BACKGROUND Bone-to-tendon healing after rotator cuff repairs is mainly impaired by poor tissue quality. The tenocytes of chronic rotator cuff tendon tears are not able to synthesize normal fibrocartilaginous extracellular matrix (ECM). We hypothesized that in the presence of platelet-released growth factors (PRGF), tenocytes from chronically retracted rotator cuff tendons proliferate and synthesize the appropriate ECM proteins. MATERIALS AND METHODS Tenocytes from 8 patients with chronic rotator cuff tears were cultured for 4 weeks in 2 different media: standard medium (Iscove's Modified Dulbecco's Media + 10% fetal calf serum + 1% nonessential amino acids + 0.5 μg/mL ascorbic acid) and media with an additional 10% PRGF. Cell proliferation was assessed at 7, 14, 21, and 28 days. Messenger (m)RNA levels of collagens I, II, and X, decorin, biglycan, and aggrecan were analyzed using real time reverse-transcription polymerase chain reaction. Immunocytochemistry was also performed. RESULTS The proliferation rate of tenocytes was significantly higher at all time points when cultured with PRGF. At 21 days, the mRNA levels for collagens I, II, and X, decorin, aggrecan, and biglycan were significantly higher in the PRGF group. The mRNA data were confirmed at protein level by immunocytochemistry. CONCLUSIONS PRGFs enhance tenocyte proliferation in vitro and promote synthesis of ECM to levels similar to those found with insertion of the normal human rotator cuffs. CLINICAL RELEVANCE Biologic augmentation of repaired rotator cuffs with PRGF may enhance the properties of the repair tissue. However, further studies are needed to determine if application of PRGF remains safe and effective in long-term clinical studies. LEVEL OF EVIDENCE Basic Science Study, Cell Biology.

Because neuronal nitric oxide synthase (nNOS) has a well-known impact on arteriolar blood flow in skeletal muscle, we compared the ultrastructure and the hemodynamics of/in the ensuing capillaries in the extensor digitorum longus (EDL) muscle of male nNOS-knockout (KO) mice and wild-type (WT) littermates. The capillary-to-fiber (C/F) ratio (-9.1%) was lower (P ≤ 0.05) in the nNOS-KO mice than in the WT mice, whereas the mean cross-sectional fiber area (-7.8%) and the capillary density (-3.1%) varied only nonsignificantly (P > 0.05). Morphometrical estimation of the area occupied by the capillaries as well as the volume and surface densities of the subcellular compartments differed nonsignificantly (P > 0.05) between the two strains. Intravital microscopy revealed neither the capillary diameter (+3% in nNOS-KO mice vs. WT mice) nor the mean velocity of red blood cells in EDL muscle (+25% in nNOS-KO mice vs. WT mice) to significantly vary (P > 0.05) between the two strains. The calculated shear stress in the capillaries was likewise nonsignificantly different (3.8 ± 2.2 dyn/cm² in nNOS-KO mice and 2.1 ± 2.2 dyn/cm² in WT mice; P > 0.05). The mRNA levels of vascular endothelial growth factor (VEGF)-A were lower in the EDL muscle of nNOS-KO mice than in the WT littermates (-37%; P ≤ 0.05), whereas mRNA levels of VEGF receptor-2 (VEGFR-2) (-11%), hypoxia inducible factor-1α (+9%), fibroblast growth factor-2 (-14%), and thrombospondin-1 (-10%) differed nonsignificantly (P > 0.05). Our findings support the contention that VEGF-A mRNA expression and C/F-ratio but not the ultrastructure or the hemodynamics of/in capillaries in skeletal muscle at basal conditions depend on the expression of nNOS.

BACKGROUND The pathogenesis of full-thickness tears of the rotator cuff remains unclear. Apart from age and trauma, distinct scapular morphologies have been found to be associated with rotator cuff disease. The purpose of the present study was to evaluate whether a score formed using these established risk factors was able to predict the presence of a rotator cuff tear reliably. METHODS We retrospectively assessed a consecutive series of patients with a minimal age of 40 years old, who had true antero-posterior (AP) radiographs of their shoulders, as well as a magnetic resonance (MR) gadolinium-arthrography, between January and December 2011. In all of these patients, the critical shoulder angle (CSA) was determined, and MR images were assessed for the presence of rotator cuff tears. Additionally, the patients' charts were reviewed to obtain details of symptom onset. Based on these factors, the so-called rotator cuff tear (RCT) score was calculated. RESULTS Patients with full-thickness RCTs were significantly older and had significantly larger CSAs than patients with intact rotator cuffs. Multiple logistic regression, using trauma, age and CSA as independent variables, revealed areas under the curve (AUCs) for trauma of 0.55, for age of 0.65 and for CSA of 0.86. The combination of all three factors was the most powerful predictor, with an AUC of 0.92. CONCLUSION Age, trauma and the CSA can accurately predict the presence of a posterosuperior RCT. An acute injury to the triangular fibrocartilage complex (TFCC) with avulsion of the foveal attachment can produce distal radioulnar joint (DRUJ) instability. The avulsed TFCC is translated distally so the footprint will be bathed in synovial fluid from the DRUJ and will become covered in synovitis. If the TFCC fails to heal to the footprint, then persistent instability can occur. The authors describe a surgical technique indicated for the treatment of persistent instability of the DRUJ due to foveal detachment of the TFCC. The procedure utilizes a loop of palmaris longus tendon graft passed through the ulnar aspect of the TFCC and into an osseous tunnel in the distal ulna to reconstruct the foveal attachment. This technique provides stability of the distal ulna to the radius and carpus. We recommend this procedure for chronic instability of the DRUJ due to TFCC avulsion, but recommend that suture repair remain the treatment of choice for acute instability. An arthroscopic assessment includes the trampoline test, hook test, and reverse hook test. DRUJ ballottement under arthroscopic vision details the direction of instability, the functional tear pattern, and unmasks concealed tears. If the reverse hook test demonstrates a functional instability between the TFCC and the radius, then a foveal reconstruction is contraindicated, and a reconstruction that stabilizes the radial and ulnar aspects of the TFCC is required. The foveal reconstruction technique has the advantage of providing a robust anatomically based reconstruction of the TFCC to the fovea, which stabilizes the DRUJ and the ulnocarpal sag.

Background A triangular fibrocartilage complex (TFCC) injury can produce distal radioulnar joint (DRUJ) instability. If the foveal attachment is avulsed, it translates distally. The footprint is separated from its origin and will become covered in synovitis, preventing healing. The authors describe a surgical technique for the treatment of instability of the DRUJ due to chronic foveal detachment of the TFCC. Technique The procedure utilizes a loop of autologous palmaris longus tendon graft passed through the ulnar aspect of the TFCC and through an osseous tunnel in the distal ulna to reconstruct the fovel attachment. Patients and Methods We report on nine patients with a mean age of 42. Median follow-up was 13 months. Results The median pain scores measured were reduced from 8 to 3 postoperatively, and all had a stable DRUJ. Conclusions This technique provides stability of the distal ulna to the radius and carpus, with potential for biologic healing through osseous integration. It is a robust, anatomically based reconstruction of the TFCC to the fovea that stabilizes the DRUJ and the ulnar-carpal sag.

BACKGROUND Symptoms associated with pes planovalgus or flatfeet occur frequently, even though some people with a flatfoot deformity remain asymptomatic. Pes planovalgus is proposed to be associated with foot/ankle pain and poor function. Concurrently, the multifactorial weakness of the tibialis posterior muscle and its tendon can lead to a flattening of the longitudinal arch of the foot. Those affected can experience functional impairment and pain. Less severe cases at an early stage are eligible for non-surgical treatment and foot orthoses are considered to be the first line approach. Furthermore, strengthening of arch and ankle stabilising muscles are thought to contribute to active compensation of the deformity leading to stress relief of soft tissue structures. There is only limited evidence concerning the numerous therapy approaches, and so far, no data are available showing functional benefits that accompany these interventions. METHODS After clinical diagnosis and clarification of inclusion criteria (e.g., age 40-70, current complaint of foot and ankle pain more than three months, posterior tibial tendon dysfunction stage I & II, longitudinal arch flattening verified by radiography), sixty participants with posterior tibial tendon dysfunction associated complaints will be included in the study and will be randomly assigned to one of three different intervention groups: (i) foot orthoses only (FOO), (ii) foot orthoses and eccentric exercise (FOE), or (iii) sham foot orthoses only (FOS). Participants in the FOO and FOE groups will be allocated individualised foot orthoses, the latter combined with eccentric exercise for ankle stabilisation and strengthening of the tibialis posterior muscle. Participants in the FOS group will be allocated sham foot orthoses only. During the intervention period of 12 weeks, all participants will be encouraged to follow an educational program for dosed foot load management (e.g., to stop activity if they experience increasing pain). Functional impairment will be evaluated pre- and post-intervention by the Foot Function Index. Further outcome measures include the Pain Disability Index, Visual Analogue Scale for pain, SF-12, kinematic data from 3D-movement analysis and neuromuscular activity during level and downstairs walking. Measuring outcomes pre- and post-intervention will allow the calculation of intervention effects by 3×3 Analysis of Variance (ANOVA) with repeated measures. DISCUSSION The purpose of this randomised trial is to evaluate the therapeutic benefit of three different non-surgical treatment regimens in participants with posterior tibial tendon dysfunction and accompanying pes planovalgus. Furthermore, the analysis of changes in gait mechanics and neuromuscular control will contribute to an enhanced understanding of functional changes and eventually optimise conservative management strategies for these patients. Las respuestas que se generan a partir de cuestionamientos del origen del hombre nos permitirán especular hacia dónde evolucionará como especie. Los grandes saltos evolutivos que diferencian a los primates humanos de los no humanos, se podrían describir entre otras características por una eficiente memoria para el uso de herramientas, la dominancia para el uso de la mano junto al desarrollo de la oposición del pulgar y el lenguaje. Se ha descrito un gen, el HSR que expresa para la dominancia para el uso de la mano derecha, habilidades cognitivas relacionadas con el lenguaje y asimetría cerebral en humanos. Este es un gen imprintado, es decir, que se hereda su expresión según el origen parental y cuya expresión está regulada por factores epigenéticos. Estos factores, modifican la expresión del gen sin afectar la estructura primaria del ADN. Se ha estudiado la expresión fenotípica del gen HSR en una población de niños escolarizados de La Rioja, dividida en dos regiones (Región 1 y Región 2). Los resultados obtenidos, que muestran una alteración de las proporciones fenotípicas del gen en la Región 2, apoyan fuertemente la posibilidad de que un factor ambiental estaría condicionando el epigenotipo del gen HSR. Se piensa que el estudio de estos mecanismos regulatorios en estos genes recientemente adquiridos por la evolución y blanco de funciones también recientemente adquiridas, podría dar información de hacia dónde la evolución del hombre podría proyectarse en el futuro.

Este artículo describe una prueba empírica de la asunción común que confianza en la policía afecte percepciones de justicia por mano propia. Aparte de testear el papel de la confianza difusa (general) en la policía, también testeamos si la reacción de la policía en un nivel situacional afecte como el público ve un acto subsiguiente de justicia por mano propia. En un estudio experimental (N = 385), participantes recibieron una viñeta (un caso específico) sobre justicia por mano propia. Usando un diseño inter-sujeto, variamos la reacción policial al delito original (activa/pasiva) y la violencia del justiciero (alta/baja). Encontramos que con más confianza difusa en la policía hubo menos apoyo para el vigilantismo. Adicionalmente, los otros factores experimentales tuvieron un impacto importante. Los ciudadanos están sensitivos a la variación situacional cuando juzgan un delito. Además, nuestros resultados ponen énfasis en la importancia de la acción policial al nivel situacional en la formación de la opinión pública

En esta ponencia presentamos un primer acercamiento exploratorio en el sector comercio. Concretamente, y a partir del análisis de un subsector -shopping centers-, buscamos observar cuestiones vinculadas con las estrategias de flexibilidad laboral, precariedad y subcontratación a través del estudio de las modalidades de gestión de mano de obra que allí operan. Para ello, analizamos algunas de las formas contractuales y las condiciones de trabajo que se presentan en un sector caracterizado por la heterogeneidad de los actores, la multiplicidad de actividades y el relevante lugar que ocupa en la economía argentina. La elección del sector mercantil parte de considerar las grandes transformaciones que la actividad ha experimentado a lo largo de su historia. E0sto se debe, entre otros factores, a los cambios económicos y sociales que impactaron directamente en las actividades comerciales. Y, a su vez, incidieron en la composición de los trabajadores y en la organización del trabajo que integran el sector. Dada la complejidad de la actividad seleccionada, distinguimos para el análisis del presente trabajo el subsector centros de compras (shopping centers). En primer lugar, nos proponemos una presentación general del sector comercio. Luego, nos focalizaremos en el subsector seleccionado a fin de identificar las principales características que operan en él

El propósito de ésta ponencia es revalorizar aquellos aspectos anatómicos y biomecánicos imprescindibles para el correcto funcionamiento de la muñeca y mano en los movimientos deportivos en los pases. Es imprescindible una revisión y una revalorización de la anatomía funcional desde el movimiento para su interpretación en relación a los soportes dinámicos de la muñeca y mano. La función de la muñeca y de la mano es importante en los gestos técnicos de los pases aunque muchas veces pase inadvertido y es importante entender el ajuste que realiza este complejo articular en los diferentes tipos de pases que hay y en los diferentes gestos técnicos deportivos que lo involucren (arqueros de futbol, voleibolistas, lanzamientos, etc.), que despliegan concretamente fuerzas de tensión de circunducción, de abducción-aducción y de flexo-extensión en la muñeca. A partir de aquí, es de vital importancia destacar el papel que desarrollan los músculos y ligamentos tanto en la estabilización de las estructuras que forman parte de la muñeca, como también en la prevención de lesiones a las que está expuesta cuando se la pone en funcionamiento durante la ejecución de movimientos que implican la coordinación y de manera explosiva como se produce en los deportes (en el atletismo, o en deportes de conjunto). La funcionalidad de la muñeca se constituye a través de segmentos con funciones de soporte estática y dinámica y segmentos óseos pasivos y motores que posibilita la relación entre los segmentos donde aparecen elementos anatómicos imprescindibles para entender los movimientos que se producen. Se intenta describir, no solo la importancia funcional de la muñeca y mano en los movimientos de pases, sino también determinar la importancia de las estructuras que la protegen ante las exigencias deportivas
