RESUMO: Introdução: O treino de exercício assume uma importância fundamental nos programas de reabilitação respiratória, contudo permanece desconhecida qual a intensidade de treino efectiva na mudança dos resultados centrados no doente. Objectivos: Foram estudados os efeitos de duas intensidades de treino aeróbio na qualidade de vida relacionada com a saúde, no controlo de sintomas e na tolerância ao exercício, num programa de reabilitação respiratória para doentes com DPOC. Métodos: Trinta e quatro doentes com DPOC de estádios GOLD de ligeiro a muito grave, foram alocados aleatoriamente a intensidades de treino de exercício aeróbio de 60% ou 80% da intensidade máxima avaliada na prova de esforço num programa ambulatório de reabilitação respiratória de 20 sessões. Foram avaliados resultados centrados no doente pela aplicação do questionário respiratório de St.George, do índice de dispneia de Mahler, do questionário de actividades da vida diária do London Chest, da prova de 6 minutos, da prova de endurance a carga constante e da prova de esforço máxima incremental. Resultados: Apesar de existirem melhorias significativas em todos os resultados para ambos os grupos, a diferença média intergrupal por efeito da intensidade de treino aeróbio a 60% ou 80% da carga máxima em prova de esforço não foi significativa no questionário respiratório de St.George (p=0,306), no índice de dispneia de Mahler (p=0,378), no questionário de actividades da vida diária do London Chest (p=0,425), na prova de marcha de 6 minutos (p=0,917), na prova de endurance a carga constante (p=0,504) e na prova de esforço máxima incremental (p=0,117). Verificou-se ainda que a média das diferenças intergrupais na prova de endurance a carga constante e na prova de marcha de 6 minutos não apresentou qualquer associação com a idade dos doentes. Conclusões: O estudo permite concluir que não existem diferenças significativas nas alterações médias da qualidade de vida relacionada com a saúde, do controle de sintomas e da tolerância ao exercício em doentes com DPOC como efeito de duas intensidades de treino aeróbio. Os autores propõem para a prática clínica em reabilitação respiratória, a aplicação de intensidades de treino aeróbio individualmente adaptadas, de pelo menos 60% da carga máxima na prova de esforço inicial. -------------------------------ABSTRACT: Introduction: Exercise training is an important component of pulmonary rehabilitation, but it remains questionable as how training intensity changes patient-centered outcomes. Aims and objectives: The effects of two aerobic exercise training intensities in health-related quality of life, symptoms control and exercise tolerance in COPD patients were studied. Methods: Thirty-four COPD patients from mild to very severe GOLD stages were randomly assigned to a 60% or 80% aerobic exercise training intensity in a twenty-session outpatient pulmonary rehabilitation program. Patient-centered outcomes were assessed with St.George's respiratory questionnaire, Mahler dyspnoea index, London Chest activity of daily living, six minute walk test, constant work rate test and maximal incremental test. Results: Although there were significant improvements in all outcomes for both groups, there were no differences in mean change in St.George's respiratory questionnaire (p=0,306), Mahler dyspnoea index (p=0,378), London Chest activity of daily living (p=0,917), six-minute walk test (p=0,504), constant work rate test (p=0,504) and maximal incremental test (p=0,117), as an effect of aerobic exercise training intensity of 60% or 80%. Mean changes in constant work rate and six-minute walk test were not age-related. Conclusions: It is concluded that there are no differences in mean changes in health-related quality of life, symptoms control and exercise tolerance in COPD patients as an effect of two aerobic exercise training intensities. Therefore, the authors suggest that individually tailored exercise intensities above 60% should guide clinical practice in pulmonary rehabilitation.

We prove a one-to-one correspondence between (i) C1+ conjugacy classes of C1+H Cantor exchange systems that are C1+H fixed points of renormalization and (ii) C1+ conjugacy classes of C1+H diffeomorphisms f with a codimension 1 hyperbolic attractor Lambda that admit an invariant measure absolutely continuous with respect to the Hausdorff measure on Lambda. However, we prove that there is no C1+alpha Cantor exchange system, with bounded geometry, that is a C1+alpha fixed point of renormalization with regularity alpha greater than the Hausdorff dimension of its invariant Cantor set.

Many-core platforms are an emerging technology in the real-time embedded domain. These devices offer various options for power savings, cost reductions and contribute to the overall system flexibility, however, issues such as unpredictability, scalability and analysis pessimism are serious challenges to their integration into the aforementioned area. The focus of this work is on many-core platforms using a limited migrative model (LMM). LMM is an approach based on the fundamental concepts of the multi-kernel paradigm, which is a promising step towards scalable and predictable many-cores. In this work, we formulate the problem of real-time application mapping on a many-core platform using LMM, and propose a three-stage method to solve it. An extended version of the existing analysis is used to assure that derived mappings (i) guarantee the fulfilment of timing constraints posed on worst-case communication delays of individual applications, and (ii) provide an environment to perform load balancing for e.g. energy/thermal management, fault tolerance and/or performance reasons.

A low-cost disposable was developed for rapid detection of the protein biomarker myoglobin (Myo) as a model analyte. A screen printed electrode was modified with a molecularly imprinted material grafted on a graphite support and incorporated in a matrix composed of poly(vinyl chloride) and the plasticizer o-nitrophenyloctyl ether. The protein-imprinted material (PIM) was produced by growing a reticulated polymer around a protein template. This is followed by radical polymerization of 4-styrenesulfonic acid, 2-aminoethyl methacrylate hydrochloride, and ethylene glycol dimethacrylate. The polymeric layer was then covalently bound to the graphitic support, and Myo was added during the imprinting stage to act as a template. Non-imprinted control materials (CM) were also prepared by omitting the Myo template. Morphological and structural analysis of PIM and CM by FTIR, Raman, and SEM/EDC microscopies confirmed the modification of the graphite support. The analytical performance of the SPE was assessed by square wave voltammetry. The average limit of detection is 0.79 μg of Myo per mL, and the slope is −0.193 ± 0.006 μA per decade. The SPE-CM cannot detect such low levels of Myo but gives a linear response at above 7.2 μg · mL−1, with a slope of −0.719 ± 0.02 μA per decade. Interference studies with hemoglobin, bovine serum albumin, creatinine, and sodium chloride demonstrated good selectivity for Myo. The method was successfully applied to the determination of Myo urine and is conceived to be a promising tool for screening Myo in point-of-care patients with ischemia.

In today’s healthcare paradigm, optimal sedation during anesthesia plays an important role both in patient welfare and in the socio-economic context. For the closed-loop control of general anesthesia, two drugs have proven to have stable, rapid onset times: propofol and remifentanil. These drugs are related to their effect in the bispectral index, a measure of EEG signal. In this paper wavelet time–frequency analysis is used to extract useful information from the clinical signals, since they are time-varying and mark important changes in patient’s response to drug dose. Model based predictive control algorithms are employed to regulate the depth of sedation by manipulating these two drugs. The results of identification from real data and the simulation of the closed loop control performance suggest that the proposed approach can bring an improvement of 9% in overall robustness and may be suitable for clinical practice.

This paper follows those expressions to design an efficient algorithm for the approximate calculation of expressions usual in fractional-order control systems. The numerical experiments demonstrate the superior efficiency of the proposed method.

Introdução: Existe alguma controvérsia respeitante ao tipo de revascularização a efectuar no contexto de angioplastia (PCI) primária no enfarte agudo do miocárdio (EAM). A presença de lesões coronárias adicionais, particularmente complexas, poderá ter impacto no prognóstico. Objectivos: Avaliar o prognóstico a médio-prazo (1 ano) face à presença de lesões adicionais complexas após PCI primária. População e Métodos: Estudaram-se retrospectivamente 138 doentes consecutivos admitidos na nossa Unidade por EAM com elevação do segmento ST e submetidos a PCI primária. Os doentes foram seguidos por um período de 1 ano e divididos em 2 grupos: sem lesões adicionais complexas (n=69, 61 ± 14 anos, 62% sexo masculino) e com lesões adicionais complexas (n=69, 65 ± 13 anos, 73% sexo masculino, p=NS). Avaliaram-se as características demográficas, factores de risco para doença coronária, história prévia cardíaca, e presença de sinais de insuficiência cardíaca na admissão. Foram também avaliadas características angiográficas, medicação efectuada e resultado da PCI. Avaliou-se o impacto das variáveis na ocorrência combinada de morte/re-enfarte/revascularização miocárdica ao primeiro ano. Resultados: A taxa de sucesso angiográfico foi de 96,4%. O grupo com idade igual ou superior a 75 anos representa 24% da população e 4,3% apresentaram-se em classe Killip IV. A localização anterior foi ligeiramente superior no grupo sem lesões adicionais complexas (60% vs. 44%, p=0,06), a inferior no grupo com lesões adicionais complexas (26% vs. 42%, p=0,07). A doença de 1 vaso foi mais prevalente no grupo sem lesões adicionais complexas como esperado (86% vs. 11%, p<0,001). A utilização de stent foi mais frequente no grupo sem lesões adicionais complexas (96% vs. 86%, p=0,08). Não houve diferenças nas restantes variáveis. A taxa de morte/re-enfarte/revascularização foi superior no grupo com lesões adicionais complexas (13% vs. 32%, p=0,014). Até aos 13 dias de seguimento, ocorreram 67% dos eventos. Na análise univariada, os factores predizentes de eventos foram a classe Killip 2, fluxo TIMI < 3 no vaso relacionado com enfarte após PCI, a não utilização de antagonistas da glicoproteína IIb/IIIa, bloqueadores beta e estatinas, doença multivaso e presença de lesões adicionais complexas (Log-rank, p=0,003). Na análise multivariável, os factores predizentes independentes de prognóstico a 1 ano foram a classe Killip 2 (Odds ratio 0,28%; IC 95% 0,08-0,93, p=0,037) e a presença de lesões adicionais complexas (OR 0,32; IC 95% 0,12-0,84, p=0,020). Conclusões: A presença de lesões adicionais complexas após PCI primária tem um pior prognóstico ao primeiro ano, sugerindo a necessidade de intervenção para a sua estabilização, particularmente nos primeiros 30 dias após enfarte.

This paper analyzes several natural and man-made complex phenomena in the perspective of dynamical systems. Such phenomena are often characterized by the absence of a characteristic length-scale, long range correlations and persistent memory, which are features also associated to fractional order systems. For each system, the output, interpreted as a manifestation of the system dynamics, is analyzed by means of the Fourier transform. The amplitude spectrum is approximated by a power law function and the parameters are interpreted as an underlying signature of the system dynamics. The complex systems under analysis are then compared in a global perspective in order to unveil and visualize hidden relationships among them.

The decomposition of a fractional linear system is discussed in this paper. It is shown that it can be decomposed into an integer order part, corresponding to possible existing poles, and a fractional part. The first and second parts are responsible for the short and long memory behaviors of the system, respectively, known as characteristic of fractional systems.

Inspired in dynamic systems theory and Brewer’s contributions to apply it to economics, this paper establishes a bond graph model. Two main variables, a set of inter-connectivities based on nodes and links (bonds) and a fractional order dynamical perspective, prove to be a good macro-economic representation of countries’ potential performance in nowadays globalization. The estimations based on time series for 50 countries throughout the last 50 decades confirm the accuracy of the model and the importance of scale for economic performance.

Ollier Disease and Maffucci Syndrome are two rare diseases that can cause tumors in several organs, having a special predilection for the hand. However, there have been very few reports in the literature focusing on hand manifestations of these diseases. We report the cases of three female patients: one with Ollier Disease, and two other with Maffucci Syndrome. All patients had hand involvement as their initial primary complaint. The Ollier Disease patient developed chondrosarcomas of two digits and had to have these fingers amputated. One of the Maffucci patients died one year after presentation from a brain glioblastoma. These cases emphasize the importance of early diagnosis of Ollier Disease and Maffucci Syndrome, as these two conditions are associated not only to crippling hand deformity, but also to a significant risk of chondrosarcoma, and other malignant tumors.

During the year of 2001, a retrospective, descriptive study in order to determine the influence of the antiretroviral therapy received by 111 HIV-HCV coinfected patients who had undergone at least one liver biopsy was conduced, 74 of them were treated with a protease inhibitor regimen (WPI), and 37 with a non-protease inhibitor regimen (NPI). The main characteristics found were: a young patient population (mean age 41 years old in both groups), composed in most part of male individuals (74.3% WPI and 51.4% NPI) with previous risk factors for both infections (WPI 93.2% and NPI 89.2%). The most significant findings included AIDS-defining disease (WPI 18.9% and NPI 13.5% of the cases), elevated hepatic enzyme levels (WPI: SGOT 52.1 and NPI 53.2), absence of liver disease-related symptoms (16.2% for both groups), average CD4 count > 350 for both groups (WPI 362.2 and NPI 378.1), predominantly low-grade fibrosis in both populations (0-2 in 63.6% of WPI patients and in 80% of NPI patients), with necro-inflammatory activity ranging from 5-7 in 51.3% and 42.9% of WPI patients and NPI patients, respectively. It is suggested a sequential biopsy to better evaluate the evolution of the hepatic disease, according to the HAART regimen received.

INTRODUCTION: Low-dose dobutamine stress echocardiography is a common and useful technique to assess myocardial viability in patients with ischemic cardiomyopathy. OBJECTIVE: To evaluate the use of low-dose dobutamine stress echocardiography in determining the functional status of patients with idiopathic dilated cardiomyopathy (IDCM). METHODS: Prospective study of 28 patients with IDCM by transthoracic echocardiography (2D), low-dose dobutamine stress echocardiography, cardiopulmonary exercise testing (CPET) and measurement of pro-BNP. RESULTS: The mean age of the population was 50.3 +/- 11.5 years, 9 female and 19 male. Mean ejection fraction was 32.1 +/- 9.8%. All were in sinus rhythm. The following parameters were analyzed in 2D echocardiography and after dobutamine: dimensions of left atrium (LA) and of left ventricle in diastole and systole, shortening fraction (%), left ventricular end-diastolic (EDV) and end-systolic volumes (ESV), ejection fraction (EF), and mitral inflow (E, A, E/A ratio and deceleration time). In CPET, we considered the following parameters: peak VO2 and % maximal peak VO2 attained. We compared echo results with CPET. There was a correlation between age and peak VO2 (r = -0.38 with p = 0.049). In 2D echo, there was a correlation between baseline EF and LA dimensions and peak VO2 (r = 0.45 / p = 0.004 and r = -0.49 / p = 0.014, respectively). After dobutamine echo, there was a correlation between some echo parameters and peak VO2: EF - r = 0.59 / p = 0.001, LA dimensions - r = 0.56 / p = 0.007, and ESV - r = -0.45 / p = 0.026. Percentage maximal peak VO2 attained correlated with LA dimensions measured in 2D echo and after dobutamine (r = -0.398 / p = 0.036 and r = -0.674 / p = 0.02 respectively) and EF after dobutamine (r = -0.389 / p = 0.04). The value of pro-BNP correlated with LA dimensions and baseline EF (r = 0.44 / p = 0.02 and r = -0.57 / p = 0.002, respectively), and the correlation was maintained after inotropic stimulation with dobutamine (r = 0.57 / p = 0.001 and r = -0.55 / p = 0.0039). CONCLUSION: Low-dose dobutamine stress echocardiography showed stronger correlations with cardiopulmonary exercise testing than the parameters evaluated by conventional echocardiography and could be used to determine the functional status of patients with dilated cardiomyopathy; patients with greater ejection fraction after inotropic stimulation had better cardiopulmonary tests.

INTRODUCTION: Obesity is an important risk factor for the development of diabetes, hypertension, coronary disease, left ventricular dysfunction, stroke and cardiac arrhythmias. Paradoxically, previous studies in patients undergoing elective coronary angioplasty showed a reduction in hospital and long-term mortality in obese patients. The relation with body mass index (BMI) has been less studied in the context of primary angioplasty. OBJECTIVES: To evaluate the impact of obesity on the results of ST-segment elevation acute myocardial infarction treated by primary angioplasty. METHODS: This was a study of 464 consecutive patients with ST-segment elevation acute myocardial infarction undergoing primary angioplasty, 78% male, mean age 61 +/- 13 years. We assessed in-hospital, 30-day and one-year mortality according to BMI. Patients were divided into three groups according to BMI: normal--18-24.9 kg/m2 (n = 171); overweight--25-29.9 kg/m2 (n = 204); and obese-- > 30 kg/m2 (n = 89). RESULTS: Obese patients were younger (ANOVA, p < 0.001) and more frequently male (p = 0.014), with more hypertension (p = 0.001) and dyslipidemia (p = 0.006). There were no differences in the prevalence of diabetes, previous cardiac history, heart failure on admission, anterior location, multivessel disease, peak total CK or medication prescribed, except that obese patients received more beta-blockers (p = 0.049). In-hospital mortality was 9.9% for patients with normal BMI, 3.4% for overweight patients and 6.7% for obese patients (p = 0.038). Mortality at 30 days was 11 4.4% and 7.8% (p = 0.032) and at one year 12.9%, 4.9% and 9% (p = 0.023), respectively. On univariate analysis, overweight was the only BMI category with a protective effect; however, after multivariate logistic regression analysis, adjusted for confounding variables, none of the BMI categories could independently predict outcome. CONCLUSIONS: Overweight patients had a better prognosis after primary angioplasty for ST-segment elevation acute myocardial infarction compared with other BMI categories, but this was dependent on other potentially confounding variables.
