qualquer uma das alternativas.

Little information is currently available from the various societies of cardiology on primary percutaneous coronary intervention (PCI) for acute myocardial infarction (AMI). Since primary PCI is the main method of reperfusion in AMI in many centres, and since of all cardiac emergencies AMI represents the most urgent situation for PCI, recommendations based on scientific evidence and expert experience would be useful for centres practising primary PCI, or those looking to establish a primary PCI programme. To this aim, a task force for primary PCI in AMI was formed to develop a set of recommendations to complement and assist clinical judgment. This paper represents the product of their recommendations.

Brief interventions (BI) commonly employ screening and target a single substance. Multi-substance interventions are a more adequate reflection of risk behaviors in adolescents and young adults. Systematic screening complicates BI in many settings. The effectiveness of a voluntary multi-substance intervention among 19-year-old men and the incremental impact of booster sessions were analyzed. Participants were enrolled during mandatory army conscription in Switzerland. Compared with 461 controls, 392 BI subjects showed reduced substance use on 10 of 12 measures (4 tobacco, 4 cannabis, and 2 alcohol measures). Between-group effects were small and non-significant (except for cannabis use prevalence). Three-month booster sessions were not effective and even contraindicated. The usefulness of targeting multi-substances during BIs without prior screening depends on the value of small effects. The addition of booster sessions was not effective and therefore is not recommended.

PURPOSE OF REVIEW: Despite progress in the understanding of the pathophysiology of invasive candidiasis, and the development of new classes of well tolerated antifungals, invasive candidiasis remains a disease difficult to diagnose, and associated with significant morbidity and mortality. Early antifungal treatment may be useful in selected groups of patients who remain difficult to identify prospectively. The purpose of this review is to summarize the recent development of risk-identification strategies targeting early identification of ICU patients susceptible to benefit from preemptive or empirical antifungal treatment. RECENT FINDINGS: Combinations of different risk factors are useful in identifying high-risk patients. Among the many risk factors predisposing to invasive candidiasis, colonization has been identified as one of the most important. In contrast to prospective surveillance of the dynamics of colonization (colonization index), integration of clinical colonization status in risk scores models significantly improve their accuracy in identifying patients at risk of invasive candidiasis. SUMMARY: To date, despite limited prospective validation, clinical models targeted at early identification of patients at risk to develop invasive candidiasis represent a major advance in the management of patients at risk of invasive candidiasis. Moreover, large clinical studies using such risk scores or predictive rules are underway.
