Chemical therapy for the treatment of leishmaniasis is still inadequate, and a number of drugs and therapeutic programs are being tested. Besides treatment, the ultimate goal is an effective cure, and histopathological analyses of the lesion cicatrices constitute an important measure of treatment success, or otherwise, in this respect. In this paper, we describe histopathological patterns in cases of American cutaneous leishmaniasis in 32 patients from the municipality of Caratinga, Minas Gerais, Brazil, before and after treatment with the following therapeutic methodos: l) leishvacin + glucantime; 2) leishvacin + BCG associated with glucantime; 3) glucantime; 4) leishvacin + BCG. Lesion fragments were collected from all patients by biopsy prior to, and approximately 30 days after, each treatment which resulted in a clinical diagnosis of cure. Following the analysis of slides, the preparations were described from a histopathological point of view and grouped taking into account the prevalence or significance of the characteristic elements. This process resulted in the following classification: 1. exsudative reaction (ER); 2. exsudative giant cell reaction (EGCR); 3. exsudative productive reaction (EPR); 4. exsudative productive giant cell reaction (EPGCR); 5. exsudative productive necrotic reaction (EPNR); 6. necrotic exsudative reaction (NER); 7. productive exsudative reaction (PER), 8. productive giant cell reaction (PGCR); 9. productive exsudative giant cell reaction (PEGCR); 10. productive exsudative giant cell granulomatous reaction (PEGCGR); 11. productive reaction (PR) and 12. productive cicatricial (cure) reaction (PCR). After this analysis, it was noted that clinical cure did not always coincide with histopathological cure.

A escolha da terapia mais apropriada para o paciente com bancroftose requer um conhecimento das diversas características clínicas da doença filarial e de sua patogênese. Como resultado de novos testes diagnósticos e avanços clínicos, não apenas nosso entendimento sobre a filariose bancroftiana mudou rapidamente de forma, como as nossas idéias sobre o tratamento. No passado, acreditava-se que a elefantíase era causada pela reação imunológica do hospedeiro ao parasita filarial. Posta dessa maneira, essa forma da doença seria o ponto final de uma inter-relação hóspede-hospedeiro imutável, dada a inexistência de medicamentos ou de condutas que possibilitassem a sua involução nos denominados "indivíduos imunologicamente predispostos". Entretanto, nos últimos anos, surgiram evidências de que o linfedema e a elefantíase tinham outro agente etiológico. O principal fator de evolução para os quadros de linfedema e elefantiásicos seria o desenvolvimento de infecções bacterianas secundárias de repetição. Hoje, é perfeitamente claro que outras formas de terapia de suporte (incluindo a educação e o aconselhamento psicológico) são necessárias e são, muitas vezes, mais importantes que a terapia antiparasitária.

The case of a man bitten by a South American rattlesnake (Crotalus durissus) and who developed an abscess at the site of the bite is reported. Abcesses are a rare complication of this type of envenoming, possibly due to the lack of a strong cytotoxic action of Crotalus durissus venom.

"Quantitative Buffy Coat" (QBC®) is a direct and fast fluorescent method used for the identification of blood parasites. Since Leishmania chagasi circulates in blood, we decided to test it in American visceral leishmaniasis (AVL). Bone marrow (BM) and peripheral blood (PB) of 49 persons and PB of 31 dogs were analyzed. QBC® was positive in BM of 11/11 patients with AVL and in 1/6 patients with other diseases. Amastigotes were identified in PB of 18/22 patients with AVL and in none without AVL. The test was positive in 30 out of the 31 seropositive dogs and in 28/28 dogs with Leishmania identified in other tissues. QBC® is a promising method for diagnosis of human AVL, and possibly for the exam of PB of patients with AVL/AIDS, for the control of the cure and for the identification of asymptomatic carriers. Because it is fast and easy to collect and execute, QBC® should be evaluated for programs of reservoir control.

Although treatment of visceral leishmaniasis with pentavalent antimony is usually successful, some patients require second-line drug therapy, most commonly with amphotericin B. To identify the clinical characteristics that predict an inadequate response to pentavalent antimony, a case-control study was undertaken in Teresina, Piaui, Brazil. Over a two-year period, there were 19 cases of VL in which the staff physicians of a hospital prescribed second-line therapy with amphotericin B after determining that treatment with pentavalent antimony had failed. The control group consisted of 97 patients that were successfully treated with pentavalent antimony. A chart review using univariate and multivariate analysis was performed. The cure rate was 90% with amphotericin B. The odds ratio for the prescription of amphotericin B was 10.2 for children less than one year old, compared with individuals aged over 10 years. Patients who presented coinfection had an OR of 7.1 while those on antibiotics had an OR of 2.8. These data support either undertaking a longer course of therapy with pentavalent antimony for children or using amphotericin B as a first-line agent for children and individuals with coinfections. It also suggests that chemoprophylaxis directed toward bacterial coinfection in small children with VL may be indicated.

The first choice of treatment for American cutaneous leishmaniasis is the pentavalent antimonial drug. Although it has been shown that this treatment is mostly effective and indicated, some disadvantages should be taken into account such as side effects, long term treatment inconveniences and counter-indication for patients suffering from cardiopathy, nephropathy; yet, aging, pregnancy and other conditions. With the advent of the vaccine anti-American cutaneous leishmaniasis as a prophylactic measure, studies on therapy using the vaccine associated or not with other drugs have been performed by many investigators and it is currently among the alternative treatments and prevention measures for American cutaneous leishmaniasis. In conclusion, the association between antimony and vaccine (immunochemotherapy) showed the same cure rate when compared with the standard treatment (100%) and it was also able to reduce the salt volume in 17.9% and treatment length from 87 to 62 days, decreasing side effects.

The diagnosis for American cutaneous leishmaniasis is based on an association of clinical, epidemiological and laboratory characteristics. The present study identified the circulating species of Leishmania in the State of Pernambuco, described its clinical-epidemiological characteristics and diagnosed the disease. Nineteen patients presenting active lesions who had been diagnosed through clinical evaluation and laboratory tests were selected. The tests included direct investigation, in vitro culturing, Montenegro skin test, indirect immunofluorescence and polymerase chain reaction. The Montenegro Skin Test showed positive results in 89% of the patients; indirect immunofluorescence, in 79%; direct investigation, in 58%; and polymerase chain reaction in 75%. Seven Leishmania (Viannia) braziliensis samples were isolated from these patients and were characterized by means of specific monoclonal antibodies. These data confirm that a combination of different diagnosis techniques is needed in order to obtain efficient results and that, so far, Leishmania (Viannia) braziliensis is the only species responsible for American cutaneous leishmaniasis infection in Pernambuco. Thus, it is essential to identify the parasite species involved in cases of human disease in an endemic area in order to determine the clinical and epidemiological characteristics, especially with regard to diagnosis, therapy development and disease prognosis.

Hybrid knowledge bases are knowledge bases that combine ontologies with non-monotonic rules, allowing to join the best of both open world ontologies and close world rules. Ontologies shape a good mechanism to share knowledge on theWeb that can be understood by both humans and machines, on the other hand rules can be used, e.g., to encode legal laws or to do a mapping between sources of information. Taking into account the dynamics present today on the Web, it is important for these hybrid knowledge bases to capture all these dynamics and thus adapt themselves. To achieve that, it is necessary to create mechanisms capable of monitoring the information flow present on theWeb. Up to today, there are no such mechanisms that allow for monitoring events and performing modifications of hybrid knowledge bases autonomously. The goal of this thesis is then to create a system that combine these hybrid knowledge bases with reactive rules, aiming to monitor events and perform actions over a knowledge base. To achieve this goal, a reactive system for the SemanticWeb is be developed in a logic-programming based approach accompanied with a language for heterogeneous rule base evolution having as its basis RIF Production Rule Dialect, which is a standard for exchanging rules over theWeb.

O litoral, espaço geográfico de interface entre o meio marinho e terrestre, reúne múltiplos recursos que têm vindo a ser explorados ao longo dos tempos de uma forma cada vez mais intensa. As zonas costeiras são os locais mais acessíveis do ambiente marinho, oferecendo inúmeras oportunidades para a descoberta de uma grande variedade de espécies. A exploração descontrolada dos recursos naturais aliada a fatores de origem natural, aumentam a dificuldade na gestão, levando a uma degradação progressiva. O litoral analisado, do Município de Torres Vedras, tem aproximadamente 20 quilómetros de costa e carateriza-se como uma zona complexa, com elevados valores naturais. Dada a qualidade paisagística e diversidade ecológica é considerado como uma zona bastante atrativa, o que permite o desenvolvimento de atividades de cariz económico, social e ambiental. O procedimento adotado foi sobretudo as visitas ao terreno e a revisão bibliográfica, elementos fundamentais para uma avaliação do território eficaz e ajustada à realidade atual com base nas tendências evolutivas. A dissertação foi desenvolvida em estreita colaboração com o Município de Torres Vedras e pretende contribuir para a elaboração de uma estratégia de utilização sustentável dos recursos costeiros e marinhos. Pretende igualmente equacionar a possibilidade de criação de uma área marinha protegida de âmbito local. Foi assim que se definiu o objetivo, criar uma estratégia assente em dois pilares fundamentais, a conservação e valorização dos recursos naturais. Só desta forma foi possível conciliar as políticas que interferem na gestão da zona costeira e apresentar um projeto fundamental para a região de Torres Vedras. Este trabalho foi desenvolvido tendo em conta as principais necessidades, não só do território como também das populações, com a realização de uma sessão de participação pública e um inquérito. Os resultados obtidos foram bastante interessantes, pois existe uma preocupação efetiva na proteção dos recursos naturais por parte da população local (das “Ações para valorizar o litoral” perguntadas aos inquiridos, a maior percentagem obtida foi de 19% na opção “Conservação dos Recursos Naturais”), fator fundamental para a definição de uma estratégia de valorização do território. Mostrando desta forma a boa relação da comunidade local à conservação do património natural, e vontade de fortalecer a identidade local. A criação de uma Reserva Natural Marinha foi a opção encontrada tendo em conta a estratégia definida e o território analisado. A implementação desta proposta depende de um processo burocrático e de participação pública, que pode ser um pouco moroso até à sua concretização.

INTRODUCTION: Chagas disease is caused by Trypanosoma cruzi. Wild and perianthropic mammals maintain the infection/transmission cycle, both in their natural habitat and in the peridomestic area. The aim of this paper was to present the results from a study on wild rodents in the central and northern regions of San Luis province, Argentina, in order to evaluate the prevalence of this infection. METHODS: Sherman traps were set up in capture areas located between latitudes 32º and 33º S, and longitudes 65º and 66º W. The captured rodents were taxonomically identified and hemoflagellates were isolated. Morphological, biometric and molecular studies and in vitro cultures were performed. Infection of laboratory animals and histological examination of the cardiac muscle and inoculation area were also carried out. Parasites were detected in circulating blood in Calomys musculinus, Graomys griseoflavus, Phyllotis darwini and Akodon molinae. The parasites were identified using biological criteria. Molecular PCR studies were performed on some isolates, which confirmed the characterization of these hemoflagellates as Trypanosoma cruzi. RESULTS AND CONCLUSIONS: Forty-four percent of the 25 isolates were identified as Trypanosoma cruzi, and the remaining 56% as Trypanosoma cruzi-like. These findings provide evidence that wild rats infected with Trypanosoma cruzi and Trypanosoma cruzi-like organisms are important in areas of low endemicity.

Nowadays there is a big percentage of the population, specially young users, which are smartphone users and there is a lot of information to be provided within the applications, information provision should be done carefully and should be accurate, otherwise an overload of information will be produced, and the user will discard the app which is providing the information. Mobile devices are becoming smarter and provide many ways to filter information. However, there are alternatives to improve information provision from the side of the application. Some examples are, taking into account the local time, considering the battery level before doing an action and checking the user location to send personalized information attached to that location. SmartCampus and SmartCities are becoming a reality and they have more and more data integrated every day. With all this amount of data it is crucial to decide when and where is the user going to receive a notification with new information. Geofencing is a technique which allows applications to deliver information in a more useful way, in the right time and in the right place. It consists of geofences, physical regions delimited by boundaries, and devices that are eligible to receive the information assigned to the geofence. When devices cross one of these geofences an alert is pushed to the mobile device with the information.
