The purpose of this study was to identify the boundary of submaximal speed zones (i.e., exercise intensity domains) between maximal aerobic speed (S-400) and lactate threshold (LT) in swimming. A 400-m all-out test, a 7 × 200 m incremental step test, and two to four 30-minute submaximal tests were performed by 12 male endurance swimmers (age = 24.5 ± 9.6 years; body mass = 71.3 ± 9.8 kg) to determine S-400, speed corresponding to LT, and maximal lactate steady state (MLSS). S-400 was 1.30 ± 0.09 m·s -1 (400 m-5:08 minutes:seconds). The speed at LT (1.08 ± 0.02 m·s-1; 83.1 ± 2.2 %S-400) was lower than the speed at MLSS (1.14 ± 0.02 m·s-1; 87.5 ± 1.9 %S-400). Maximal lactate steady state occurred at 26 ± 10% of the difference between the speed at LT and S-400. Mean blood lactate values at the speeds corresponding to LT and MLSS were 2.45 ± 1.13 mmol·L-1 and 4.30 ± 1.32 mmol·L-1, respectively. The present findings demonstrate that the range of intensity zones between LT and MLSS (i.e., heavy domain) and between MLSS and S-400 (i.e., severe domain) are very narrow in swimming with LT occurring at 83% S-400 in trained swimmers. Precision and sensitivity of the measurement of aerobic indexes (i.e., LT and MLSS) should be considered when conducting exercise training and testing in swimming. © 2013 National Strength and Conditioning Association.

Objectives: To investigate the test-retest reliability of mechanical parameters derived from a 3-min isokinetic all-out test, performed at 60 and 100 rpm. Reliability and validity of the peak oxygen uptake derived from 3-min isokinetic all-out test were also tested. Design: 14 healthy male subjects completed an incremental ramp testing and four randomized 3-min isokinetic all-out test (two at 60 rpm and two at 100 rpm). Methods: The absolute and relative reliability of the following parameters were analyzed: peak power, mean power, end power, fatigue index, work performed above end power and peak oxygen uptake. Results: No difference was found between each two sets of data, although there were between-cadence differences for peak power, mean power, end power, and fatigue index. Higher intra-class correlation (ICC) and lower coefficient of variation (CV) were found for end power (ICC = 0.91 and 0.95; CV = 5.6 and 5.7%) and mean power (ICC = 0.97 and 0.98; CV = 2.4 and 3.1%), than for peak power (ICC = 0.81 and 0.84; CV = 8.7 and 10%) and work performed above end power (ICC = 0.79 and 0.84; CV = 7.9 and 10.6%; values reported for 60 rpm and 100 rpm, respectively). High reliability scores were also observed for peak oxygen uptake at both cadences (60 rpm, CV = 3.2%; 100 rpm, CV = 2.3%,) with no difference with the incremental ramp testing peak oxygen uptake. Conclusions: The power profile and peak oxygen uptake of a 3-min isokinetic all-out test are both highly reliable, whether the test is performed at 60 or 100 rpm. Besides, peak oxygen uptake and work performed above end power were not affected by the change in cadence while peak power, mean power, end power, and fatigue index were. © 2013.

This study aims to evaluate and correlate the vascular, sensory and motor components related to the plantar surface in individuals with diabetic peripheral neuropathy. 68 patients were categorized into two groups: 28 in the neuropathic group and 40 in the control group. In each patient, we assessed: circulation and peripheral perfusion of the lower limbs; somatosensory sensitivity; ankle muscle strength; and pressure on the plantar surface in static, dynamic and gait states. We used the Mann-Whitney test and analysis of variance (ANOVA and MANOVA) for comparison between groups, and performed Pearson and Spearman linear correlations amongst the variables (P < 0.05). The somatosensory sensitivity, peripheral circulation and ankle muscle strength were reduced in the neuropathic group. In full peak plantar pressures, no differences were seen between groups, but differences did appear when the foot surface was divided into regions (forefoot, midfoot and hindfoot). In the static condition, the plantar surface area was greater in the neuropathic group. In the dynamic state, peak pressures in the neuropathic group, were higher in the forefoot and lower in the hindfoot, as well as lower in the hindfoot during gait. There were positive or negative correlations between the sensitivity deficit, dorsal ankle flexor strength, plantar surface area, and peak pressure by plantar region. The sensitivity deficit contributed to the increased plantar surface area.

The characterization of the hyperbolic power-time (P-tlim) relationship using a two-parameter model implies that exercise tolerance above the asymptote (Critical Power; CP), i.e. within the severe intensity domain, is determined by the curvature (W') of the relationship. The purposes of this study were (1) to test whether the amount of work above CP (W>CP) remains constant for varied work rate experiments of high volatility change and (2) to ascertain whether W' determines exercise tolerance within the severe intensity domain. Following estimation of CP (208 ± 19 W) and W' (21.4 ± 4.2 kJ), 14 male participants (age: 26 ± 3; peak [Formula: see text]: 3708 ± 389 ml.min-1) performed two experimental trials where the work rate was initially set to exhaust 70% of W' in 3 ('THREE') or 10 minutes ('TEN') before being subsequently dropped to CP plus 10 W. W>CP for TEN (104 ± 22% W') and W' were not significantly different (P>0.05) but lower than W>CP for THREE (119 ± 17% W', P<0.05). For both THREE (r = 0.71, P<0.01) and TEN (r = 0.64, P<0.01), a significant bivariate correlation was found between W' and tlim. W>CP and tlim can be greater than predicted by the P-tlim relationship when a decrement in the work rate of high-volatility is applied. Exercise tolerance can be enhanced through a change in work rate within the severe intensity domain. W>CP is not constant.

Herbivory assessments were conducted on seven reforested sites that were less than one year old in the following Mississippi counties: Bolivar, Leflore, and Attala. At each site, 100ft. x 100 ft. plots were established and randomly selected seedlings were marked and measured to determine seedling species, height, condition, survival, and type and extent of animal feeding sign. Surveys were conducted in March/April, May, and August 2004. Herbivory rates were highest during May with approximately 47% of seedlings showing signs of herbivory. In March/April and August, the percentage of seedlings exhibiting signs of herbivory was 37% and 30%, respectively. Foraging by white-tailed deer (Odocoileus virginianus) was recorded on > 90% of the damaged seedlings during each survey. Tree mortality for all study sites and tree species was negligible, with the highest amount (7%) recorded during August, despite the recorded rates of herbivory by white-tailed deer. Herbivory by rabbits (Sylvilagus spp.) and rodents occurred on approximately 6% of the seedlings throughout the 2004 growing season.

We ascertained a Brazilian family with nine individuals affected by autosomal dominant nonsyndromic sensorineural hearing loss. The bilateral hearing loss affected mainly mid-high frequencies, was apparently stable with an early onset. Microsatellites close to the DFNA8/DFNA12 locus, which harbors the TECTA gene, showed significant multipoint lod scores (32) close to marker D11S4107. Sequencing of the exons and exon-intron boundaries of the TECTA gene in one affected subject revealed the deletion c.5383 + 5delGTGA in the 5' end of intron 16, that includes the last two bases of the donor splice site consensus sequence. This mutation segregates with deafness within the family. To date, 33 different TECTA mutations associated with autossomal dominant hearing loss have been described. Among them is the mutation reported herein, first described by Hildebrand et al. (2011) in a UK family. The audioprofiles from the UK and Brazilian families were similar. In order to investigate the transcripts produced by the mutated allele, we performed cDNA analysis of a lymphoblastoid cell line from an affected heterozygote with the c.5383 + 5delGTGA and a noncarrier from the same family. The analysis allowed us to identify an aberrant transcript with skipping of exon 16, without affecting the reading frame. One of the dominant TECTA mutations already described, a synonymous substitution in exon 16 (c.5331 G<A), was also shown to affect splicing resulting in an aberrant transcript lacking exon 16. Despite the difference in the DNA level, both the synonymous substitution in exon 16 (c.5331 G<A) and the mutation described herein affect splicing of exon 16, leading to its skipping. At the protein level they would have the same effect, an in-frame deletion of 37 amino-acids (p.S1758Y/G1759_N1795del) probably leading to an impaired function of the ZP domain. Thus, like the TECTA missense mutations associated with dominant hearing loss, the c5383 + 5delGTGA mutation does not have an inactivating effect on the protein. (C) 2012 Elsevier B.V. Purpose: There are no current guidelines for diagnosing and managing mild prenatal hydronephrosis. Variations in physician approach make it difficult to analyze outcomes and establish optimal management. We determined the variability of diagnostic approach and management regarding prenatal hydronephrosis among maternal-fetal medicine obstetricians, pediatric urologists and pediatric radiologists. Materials and Methods: Online surveys were sent to mailing lists for national societies for each specialty. Participants were surveyed regarding criteria for diagnosing mild prenatal hydronephrosis and recommendations for postnatal management, including use of antibiotic prophylaxis, followup scheduling and type of followup imaging. Results: A total of 308 maternal-fetal medicine obstetricians, 126 pediatric urologists and 112 pediatric radiologists responded. Pediatric urologists and radiologists were divided between Society for Fetal Urology criteria and use of anteroposterior pelvic diameter for diagnosis, while maternal-fetal medicine obstetricians preferred using the latter. For postnatal evaluation radiologists preferred using personal criteria, while urologists preferred using anteroposterior pelvic diameter or Society for Fetal Urology grading system. There was wide variation in the use of antibiotic prophylaxis among pediatric urologists. Regarding the use of voiding cystourethrography/radionuclide cystography in patients with prenatal hydronephrosis, neither urologists nor radiologists were consistent in their recommendations. Finally, there was no agreement on length of followup for mild prenatal hydronephrosis. Conclusions: We observed a lack of uniformity regarding grading criteria in diagnosing hydronephrosis prenatally and postnatally among maternal-fetal medicine obstetricians, pediatric urologists and pediatric radiologists. There was also a lack of agreement on the management of mild intermittent prenatal hydronephrosis, resulting in these cases being managed inconsistently. A unified set of guidelines for diagnosis, evaluation and management of mild intermittent prenatal hydronephrosis would allow more effective evaluation of outcomes.

Objectives Clinical significance and management of prenatal hydronephrosis (PNH) are sources of debate. Existing studies are flawed with biased cohorts or inconsistent follow-up. We aimed to evaluate the incidence of pathology in a large cohort of PNH and assess the biases and outcomes of this population. Methods We reviewed 1034 charts of fetuses with PNH. Records of delivered offspring were reviewed at a pediatric center and analyzed with respect to prenatal and postnatal pathology and management. Results Prenatal resolution of hydronephrosis occurred in 24.7% of pregnancies. On first postnatal ultrasound, some degree of dilatation was present in 80%, 88% and 95% of mild, moderate and severe PNH cases, respectively. At the end of follow-up, hydronephrosis persisted in 10%, 25% and 72% of children, respectively. Incidence of vesicoureteral reflux did not correlate with severity of PNH. Children with postnatal workup had more severe PNH than those without. Conclusions Despite prenatal resolution totalizing 25%, pelvic dilatation persisted on first postnatal imaging in most cases, thus justifying postnatal ultrasound evaluation. Whereas most mild cases resolved spontaneously, a quarter of moderate and more than half of severe cases required surgery. Patients with postnatal imaging and referral had more severe PNH, which could result in overestimation of pathology. (c) 2012 John Wiley & Sons, Ltd.

Purpose: Dynamic near infrared fluorescence imaging of the urinary tract provides a promising way to diagnose ureteropelvic junction obstruction. Initial studies demonstrated the ability to visualize urine flow and peristalsis in great detail. We analyzed the efficacy of near infrared imaging in evaluating ureteropelvic junction obstruction, renal involvement and the anatomical detail provided compared to conventional imaging modalities. Materials and Methods: Ten swine underwent partial or complete unilateral ureteral obstruction. Groups were survived for the short or the long term. Imaging was performed with mercaptoacetyltriglycine diuretic renogram, magnetic resonance urogram, excretory urogram, ultrasound and near infrared imaging. Scoring systems for ureteropelvic junction obstruction were developed for magnetic resonance urogram and near infrared imaging. Physicians and medical students graded ureteropelvic junction obstruction based on magnetic resonance urogram and near infrared imaging results. Results: Markers of vascular and urinary dynamics were quantitatively consistent among control renal units. The same markers were abnormal in obstructed renal units with significantly different times of renal phase peak, start of pelvic phase and start of renal uptake. Such parameters were consistent with those obtained with mercaptoacetyltriglycine diuretic renography. Near infrared imaging provided live imaging of urinary flow, which was helpful in identifying the area of obstruction for surgical planning. Physicians and medical students categorized the degree of obstruction appropriately for fluorescence imaging and magnetic resonance urogram. Conclusions: Near infrared imaging offers a feasible way to obtain live, dynamic images of urine flow and ureteral peristalsis. Qualitative and quantitative parameters were comparable to those of conventional imaging. Findings support fluorescence imaging as an accurate, easy to use method of diagnosing ureteropelvic junction obstruction.
