The failing heart is characterized by complex tissue remodelling involving increased cardiomyocyte death, and impairment of sarcomere function, metabolic activity, endothelial and vascular function, together with increased inflammation and interstitial fibrosis. For years, therapeutic approaches for heart failure (HF) relied on vasodilators and diuretics which relieve cardiac workload and HF symptoms. The introduction in the clinic of drugs interfering with beta-adrenergic and angiotensin signalling have ameliorated survival by interfering with the intimate mechanism of cardiac compensation. Current therapy, though, still has a limited capacity to restore muscle function fully, and the development of novel therapeutic targets is still an important medical need. Recent progress in understanding the molecular basis of myocardial dysfunction in HF is paving the way for development of new treatments capable of restoring muscle function and targeting specific pathological subsets of LV dysfunction. These include potentiating cardiomyocyte contractility, increasing cardiomyocyte survival and adaptive hypertrophy, increasing oxygen and nutrition supply by sustaining vessel formation, and reducing ventricular stiffness by favourable extracellular matrix remodelling. Here, we consider drugs such as omecamtiv mecarbil, nitroxyl donors, cyclosporin A, SERCA2a (sarcoplasmic/endoplasmic Ca(2 +) ATPase 2a), neuregulin, and bromocriptine, all of which are currently in clinical trials as potential HF therapies, and discuss novel molecular targets with potential therapeutic impact that are in the pre-clinical phases of investigation. Finally, we consider conceptual changes in basic science approaches to improve their translation into successful clinical applications.

This paper assesses the impact of the location and configuration of Battery Energy Storage Systems (BESS) on Low-Voltage (LV) feeders. BESS are now being deployed on LV networks by Distribution Network Operators (DNOs) as an alternative to conventional reinforcement (e.g. upgrading cables and transformers) in response to increased electricity demand from new technologies such as electric vehicles. By storing energy during periods of low demand and then releasing that energy at times of high demand, the peak demand of a given LV substation on the grid can be reduced therefore mitigating or at least delaying the need for replacement and upgrade. However, existing research into this application of BESS tends to evaluate the aggregated impact of such systems at the substation level and does not systematically consider the impact of the location and configuration of BESS on the voltage profiles, losses and utilisation within a given feeder. In this paper, four configurations of BESS are considered: single-phase, unlinked three-phase, linked three-phase without storage for phase-balancing only, and linked three-phase with storage. These four configurations are then assessed based on models of two real LV networks. In each case, the impact of the BESS is systematically evaluated at every node in the LV network using Matlab linked with OpenDSS. The location and configuration of a BESS is shown to be critical when seeking the best overall network impact or when considering specific impacts on voltage, losses, or utilisation separately. Furthermore, the paper also demonstrates that phase-balancing without energy storage can provide much of the gains on unbalanced networks compared to systems with energy storage.

We evaluated the development of arterial hypertension, cardiac function, and collagen deposition, as well as the level of components of the renin-angiotensin system in the heart of transgenic rats that overexpress an angiotensin (Ang)-(1-7)-producing fusion protein, TGR(A1-7)3292 (TG), which induces a lifetime increase in circulating levels of this peptide. After 30 days of the induction of the deoxycorticosterone acetate (DOCA)-salt hypertension model, DOCA-TG rats were hypertensive but presented a lower systolic arterial pressure in comparison with DOCA-Sprague-Dawley (SD) rats. In contrast to DOCA-SD rats that presented left ventricle (LV) hypertrophy and diastolic dysfunction, DOCA-TG rats did not develop cardiac hypertrophy or changes in ventricular function. In addition, DOCA-TG rats showed attenuation in mRNA expression for collagen type I and III compared with the increased levels of DOCA-SD rats. Ang II plasma and LV levels were reduced in SD and TG hypertensive rats in comparison with normotensive animals. DOCA-TG rats presented a reduction in plasma Ang-(1-7) levels; however, there was a great increase in Ang-(1-7) (approximate to 3-fold) accompanied by a decrease in mRNA expression of both angiotensin-converting enzyme and angiotensin-converting enzyme 2 in the LV. The mRNA expression of Mas and Ang II type 1 receptors in the LV was not significantly changed in DOCA-SD or DOCA-TG rats. This study showed that TG rats with increased circulating levels of Ang-(1-7) are protected against cardiac dysfunction and fibrosis and also present an attenuated increase in blood pressure after DOCA-salt hypertension. In addition, DOCA-TG rats showed an important local increase in Ang-(1-7) levels in the LV, which might have contributed to the attenuation of cardiac dysfunction and prefibrotic lesions. P>1. Impairmant of baroreflex sensitivity (BRS) has been implicated in the reduction of heart rate variability (HRV) and in the increased risk of death after myocardial infarction (MI). In the present study, we investigated whether the additional impairment in BRS induced by sinoaortic baroreceptor denervation (SAD) in MI rats is associated with changes in the low-frequency (LF) component of HRV and increased mortality rate. 2. Rats were randomly divided into four groups: control, MI, denervated (SAD) and SAD + MI rats. Left ventricular (LV) function was evaluated by echocardiography. Autonomic components were assessed by power spectral analysis and BRS. 3. Myocardial infarction (90 days) reduced ejection fraction (by similar to 42%) in both the MI and SAD + MI groups; however, an increase in LV mass and diastolic dysfunction were observed only in the SAD + MI group. Furthermore, BRS, HRV and the LF power of HRV were reduced after MI, with an exacerbated reduction seen in SAD + MI rats. The LF component of blood pressure variability (BPV) was increased in the MI, SAD and SAD + MI groups compared with the control group. Mortality was higher in the MI groups compared with the non-infarcted groups, with an additional increase in mortality in the SAD + MI group compared with the MI group. Correlations were obtained between BRS and the LF component of HRV and between LV mass and the LF component of BPV. 4. Together, the results indicate that the abolishment of BRS induced by SAD in MI rats further reduces the LF band of HRV, resulting in a worse cardiac remodelling and increased mortality in these rats. These data highlight the importance of this mechanism in the prognosis of patients after an ischaemic event.

Background/Aim. Granulocyte colony-stimulating factor (G-CSF) reduces myocardial injury and improves cardiac function after myocardial infarction (MI). We investigated the early alterations provided by G-CSF and the chronic repercussions in infarcted rats. Methods. Male Wistar rats (200-250g) received vehicle (MI) or G-CSF (MI-GCSF) (50 mu g/kg, sc) at 7, 3 and 1 days before MI surgery. Afterwards MI was produced and infarct size was measured 1 and 15 days after surgery. Expression of anti-and proapoptotic proteins was evaluated immediately before surgery. 24 hours after surgery, apoptotic nuclei were evaluated. Two weeks after MI, left ventricular (LV) function was evaluated, followed by in situ LV diastolic pressure-volume evaluation. Results. Infarct size was decreased by 1 day pretreatment before occlusion (36 +/- 2.8 vs. 44 +/- 2.1% in MI; P<0.05) and remained reduced at 15 days after infarction (28 +/- 2.2 vs. 36 +/- 1.4% in MI; P<0.05). G-CSF pretreatment increased Bcl-2 and Bcl-xL protein expression, but did not alter Bax in LV. Apoptotic nuclei were reduced by treatment (Sham: 0.46 +/- 0.42, MI: 15.5 +/- 2.43, MI-GCSF: 5.34 +/- 3.34%; P<0.05). Fifteen days after MI, cardiac function remained preserved in G-CSF pretreated rats. The LV dilation was reduced in MI-G-CSF group as compared to MI rats, being closely associated with infarct size. Conclusion. The early beneficial effects of G-CSF were essentials to preserve cardiac function at a chronic stage of myocardial infarction. Copyright (C) 2011 S. Karger AG, Basel

Background/Aim: Nitric oxide (NO) modulates the expression of the chaperone Hsp72 in the heart, and exercise stimulates both NO production and myocardial Hsp72 expression. The main purpose of the study was to investigate whether NO interferes with an exercise-induced myocardial Hsp72 expression. Methods: Male Wistar rats (70-100 days) were divided into control (C, n= 12), L-NAME-treated (L, n= 12), exercise (E, n= 13) and exercise plus L-NAME-treated (EL, n= 20) groups. L-NAME was given in drinking water (700 mg. L(-1)) and the exercise was performed on a treadmill (15-25 m.min(-1), 40-60 min. day(-1)) for seven days. Left ventricle (LV) protein Hsp content, NOS and phosphorylated-NOS (p-NOS) isoforms were measured using Western blotting. The activity of NOS was assayed in LV homogenates by the conversion of [(3)H] L-arginine to [(3)H] L-citrulline. Results: Hsp72 content was increased significantly (223%; p < 0.05) in the E group compared to the C group, but exercise alone did not alter the NOS content, p-NOS isoforms or NOS activity. Contrary to our expectation, L-NAME enhanced (p < 0.05) the exercise-induced Hsp72 content (EL vs. C, L and E groups = 1019%, 548% and 457%, respectively). Although the EL group had increased stimulatory p-eNOS(Ser1177) (over 200%) and decreased inhibitory p-nNOS(Ser852) (similar to 50%) compared to both the E and L groups (p < 0.05), NOS activity was similar in all groups. Conclusions: Our results suggest that exercise-induced cardiac Hsp72 expression does not depend on NO. Conversely, the in vivo L-NAME treatment enhances exercise-induced Hsp72 production. The aim of the present study was to evaluate the effect of overstimulation of beta-adrenoceptors on vascular inflammatory mediators. Wistar rats were treated with the beta-adrenoceptor agonist isoproterenol (0.3 mg(.)kg(-1.)day(-1) sc) or vehicle (control) for 7 days. At the end of treatment, the right carotid artery was catheterized for arterial and left ventricular (LV) hemodynamic evaluation. Isoproterenol treatment increased LV weight but did not change hemodynamic parameters. Aortic mRNA and protein expression were quantified by real-time RT-PCR and Western blot analysis, respectively. Isoproterenol enhanced aortic mRNA and protein expression of IL-1 beta (124% and 125%) and IL-6 (231% and 40%) compared with controls but did not change TNF-alpha expression. The nuclear-to-cytoplasmatic protein expression ration of the NF-beta B p65 subunit was increased by isoproterenol treatment (51%); in addition, it reduced the cytoplasmatic expression of I kappa B-alpha (52%) in aortas. An electrophoretic mobility shift assay was performed using the aorta, and increased NF-kappa B DNA binding (31%) was observed in isoproterenol-treated rats compared with controls (P < 0.05). Isoproterenol treatment increased phenylephrine-induced contraction in aortic rigs (P < 0.05), which was significantly reduced by superoxide dismutase (150 U/ml) and sodium salicylate (5 mM). Cotreatment with thalidomide (150 mg(.)kg(-1.)day(-1) for 7 days) also reduced hyperreactivity to phenylephrine induced by isoproterenol. In conclusion, overstimulation of beta-adrenoceptors increased proinflammatory cytokines and upregulated NF-kappa B in the rat aorta. Moreover, local oxidative stress and the proinflammatory state seem to play key roles in the altered vascular reactivity of the rat aorta induced by chronic beta-adrenergic stimulation.

The Global Initiative Against Asthma (GINA) was developed to meet the global challenge of asthma. GINA has been adopted in most countries and comparison of asthma management in different parts of the world may be of help when assessing the global dissemination of the guideline. The overall goals in GINA include that asthma patients should be free of symptoms, acute asthma attacks and activity limitations. The aim of the present study was to compare asthma management and asthma control in Sao Paulo, Brazil and Uppsala, Sweden. Information was collected from asthmatics in Sao Paulo and Uppsala with a questionnaire. The questionnaire dealt with the following issues: symptoms, smoking, self-management, hospital visits, effect on school/work and medication. The Sao Paulo patients were more likely to have uncontrolled asthma (36% vs 13%, P < 0.001), having made emergency room visits (57% vs 29%, P < 0.001) and having lost days at school or work because of their asthma (46% vs 28%, P = 0.03) than the asthmatics from Uppsala. There were no difference in the use of inhaled corticosteroids, but the Brazilian patients were more likely to be using theophylline (18% vs 1%, P = 0.001) and less likely to be using long-acting beta-2 agonists (18% vs 37%, P < 0.001). We conclude that the level of asthma control was lower among the patients from Sao Paulo than Uppsala. Few of the patients in either city reached the goals set up by GINA. In hypertension, left ventricular (LV) hypertrophy develops as an adaptive mechanism to compensate for increased afterload and thus preserve systolic function. Associated structural changes such as microvascular disease might potentially interfere with this mechanism, producing pathological hypertrophy. A poorer outcome is expected to occur when LV function is put in jeopardy by impaired coronary reserve. The aim of this study was to evaluate the role of coronary reserve in the long-term outcome of patients with hypertensive dilated cardiomyopathy. Between 1996 and 2000, 45 patients, 30 of them male, with 52 +/- 11 years and LV fractional shortening <30% were enrolled and followed until 2006. Coronary flow velocity reserve was assessed by transesophageal Doppler of the left anterior descending coronary artery. Sixteen patients showed >= 10% improvement in LV fractional shortening after 17 +/- 6 months. Coronary reserve was the only variable independently related to this improvement. Total mortality was 38% in 10 years. The Cox model identified coronary reserve (hazard ratio = 0.814; 95% CI = 0.72-0.92), LV mass, low diastolic blood pressure, and male gender as independent predictors of mortality. In hypertensive dilated cardiomyopathy, coronary reserve impairment adversely affects survival, possibly by interfering with the improvement of LV dysfunction. J Am Soc Hypertens 2010;4(1):14-21. A 72 cm long core was collected from Lagoa da Viracao (LV), a small poind in the Fernando de Noronha island, northern Brazil. Sediments from the lower section of the core (20-72 cm depth) contain essentially mineral matter, while in the upper section (0-20 cm depth) mineral matter is mixed with organic matter. Lithogenic conservative elements - Si, Al, Fe, Ti, Co, Cr, Cu, Ba, Ga, Hf, Nb, Ni, Y, V, Zn, Zr and REE - exhibit remarkably constant values throughout the core, with concentrations similar to those of the bedrock. The vertical distribution of soluble elements - Ca, Mg, Na, K, P, Mn and Sr - is also homogeneous, but these elements are systematically depleted in relation to the bedrock. LOI, TOC, Br, Se, Hg and Pb, although showing nearly constant values in the lower section of the core, are significantly enriched in the upper section. The concentration profiles of Br and Se suggest that they may be accounted for by natural processes, related to the slight affinity of these elements for organic matter. On the other hand, the elevated levels of Hg and Pb in recent sediments may be explained by their long-range atmospheric transport and deposition. Furthermore, the isotopic composition of Pb clearly indicates that anthropogenic sources contributed to the Pb burden in the uppermost pond sediments.

I kölvattnet av Nollvisionen uppstår problem för oskyddade trafikanter. Vid mittseparering av 13-metersvägar blir utrymmet för dessa trafikantgrupper otillräckligt och nya lösningar måste till. Detta examensarbete syftar till att förbättra framkomligheten och trafiksäkerheten för oskyddade trafikanter på s k MLV (Mötesseparerad LandsVäg). Målet med examensarbetet har varit att inventera problem, identifiera förbättringsområden, finna bra lösningar samt att beräkna vilka kostnader som dessa lösningar orsakar.Som ett resultat av arbetet har framkommit att det saknas en nationell samstämmighet i synen på hur oskyddade trafikanter på MLV, skall behandlas. Vidare existerar det en stor fokusering i centrala direktiv, kanske mest inofficiella, på bilisters och transportörers behov vid utformningen av MLV. Som en konsekvens av detta har de oskyddade trafikanterna blivit undanträngda, både bildligt och fysiskt talat, från dessa vägtyper och därmed begränsas deras förflyttnings-möjligheter. Ytterligare resultat är att det behövs mer utredning kring hänvisning för de oskyddade trafikanterna på MLV, detta för att i möjligaste mån separera trafikantgrupperna.I arbetet har också utförts en kostnadsberäkning av en GCM-lösning (GCM står för Gång, Cykel och Moped) på en vägsträcka, riksväg 80 mellan Falun och Hofors. Kostnaden blev 1,1 - 2,2 mn kr, vilket motsvarar en projektfördyring med 4,4 - 11 %, beroende på alternativ. De viktigaste slutsatserna i arbetet är att man bör anstränga sig till det yttersta för att erbjuda ett alternativ till en MLV, och att man då också måste förbättra skyltningen, för att undvika att oskyddade trafikanter irrar sig in på MLV:n, trots acceptabla alternativvägar. För detta har ett förslag till ett nytt vägmärke framtagits, Alternativ GCM-väg längs MLV, se Bilaga 5.

Syftet med denna skrift är att sammanställa den kunskap som finns beträffande kombinerade pellet- och solvärmesystem för att på så sätt stödja företagen i deras systemutveckling. Denna skrift behandlar erfarenheter som gjorts inom forskning på sol och pellet och omsätter dessa i praktiska råd för systemutformning. Förslag ges på systemutformning, olika tekniska lösningar samt hur systemen bör styras.När solvärme och pellet skall kombineras finns det många möjligheter att koppla ihop systemen. Det finns olika traditioner i olika länder, vilket gör att systemlösningarna varierar från land till land. En generell slutsats är dock att konventionella svenska pannor med inbyggd varmvattenberedning inte är lämpliga i konventionella solvärmesystem. Det ger komplicerade systemlösningar och det är svårt att åstadkomma bra skiktning i tanken.I ett solvärmesystem är det viktigt att tanken kan laddas ur på ett sådant sätt att kraftig skiktning erhålls. Det betyder att tankens botten skall kylas ner till temperaturen på ingående kallvatten och att tankens mellersta del skall kylas till samma temperatur som radiatorreturen. Om solfångaren även vintertid kan arbeta med att förvärma kallvatten av 10 till 20ºC fås en betydligt bättre verkningsgrad på solfångaren än om radiator returen skall förvärmas, som i bästa fall ligger på en temperaturnivå på mellan 30 och 40ºC. Av denna anledning skall radiator returen placeras en bra bit upp från botten i ackumulatortanken och tappvattnet skall förvärmas i en slinga som börjar i tankens botten. Om det finns ett VVC-system måste systemet anslutas på ett speciellt sätt så att inte tankens skiktning störs.En annan viktig parameter i tankens utformning är att värmeförlusterna hålls låga, detta är viktigt för att klara tappvattenlasten mulna perioder och för att hålla energianvändningen låg. I moderna hus där tanken placeras i boutrymmet blir det också en komfortfråga för att undvika över-temperaturer i rummet där tanken placeras. För att få en bra isolering måste man se till att det finns ett lufttätt skikt över hela isoleringen som dessutom sluter tätt mot röranslutningar. Ofrivillig själv-cirkulation i anslutande kretsar som kan kyla av och blanda om ackumulatortanken skall förhindras med backventiler.Vid design av solfångarkretsen måste överhettning och stagnation kunna klaras utan glykolnedbrytning eller andra skador. Partiell förångning innebär att man låter solfångaren koka på ett kontrollerat sätt så att endast ånga blir kvar i solfångaren. Vätska samlas i ett större expansionskärl och systemet återfylls när vätskan kondenserar. Dränerande system med enbart vatten är också en möjlighet, men kräver större noggrannhet vid installationen så att sönderfrysning undviks.Pelletkaminer (luftburna) ger god komfort och lågt elbehov i direktelvärmda hus med öppen planlösning, dvs. om värmen från kaminen kan spridas till alla rum utan att behöva passera genom mer än en dörröppning. Även i lågenergihus kan den luftburna kaminen vara lämplig. I hus med mer sluten planlösning krävs en vattenmantlad kamin med hög andel värme till vattenkretsen och ett vattenburet värmesystem. Det är viktigt att sådana system utformas korrekt för att komforten skall bli hög och elanvändningen låg. Brukarens aspekter och komfortkrav måste beaktas vid användning av kaminer, eftersom det krävs en temperaturskillnad mellan olika rum för att få värmespridning från det rum där kaminen är placerad.

Em uma amostra constituída de oito mandíbulas humanas secas, foram selecionados quatro sítios na região de segundo pré-molar a primeiro molar, dos de cada lado. Um come de gutta-percha foi fixado com adesivo instantâneo na face lingual da mandíbula, na região de cada sítio. Para simular a presença de tecidos moles, as mandíbulas foram incluídas em parafina. Para a correta seleção dos sítios de exame, a tomografia hipocicloidal, foi utilizazda uma incidência radiográfica oclusa total dee cada mandíbula, cuja imagem foi capturada par ao monitor. Sobre a mesma foram selecionados os sítos para a obtenção de cortes ortorradiais de tomografia hipocicloidal, com espessura de 2 mm, através do equipamento IS2000 COMM-CATR. A tomografia computadoriza helicoidal foi obtida através do equipamento Twin FlashR e as reconstruções ortorradiais, cada uma com 2 mm de espessura, foram realizadas com o softtware Denta CTr. A mandíbulas foram seccionadas nos sítios propostos e todas as imagens, digitalizadas através de scanner. As imagens de tomografia hipocicloidal foram redimensionadas de acordo com a magnificação de 26%. Dois examinadores previamente calibrados realizaram medidas de distâncias e horizontais das imagens digitais apresentadas no monitor, sob condições ideiais de visualizaação. As comparações intra e interexaminador não mostraram variabilidade significativas. As medidas obtidas pelos dois métodos foram comparadas ao padrão-ouro e as diferenças encontradas foram menores que 0,1 mm. Tais resultados permitem concluir que, na presente amostra, tanto a tomografia computadoriza helicoidal quando a tomografia hipocicloidal permite obtenção segura das medidas verticais e horizontais propostas na região posterior mandíbula.

Embora os benefícios fisiológicos da corrida sejam bastante documentados na literatura, a freqüência de lesões nos segmentos inferiores, devido ao uso excessivo, é bastante grande. A corrida em piscina funda é uma modalidade utilizada por corredores de elite para diminuir riscos de lesão por volume excessivo de treinamento. Não obstante, dados sobre as adaptações fisiológicas e biomecânicas, da corrida em piscina funda em corredores de rendimento são insuficientes. O objetivo geral deste estudo foi avaliar os efeitos da inclusão da corrida em piscina funda dentro do treinamento regular no desempenho de corredores de rendimento. Dezoito corredores foram divididos em grupo experimental 1 (GE1) e experimental 2 (GE2). O GE1 substituiu 30 por cento do volume de treinamento em terra pelo treinamento de corrida em piscina funda e o GE2 treinou apenas em terra. O período de treinamento foi de 8 semanas com 6 sessões semanais nas primeiras quatro semanas e 7 sessões semanais nas últimas quatro semanas No pré e pós-testes verificou-se o consumo máximo de oxigênio (VO2máx), limiar ventilatório (LV), volume expiratório máximo (VEmáx), economia de corrida (Eco), freqüência cardíaca máxima (FCmáx), freqüência de passada (FP), comprimento de passada (CP), comprimento de passada relativo (CPR), tempo de passada (TP), tempo de suporte (TS), tempo de vôo (TV), velocidade linear horizontal do quadril (VH). Foi usada a análise de covariância de dois caminhos (grupo x tempo) com medidas repetidas, e gênero sexual como covariante (p < 0,05). Após as 8 semanas de treinamento as determinantes fisiológicas do rendimento (VO2máx, Eco, LV’s) e a técnica de corrida durante os testes de 500m e Eco não foram diferentes tanto entre o pré-teste e pós-teste, quanto entre os tipos de treinamento. Além disso, não houve influência da variável independente tipo de treinamento sobre a variável independente tempo (pré/pós), ou seja, a modificação do treinamento (substituição de 30 por cento no volume de treinamento em terra) não foi suficiente para modificar o desempenho dos atletas. Deste modo, se conclui que a corrida em piscina funda pode servir como um efetivo complemento de treinamento em até 30 por cento no volume de treinamento em terra durante um período de até 8 semanas para atletas corredores de rendimento.
