OBJECTIVE: To estimate the prevalence of alcohol abuse/dependence and identify associated factors among demographic, family, socioeconomic and mental health variables. METHODS: A household survey was carried out in the urban area of Campinas, southeastern Brazil, in 2003. A total of 515 subjects, aged 14 years or more were randomly selected using a stratified cluster sample. The Self-Report Questionnaire and the Alcohol Use Disorder Identification Test were used in the interview. Prevalences were calculated, and univariate and multivariate logistic analyses performed by estimating odds ratios and 95% confidence intervals. RESULTS: The estimated prevalence of alcohol abuse/dependence was 13.1% (95% CI: 8.4;19.9) in men and 4.1% (95% CI: 1.9;8.6) in women. In the final multiple logistic regression model, alcohol abuse/dependence was significantly associated with age, income, schooling, religion and illicit drug use. The adjusted odds ratios were significantly higher in following variables: income between 2,501 and 10,000 dollars (OR=10.29); income above 10,000 dollars (OR=10.20); less than 12 years of schooling (OR=13.42); no religion (OR=9.16) or religion other than Evangelical (OR=4.77); and illicit drug use during lifetime (OR=4.47). Alcohol abuse and dependence patterns were different according to age group. CONCLUSIONS: There is a significantly high prevalence of alcohol abuse/dependence in this population. The knowledge of factors associated with alcohol abuse, and differences in consumption patterns should be taken into account in the development of harm reduction strategies.

OBJECTIVE: To assess risk factors for retreatment of leprosy patients. METHODS: A case-control study with patients from two reference care units in Recife, northeastern Brazil, in 2003. The case group included retreated patients (N=155) and the control group comprised those patients who were not retreated (N=155) matched by year of diagnosis and health care unit. Univariate and multivariate analyses were conducted to test the associations and odds ratios and related 95% confidence intervals were estimated. RESULTS: The following factors were found to be significantly associated (p<0.05) with retreatment: occurrence of adverse immunological reactions after treatment completion (OR=2.3; 95% CI=1.18;4.83), final bacterial index > 1 (OR=6.43; 95% CI=1.67;24.74), therapeutic regimen consisting of sulfone monotherapy (OR=10; 95% CI=0.01;0.78) and reports of household contacts (OR=2.2; 95% CI=0.24;0.85). CONCLUSIONS: The study findings reinforce that the use of dapsone monotherapy should be discontinued, and highlight the need for epidemiological monitoring of specific groups of leprosy patients after treatment completion through periodical clinical and laboratory evaluation. Further studies to explore the association between final bacterial index and retreatment are strongly recommended.

OBJECTIVE: To analyze the association between dietary patterns and oral cancer. METHODS: The study, part of a Latin American multicenter hospital-based case-control study, was conducted in São Paulo, Southeastern Brazil, between November 1998 and March 2002 and included 366 incident cases of oral cancer and 469 controls, frequency-matched with cases by sex and age. Dietary data were collected using a food frequency questionnaire. The risk associated with the intake of food groups defined a posteriori, through factor analysis (called factors), was assessed. The first factor, labeled "prudent," was characterized by the intake of vegetables, fruit, cheese, and poultry. The second factor, "traditional," consisted of the intake of rice, pasta, pulses, and meat. The third factor, "snacks," was characterized as the intake of bread, butter, salami, cheese, cakes, and desserts. The fourth, "monotonous," was inversely associated with the intake of fruit, vegetables and most other food items. Factor scores for each component retained were calculated for cases and controls. After categorization of factor scores into tertiles according to the distribution of controls, odds ratios and 95% confidence intervals were calculated using unconditional multiple logistic regression. RESULTS: "Traditional" factor showed an inverse association with cancer (OR=0.51; 95% CI: 0.32; 0.81, p-value for trend 0.14), whereas "monotonous" was positively associated with the outcome (OR=1.78; 95% CI: 1.78; 2.85, p-value for trend <0.001). CONCLUSIONS: The study data suggest that the traditional Brazilian diet, consisting of rice and beans plus moderate amounts of meat, may confer protection against oral cancer, independently of any other risk factors such as alcohol intake and smoking.

OBJETIVO: Descrever o perfil dos casos notificados de tuberculose e analisar os fatores associados ao abandono do tratamento. MÉTODOS: Foram descritos 178 casos de tuberculose notificados na Regional Oeste de Belo Horizonte, em 2001-2002, e registrados no Sistema Nacional de Agravos de Notificação. Realizou-se estudo caso-controle não pareado, com dados coletados por entrevistas, comparando pacientes que abandonaram o tratamento com aqueles que evoluíram para cura. Foram analisadas as variáveis: características sociodemográficas, comportamentais, associação com Aids, efeitos colaterais, informação sobre a doença e interesse no tratamento. Utilizou-se análise univariada e regressão logística não condicional na análise multivariada, com odds ratio ajustado como medida de associação e intervalo de confiança de 95%. RESULTADOS: O coeficiente de incidência foi de 56,6/100.000 habitantes. Houve predomínio de homens, de 30 a 49 anos, da forma pulmonar (76,4%), e 72,5% de bacilíferos. Entre as notificações verificou-se 65,2% de curas, 12,4% abandonos e 9,6% óbitos, sem influência do local do tratamento. No estudo caso-controle não houve diferença quanto ao gênero, cor, escolaridade, renda, ocupação, apoio familiar, associação com Aids e etilismo. O uso de drogas, interesse em se tratar e informação sobre a doença mostraram-se independentemente associados ao abandono. CONCLUSÕES: A adesão ao tratamento representa um desafio no controle da tuberculose. Os fatores de proteção - interesse em se tratar e nível de informação sobre a doença - e o reconhecimento do uso de droga como fator de risco devem integrar estratégias de cuidado ao doente, buscando reduzir os índices de abandono para recuperação da saúde.

The aim of this work is to study the risk of obesity posed by two genetic factors: haptoglobin phenotype and acid phosphatase phenotype, one enzymatic activity: acid phosphatase activity (ACP1), age and gender. Haptoglobin (Hp) is a protein of the immune system, and three phenotypes of Hp are found in humans: Hp1-1, Hp2-1, and Hp2-2. This protein is associated with a susceptibility to common pathological conditions, such as obesity. ACP1 is an intracellular enzyme The phenotypes of ACP1 (AA, AB, AC, BB, BC, CC) are also considered. We took a sample of 127 subjects with complete data from 714 registers. Since we intend to identify risk factors for obesity, an ordinal regression model is adjusted, using the Body Mass Index, BMI, to define weight categories. Haptoglobin phenotype, enzymatic activity of ACP1, acid phosphatase phenotype, age and gender are considered as regressor variables. We found three factors associated with an increased risk of obesity: phenotype Hp2-1 of haptoglobin (estimated odds ratio OR 11.54), phenotype AA of acid phosphatase (OR 33.788) and age (OR 1.39). The interaction between phenotype Hp2-1 and phenotype AC is associated with a decreased risk of obesity (OR 0.032); The interaction between phenotype AA and ACP1 activity is associated with a decreased risk of obesity (OR 0.954).

OBJETIVO: Avaliar os fatores de risco para a interrupção de aleitamento materno e desmame em crianças menores de um ano. MÉTODOS: Estudo transversal analítico com crianças menores de um ano de idade que compareceram aos postos de vacinação do município de Cuiabá, Mato Grosso, em 2004, acompanhadas de seus responsáveis. Para definição da amostra, foram sorteadas as unidades de vacinação, seguindo-se o sorteio das crianças em cada unidade, de forma sistemática. Para coleta de dados, aplicou-se um questionário semi-estruturado aos acompanhantes das crianças, investigando variáveis características sociodemográficas, referentes ao nascimento da criança e maternas, uso de chupeta e alimentação no primeiro dia em casa. Foram realizadas análise descritiva e regressão logística dos fatores de risco para cada faixa etária, apresentadas em odds ratio e intervalos de confiança. RESULTADOS: No total, 920 crianças menores de um ano foram avaliadas, das quais 205 menores de 120 dias e 275 menores de 180 dias. Verificou-se que usar chupeta, tomar chá no primeiro dia em casa, ter mãe com escolaridade até o primeiro ou segundo graus ou primípara, representam maior risco de não estar em amamentação exclusiva aos 120 dias de vida. Tais fatores se mostraram significativos também para menores de 180 dias, com exceção do consumo de chá, que não foi indagado para essa faixa etária. Nos menores de um ano, o uso de chupeta foi a única variável que manteve significância estatística. CONCLUSÕES: Fatores socioculturais mostraram-se determinantes da situação de aleitamento materno. Ressalta-se a importância da instrução e conscientização maternas, que refletem sobre as práticas que podem prejudicar a amamentação.

Objectives - The aim of this work was to study the interaction between genetic polymorphisms (single-nucleotide polymorphisms, SNPs) of pro- and anti-inflammatory cytokines and fat intake on the risk of developing Crohn's disease (CD) or modifying disease activity. Methods - Seven SNPs in interleukin 1 (IL1), tumor necrosis factor alpha (TNFalpha), lymphotoxin alpha (LTalpha), and IL6 genes were analyzed in 116 controls and 99 patients with CD. The type of fat intake was evaluated, and the interaction between SNPs and dietary fat in modulating disease activity was analyzed. Results - Individuals who were homozygous for the IL6-174G/C polymorphism had a six-fold higher risk for CD (odds ratio (OR)=6.1; 95% confidence interval (95% CI)=1.9-19.4), whereas the TT genotype on the TNFalpha-857C/T polymorphism was associated with more active disease (OR=10.4; 95% CI=1.1-94.1). A high intake of total, saturated, and monounsaturated fats, as well as a higher ratio of n-6/n-3 polyunsaturated fatty acid (PUFA), was associated with a more active phenotype (P<0.05). Furthermore, there was an interaction between dietary fat intake and SNPs, with a high intake of saturated and monounsaturated fats being associated with active disease, mainly in patients carrying the variant alleles of the 857 TNFalpha polymorphism (OR=6.0, 95% CI=1.4-26.2; OR=5.17; 95% CI=1.4-19.2, respectively) and the 174 IL6 polymorphism (OR=2.95; 95% CI=1.0-9.1; OR=3.21; 95% CI=1.0-10.4, respectively). Finally, low intake of n-3 PUFA and high n-6/n-3 PUFA ratio in patients with the TNFalpha 857 polymorphism were associated with higher disease activity (OR=3.6; 95% CI=1.0-13.0; OR=5.92; 95% CI=1.3-26.5, respectively). Conclusions - These results show that different types of fat may interact with cytokine genotype, modulating disease activity.

Background: Polymorphisms located in genes involved in the metabolism of folate and some methyl-related nutrients are implicated in colorectal cancer (CRC). Objective: We evaluated the association of 3 genetic polymorphisms [C677T MTHFR (methylene tetrahydrofolate reductase), A2756G MTR (methionine synthase), and C1420T SHMT (serine hydroxymethyltransferase)] with the intake of methyl-donor nutrients in CRC risk. Design: Patients withCRC(n 196) and healthy controls (n 200) matched for age and sex were evaluated for intake of methyl-donor nutrients and the 3 polymorphisms. Results: Except for folate intake, which was significantly lower in patients (P 0.02), no differences were observed in the dietary intake of other methyl-donor nutrients between groups. High intake of folate ( 406.7 g/d) was associated with a significantly lower risk of CRC (odds ratio: 0.67; 95% CI: 0.45, 0.99). The A2756G MTR polymorphism was not associated with the risk of developing CRC. In contrast, homozygosity for the C677TMTHFRvariant (TT) presented a 3.0-fold increased risk of CRC (95% CI: 1.3, 6.7). Similarly, homozygosity for the C1420T SHMT polymorphism also had a 2.6-fold increased risk (95% CI: 1.1, 5.9) of developing CRC. When interactions between variables were studied, low intake of all methyl-donor nutrients was associated with an increased risk ofCRC in homozygous participants for the C677T MTHFR polymorphism, but a statistically significant interaction was only observed for folate (odds ratio: 14.0; 95% CI: 1.8, 108.5). No significant associations were seen for MTR or SHMT polymorphisms. Conclusion: These results show an association between the C677T MTHFR variant and different folate intakes on risk of CRC.

Introduction - Obesity became a major public health problem as a result of its increasing prevalence worldwide. Paraoxonase-1 (PON1) is an esterase able to protect membranes and lipoproteins from oxidative modifications. At the PON1 gene, several polymorphisms in the promoter and coding regions have been identified. The aims of this study were i) to assess PON1 L55M and Q192R polymorphisms as a risk factor for obesity in women; ii) to compare PON1 activity according to the expression of each allele in L55M and Q192R polymorphisms; iii) to compare PON1 activity between obese and normal-weight women. Materials and methods - We studied 75 healthy (35.9±8.2 years) and 81 obese women (34.3±8.2 years). Inclusion criteria for obese subjects were body mass index ≥30 kg/m2 and absence of inflammatory/neoplasic conditions or kidney/hepatic dysfunction. The two PON1 polymorphisms were assessed by real-time PCR with TaqMan probes. PON1 enzymatic activity was assessed by spectrophotometric methods, using paraoxon as a substrate. Results - No significant differences were found for PON1 activity between normal and obese women. Nevertheless, PON1 activity was greater (P<0.01) for the RR genotype (in Q192R polymorphism) and for the LL genotype (in L55M polymorphism). The frequency of allele R of Q192R polymorphism was significantly higher in obese women (P<0.05) and was associated with an increased risk of obesity (odds ratio=2.0 – 95% confidence interval (1.04; 3.87)). Conclusion - 55M and Q192R polymorphisms influence PON1 activity. The allele R of the Q192R polymorphism is associated with an increased risk for development of obesity among Portuguese Caucasian premenopausal women.

It is now widely recognized that translation factors are involved in cancer development and that components of the translation machinery that are deregulated in cancer cells may become targets for cancer therapy. The eukaryotic Release Factor 3 (eRF3) is a GTPase that associates with eRF1 in a complex that mediates translation termination. eRF3a/GSPT1 first exon contains a (GGC)n expansion coding for proteins with different N-terminal extremities. Herein we show that the longer allele (12-GGC) is present in 5.1% (7/137) of the breast cancer patients analysed and is absent in the control population (0/135), corresponding to an increased risk for cancer development, as revealed by Odds Ratio analysis. mRNA quantification suggests that patients with the 12-GGC allele overexpress eRF3a/GSPT1 in tumor tissues relative to the normal adjacent tissues. However, using an in vivo assay for translation termination in HEK293 cells, we do not detect any difference in the activity of the eRF3a proteins encoded by the various eRF3a/GSPT1 alleles. Although the connection between the presence of eRF3a/GSPT1 12-GGC allele and tumorigenesis is still unknown, our data suggest that the presence of the 12-GGC allele provides a potential novel risk marker for various types of cancer.

Background - Both genetic and environmental factors affect the risk of colorectal cancer (CRC). Objective - We aimed to examine the interaction between the D1822V polymorphism of the APC gene and dietary intake in persons with CRC. Design - Persons with CRC (n = 196) and 200 healthy volunteers, matched for age and sex in a case-control study, were evaluated with respect to nutritional status and lifestyle factors and for the D1822V polymorphism. Results - No significant differences were observed in energy and macronutrient intakes. Cases had significantly (P < 0.05) lower intakes of carotenes, vitamins C and E, folate, and calcium than did controls. Fiber intake was significantly (P = 0.004) lower in cases than in controls, whereas alcohol consumption was associated with a 2-fold risk of CRC. In addition, cases were significantly (P = 0.001) more likely than were controls to be sedentary. The homozygous variant for the APC gene (VV) was found in 4.6% of cases and in 3.5% of controls. Examination of the potential interactions between diet and genotype found that a high cholesterol intake was associated with a greater risk of colorectal cancer only in noncarriers (DD) of the D1822V APC allele (odds ratio: 1.66; 95% CI: 1.00, 2.76). In contrast, high fiber and calcium intakes were more markedly associated with a lower risk of CRC in patients carrying the polymorphic allele (DV/VV) (odds ratio: 0.50; 95% CI: 0.27, 0.94 for fiber; odds ratio: 0.51; 95% CI: 0.28, 0.93 for calcium) than in those without that allele. Conclusion - These results suggest a significant interaction between the D1822V polymorphism and the dietary intakes of cholesterol, calcium, and fiber for CRC risk.

OBJETIVO: Avaliar os eventos adversos pós-vacina contra a difteria, coqueluche e tétano (EAPV-DPT) e os fatores associados à sua gravidade. MÉTODOS: Estudo transversal com componente descritivo e analítico, abrangendo os eventos adversos pós-vacina DPT notificados no Estado de São Paulo, de 1984 a 2001, entre crianças menores de sete anos de idade. A definição de caso foi a adotada pela vigilância de EAPV-DPT; os dados obtidos foram originados da vigilância passiva desses eventos. No cálculo das taxas, o numerador foram os EAPV e o denominador o número de doses aplicadas. A associação entre a gravidade dos EAPV-DPT e as exposições de interesse foi investigada pelas estimativas não ajustadas e ajustadas da odds ratio, com os respectivos intervalos de 95% de confiança, usando regressão logística não condicional. RESULTADOS: Identificaram-se 10.059 EAPV-DPT relativos a 6.266 crianças, apresentando um ou mais EAPV, 29,5% foram internadas e em 68,2% houve contra-indicação das doses subseqüentes de DPT. Cerca de 75% dos eventos ocorreram nas primeiras seis horas após a aplicação da vacina. Os eventos mais freqüentes foram: febre<39,5°C, reação local, evento hipotônico hiporresponsivo e convulsão. Mostraram-se independentemente associadas à gravidade: intervalo inferior a uma hora entre a aplicação da vacina e o evento (OR=2,1), primeira dose aplicada (OR=5,8), antecedentes neurológicos pessoais (OR=2,2) e familiares (OR=5,3). CONCLUSÕES: A vigilância passiva de EAPV mostrou-se útil no monitoramento da segurança da vacina DPT, descrevendo as características e a magnitude desses eventos, assim como permitiu identificar possíveis fatores associados às formas graves.

Este artigo apresenta e discute alguns aspetos sobre a aprendizagem da divisão com números naturais, focando-se nos procedimentos usados por alunos de uma turma do 3.º ano na resolução de tarefas de divisão. Os resultados apresentados fazem parte de uma investigação mais abrangente que teve como finalidade a compreensão do modo como os alunos aprofundam a aprendizagem da multiplicação numa perspetiva de desenvolvimento do sentido do número. A investigação realizada seguiu uma metodologia de design research, na modalidade de experiência de ensino. A análise das produções escritas dos alunos e de episódios de sala de aula relativos às discussões coletivas sobre as resoluções das tarefas propostas mostra que os alunos usam uma diversidade de procedimentos e que estes evoluem significativamente ao longo da experiência de ensino. Esta evolução parece ser suportada pelas características das tarefas, os seus contextos e números, assim como pela articulação, desde logo estabelecida, entre a divisão e a multiplicação. Além disso, o recurso ao modelo retangular parece, também, ter contribuído para a progressão para procedimentos multiplicativos, baseados na decomposição de um dos fatores. Os resultados do estudo permitem ainda perceber que a evolução dos procedimentos usados pelos alunos e a sua diversidade não são alheias ao ambiente de sala de aula construído.

OBJETIVO: Analisar as evidências na literatura do efeito do peso ao nascer sobre a ocorrência de síndrome metabólica em adultos. MÉTODOS: Foram pesquisados nas bases PubMed and LILACS, no período de 1966 a maio de 2006, artigos publicados usando os seguintes descritores: "birth weight" , "birthweight" , "intra-uterine growth restriction (IUGR)", "fetal growth retardation", "metabolic syndrome", "syndrome X", "Reaven's X syndrome". Foram selecionados 224 estudos considerados elegíveis que relatavam estimativas de associação entre peso ao nascer e síndrome metabólica ou seus componentes. Desses, 11 apresentavam razões de odds e foram usados na meta-análise. RESULTADOS: Com exceção de dois estudos, os demais relataram associação inversa entre peso ao nascer e síndrome metabólica. Comparadas com pessoas de peso normal, a razão de odds do efeito combinado naquelas que nasceram com baixo peso foi de 2,53 (IC 95%: 1,57;4,08). O gráfico de funil sugere viés de publicação e o resultado permanece estatisticamente significativo mesmo em estudos com mais de 400 pessoas (efeito combinado 2,37; IC 95%: 1,15;4,90). CONCLUSÕES: Baixo peso ao nascer aumenta o risco de síndrome metabólica na idade adulta.

OBJETIVO: Avaliar como as características demográficas, sociais e comportamentais se associam a diferentes tipos de atividade física. MÉTODOS: Avaliaram-se 37.692 indivíduos de amostra representativa da população portuguesa, no âmbito do Inquérito Nacional de Saúde, 1998-99. A maioria era constituída por mulheres (53,1%) e idade 20 anos. A avaliação da atividade física diária foi baseada em questionário e classificada como: total, de lazer e exercício. Cada tipo foi dicotomizado em baixa intensidade (atividades leves/moderadas) e alta intensidade (atividades pesadas/muito pesadas). Calcularam-se odds ratios (OR) e respectivos intervalos de confiança de 95% por regressão logística não condicional. RESULTADOS: Em ambos os sexos, verificou-se associação inversa significativa entre idade e diferentes tipos de atividade física, e entre a obesidade e a atividade de lazer e exercício. A escolaridade (<4; 5-11; 12 anos) associou-se positivamente com a atividade física de lazer (OR 1; 1,58; 2,39 nas mulheres e OR 1; 1,44; 2,08 nos homens) e com o exercício (OR 1; 3,50; 9,77 nas mulheres e OR 1; 3,42; 7,61 nos homens) e de forma inversa com a AF total (OR 1; 0,65; 0,20 nas mulheres e 1; 0,47; 0,09 nos homens). Independentemente da idade, os solteiros eram frequentemente mais ativos. A atividade de lazer associou-se negativamente ao consumo de bebidas alcoólicas para ambos os sexos e nos homens, ao consumo de tabaco. CONCLUSÕES: Os jovens, normoponderais, solteiros, não-bebedores e os homens não-fumadores apresentaram maior probabilidade de serem fisicamente ativos. Em ambos os sexos, observou-se um efeito diferencial da escolaridade segundo os tipos de atividade física.
