Este artigo aborda a necessidade de se dar um tratamento estratatégico à Administração de Recursos Humanos, da mesma forma como normalmente ocorre em outras áreas da Administração, de maneira que as Organizações possam estabelecer um relacionamento positivo e duradouro com o seu ambiente. O artigo sugere, de forma prática, algumas atividades de Recursos Humanos que deverão estar presentes nas diferentes etapas de um plano estratégico.

OBJETIVO: Evaluar la relación entre consumo precoz de tabaco y alcohol y el riesgo de consumir marihuana en escolares. MÉTODOS: Estudio transversal con datos del IV Estudio Nacional de Consumo de Drogas en Población Escolar, año 2001. Se analizó 54,001 escolares de ocho a 20 años de edad. Como variables predictoras se consideró el autoreporte de consumo de tabaco y alcohol, la edad de inicio de consumo de tabaco y/o alcohol, y la intensidad de uso del tabaco. Como variable resultado se utilizó el autoreporte de uso de marihuana y la edad de inicio de consumo de ésta. Para el análisis de los datos se utilizó regresión de Poisson y regresión de Weibull. RESULTADOS: Los escolares presentaron alta prevalencia de consumo de tabaco, alcohol y marihuana (77%, 79% y 23% respectivamente). El consumo de tabaco en presencia de consumo de alcohol fue un factor de riesgo de uso de marihuana (RP=10.4; IC 95%: 8.9;12.2). El inicio tardío de consumo de tabaco (HR=0.85; IC 95%: 0.84;0.86) y alcohol (HR=0.90; IC 95%: 0.89;0.91) resultó ser un factor protector del uso de marihuana. La probabilidad de consumo de marihuana fue mayor en quienes fumaban todos o casi todos los días en relación a quienes fumaban sólo los fines de semana (RP=3.11; IC 95%: 2.96;3.26 vs. RP=1.70; IC 95%: 1.58;1.83). CONCLUSIONES: El riesgo de consumo de marihuana se asoció significativamente con la edad de inicio de consumo de tabaco, la frecuencia de consumo de tabaco y el consumo simultáneo de alcohol. Las estrategias de prevención deberían orientarse a evitar el consumo precoz de tabaco en escolares.

Outbreaks of rapidly growing mycobacteria have been occasionally described. The article reports an outbreak of cutaneous abscesses due to Mycobacterium chelonae following mesotherapy in Lima, Peru. From December 2004 through January 2005, 35 subjects who had participated in mesotherapy training sessions presented with persistent cutaneous abscesses. Thirteen (37%) of these suspected cases consented to underwent clinical examination. Skin punch-biopsies were collected from suspicious lesions and substances injected during mesotherapy were analyzed. Suspected cases were mainly young women and lesions included subcutaneous nodules, abscesses and ulcers. Mycobacterium chelonae was isolated from four patients and from a procaine vial. In conclusion, it is important to consider mesotherapy as a potential source of rapidly growing mycobacteria infections.

El objetivo fue describir un brote de faringo-amigdalitis causado por estreptococos β-hemolíticos del grupo A (EGA) en profesionales de la salud. El estudio que se transmite de persona-persona o por vía alimentaria. El estudio transversal descriptivo se realizó en 17 clientes, localizados en la misma mesa, que participaron de una cena en restaurante de Barcelona, España, en julio de 2012. Se analizaron, la frecuencia de síntomas de los afectados, el tiempo y la severidad de los síntomas, variables demográficas y alimentos ingeridos, entre otros factores. La tasa de ataque (TA) en los comensales fue del 58,8% (10/17). El 60,0% (6/10) de los comensales fueron positivos para EGA. El 46,2% (6/13) de los manipuladores de alimentos suministrados en la cena presentaron síntomas. No se identificó asociación con los alimentos ingeridos. Existen evidencias epidemiológicas de la transmisión alimentaria del EGA, pero no podría descartarse la transmisión respiratoria.

A eficiência do uso de uma alça metálica para a remoção de elementos parasitários flutuantes foi investigada. A quantidade de elementos removidos a cada alçada sucessiva foi confrontada com um modelo logarítmico, o qual indicou que cada alçada retirou cerca de 30% dos elementos parasitários flutuantes remanescentes. Estimou-se que as 5 primeiras alçadas retiraram um total de 83% dos elementos parasitários flutuantes, sendo este um procedimento adequado para o método sob investigação.

Three cases of the juvenile form of paracoccidioidomycosis are reported. Emphasis has been given to the oral manifestations, particularly the periodontal involvement. The main periodontal findings were: generalized and progressive alveolar bone destruction leading to gingival recession with exposure of the tooth roots, and spontaneous tooth losses. The gingival mucosa was predominantly smooth, erithematous and slightly swollen. These aspects, although rare, may be the earliest signs of the disease and sometimes its only manifestation.

The objective of the present study was to evaluate the serum viral load in chronically infected Hepatitis B virus (HBV) patients and to investigate the distribution of HBV genotypes in São Paulo city. Quantitative HBV-DNA assays and HBV genotyping have gained importance for predicting HBV disease progression, have been employed for assessing infectivity, for treatment monitoring and for detecting the emergence of drug resistance. Twenty-nine Brazilian patients with suspected chronic hepatitis B were studied, using real time PCR for viral load determination and direct DNA sequencing for the genotyping. The serology revealed chronic HBV infection in 22 samples. The HBV-DNA was positive in 68% samples (15/22). The phylogenetic analysis disclosed that eleven patients were infected with HBV genotype A, two with genotype F and two with genotype D. Thus, the genotype A was the most prevalent in our study.

OBJETIVO: Avaliar a produção científica brasileira, no ano de 2005, nas 20 revistas de psiquiatria com maior fator de impacto (FI) segundo o Journal of Citation Report (JCR), 2004. MÉTODOS: Do total de 90 periódicos com FI variando de 11,207 até 0,13, selecionamos todos os artigos dos periódicos com FI acima do percentil 75 (n = 20). Avaliamos, entre esses artigos, quantos apresentavam pelo menos um autor brasileiro, quantos eram compostos por equipe exclusivamente brasileira ou tinham pelo menos o primeiro autor e/ou o autor sênior filiado à instituição nacional. Avaliamos também a respectiva distribuição das publicações por estados de acordo com a instituição de filiação do(s) autor(es). RESULTADOS: Entre 4.859 artigos avaliados, 54 (1,11%) tinham pelo menos um autor com afiliação a instituição brasileira e 22 tinham autor sênior brasileiro, excluindo-se as 32 publicações compostas exclusivamente por pesquisadores brasileiros. Ao se determinar a distribuição, por estados, da instituição de pesquisa à qual está(ão) vinculado(s) o(s) autor(res) brasileiro(s), São Paulo, com 34 publicações (55,73%), ficou na primeira posição, com predominância absoluta do eixo Sudeste-Sul (96,72%). CONCLUSÃO: Apesar de as publicações brasileiras em psiquiatria terem apresentado aparente crescimento quantitativo e qualitativo, a produção científica da área é ainda pequena e altamente concentrada em alguns poucos estados.

OBJECTIVE: To analyze the early and late results of cardiopulmonary resuscitation in a cardiology hospital and to try to detect prognostic determinants of both short- and long-term survival. METHODS: A series of 557 patients who suffered cardiorespiratory arrest (CRA) at the Dante Pazzanese Cardiology Institute over a period of 5 years was analyzed to examine factors predicting successful resuscitation and long-term survival. RESULTS: Ressuscitation maneuvers were tried in 536 patients; 281 patients (52.4%) died immediately, and 164 patients (30.6%) survived for than 24 hours. The 87 patients who survived for more than 1 month after CRA were compared with nonsurvivors. Coronary disease, cardiomyopathy, and valvular disease had a better prognosis. Primary arrhythmia occurred in 73.5% of the >1-month survivor group and heart failure occurred in 12.6% of this group. In those patients in whom the initial mechanism of CRA was ventricular fibrillation, 33.3% survived for more than 1 month, but of those with ventricular asystole only 4.3% survived. None of the 10 patients with electromechanical dissociation survived. There was worse prognosis in patients included in the extreme age groups (zero to 10 years and 70 years or more). The best results occurred when the cardiac arrest took place in the catheterization laboratories. The worst results occurred in the intensive care unit and the hemodialysis room. CONCLUSION: The results in our series may serve as a helpful guide to physicians with the difficult task of deciding when not to resuscitate or when to stop resuscitation efforts.

OBJECTIVE: To evaluate the relationship between 24-hour ambulatory arterial blood pressure monitoring and the prognosis of patients with advanced congestive heart failure. METHODS: We studied 38 patients with NYHA functional class IV congestive heart failure, and analyzed left ventricular ejection fraction, diastolic diameter, and ambulatory blood pressure monitoring data. RESULTS: Twelve deaths occurred. Left ventricular ejection fraction (35.2±7.3%) and diastolic diameter (72.2±7.8mm) were not correlated with the survival. The mean 24-hour (SBP24), waking (SBPw), and sleeping (SBPs) systolic pressures of the living patients were higher than those of the deceased patients and were significant for predicting survival. Patients with mean SBP24, SBPv, and SBPs > or = 105mmHg had longer survival (p=0.002, p=0.01 and p=0.0007, respectively). Patients with diastolic blood pressure sleep decrements (dip) and patients with mean blood pressure dip <=6mmHg had longer survival (p=0.04 and p=0.01, respectively). In the multivariate analysis, SBPs was the only variable with an odds ratio of 7.61 (CI: 1.56; 3704) (p=0.01). Patients with mean SBP<105mmHg were 7.6 times more likely to die than those with SBP > or = 105 mmHg CONCLUSION: Ambulatory blood pressure monitoring appears to be a useful method for evaluating patients with congestive heart failure.
Electrocardiographic and Blood Pressure Alterations During Electroconvulsive Therapy in Young Adults

OBJECTIVE - To study cardiovascular alterations in young patients with no apparent organic disease who underwent electroconvulsive therapy. METHODS - The study comprised 47 healthy patients (22 males and 25 females) with a mean age of 30.3 years, who underwent electroconvulsive therapy. Ambulatory blood pressure monitoring and continuous electrocardiographic monitoring (Holter monitor) were performed during 24 hours. Blood pressure and heart rate were assessed 4 hours prior to electric shock administration, during electric shock administration, and 3 hours after electric shock administration. Arrhythmias and alterations in the ST segment in 24 hours were recorded. RESULTS - On electroconvulsive therapy, a significant increase in blood pressure and heart rate was observed and the measurements returned to basal values after 25 minutes. Three females had tracings with depression of the ST segment suggesting myocardial ischemia prior to and after electroconvulsive therapy. Coronary angiography was normal. No severe cardiac arrhythmias were diagnosed. CONCLUSION - 1) Electroconvulsive therapy is a safe therapeutic modality in psychiatry; 2) it causes a significant increase in blood pressure and heart rate; 3) it may be associated with myocardial ischemia in the absence of coronary obstructive disease; 4) electroconvulsive therapy was not associated with the occurrence of severe cardiac arrhythmias.
