Background Estimates of the disease burden due to multiple risk factors can show the potential gain from combined preventive measures. But few such investigations have been attempted, and none on a global scale. Our aim was to estimate the potential health benefits from removal of multiple major risk factors. Methods We assessed the burden of disease and injury attributable to the joint effects of 20 selected leading risk factors in 14 epidemiological subregions of the world. We estimated population attributable fractions, defined as the proportional reduction in disease or mortality that would occur if exposure to a risk factor were reduced to an alternative level, from data for risk factor prevalence and hazard size. For every disease, we estimated joint population attributable fractions, for multiple risk factors, by age and sex, from the direct contributions of individual risk factors. To obtain the direct hazards, we reviewed publications and re-analysed cohort data to account for that part of hazard that is mediated through other risks. Results Globally, an estimated 47% of premature deaths and 39% of total disease burden in 2000 resulted from the joint effects of the risk factors considered. These risks caused a substantial proportion of important diseases, including diarrhoea (92%-94%), lower respiratory infections (55-62%), lung cancer (72%), chronic obstructive pulmonary disease (60%), ischaemic heart disease (83-89%), and stroke (70-76%). Removal of these risks would have increased global healthy life expectancy by 9.3 years (17%) ranging from 4.4 years (6%) in the developed countries of the western Pacific to 16.1 years (43%) in parts of sub-Saharan Africa. Interpretation Removal of major risk factors would not only increase healthy life expectancy in every region, but also reduce some of the differences between regions, The potential for disease prevention and health gain from tackling major known risks simultaneously would be substantial.

Objective: To test the hypothesis that the presence of national mental health policies, programs and legislation would be associated with lower national suicide rates. Method: Suicide rates from 100 countries were regressed on mental health policy, program and legislation indicators. Results: Contrary to the hypothesized relationship, the study found that after introducing mental health initiatives (with the exception of substance abuse policies), countries' suicide rates rose. Conclusion: It is of concern that most mental health initiatives are associated with an increase in suicide rates. However, there may be acceptable reasons for the observed findings, for example initiatives may have been introduced in areas of increasing need, or a case-finding effect may be operating. Data limitations must also be considered.

A large longitudinal dataset on women's health in Australia provided the basis of analysis of potential positive health effects of living with a companion animal. Age, living arrangements, and housing all strongly related to both living with companion animals and health. Methodological problems in using data from observational studies to disentangle a potential association in the presence of substantial effects of demographic characteristics are highlighted. Our findings may help to explain some inconsistencies and contradictions in the literature about the health benefits of companion animals, as well as offer suggestions for ways to more forward in future investigations of human-pet relationships.

Background: The effective evaluation of physical activity interventions for older adults requires measurement instruments with acceptable psychometric properties that are sufficiently sensitive to detect changes in this population. Aim: To assess the measurement properties (reliability and validity) of the Community Healthy Activities Model Program for Seniors (CHAMPS) questionnaire in a sample of older Australians. Methods: CHAMPS data were collected from 167 older adults (mean age 79.1 S.D. 6.3 years) and validated with tests of physical ability and the SF-12 measures of physical and mental health. Responses from a sub-sample of 43 older adults were used to assess 1-week test-retest reliability. Results: Approximately 25% of participants needed assistance to complete the CHAMPS questionnaire. There were low but significant correlations between the CHAMPS scores and the physical performance measures (rho=0.14-0.32) and the physical health scale of the SF-12 (rho=0.12-0.24). Reliability coefficients were highest for moderate-intensity (ICC=0.81-0.88) and lowest for vigorous-intensity physical activity (ICC=0.34-0.45). Agreement between test-retest estimates of sufficient physical activity for health benefits (>= 150 min and >= 5 sessions per week) was high (percent agreement = 88% and Cohen's kappa = 0.68). Conclusion: These findings suggest that the CHAMPS questionnaire has acceptable measurement properties, and is therefore suitable for use among older Australian adults, as long as adequate assistance is provided during administration. Background: Kiribati, a remote atoll island country of the Pacific, has serious problems of vitamin A deficiency (VAD). Thus, it is important to identify locally grown acceptable foods that might be promoted to alleviate this problem. Pandanus fruit (Pandanus tectorius) is a well-liked indigenous Kiribati food with many cultivars that have orange/yellow flesh, indicative of carotenoid content. Few have been previously analysed. Aim: This study was conducted to identify cultivars of pandanus and other foods that could be promoted to alleviate VAD in Kiribati. Method: Ethnography was used to select foods and assess acceptability factors. Pandanus and other foods were analysed for beta- and alpha-carotene, beta-cryptoxanthin, lutein, zeaxanthin, lycopene and total carotenoids using high-performance liquid chromatography. Results: Of the nine pandanus cultivars investigated there was a great range of provitamin A carotenoid levels (from 62 to 19 086 mu g beta-carotene/100 g), generally with higher levels in those more deeply coloured. Seven pandanus cultivars, one giant swamp taro (Cyrtosperma chamissonis) cultivar and native fig (Ficus tinctoria) had significant provitamin A carotenoid content, meeting all or half of estimated daily vitamin A requirements within normal consumption patterns. Analyses in different laboratories confirmed high carotenoid levels in pandanus but showed that there are still questions as to how high the levels might be, owing to variation arising from different handling/preparation/analytical techniques. Conclusions: These carotenoid-rich acceptable foods should be promoted for alleviating VAD in Kiribati and possibly other Pacific contexts where these foods are important. Further research in the Pacific is needed to identify additional indigenous foods with potential health benefits.

No campo científico, a busca por uma maior compreensão acerca de bem-estar no âmbito do trabalho é a tônica do momento, reunindo estudiosos das mais diferentes áreas que demonstram interesse pelo tema. No contexto empresarial, o papel dos programas de promoção da saúde e qualidade de vida torna-se marcante quando se deseja o crescimento da produtividade e do bem-estar dos trabalhadores, considerando a obtenção de resultados crescentes, pela empresa, como fundamentais para que as melhorias nas condições, organização e relações de trabalho ocorram permanentemente. Ou seja, as empresas entendem que com o investimento nesses programas a produtividade deve aumentar, gerando recursos, para que esta se sinta estimulada a continuar investindo no bem-estar dos empregados. Este estudo teve como objetivo promover uma análise sobre as relações entre bem-estar no trabalho, representado por satisfação com o trabalho, envolvimento com o trabalho e comprometimento organizacional afetivo; e freqüência a programas organizacionais de promoção da saúde. A amostra foi constituída por 117 trabalhadores de uma indústria multinacional química e farmacêutica, situada na Grande São Paulo, sendo 53 do sexo masculino e 64 do sexo feminino, com idade média de 30,28 anos, dos quais 63 eram solteiros, 46 casados e oito registram outro estado civil. Para desenvolvimento deste estudo, utilizou-se como instrumento um questionário composto por quatro escalas que mediram satisfação no trabalho, envolvimento com o trabalho, comprometimento organizacional afetivo e freqüência de participação em programas de promoção da saúde, realizados na empresa. Todos os dados recolhidos por meio do instrumento eram numéricos, o que permitiu sua análise eletrônica pelo SPSS, versão 13.0 para Windows. Nessa categoria de dados incluem-se os de descrição da amostra, os referentes a bem-estar no trabalho e freqüência aos programas de promoção da saúde. Foi criado um banco de dados para realização das análises descritivas (freqüências, percentuais, médias e desvios-padrão) e de correlação bivariada (r de Pearson). Os dados que permitiram descrever os programas de promoção da saúde promovidos pela empresa foram obtidos por meio de entrevista com a gerente de responsabilidade social, análise do relatório da empresa e de folhetos elaborados para divulgação e registro desses programas. Um total de cinco categorias de programas foi identificado: benefícios, atividades físicas, saúde, atividades culturais/lazer e ações comunitárias. A maior freqüência aos programas ocorreu na categoria benefícios, pela presença constante no restaurante da empresa. Os resultados deste estudo revelaram que grande parcela do bem-estar no trabalho especialmente no que tange o envolvimento com o trabalho, comprometimento afetivo com a organização, bem como satisfações com os colegas, com a chefia e com o próprio trabalho praticamente independe dos programas de promoção da saúde promovidos pela empresa. Por outro lado, participação em programas de promoção da saúde que incluem as categorias de benefícios, saúde e ações comunitárias, relacionou-se a satisfações com promoções e com salário

People and their performance are key to an organization's effectiveness. This review describes an evidence-based framework of the links between some key organizational influences and staff performance, health and well-being. This preliminary framework integrates management and psychological approaches, with the aim of assisting future explanation, prediction and organizational change. Health care is taken as the focus of this review, as there are concerns internationally about health care effectiveness. The framework considers empirical evidence for links between the following organizational levels: 1. Context (organizational culture and inter-group relations; resources, including staffing; physical environment) 2. People management (HRM practices and strategies; job design, workload and teamwork; employee involvement and control over work; leadership and support) 3. Psychological consequences for employees (health and stress; satisfaction and commitment; knowledge, skills and motivation) 4. Employee behaviour (absenteeism and turnover; task and contextual performance; errors and near misses) 5. Organizational performance; patient care. This review contributes to an evidence base for policies and practices of people management and performance management. Its usefulness will depend on future empirical research, using appropriate research designs, sufficient study power and measures that are reliable and valid.
