The rhesus-human reassortant, tetravalent rotavirus vaccine (RRV-TV) was licensed for routine use in the United States of America but it was recently withdrawn from the market because of its possible association with intussusception as an adverse event. The protective efficacy of 3 doses of RRV-TV, in its lower-titer (4 x 10(4) pfu/dose) formulation, was evaluated according to the nutritional status of infants who participated in a phase III trial in Belém, Northern Brazil. A moderate protection conferred by RRV-TV was related to weight-for-age Z-scores (WAZ) greater than -1 only, with rates of 38% (p = 0.04) and 40% (p = 0.04) for all- and- pure rotavirus diarrhoeal cases, respectively. In addition, there was a trend for greater efficacy (43%, p = 0.05) among infants reaching an height-for-age Z-score (HAZ) of > -1. Taking WAZ, HAZ and weight-for-height Z-score (WHZ) indices <= -1 together, there was no significant protection (p > 0.05) if both placebo and vaccine groups are compared. There was no significant difference if rates of mixed and pure rotavirus diarrhoeal cases are compared in relation to HAZ, WAZ and weight-for-height Z-score (WHZ) indices. Although a low number of malnourished infants could be identified in the present study, our data show some evidence that malnutrition may interfere with the efficacy of rotavirus vaccines in developing countries.

All over the world, the liberalization of electricity markets, which follows different paradigms, has created new challenges for those involved in this sector. In order to respond to these challenges, electric power systems suffered a significant restructuring in its mode of operation and planning. This restructuring resulted in a considerable increase of the electric sector competitiveness. Particularly, the Ancillary Services (AS) market has been target of constant renovations in its operation mode as it is a targeted market for the trading of services, which have as main objective to ensure the operation of electric power systems with appropriate levels of stability, safety, quality, equity and competitiveness. In this way, with the increasing penetration of distributed energy resources including distributed generation, demand response, storage units and electric vehicles, it is essential to develop new smarter and hierarchical methods of operation of electric power systems. As these resources are mostly connected to the distribution network, it is important to consider the introduction of this kind of resources in AS delivery in order to achieve greater reliability and cost efficiency of electrical power systems operation. The main contribution of this work is the design and development of mechanisms and methodologies of AS market and for energy and AS joint market, considering different management entities of transmission and distribution networks. Several models developed in this work consider the most common AS in the liberalized market environment: Regulation Down; Regulation Up; Spinning Reserve and Non-Spinning Reserve. The presented models consider different rules and ways of operation, such as the division of market by network areas, which allows the congestion management of interconnections between areas; or the ancillary service cascading process, which allows the replacement of AS of superior quality by lower quality of AS, ensuring a better economic performance of the market. A major contribution of this work is the development an innovative methodology of market clearing process to be used in the energy and AS joint market, able to ensure viable and feasible solutions in markets, where there are technical constraints in the transmission network involving its division into areas or regions. The proposed method is based on the determination of Bialek topological factors and considers the contribution of the dispatch for all services of increase of generation (energy, Regulation Up, Spinning and Non-Spinning reserves) in network congestion. The use of Bialek factors in each iteration of the proposed methodology allows limiting the bids in the market while ensuring that the solution is feasible in any context of system operation. Another important contribution of this work is the model of the contribution of distributed energy resources in the ancillary services. In this way, a Virtual Power Player (VPP) is considered in order to aggregate, manage and interact with distributed energy resources. The VPP manages all the agents aggregated, being able to supply AS to the system operator, with the main purpose of participation in electricity market. In order to ensure their participation in the AS, the VPP should have a set of contracts with the agents that include a set of diversified and adapted rules to each kind of distributed resource. All methodologies developed and implemented in this work have been integrated into the MASCEM simulator, which is a simulator based on a multi-agent system that allows to study complex operation of electricity markets. In this way, the developed methodologies allow the simulator to cover more operation contexts of the present and future of the electricity market. In this way, this dissertation offers a huge contribution to the AS market simulation, based on models and mechanisms currently used in several real markets, as well as the introduction of innovative methodologies of market clearing process on the energy and AS joint market. This dissertation presents five case studies; each one consists of multiple scenarios. The first case study illustrates the application of AS market simulation considering several bids of market players. The energy and ancillary services joint market simulation is exposed in the second case study. In the third case study it is developed a comparison between the simulation of the joint market methodology, in which the player bids to the ancillary services is considered by network areas and a reference methodology. The fourth case study presents the simulation of joint market methodology based on Bialek topological distribution factors applied to transmission network with 7 buses managed by a TSO. The last case study presents a joint market model simulation which considers the aggregation of small players to a VPP, as well as complex contracts related to these entities. The case study comprises a distribution network with 33 buses managed by VPP, which comprises several kinds of distributed resources, such as photovoltaic, CHP, fuel cells, wind turbines, biomass, small hydro, municipal solid waste, demand response, and storage units.

DEET (N,N-diethyl-3-methylbenzamide) is nowadays the most effective mosquito repellent available, however, its use can present some topical and systemic side effects. Some botanical compositions, as Andiroba (Carapa guianensis), have been proved repellent properties at low cost and toxicity. An experimental study was driven involving four volunteers submitting their forearms covered with Andiroba oil at 100%, DEET 50%, refined soy oil, Andiroba oil 15% and in the absence of products, directly to healthy females of Aedes sp. The times of first and third bites were checked. The results showed that the median of the first bite without any product was 17.5s and the third bite, 40.0s. In the soy oil, the bites happened in 60.0s and 101.5s, in the presence of Andiroba oil 100%, in 56.0s and 142.5s and in Andiroba oil 15%, in 63.0s and 97.5s. The volunteers using DEET 50% had not received bites after 3600s in most of the experiments (p < 0.001 Wilcoxon). Pure Andiroba oil compared to the soy oil, forearm without product and Andiroba oil 15%, showed discreet superiority (p < 0.001 Wilcoxon). Our conclusion is that this study demonstrated that the pure Andiroba oil presents discreet repellent effect against bite of Aedes sp., being significantly inferior to DEET 50%.

Em Galileu (1564-1642) a noção de tempo revela-se associada a movimento no sistema solar do Universo. Pouco tempo depois, Isaac Newton (1642-1727) considera o Espaço e o Tempo em termos absolutos na Lei da gravitação e Leibnitz (1646- -1716), contestando o caracter absoluto do tempo, afirmou o caracter relacionai do Espaço e do Tempo, considerando-o importante como contexto em que os objectos têm uma posição e relação lógica no Espaço e no Tempo. Por influência dos Naturalistas no século XVIII, a noção de sistema e de organismo integra os fenômenos da Natureza e da Sociedade em que o Tempo é relevante com o significado imprescindível de Evolução e Evolucionismo. À superfície da Terra, a evolução no Tempo era cíclica e estacionai, a variação no Espaço era zonal de acordo com a proximidade do Sol em relação ao Equador.

A questão do tempo é tão antiga como a Filosofia. Ela surge já nos pre-socráticos intimamente associada à relação ontológica fundamental Ser/Devir, é retomada dentro da filosofia platônica principalmente no "Timeu" e posteriormente na "Física" de Aristóteles. Agostinho de Hipona e Tomás de Aquino não deixam também de reflectir sobre o tempo, sendo a contribuição do primeiro tão notável que ainda continua a influenciar o pensamento contemporâneo. O espírito da modernidade, sob a influência de Galileu, Descartes e Newton retoma a questão propondo um tempo abstracto, matemático e mecanicista que passou a designar-se por Tempo Absoluto e dominou o pensamento ocidental por cerca de duzentos anos. Porém, este tempo é um tempo reversível totalmente inadequado às ciências da vida que foram surgindo durante o Sec.XIX. A Teoria da Relatividade recusa o Tempo Absoluto e propõe um Tempo Relativo, mas este ainda é reversível. A Biologia e a Sistêmica contemporâneas, além da Mecânica Estatística, insistem numa Flecha do Tempo sem a qual os fenômenos essenciais da ciência de hoje seriam inexplicáveis. É neste quadro de reflexão que a proposta do super-tempo eónico feita pela ontóloga e fenomenôloga Hedwig Conrad-Martius, de Munique, tem o maior interesse.

The purpose of this work is to present an algorithm to solve nonlinear constrained optimization problems, using the filter method with the inexact restoration (IR) approach. In the IR approach two independent phases are performed in each iteration—the feasibility and the optimality phases. The first one directs the iterative process into the feasible region, i.e. finds one point with less constraints violation. The optimality phase starts from this point and its goal is to optimize the objective function into the satisfied constraints space. To evaluate the solution approximations in each iteration a scheme based on the filter method is used in both phases of the algorithm. This method replaces the merit functions that are based on penalty schemes, avoiding the related difficulties such as the penalty parameter estimation and the non-differentiability of some of them. The filter method is implemented in the context of the line search globalization technique. A set of more than two hundred AMPL test problems is solved. The algorithm developed is compared with LOQO and NPSOL software packages.

A new iterative algorithm based on the inexact-restoration (IR) approach combined with the filter strategy to solve nonlinear constrained optimization problems is presented. The high level algorithm is suggested by Gonzaga et al. (SIAM J. Optim. 14:646–669, 2003) but not yet implement—the internal algorithms are not proposed. The filter, a new concept introduced by Fletcher and Leyffer (Math. Program. Ser. A 91:239–269, 2002), replaces the merit function avoiding the penalty parameter estimation and the difficulties related to the nondifferentiability. In the IR approach two independent phases are performed in each iteration, the feasibility and the optimality phases. The line search filter is combined with the first one phase to generate a “more feasible” point, and then it is used in the optimality phase to reach an “optimal” point. Numerical experiences with a collection of AMPL problems and a performance comparison with IPOPT are provided.

Exames sorológicos de Imunofluorescència, Fixação do Complemento e Hemoaglutinação para a Doença de Chagas foram rezalidos em 1609 pacientes. Em 154 casos, as provas foram positivas, com um índice percentual de 9,57%. Posteriormente, 50 casos sorològicamente positivos foram submetidos ao xenodiagnústico. Os xeno-positivos revelaram um índice de 5,35% sobre o total de casos examinados e de 56% sôbre os sorològicamente positivos. Chama-se a atenção para a alta incidência de chagásicos no Hospital São Pedro, ao lado de outras considerações.

Foram selecionados 25 pacientes chagásicos crônicos que apresentavam sorologia positiva e elevada parasitemia. A 15 deles, durante 120 dias, foi ministrado o Nitrofurfurilidene na dose diária de 8 a 10 mg por quilo de peso. Aos 10 restantes deu-se placebo. No trans e pós-tratamento não se verificaram alterações nas funções hepática, hematopoiética ou renal de caráter iatrogênico. Xenodiagnóstico e provas sorológicas foram realizadas mensalmente. O xenodiagnóstico, nos 15 doentes tratados, aos 60º dias da fase trans-terapêutica, mostrou-se negativo e assim se manteve durante 4 anos, período de tempo que durou a nossa observação. Quanto às reações sorodiagnósticas o mesmo não ocorreu, embora a partir dos 12 meses do término do tratamento o grupo que recebeu a droga tenha passado a apresentar níveis mais baixos de anticorpos. Os A.A. comentam o desacordo entre os achados do xenodiagnóstico e os das provas sorológicas e sugerem uma hipótese para justificá-lo. Concluem que, embora com menos evidência do que nas formas agudas, o Nitrofurfurilidene revela certa eficácia na fase crônica da infecção chagásica.

Os autores relatam um caso de entomoftoromicose intestinal causada por Entomophthorales, em indivíduo de 19 anos, agricultor e sem doença associada. O paciente foi submetido a ressecção intestinal e o diagnóstico foi feito após análise da peça cirúrgica. Após revisão da literatura, são discutidos a evolução clínica, as características clinicopatológicas, as dificuldades no diagnóstico e o tratamento dessa entidade rara.

The proliferation of Aedes aegypti, a species of mosquito that is the vector of the dengue pathogen, is being augmented by the population's lack of care in allowing the formation of larval habitats. One form of controlling dengue is the distribution of information on the mosquito to improve awareness and to provide the means necessary for the elimination of its reproductive habitats. To evaluate a teaching method concerning the vector and dengue, students from the 5th and 6th years of primary education were compared before and after didactic intervention with a group of control students. The students who received intervention were more successful in identifying the stages of the cycle, biological and morphological characteristics of the adult insect and the importance of the mosquito in health issues. The didactic intervention was successful in developing knowledge leading to increased awareness of the importance of preventative measures that should be taken against the vector and the disease.

A presença de formigas (Hymenoptera: Formicidae) em ambientes hospitalares pode constituir um problema de saúde pública, especialmente por serem vetores mecânicos de organismos patogênicos. O trabalho teve como objetivo realizar o levantamento de formigas e analisar a presença de bactérias a elas associadas em dois hospitais regionais de médio porte da cidade de Divinópolis, MG. As coletas foram realizadas mensalmente, durante um período de seis meses. Foram coletadas formigas Pheidole sp1 e sp2, Linepithema humile, Wasmannia auropunctata, Camponotus sp1 e sp2, Odontomachus sp, Solenopsis sp, Acromyrmex sp e Tapinoma melenocephalum. Observou-se que estas transportavam mecanicamente Pseudomonas aeruginosa, Enterococcus, Streptococcus, Staphylococcus patogênico e não patogênico e Escherichia coli. Tais resultados evidenciam a propensão à ocorrência de infecções hospitalares nesses locais pela transmissão mecânica de agentes patogênicos por formigas.

Henrique, ou Jacob, de Castro Sarmento (1691-1762), médico judeu, nascido em Bragança e exilado em Inglaterra a partir de 1721, devido às perseguições da Inquisição aos cristãos-novos, é considerado como o introdutor das ideias de Isaac Newton (1662-1727) em Portugal e um dos principais responsáveis pela disseminação da iatroquímica entre os médicos portugueses, contribuindo para o desenvolvimento da medicina e ciência portuguesas do século XVIII. Eleito para sócio da Royal Society em 1730, o médico português tornou-se o principal interlocutor científico entre Portugal e Inglaterra, sendo a sua actividade exemplar do modo como ocorreram as trocas de conhecimento entre o centro da Europa e a sua periferia. Autor de diversas obras médicas, Sarmento abordou algumas das temáticas médicas mais relevantes do século das luzes, como a inoculação das bexigas ou a utilização terapêutica das águas minerais, veiculando uma nova forma de encarar a prática médica. A sua obra Theorica Verdadeira das Mares, editada em 1737, foi a primeira publicada em Portugal onde as ideias de Newton são consideradas com o devido detalhe. Paradoxalmente, Sarmento foi também produtor de medicamentos de segredo, tendo a sua Água de Inglaterra alcançado um sucesso significativo em Portugal. Figura polémica e multifacetada, Sarmento dedicou uma parte considerável da sua vida à divulgação e disseminação das ideias dos autores modernos, dos quais se destacam Boyle, Newton e Boerhaave. O presente trabalho tem como principal objectivo elucidar os principais aspectos da sua vida e obra, procurando enquadrar as suas actividades no contexto da medicina e ciência europeias de setecentos.
