To clarify the mechanism responsible for the transient sinus tachycardia in rats with acute chagasic myocarditis, we have examined the cardiac sympathetic-parasympathetic balance of 29 rats inoculated with 200,000 parasites (Trypanosoma cruzi). Sixteen infected animals and 8 controls were studied between days 18 and 21 after inoculation (acute stage). The remaining 13 infected animals and 9 controls were studied between days 60 and 70 after inoculation (sub-acute stage). Under anesthesia (urethane 1.25 g/kg), all animals received intravenous atenolol (5 mg/kg) and atropine (10 mg/kg). Acute stage: The baseline heart rate of the infected animals was significantly higher than that of the controls (P < 0.0001). The magnitude of the negative chronotropic response to atenolol was 4 times that of the controls (P < 0.00001). This response correlated with the baseline heart rate (r= - 0.72, P < 0.001). The heart rate responses to the beta-blocker and to atropine, of the infected animals studied during the sub-acute stage, were not different from controls. These findings suggest that cardiac sympathetic activity is transiently enhanced and cardiac parasympathetic activity is not impaired, in rats with acute chagasic myocarditis. The transient predominance of cardiac sympathetic activity could explain, in part, the sinus tachycardia observed in the acute stage of experimentally-induced chagasic myocarditis.

From January, 1984 to December, 1996, 422 patients (ages 9 m-99 y, median 29 y) were admitted after being bitten by spiders which were brought and identified as Phoneutria spp. Most of the bites occurred at March and April months (29.2%), in the houses (54.5%), during the day (76.5%), and in the limbs (feet 40.9%, hands 34.3%). Upon hospital admission, most patients presented only local complaints, mainly pain (92.1%) and edema (33.1%) and were classified as presenting mild (89.8%), moderate (8.5%) and severe (0.5%) envenomation. Few patients (1.2%) did not present signs of envenomation. Severe accidents were only confirmed in two children (9 m, 3 y). Both developed acute pulmonary edema, and the older died 9 h after the accident. Patients more than 70 year-old had a significantly greater (p<0.05) frequency of moderate envenomations compared to the 10-70-year-old individuals. Proceedings to relief local pain were frequently performed (local anesthesia alone 32.0%, local anesthesia plus analgesics 20.6% and oral analgesics alone 25.1%). Only 2.3% of the patients (two cases classified as severe and eight as moderate, eight of them in children) were treated with i.v. antiarachnid antivenom. No antivenom early reaction was observed. In conclusion, accidents involving the genus Phoneutria are common in the region of Campinas, with the highest risk groups being children under 10 years of age and adults over 70 years of age. Cases of serious envenomation are rare (0.5%).

Chiropractic's popularity is rising among the general population. Moreover, few studies have been conducted to properly evaluate its safety. We report three cases of serious neurological adverse events in patients treated with chiropractic manipulation. The first case is a 41 years old woman who developed a vertebro-basilar stroke 48 h after cervical manipulation. The second case represents a 68 years old woman who presented a neuropraxic injury of both radial nerves after three sessions of spinal manipulation. The last case is a 34 years old man who developed a cervical epidural haematoma after a chiropractic treatment for neck pain. In all three cases there were criteria to consider a causality relation between the neurological adverse events and the chiropractic manipulation. The described serious adverse events promptly recommend the implementation of a risk alert system.

OBJECTIVE: The aim of this study was to evaluate the efficacy of post-caesarean analgesia comparing three techniques most frequently used. PATIENTS AND METHODS: For three months all pregnant women submitted to elective or urgent caesarean section, under general or regional anaesthesia, were evaluate with a total of 129 parturient. These parturient were divided into three groups with different techniques of postoperative analgesia: Group 1 (n = 26) received intravenous pethidine and paracetamol per os, group 2 (n = 58) received epidural morphine and group 3 (n = 45) epidural morphine and intravenous propacetamol. Pain was assessed at rest and during mobilisation using a scale of 0-without pain, 1-mild pain, 2-moderate pain and 3-severe pain. Overall satisfaction was assessed with a verbal qualitative scale of very good, good, sufficient and bad. Side effects were analysed. RESULTS: The records of pain at rest and during mobilisation were significantly lower with epidural analgesia compared with intravenous pethidine. There were no significant differences between groups 2 and 3. Similar results were observed in the degree of satisfaction. For 50% of parturient of epidural analgesia (groups 2 and 3) and only 4% of intravenous pethidine (group 1) the analgesic technique was very good. Propacetamol and epidural morphine (group 3) had better pain scores (very good and good) when compared with morphine alone (group 2) but there were no significant differences. Epidural morphine was associated with more pruritus. CONCLUSION: From this study we are able to conclude that epidural morphine offers a good quality of analgesia with better satisfaction and minimal side effects.

Frontal osteomyelitis is a rare complication of sinusitis. Common intracranial complications of the frontal osteomyelitis are meningitis, epidural empyema, subdural empyema and brain abscess. We described a case of frontal osteomyelitis with brain abscess caused by Staphylococcus aureus with improve after needle aspiration and antibiotics to brain abscess for eight weeks and for chronic osteomyelitis for four months.

Estas recomendações resultam do trabalho de um conjunto de 32 Anestesiologistas portugueses, provenientes de hospitais com diversas realidades na prática da Cirurgia Ambulatória em Portugal. Esse grupo começou por reunir e analisar documentação científica posterior à publicação pela SAMBA (Society of Ambulatory Anesthesia) das suas Guidelines para profilaxia das náuseas e vómitos no pós-operatório (NVPO) em cirurgia ambulatória. Numa Reunião Nacional de Consenso, estes 32 Especialistas discutiram as adaptações e actualizações mais adequadas à realidade portuguesa. As conclusões desta reunião foram posteriormente divulgadas e novamente discutidas em reuniões mais abrangentes nos Congressos da SPA (Sociedade Portugesa de Anestesiologia) e APCA (Associação Portuguesa de Cirurgia Ambulatória) e o resultado final é o que se apresenta neste artigo. O objectivo fundamental deste trabalho é tornar estas recomendações numa ferramenta de aplicação simples e prática em todas as cirurgias efectuadas em regime de ambulatório, procurando reduzir cada vez mais a incidência das NVPO, aumentando a eficiência deste regime cirúrgico e a satisfação dos doentes.

Introdução: A incidência de síndrome da cauda equina foi quantificada em <0,23: 10 000 na anestesia obstétrica. Apesar da raridade, as suas consequências podem ser geradoras de limitações importantes na qualidade de vida das pacientes, daí a pertinência da análise deste caso clínico. Grávida de 34 anos com antecedentes de VIH2, VHC e linfoma não Hodgkin em remissão desde há 5 anos submetida a bloqueio subaracnoideu e colocação de cateter epidural para cesariana. A abordagem do espaço epidural foi difícil. Administrou-se 7,5mg de levobupivacaína e 0,0025mg sufentanil no espaço subaracnoideu e a paciente queixou-se de dor tipo choques eléctricos e parestesias nos membros inferiores durante segundos. Após 1:30h, na UCPA, a paciente iniciou o mesmo tipo de dor na região sacrococcígea e coxas após administração morfina e metilprednisolona no espaço epidural, a qual foi debelada com paracetamol, cetorolac, dipirona e petidina. A remoção do cateter epidural foi difícil. No pós-operatório a paciente sentiu tremor ao nível da coxa esquerda e parestesias na coxa direita. Pela raridade do síndrome da cauda equina, a análise do ocorrido é pertinente contrapondo com descrições prévias, em virtude duma intervenção anestésica mais segura.

As diversas manifestações clínicas do abuso de drogas combinadas com as alterações fisiológicas da gravidez e a fisiopatologia das doenças coexistentes específicas da gravidez, podem conduzir a complicações ameaçadoras da vida com impacto significativo na prática da anestesia obstétrica. As implicações anestésicas específicas são difíceis de predizer em doentes quimicamente dependentes, independentemente da droga consumida. A avaliação pré-anestésica cuidada e o interrogatório não recriminatório da grávida são fundamentais para a identificação do problema. Os anestesiologistas intervêm na prestação de cuidados a estas grávidas na analgesia do trabalho de parto e/ou parto cirúrgico, quer em situações programadas, quer em situações de emergência, com morbi-mortalidade significativas.

A punção acidental da dura-mater (PD) é uma complicação ocasional na realização do bloqueio epidural. A sua incidência pode chegar aos 4,4%, sendo as grávidas um grupo de risco pela idade, alterações fisiológicas e anatómicas da gravidez. O sintoma mais frequente é a cefaleia (Cefaleia pós-punção da dura – CPPD), que tem como principal característica ser postural, melhorar com o decúbito e a compressão abdominal. Esta pode ser incapacitante, privando a mãe do contacto com o bebé e impossibilitando-a na prestação de cuidados. Além da cefaleia, a punção da dura pode apresentar-se com outros sintomas tais como náuseas, fotofobia, rigidez cervical e tonturas. Estes sintomas (meníngeos e tentoriais) são associados a tracção de estruturas. Neste artigo são abordados o quadro clínico, as medidas profilácticas e de tratamento. É feita uma proposta de revisão do protocolo de actuação perante um quadro de punção acidental da Dura.

Objective: To investigate the results of vaginal obliterate surgery in elderly women with pelvic organ prolapse. Design: observational retrospective study. Material and Methods: a total of 69 women with the diagnosis of pelvic prolapse were submitted to obliterative surgery in the urogynecology unit of a tertiary care hospital centre over the course of 8 years (2001 to 2008). The following data were collected from their clinical records: age, number of vaginal births, body mass index (BMI), hormone therapy, other existing diseases, type of prolapse and stage, anaesthetic risk score, duration of surgery, length of hospital stay, and short-term complications. Results: Of the 69 women studied, 31 were submitted to colpocleisis and the remaining 38 were managed by the LeFort technique. Mean age was 74.8 years with a standard deviation (sd) of 7.14 years. Average BMI was 26.2 (sd =3.76). Vaginal births were recorded in all patients. Only three patients were taking hormone therapy at the time of surgery. Sixty-three women were classified as having and anesthetic risk of II or III and 55 underwent local-regional anesthesia. Complications were reported in five cases, four of which in the first days after surgery. Nearly all were mild and resolved within the first 6 weeks. Conclusion: Complication rates appear to be low after obliterative surgery for pelvic organ prolapse in elderly women.

Resumo: Por intermédio deste estudo, pretendeu-se verificar qual a realidade em termos de analgesia e anestesia obstétricas na Maternidade do Hospital D. Estefânia, ao longo de 4 anos. Para isso, foi realizado um estudo retrospectivo no período entre Janeiro de 2005 e Dezembro de 2008. Foram avaliados a classificação ASA das parturientes, o número total de partos, o número de parturientes submetidas a técnicas locorregionais e suas complicações, número de cesarianas e a necessidade de conversão de técnica regional para anestesia geral. No período do estudo, houve um total de 8291 partos, maioritariamente em mulheres ASA I, dos quais 2643 foram cesarianas. A maioria das parturientes (77%) foi submetida a uma técnica locorregional, para analgesia de trabalho de parto ou anestesia para cesariana, com baixa taxa de complicações (2,9%) e com rara necessidade de conversão para anestesia geral (3,5%). As autoras concluiram que, na Maternidade do Hospital D. Estefânia, a epidural continua a ser a técnica gold standard para analgesia do trabalho de parto, com raras complicações e permitindo a fácil conversão para anestesia para cesariana.

INTRODUCTION: Ryanodine receptor gene (RYR1) mutations have been associated with central core disease (CCD), multiminicore/minicore/multicore disease (MmD), and susceptibility to malignant hyperthermia (MH). METHODS: Patients with muscle symptoms in adulthood, who had features compatible with CCD/MmD, underwent clinical, histological, and genetic (RYR1 and SEPN1 genes) evaluations. Published cases of CCD and MmD with adult onset were also reviewed. RESULTS: Eight patients fulfilled the criteria for further analysis. Five RYR1 mutations, 4 of them unreported, were detected in 3 patients. Compound heterozygosity was proven in 1 case. CONCLUSIONS: To our knowledge, this is the only report of adult onset associated with recessive RYR1 mutations and central core/multiminicores on muscle biopsy. Although adult patients with CCD, MmD, and minimally symptomatic MH with abnormal muscle biopsy findings usually have a mild clinical course, differential diagnosis and carrier screening is crucial for prevention of potentially life-threatening reactions to general anesthesia.
