The objective of the present study was to estimate the frequency of infection by Cryptosporidium spp and other intestinal parasites in dehydrated children with gastroenteritis who were admitted to a pediatric hospital. Stool examinations from 218 children were performed. Cryptosporidium spp was identified in eighteen out of 193 stool samples (9.3%) subjected to safranin-methylene blue staining. Giardia lamblia was detected in ten out of 213 (4.7%) samples examined via the direct or Ritchie methods. Other parasites identified were Ascaris lumbricoides (4.2%), Blastocystis hominis (1.4%), Entamoeba coli (0.9%), Entamoeba histolytica/Entamoeba dispar (0.5%), Endolimax nana (0.5%), Trichuris trichiura (0.5%) and Enterobius vermicularis (0.5%).

Realizou-se coletas de flebotomíneos de maio de 2005 a abril de 2006. Os resultados foram comparados com os das coletas realizadas entre abril de 2001 e setembro de 2002, para avaliar as medidas empregadas para diminuir a densidade destes insetos, no Recanto Marista, município de Doutor Camargo, Estado do Paraná. As coletas foram feitas com armadilhas de Falcão, em domicílios e galinheiros, das 22 às 2 horas, semanalmente, quatro vezes ao mês. Em 2005 e 2006, coletaram-se 213.195 flebotomíneos, 1.113,8 em média por hora (MH) e em 2001 e 2002, 199.821 (MH=1.653,5). Nyssomyia neivai prevaleceu (75,4%) em todos os ecótopos que, juntamente com Nyssomyia whitmani (23,4%), Migonemyia migonei (0,8%) e Pintomyia fischeri (0,4%), representaram 99,7% do total coletado. Nos ecótopos representados por galinheiros foram coletados 88,7% dos flebotomíneos. Constatou-se que houve queda na densidade de flebotomíneos no período de coletas 2005 e 2006 em relação à de 2001 e 2002, especialmente no domicílio.

Foram estudados 233 casos de fase aguda da doença de Chagas, oriundos do Pará, Amapá e Maranhão, observados no período de 1988 a 2005, cento e sessenta deles retrospectivamente de 1988 a 2002 e setenta e três prospectivamente de 2003 a 2005. Entre os casos estudados 78,5% (183/233) faziam parte de surtos provavelmente por transmissão oral, acometendo em média 4 pessoas e 21,5% (50/233) eram casos isolados. Foram considerados casos agudos aqueles que apresentaram exames parasitológicos diretos (a fresco, gota espessa ou Quantitative Buffy Coat - QBC) e/ou IgM anti-Trypanosoma cruzi positivos. Foram feitos ainda xenodiagnósticos em 224 pacientes e hemoculturas em 213. Todos foram avaliados clinica e epidemiologicamente. As manifestações clínicas mais freqüentes foram febre (100%), cefaléia (92,3%), mialgia (84,1%), palidez (67%), dispnéia (58,4%), edema de membros inferiores (57,9%), edema de face (57,5%) dor abdominal (44,2%), miocardite (39,9%) e exantema (27%). O eletrocardiograma mostrou alterações de repolarização ventricular em 38,5% dos casos, baixa voltagem de QRS em 15,4% e desvio de SAQRS em 11,5%, extra-sístoles ventriculares em 5,8%, bradicardia em 5,8% e taquicardia em 5,8%, bloqueio de ramo direito em 4,8% e fibrilação atrial em 4,8%. A alteração mais freqüente vista no ecocardiograma foi o derrame pericárdico em 46,2% dos casos. Treze (5,6%) pacientes evoluíram para o óbito, 10 (76,9%) dos quais por comprometimento cardiovascular, dois por complicações de origem digestiva e um de causa mal definida.

INTRODUCTION: Human serofrequency of antibodies against Taenia solium antigens was determined and risk factors for cysticercosis transmission were identified. METHODS: Individuals (n=878) from periurban and rural locations of Lages, SC, were interviewed to gather demographic, sanitary and health information. Interviews and blood sample collections by finger prick on Whatman filter paper were performed from August 2004 to May 2005. Observation determined that 850 samples were suitable for analysis and were tested by ELISA using vesicular fluid of Taenia crassiceps heterologous antigen. To ensure the reliability of the results, 77 samples of the dried blood were matched with sera. The reactive samples were submitted to a serum confirmatory immunoblot (IB) test using purified Taenia crassiceps glycoproteins. RESULTS: The ELISA results for the dried blood and serum samples were statistically consistent. ELISA was positive in 186 (21.9%) out of 850 individuals. A group of 213 individuals were asked to collect vein blood for IB (186 with positive result in ELISA and 27 with inappropriate whole blood samples) and 130 attended the request. The IB was positive in 29 (3.4%) out of 850 individuals. A significant correlation (p = 0.0364) was determined among individuals who tested positive in the IB assay who practiced both pig rearing and kitchen gardening. CONCLUSIONS: ELISA with dried blood eluted from filter paper was suitable for cysticercosis population surveys. In Lages, human infection was associated with pig rearing and kitchen gardening. The prevalence index was compatible with other Latin American endemic areas.

INTRODUCTION: Paracoccidioidomycosis (PCM) is the most important systemic mycosis in South America. Central nervous system involvement is potentially fatal and can occur in 12.5% of cases. This paper aims to contribute to the literature describing eight cases of neuroparacoccidioidomycosis (NPMC) and compare their characteristics with patients without neurological involvement, to identify unique characteristics of NPCM. METHODS: A cohort of 213 PCM cases was evaluated at the Infectious Diseases Clinic of the University Hospital, Federal University of Minas Gerais, Brazil, from October 1976 to August 2008. Epidemiological, clinical, laboratory, therapeutic and follow-up data were registered. RESULTS: Eight patients presented NPCM. The observed NPCM prevalence was 3.8%. One patient presented the subacute form of PCM and the other seven presented the chronic form of the disease. The parenchymatous form of NPCM occurred in all patients. 60% of the patients who proceeded from the north/ northeast region of Minas Gerais State developed NPCM. The neurological involvement of a mother and her son was observed. NPCM patients exhibited demographical and clinical profiles similar to what is described in the literature. When NPCM cases were compared to PCM patients, there were differences in relation to origin and positive PCM family history. CONCLUSIONS: The results corroborate the clinical view that the neurological findings are extremely important in the evaluation of PCM patients. Despite the limitations of this study, the differences in relation to patient's origins and family history point to the need of further studies to determine the susceptibility factors involved in the neurological compromise.

INTRODUCTION: Operational classification of leprosy based on the number of skin lesions was conceived to screen patients presenting severe forms of the disease to enable their reception of a more intense multidrug regimen without having to undergo lymph smear testing. We evaluated the concordance between operational classification and bacilloscopy to define multibacillary and paucibacillary leprosy. METHODS: We selected 1,213 records of individuals with leprosy, who were untreated (new cases) and admitted to a dermatology clinic in Recife, Brazil, from 2000 to 2005, and who underwent bacteriological examination at diagnosis for ratification of the operational classification. RESULTS: Compared to bacilloscopy, operational classification demonstrated 88.6% sensitivity, 76.9% specificity, a positive predictive value of 61.8%, and a negative predictive value of 94.1%, with 80% accuracy and a moderate kappa index. Among the bacilloscopy-negative cases, 23% had more than 5 skin lesions. Additionally, 11% of the bacilloscopy-positive cases had up to 5 lesions, which would have led to multibacillary cases being treated as paucibacillary leprosy if the operational classification had not been confirmed by bacilloscopy. CONCLUSIONS: Operational classification has limitations that are more obvious in borderline cases, suggesting that in these cases, lymph smear testing is advisable to enable the selection of true multibacillary cases for more intense treatment, thereby contributing to minimization of resistant strain selection and possible relapse.

RESUMO: Objectivo: O presente estudo tem como principal objectivo a caracterização da vulnerabilidade individual em pessoas idosas residentes na comunidade. Metodologia: Trata-se de um estudo não experimental, exploratório, quantitativo, e transversal numa amostra probabilística de aleatorização simples (n=213). A vulnerabilidade individual foi avaliada com recurso ao instrumento Vulnerable Elders Survey 13 e as restantes variáveis sócio-demográficas por questionário, ambas por via telefónica. Resultados: A amostra era predominantemente feminina, com uma maior prevalência das pessoas com 75 ou mais anos, viúvas, sem escolaridade, maioritariamente a viverem sós e mais de com um rendimento mensal igual ou inferior a 485€. Relativamente à vulnerabilidade, numa amostra aleatorizada de 213 pessoas, apenas 15 não apresentavam vulnerabilidade, avaliada pelo Vulnerable Elders Survey-13 (VES-13). Das 198 pessoas vulneráveis, correspondendo a 93% da amostra, 164, ou seja, 77% deste grupo, tinham uma pontuação total igual ou superior a 7 em 10, revelando um grau elevado de vulnerabilidade. Conclusão: As variáveis preditoras da vulnerabilidade são a idade, quanto maior a idade maior a vulnerabilidade, e o ser-se viúvo. As variáveis preditoras da dificuldade na realização das actividades físicas (AF) são a idade e pessoa isolada em alojamento colectivo. Para as actividades de vida diária (AVD), as variáveis que demostram ser preditoras são a idade, pessoa isolada em alojamento colectivo e residir em agregado familiar de casal de idosos.----------------------- ABSTRACT:Objective: The present study has as its principal objective the characterization of individual vulnerability in community-dwelling older people. Methodology: This is a non-experimental study, exploratory, qualitative, quantitative, and cross-sectional in a probabilistic sample of simple randomization (n=213). Individual vulnerability was assessed using the instrument Vulnerable Elders Survey 13 and the other socio-demographic variables by questionnaire, both by telephone. Results: the sample was predominantly female, with a higher prevalence of persons with 75 or more years, widows, without education, mostly living alone and most of all with less than or equal to 485€ per month. Concerning vulnerability, in a random sample of 213 people, only 15 doesn´t present vulnerability, evaluated by the Vulnerable Elders Survey-13 (VES-13). Of the 198 people vulnerable, accounting for 93% of the sample, 164, i.e. 77% of this group, had a total score equal to or greater than 7 in 10, revealing a high degree of vulnerability. Conclusion: The predictors of vulnerability are the age, where higher the age means greater vulnerability as well as being a widow. The predictors of the difficulty in physical activities are old age and isolated persons in collective accommodation. For the activities of daily life, the variables that demonstrate as being predictors are old age, isolated persons in collective accommodation and living in elderly couple.

Stage IV non-small cell lung cancer is a fatal disease, with a median survival of 14 months. Systemic chemotherapy is the most common approach. However the impact in overall survival and quality of life still a controversy. OBJECTIVES: To determine differences in overall survival and quality of life among patients with stage IV non-small cell lung cancer non-metastatic to the brain treated with best supportive care versus systemic chemotherapy. PATIENTS: From February 1990 through December 1995, 78 eligible patients were admitted with the diagnosis of stage IV non-small cell lung cancer . Patients were divided in 2 groups: Group A (n=31 -- treated with best supportive care ), and Group B (n=47 -- treated with systemic chemotherapy). RESULTS: The median survival time was 23 weeks (range 5 -- 153 weeks) in Group A and 55 weeks (range 7.4 -- 213 weeks) in Group B (p=0.0018). In both groups, the incidence of admission for IV antibiotics and need of blood transfusions were similar. Patients receiving systemic chemotherapy were also stratified into those receiving mytomycin, vinblastin, and cisplatinum, n=25 and those receiving other combination regimens (platinum derivatives associated with other drugs, n=22). Patients receiving mytomycin, vinblastin, and cisplatinum, n=25 had a higher incidence of febrile neutropenia and had their cycles delayed for longer periods of time than the other group. These patients also had a shorter median survival time (51 versus 66 weeks, p=0.005). CONCLUSION: In patients with stage IV non-small cell lung cancer, non-metastatic to the brain, chemotherapy significantly increases survival compared with best supportive care.

RESUMO - Os resíduos hospitalares (RH) perigosos — Grupos III e IV — produzidos na prestação de cuidados domiciliários (CD), dada a sua composição, infecciosidade, toxicidade, mobilidade e persistência, constituem um perigo relevante. A exposição a estes resíduos traduz-se num risco importante para os profissionais de saúde, doentes e seus familiares. Dado que em muitas situações estes resíduos ficam no domicílio dos doentes, sendo posteriormente depositados nos contentores camarários, o risco é alargado ao público em geral, aos catadores e aos profissionais de recolha de resíduos sólidos urbanos dos municípios. Através de um estudo observacional, transversal, com componente analítica, da produção de RH pretende-se determinar e caracterizar os quantitativos dos Grupos III e IV produzidos na prestação de CD em 2003 no concelho da Amadora, identificando também o seu destino final. Utiliza- se uma amostra aleatória do universo de doentes submetidos a tratamento domiciliário em 2003 e efectua-se a análise da associação estatística das variáveis peso do Grupo III e peso do Grupo IV com as variáveis relativas às características do doente (sexo, idade e doença), do tratamento (duração e periodicidade) e sazonais (época do ano). A média do peso produzido dos RH por acto prestado é de 213,1 g para o Grupo III e de 3,8 g para o Grupo IV. Estima--se uma produção de RH do Grupo III na prestação de CD, em 2003, no concelho da Amadora entre 8,8 e 11,4 t e para os RH do Grupo IV um valor de 10,2 kg. Verifica-se que, por acto prestado, a produção média de resíduos do Grupo III é maior nos doentes mais idosos, nas úlceras varicosas, no pé diabético, na escara de pressão, nas situações de maior duração do tratamento e nos doentes submetidos a três tratamentos por semana. Também por acto prestado, a produção média de RH do Grupo IV é maior nos doentes mais novos, na patologia osteo-articular, na infecção, no acidente, no pós-operatório, nas situações de menor duração do tratamento e nos doentes submetidos a seis tratamentos por semana (o que está relacionado com as patologias em causa). As produções médias, por acto prestado, de ambos os grupos não apresentam relação com as variáveis idade e época do ano. Todos os RH produzidos nos actos prestados em CD, em 2003, no concelho da Amadora foram depositados nos contentores municipais. Recomendam-se acções de formação e de informação dirigidas aos profissionais de saúde e ao público em geral, a criação de condições para que os RH produzidos nos CD sejam transportados, em condições adequadas, para os centros de saúde e uma articulação entre os órgãos de gestão dos centros de saúde, a autarquia, os operadores de gestão de RH e os serviços de saúde pública no sentido de serem encontradas soluções apropriadas e inovadoras relativamente à gestão dos RH produzidos na prestação de CD.

Here, we evaluate the diagnostic and prognostic role of liquid-based cytology (LBC) in different body lesions, including thyroid, lung, effusions and malignant breast lesions. LBC has gained consensus after being applied to both non-gynecologic and fine-needle aspiration cytology. Although some remain sceptical regarding the diagnostic efficacy of LBC, mainly when used alone, in recent years, good results have been obtained as long as it showed a high diagnostic accuracy. Here, we discuss the additional possibility of storing material for the application of ancillary techniques (immunocytochemistry–molecular analysis) with several diagnostic and prognostic advantages, which may pave the way for the challenging evaluation of both monitoring responses to treatment and resistance to targeted therapies in thyroid, lung, breast carcinoma or malignant effusions. Furthermore, it provides the use of several molecular spots as specific targets for personalized therapy.

OBJECTIVE: To verify if adaptive left ventricle (LV) characteristics are also present in individuals under 70 years of age with severe aortic stenosis (AS). METHODS: The study comprised 40 consecutive patients under 70 years of age with AS and no associated coronary artery disease, referred for valve surgery. Out of the 40 patients, 22 were men and 18 women, and the mean age was 49.8±14.3 years. Cardiac symptoms, presence of systemic hypertension (SH), functional class according to the New York Heart Association (NYHA), and valve lesion etiology were considered. LV cavity dimensions, ejection fraction (EF), fractional shortening (FS), mass (MS), and relative diastolic thickness (RDT) were examined by Doppler echocardiography. RESULTS: Fourteen (63.6%) men and 11 (61.6%) women were classified as NYHA class III/IV (p=0.70). There was no difference in the frequency of angina, syncope or dyspnea between genders. The incidence of SH was greater in women than in men (10 versus 2, p=0.0044). Women had a smaller LV end-diastolic diameter index (32.1±6.5 x 36.5±5.3mm/m², p=0.027), LV end-systolic diameter index (19.9±5.9 x 26.5±6.4mm/m², p=0.0022) and LV mass index (MS) (211.4±71.1 x 270.9±74.9g/m², p=0.017) when compared with men. EF (66.2±13.4 x 52.0±14.6%, p=0.0032), FS (37.6±10.7 x 27.9±9.6%, p=0.0046) and RDT (0.58±0.22 x 0.44±0.09, p=0.0095) were significantly greater in women than in men. CONCLUSION: It is the patient gender rather than age that influences left ventricular adaptive response to AS.

The use of chemicals and chemical derivatives in agriculture and industry has contributed to their accumulation and persistence in the environment. Persistent organic pollutants (POPs) are among the environmental pollutants of most concern since, when improperly handled and disposed, they can persist in the environment, bioaccumulate through the food web, and may create serious public health and environmental problems. Development of an effective degradation process has become an area of intense research. The physical/chemical methods employed, such as volatilization, evaporation, photooxidation, adsorption, or hydrolysis, are not always effective, are very expensive, and, sometimes, lead to generation/disposal of other contaminants. Biodegradation is one of the major mechanisms by which organic contaminants are transformed, immobilized, or mineralized in the environment. A clear understanding of the major processes that affect the interactions between organic contaminants, microorganisms, and environmental matrix is, thus, important for determining persistence of the compounds, for predicting in situ transformation rates, and for developing site remediation. Information on their risks and impact and occurrence in the different environmental matrices is also important, in order to attenuate their impact and apply the appropriate remediation process. This chapter provides information on the fate of pesticides and polycyclic aromatic hydrocarbons (PAHs), their impact, bioavailability, and biodegradation. © Springer Science+Business Media Dordrecht 2014.

Between 2011 and 2012, 213 heterosexual couples undergoing fertility treatments in a Portuguese public fertility centre were systematically recruited to assess factors associated with willingness to donate embryos for research. Data were collected by questionnaire. Most couples (87.3%; 95% CI 82.1 to 91.5) were willing to donate embryos for research, citing benefits for science, health and infertile patients. Almost all couples (94.3%; 95% CI 89.8 to 96.7) reached consensus about the decision. Willingness to donate was more frequent in women younger than 36 years (adjusted OR 3.06; 95% CI 1.23 to 7.61) and who considered embryo research to be very important (adjusted OR: 6.32; 95% CI 1.85 to 21.64), and in Catholic men (adjusted OR 4.16; 95% CI 1.53 to 11.30). Those unwilling to donate reported conceptualizing embryos as children or living beings and a lack of information or fears about embryo research. Men with higher levels of trait anxiety (adjusted OR 0.90; 95% CI 0.84 to 0.96) were less frequently willing to donate. Future research on embryo disposition decision-making should include the assessment of gender differences and psychosocial factors. Ethically robust policies and accurate information about the results of human embryo research are required.

Background: The Neonatal Behavioral Assessment Scale (NBAS, Brazelton & Nugent, 1995) is an instrument conceived to observe the neonatal neurobehavior. Data analysis is usually performed by organizing items into groups. The most widely used data reduction for the NBAS was developed by Lester, Als, and Brazelton (1982). Objective: Examine the psychometric properties of the NBAS items in a sample of 213 Portuguese infants. Method: The NBAS was performed in the first week of infant life (3 days±2) and in the seventh week of life (52 days±5). Results: Principal component analyses yielded a solution of four components explaining 55.13% of total variance. Construct validity was supported by better neurobehavioral performance of 7-week-old infants compared with 1-week-old infants. Conclusion: Changes in the NBAS structure for the Portuguese sample are suggested compared to Lester factors in order to reach better internal consistency of the scale.
