BACKGROUND: Considering the high rates of pain as well as its under-management in long-term care (LTC) settings, research is needed to explore innovations in pain management that take into account limited resource realities. It has been suggested that nurse practitioners, working within an inter-professional model, could potentially address the under-management of pain in LTC.
OBJECTIVES: This study evaluated the effectiveness of implementing a nurse practitioner-led, inter-professional pain management team in LTC in improving (a) pain-related resident outcomes; (b) clinical practice behaviours (e.g., documentation of pain assessments, use of non-pharmacological and pharmacological interventions); and, (c) quality of pain medication prescribing practices.
METHODS: A mixed method design was used to evaluate a nurse practitioner-led pain management team, including both a quantitative and qualitative component. Using a controlled before-after study, six LTC homes were allocated to one of three groups: 1) a nurse practitioner-led pain team (full intervention); 2) nurse practitioner but no pain management team (partial intervention); or, 3) no nurse practitioner, no pain management team (control group). In total, 345 LTC residents were recruited to participate in the study; 139 residents for the full intervention group, 108 for the partial intervention group, and 98 residents for the control group. Data was collected in Canada from 2010 to 2012.
RESULTS: Implementing a nurse practitioner-led pain team in LTC significantly reduced residents' pain and improved functional status compared to usual care without access to a nurse practitioner. Positive changes in clinical practice behaviours (e.g., assessing pain, developing care plans related to pain management, documenting effectiveness of pain interventions) occurred over the intervention period for both the nurse practitioner-led pain team and nurse practitioner-only groups; these changes did not occur to the same extent, if at all, in the control group. Qualitative analysis highlighted the perceived benefits of LTC staff about having access to a nurse practitioner and benefits of the pain team, along with barriers to managing pain in LTC.
CONCLUSIONS: The findings from this study showed that implementing a nurse practitioner-led pain team can significantly improve resident pain and functional status as well as clinical practice behaviours of LTC staff. LTC homes should employ a nurse practitioner, ideally located onsite as opposed to an offsite consultative role, to enhance inter-professional collaboration and facilitate more consistent and timely access to pain management.

Résumé : Les eaux souterraines ont un impact majeur sur la vie terrestre, les besoins domestiques et le climat. Elles sont aussi un maillon essentiel du cycle hydrologique. Au Canada par exemple, plus de 30 % de la population est tributaire des eaux souterraines pour leur alimentation en eau potable. Ces ressources subissent de nombreuses pressions sous l’influence de certains facteurs comme la salinisation, la contamination et l’épuisement. La variabilité du climat et la demande croissante sur ces ressources imposent l'amélioration de nos connaissances sur les eaux souterraines. L’objectif principal du projet de recherche est d’exploiter les données d’anomalies (TWS) de la mission Gravity Recovery And Climate Experiment (GRACE) pour localiser, quantifier et analyser les variations des eaux souterraines à travers les bassins versants du Bas-Mackenzie, du Saint-Laurent, du Nord-Québec et du Labrador. Il s’agit aussi d’analyser l’influence des cycles d’accumulation et de fonte de neige sur les variations du niveau des eaux souterraines. Pour estimer les variations des eaux souterraines, la connaissance des autres paramètres du bilan hydrologique est nécessaire. Ces paramètres sont estimés à l’aide des sorties du modèles de surface CLM du Système Global d’Assimilation des Données de la Terre (GLDAS). Les données GRACE qui ont été utilisées sont celles acquises durant la période allant de mars 2002 à août 2012. Les résultats ont été évalués à partir d’enregistrements de niveaux piézométriques provenant de 1841 puits localisés dans les aquifères libres du bassin des réseaux de suivi des eaux souterraines au Canada. Les valeurs de rendements spécifiques des différents types d’aquifères de chaque puits et celles des variations mensuelles du niveau d’eau dans ces puits ont été utilisées pour estimer les variations des anomalies des eaux souterraines in-situ. L’étude de corrélation entre les variations des anomalies des eaux souterraines estimées à partir de la combinaison GRACE-GLDAS et celles issues de données in-situ révèle des concordances significatives avec des valeurs de

L’objectif de ce mémoire est d’identifier les déterminants de la générosité de l’aide sociale au Canada. Plus précisément, quels sont les facteurs qui expliquent les variations entre les montants d’aide sociale entre les provinces canadiennes de 1990 à 2009? Pourquoi le Québec, la Saskatchewan et Terre-Neuve-Labrador sont plus généreux que le Nouveau-Brunswick et l’Alberta? L’analyse de ces 10 politiques distinctes est produite à partir d’un cadre théorique quadripartite qui inclut le rôle des acteurs (partis politiques et syndicats), les traits institutionnels (dépenses publiques et engagement à la redistribution), les contraintes budgétaires (taux d’assistance sociale, dette, économie) et le rôle du gouvernement fédéral (montant et type de transfert). Les résultats démontrent que l’aide sociale est une politique hautement dépendante au sentier et incrémentale. Des transferts fédéraux à coût partagé et un taux de syndicalisation élevé sont des facteurs qui exercent une influence positive sur la générosité des provinces. À l’inverse, les partis de droite ainsi qu’une situation budgétaire difficile ont un impact négatif. Il faut noter que la richesse économique des provinces n’est pas associée à une plus grande générosité de l’aide sociale, au contraire les prestations d’aide sociale étaient plus faibles en 2009 qu’en 1990 malgré un PIB qui a presque doublé. De plus, des provinces riches comme l’Alberta et la Colombie-Britannique sont peu généreuses. Finalement, il faut noter que les partis politiques de gauche n’ont pas l’effet positif escompté sur la générosité des politiques de revenu minimum.

Polychlorinated biphenyls (PCBs) and substituted phenylamine antioxidants (SPAs) are two chemical groups that have been used in multiple Canadian industrial processes. Despite the production ban of PCBs in North America in 1977, they are still ubiquitous in the environment and in wildlife tissues. Previous studies of fish, amphibians, birds, and mammals have shown that PCBs are toxic and act as endocrine disruptors. In contrast, SPAs, specifically N-phenyl-1-naphthylamine (PANA), have received very little attention despite their current use in Canada and their expected environmental releases. The effects of PCB and PANA exposures in reptiles remain unknown thus, juvenile Chelydra serpentina were used in this thesis as a model vertebrate to fill in missing toxicity research gaps due to their importance as an environmental indicator. First, food pellets were spiked at an environmentally relevant concentration of the PCB mixture Aroclor 1254 (A1254) to model hepatic bioaccumulation (0.45 μg/g A1254 for 31 days) and depuration (clean food for 50 days) of PCBs in turtles. No significant differences in PCB concentrations were observed between the control and treated animals, suggesting that juvenile turtles exposed to environmentally relevant concentrations of PCBs can likely detoxify low concentrations of PCBs. Additionally, two dose-response experiments were performed using A1254 or PANA spiked food (0-12.7 μg/g and 0-3,446 μg/g, respectively) to determine hepatic toxicity and bioaccumulation in juvenile C. serpentina. An increase in hepatic cyp1a was observed when exposed to the highest dose of both chemicals: 1) for A1254, induction correlated to the significant increase in hepatic PCB congeners that are known to be metabolized by CYP1A; and 2) for PANA, induction suggested that CYP1A has a potential role in its detoxification. PCBs are known endocrine disruptors, but no significant changes were observed for both thyroid receptors (alpha and beta) or by estrogen and androgen receptors. This lack of response, also noted in the PANA exposure, suggests that C. serpentina is less sensitive to endocrine disruption than other vertebrates. Furthermore, the expression of genes involved in cellular stress was not altered in PCB and PANA exposed animals, supporting the resilience of turtles to oxidative stress. This is the first study to demonstrate the toxicity of PCBs and PANA in C. serpentina, demonstrating the turtle’s high tolerance to contamination.

Clinicians working in the field of congenital and paediatric cardiology have long felt the need for a common diagnostic and therapeutic nomenclature and coding system with which to classify patients of all ages with congenital and acquired cardiac disease. A cohesive and comprehensive system of nomenclature, suitable for setting a global standard for multicentric analysis of outcomes and stratification of risk, has only recently emerged, namely, The International Paediatric and Congenital Cardiac Code. This review, will give an historical perspective on the development of systems of nomenclature in general, and specifically with respect to the diagnosis and treatment of patients with paediatric and congenital cardiac disease. Finally, current and future efforts to merge such systems into the paperless environment of the electronic health or patient record on a global scale are briefly explored. On October 6, 2000, The International Nomenclature Committee for Pediatric and Congenital Heart Disease was established. In January, 2005, the International Nomenclature Committee was constituted in Canada as The International Society for Nomenclature of Paediatric and Congenital Heart Disease. This International Society now has three working groups. The Nomenclature Working Group developed The International Paediatric and Congenital Cardiac Code and will continue to maintain, expand, update, and preserve this International Code. It will also provide ready access to the International Code for the global paediatric and congenital cardiology and cardiac surgery communities, related disciplines, the healthcare industry, and governmental agencies, both electronically and in published form. The Definitions Working Group will write definitions for the terms in the International Paediatric and Congenital Cardiac Code, building on the previously published definitions from the Nomenclature Working Group. The Archiving Working Group, also known as The Congenital Heart Archiving Research Team, will link images and videos to the International Paediatric and Congenital Cardiac Code. The images and videos will be acquired from cardiac morphologic specimens and imaging modalities such as echocardiography, angiography, computerized axial tomography and magnetic resonance imaging, as well as intraoperative images and videos. Efforts are ongoing to expand the usage of The International Paediatric and Congenital Cardiac Code to other areas of global healthcare. Collaborative efforts are under-way involving the leadership of The International Nomenclature Committee for Pediatric and Congenital Heart Disease and the representatives of the steering group responsible for the creation of the 11th revision of the International Classification of Diseases, administered by the World Health Organisation. Similar collaborative efforts are underway involving the leadership of The International Nomenclature Committee for Pediatric and Congenital Heart Disease and the International Health Terminology Standards Development Organisation, who are the owners of the Systematized Nomenclature of Medicine or ""SNOMED"". The International Paediatric and Congenital Cardiac Code was created by specialists in the field to name and classify paediatric and congenital cardiac disease and its treatment. It is a comprehensive code that can be freely downloaded from the internet (http://www.IPCCC.net) and is already in use worldwide, particularly for international comparisons of outcomes. The goal of this effort is to create strategies for stratification of risk and to improve healthcare for the individual patient. The collaboration with the World Heath Organization, the International Health Terminology Standards Development Organisation, and the healthcare Industry, will lead to further enhancement of the International Code, and to Its more universal use.

Objective: First, to assess the clinical effectiveness of hylan G-F 20 in an appropriate care treatment regimen (as defined by the American College of Rheumatology (ACR) 1995 guidelines) as measured by validated disease-specific outcomes and health-related quality of life endpoints for patients with osteoarthritis (OA) of the knee. Second, to utilize the measures of effectiveness and costs in an economic evaluation (see accompanying manuscript). Design: A total of 255 patients with OA of the knee were enrolled by rheumatologists or orthopedic surgeons into a prospective, randomized, open-label, 1-year, multi-centred trial, conducted in Canada. Patients were randomized to 'Appropriate care with hylan G-F 20' (AC+H) or 'Appropriate care without hylan G-F 20' (AC). Data were collected at clinic visits (baseline, 12 months) and by telephone (1, 2, 4, 6, 8, 10, and 12 months). Results: The AC+H group was superior to the AC group for all primary (% reduction in mean Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain scale: 38% vs 13%, P=0.0001) and secondary effectiveness outcome measures. These differences were all statistically significant and exceeded the 20% difference between groups seta priori by the investigators as the minimum clinically important difference. Research reports prepared by three Australian preservice teachers--Paula Shaw, Chris Sharp and Scott McDonald--undertaking their teacher education practicum in Canada, form the basis of this paper. The reports provide critical insights into three aspects of education for young people in both Canada and Australia. They also provide critical insight into the ways in which a practicum research project, along with the opportunities afforded through an international experience, enabled the preservice teachers to broaden their understanding of the curriculum for young people, of issues relevant to the diverse needs of young people, and of themselves and their priorities as teachers. The preservice teachers investigated three topics: attempts to reduce homophobia in schools; the presence or absence of Aboriginal content in the school curricula in British Columbia and Queensland; and "schools-within-schools" as a means to meet the needs of diverse student populations. CONTEXT: Despite more than 2 decades of outcomes research after very preterm birth, clinicians remain uncertain about the extent to which neonatal morbidities predict poor long-term outcomes of extremely low-birth-weight (ELBW) infants. OBJECTIVE: To determine the individual and combined prognostic effects of bronchopulmonary dysplasia (BPD), ultrasonographic signs of brain injury, and severe retinopathy of prematurity (ROP) on 18-month outcomes of ELBW infants. DESIGN: Inception cohort assembled for the Trial of Indomethacin Prophylaxis in Preterms (TIPP). SETTING AND PARTICIPANTS: A total of 910 infants with birth weights of 500 to 999 g who were admitted to 1 of 32 neonatal intensive care units in Canada, the United States, Australia, New Zealand, and Hong Kong between 1996 and 1998 and who survived to a postmenstrual age of 36 weeks. MAIN OUTCOME MEASURES: Combined end point of death or survival to 18 months with 1 or more of cerebral palsy, cognitive delay, severe hearing loss, and bilateral blindness. RESULTS: Each of the neonatal morbidities was similarly and independently correlated with a poor 18-month outcome. Odds ratios were 2.4 (95% confidence interval [CI], 1.8-3.2) for BPD, 3.7 (95% CI, 2.6-5.3) for brain injury, and 3.1 (95% CI, 1.9-5.0) for severe ROP. In children who were free of BPD, brain injury, and severe ROP the rate of poor long-term outcomes was 18% (95% CI, 14%-22%). Corresponding rates with any 1, any 2, and all 3 neonatal morbidities were 42% (95% CI, 37%-47%), 62% (95% CI, 53%-70%), and 88% (64%-99%), respectively. CONCLUSION: In ELBW infants who survive to a postmenstrual age of 36 weeks, a simple count of 3 common neonatal morbidities strongly predicts the risk of later death or neurosensory impairment.

This article analyzes the climate policy performance of the G-8 from 1992 to 2012 based on their legal commitments (Annex-1 and Annex-B countries) under the UNFCCC (1992) and the Kyoto Protocol (1997) and their policy declarations on their GHG reduction goals until 2050. A climate paradox has emerged due to a growing implementation gap in Canada, USA and Japan, while Russia, Germany, UK, France and Italy fulfilled their GHG reduction obligation.

Este trabalho, em co-autoria, visa divulgar os processos de internacionalização da Universidade dos Açores e da Universidade do Minho, dando conta de um conjunto diversificado de projetos na área do ensino da língua portuguesa e cultura em diferentes partes do mundo.

O objetivo deste estudo foi avaliar o padrão de participação em atividades de lazer, formais e informais, de crianças e jovens com e sem incapacidade nos seus contextos imediatos – em casa, “fora de casa”, e na escola. Usamos uma adaptação do instrumento CAPE - Children’s Assessment of Participation and Enjoyment e CAP - Preferences for Activities of Children, originalmente desenvolvido no Canadá por King e seus colaboradores (2004). Participaram neste estudo 56 alunos – 19 com incapacidades e 37 sem incapacidades – de dez turmas dos três ciclos de educação básica com idades compreendidas entre os 7 e 16 anos de idade. Os resultados deste estudo revelaram que, ao nível da diversidade, apenas no contexto “fora de casa” existem diferenças significativas, isto é, as crianças/jovens com incapacidade participam em menos atividades – no total, em atividades formais, em atividades físicas e de autoaperfeiçoamento. Também se verificou existência de diferenças no padrão de participação ao nível da intensidade, com as crianças e jovens com incapacidades a reportarem uma participação mais limitada mas maiores índices nos contextos em casa e na escola. No contexto “escola”, os alunos com incapacidades participam em atividades com significativa menor dimensão social. No entanto, em relação ao nível de satisfação, verificámos que não existem diferenças. No que diz respeito à dimensão preferência verificamos que está positivamente relacionada com o padrão de participação. A consideração de outras características pessoais como a idade e o sexo poderá enriquecer este estudo, bem como a aplicação deste estudo a amostras mais representativas.

O texto tem por base a investigação realizada no quadro do projecto ““Feeling the Pulse of the community" - Identity politics and narratives of a portuguese migrant community in Canada, financiado pela Fundação para a Ciência e Tecnologia. O projecto centrou-se no estudo do programa de televisão “Gente da Nossa”, que integra as características dos “Ethnic minority media”. Colocando-se no interior do campo da Antropologia dos Media, a estratégia metodológica integrou uma etnografia da produção e da recepção. O texto coloca o programa “Gente da Nossa” no interior da problemática mais geral das relações complexas que se estabelecem entre a produção e a difusão dos conteúdos dos “media étnicos” e os processos de etnicização de comunidades migrantes: o programa é conceptualizado enquanto veículo estrategicamente usado nos processos de construção, difusão, objectificação e mercadorização de imagens da “comunidade portuguesa do Canadá”. Quando tomado no seu conjunto, o processo de etnicização surge como algo complexo: uma etnografia do detalhe permite ler e discutir as diferentes dimensões desse complexo trabalho de construção da ideia de “comunidade”.
