RESUMO: Introdução: O treino de exercício assume uma importância fundamental nos programas de reabilitação respiratória, contudo permanece desconhecida qual a intensidade de treino efectiva na mudança dos resultados centrados no doente. Objectivos: Foram estudados os efeitos de duas intensidades de treino aeróbio na qualidade de vida relacionada com a saúde, no controlo de sintomas e na tolerância ao exercício, num programa de reabilitação respiratória para doentes com DPOC. Métodos: Trinta e quatro doentes com DPOC de estádios GOLD de ligeiro a muito grave, foram alocados aleatoriamente a intensidades de treino de exercício aeróbio de 60% ou 80% da intensidade máxima avaliada na prova de esforço num programa ambulatório de reabilitação respiratória de 20 sessões. Foram avaliados resultados centrados no doente pela aplicação do questionário respiratório de St.George, do índice de dispneia de Mahler, do questionário de actividades da vida diária do London Chest, da prova de 6 minutos, da prova de endurance a carga constante e da prova de esforço máxima incremental. Resultados: Apesar de existirem melhorias significativas em todos os resultados para ambos os grupos, a diferença média intergrupal por efeito da intensidade de treino aeróbio a 60% ou 80% da carga máxima em prova de esforço não foi significativa no questionário respiratório de St.George (p=0,306), no índice de dispneia de Mahler (p=0,378), no questionário de actividades da vida diária do London Chest (p=0,425), na prova de marcha de 6 minutos (p=0,917), na prova de endurance a carga constante (p=0,504) e na prova de esforço máxima incremental (p=0,117). Verificou-se ainda que a média das diferenças intergrupais na prova de endurance a carga constante e na prova de marcha de 6 minutos não apresentou qualquer associação com a idade dos doentes. Conclusões: O estudo permite concluir que não existem diferenças significativas nas alterações médias da qualidade de vida relacionada com a saúde, do controle de sintomas e da tolerância ao exercício em doentes com DPOC como efeito de duas intensidades de treino aeróbio. Os autores propõem para a prática clínica em reabilitação respiratória, a aplicação de intensidades de treino aeróbio individualmente adaptadas, de pelo menos 60% da carga máxima na prova de esforço inicial. -------------------------------ABSTRACT: Introduction: Exercise training is an important component of pulmonary rehabilitation, but it remains questionable as how training intensity changes patient-centered outcomes. Aims and objectives: The effects of two aerobic exercise training intensities in health-related quality of life, symptoms control and exercise tolerance in COPD patients were studied. Methods: Thirty-four COPD patients from mild to very severe GOLD stages were randomly assigned to a 60% or 80% aerobic exercise training intensity in a twenty-session outpatient pulmonary rehabilitation program. Patient-centered outcomes were assessed with St.George's respiratory questionnaire, Mahler dyspnoea index, London Chest activity of daily living, six minute walk test, constant work rate test and maximal incremental test. Results: Although there were significant improvements in all outcomes for both groups, there were no differences in mean change in St.George's respiratory questionnaire (p=0,306), Mahler dyspnoea index (p=0,378), London Chest activity of daily living (p=0,917), six-minute walk test (p=0,504), constant work rate test (p=0,504) and maximal incremental test (p=0,117), as an effect of aerobic exercise training intensity of 60% or 80%. Mean changes in constant work rate and six-minute walk test were not age-related. Conclusions: It is concluded that there are no differences in mean changes in health-related quality of life, symptoms control and exercise tolerance in COPD patients as an effect of two aerobic exercise training intensities. Therefore, the authors suggest that individually tailored exercise intensities above 60% should guide clinical practice in pulmonary rehabilitation.

A true neurogenic thoracic outlet syndrome (TOS) associated with a cervical rib is considered extremely rare. The authors present their experience with 5 cases of true neurogenic TOS associated with a cervical rib. All patients were female and had a cervical rib confirmed radiographically pre-operatively. Average age was 34,8 years. Although all patients had been treated with several combinations of diverse drugs and a rehabilitation program before referral to surgery, all described their pain as intense and debilitating before surgical treatment. All patients had pre-operative electromyographic abnormalities. Patients were operated on via a supraclavicular approach and the cervical rib was resected. No intra-operative or postoperative complications were noted. Two years postoperatively, all patients mentioned improvement. However, only 2 were symptomless, and on no medication. In one patient there was significant improvement, and in the remaining 2 patients some residual pain persisted that had to be dealt with pharmacologically. All patients were able to resume their daily life activities. Recovery was poorer in the 2 patients that had been referred to surgery after a longer period of time since the beginning of symptoms.

RESUMO: Relevância e Objectivos: A educação focada na auto-gestão tem sido cada vez mais valorizada como parte integrante da abordagem terapêutica dos doentes com DPOC. No entanto poucos resultados se encontram na literatura. Este estudo investiga se um programa de educação contribui para a melhoria da funcionalidade e do estado de saúde associados à DPOC estável. Metodologia: A amostra incluiu 18 indivíduos portadores de DPOC ligeira a moderada, de acordo com os critérios GOLD, com uma média de idades de 71 ± 8 anos. Os doentes foram distribuídos por 2 grupos: um grupo experimental, constituído por 10 doentes do sexo masculino, a receber os cuidados habituais e submetidos a um programa de educação, e um grupo de controlo formado por 7 doentes do sexo masculino e uma do sexo feminino, a receber apenas os cuidados habituais. Foram avaliados os dados sócio-demográficos relativos à amostra, o nível de conhecimentos sobre a patologia e a sua auto-gestão, a dispneia, o impacto nas AVD’s, a qualidade de vida relacionada com a saúde, a ansiedade e a depressão. Foram aplicados 8 questionários, 2 realizados pela invesstigadora, um destinado a recolher dados sociodemográficos e clínicos, outro para avaliar o conhecimento da doença pelo doente e os 6 restantes estandardizados: Medical Research Council, Índice de Dispneia Basal Modificado de Mahler, Índice de Dispneia de Transição de Mahler, St. George Respiratory Questionnaire, London Chest Activity of Daily Living e o Hospital Anxiety and Depression Scale. Incluíu-se também a realização de uma espirometria, de uma prova de marcha de 6 minutos e a aplicação de 1 checklist para avaliar a evolução dos conhecimentos do doente sobre a sua doença. Os dados foram recolhidos em dois momentos: em T0 e em T1, correspondendo ao início e o final do programa de educação. Resultados: Obteve-se uma diferença com significância na melhoria do nível de conhecimentos entre o grupo experimental e o grupo de controlo com um p = 0,001. Não se alcançaram resultados significativos na melhoria da distância percorrida, da dispneia, do impacto nas AVD’s, da QVRS, da ansiedade e da depressão. Conclusões: A aplicação de um programa de educação a doentes com DPOC estável contribuiu para a melhoria de conhecimentos sobre a doença, mas não se traduziu em modificações no estado de saúde e da funcionalidade na população estudada.------------ABSTRACT: Relevance and Objectives: Self-management interventions have been increased as an important part of therapeutic approach in COPD patients. However, few results are found in literature. This study investigates whether self-management program contributes to improve functionality and health status associated with stable COPD. Methods: The sample included 18 subjects with mild to moderate COPD, according to GOLD criteria, and a mean age of 71 ± 8 years. Patients were divided into 2 groups: one experimental group, consisting of 10 male patients receiving usual care plus an education program, and a control group consisting of one female and 7 male patients, receiving only usual care. We assessed socio-demographic data, level of knowledge about the pathology and selfmanagement, dyspnea, impact on ADLs, health related quality of life, anxiety and depression. Were administered 8 questionnaires, two made by the researcher, one to collect sociodemographic and clinical data, another to assess the knowledge of the disease by the patient and the remaining 6 standardized: Medical Research Council, baseline dyspnea index Modified Mahler's, Mahler Transitional (dyspnea), St. George Respiratory Questionnaire (HRQOL),London Chest Activity of Daily Living (ADL´s) and the Hospital Anxiety and Depression Scale (anxiety and depression). Performed a spirometry, a test of 6-minute walk and a checklist for monitoring progress of the patient's knowledge about their disease. Data were collected on two times: T0 and T1, beginning and end of self-management program. Results: We found a significant improvement in the level of knowledge between the experimental and control group with p = 0.001. We didn’t achieve significant results in improving distance, dyspnea, impact on ADLs, HRQOL, anxiety and depression. Conclusions: The application of an education program in stable COPD patients contributed to the improvement of knowledge about the disease, but didn’t translate into changes in health status and functionality in population of this study.

RESUMO - Contexto: As desigualdades sociais em saúde são uma questão central de justiça social. No contexto de forte envelhecimento populacional em Portugal, as desigualdades nos idosos representam um desafio crucial para o futuro, sobre as quais existe pouca evidência. Este estudo pretende investigar a existência de desigualdades socioeconómicas em saúde nos idosos, em Portugal. Metodologia: Foram utilizados os dados para Portugal, da quarta vaga do Survey of Health, Ageing and Retirement in Europe. O estudo engloba 2017 indivíduos com 50 ou mais anos. Foram utilizados quatro indicadores de saúde: problemas de saúde, saúde auto-reportada, doenças de longa duração e atividade limitada. Foi utilizado o nível de educação como indicador socioeconómico. As desigualdades socioeconómicas foram avaliadas através de regressões logísticas multivariadas. Resultados: Existem desigualdades socioeconómicas em saúde nos idosos favoráveis aos mais educados. Os indivíduos com menor educação estão em maior risco de reportar má saúde (OR=5,5); maior risco em ter problemas de saúde, existindo um gradiente social na Hipertensão Arterial (OR=2,4) e na Artrite (OR=7,0); maior risco de doenças de longa duração (OR=1,6) e maior risco de limitação nas atividades diárias (OR=5,1). As desigualdades socioeconómicas diminuem com a idade. Conclusão: De forma a melhorar a saúde e reduzir as desigualdades socioeconómicas em saúde nos idosos, os resultados apontam para a necessidade de implementar medidas no âmbito dos problemas de saúde em que existe um gradiente social, melhorar o nível de educação da população geral e implementar medidas de educação para a saúde, aumentando a literacia em saúde nos idosos mais jovens.

Several studies have shown that people with disabilities benefit substantially from access to a means of independent mobility and assistive technology. Researchers are using technology originally developed for mobile robots to create easier to use wheelchairs. With this kind of technology people with disabilities can gain a degree of independence in performing daily life activities. In this work a computer vision system is presented, able to drive a wheelchair with a minimum number of finger commands. The user hand is detected and segmented with the use of a kinect camera, and fingertips are extracted from depth information, and used as wheelchair commands.

A prevalência de pessoas que referem dor no complexo articular do ombro, com concomitante limitação na capacidade para realizar atividades da vida diária, é elevada. Estes níveis de prevalência sobrecarregam quer os utentes, como a própria sociedade. A evidência científica atual indicia a existência de uma relação entre as alterações da articulação escápulo-torácica e as patologias associadas à articulação gleno-umeral. A capacidade de quantificar, cinemática e cineticamente, as disfunções ao nível das articulações escápulo-torácica e gleno-umeral, é algo de enorme importância, quer para a comunidade biomecânica, como para a clínica. No decorrer dos trabalhos desta tese foi desenvolvido, através do software OpenSim, um modelo tridimensional músculo-esquelético do complexo articular do ombro que inclui a representação do tórax/coluna, clavícula, omoplata, úmero, rádio, cúbito e articulações que permitem os movimentos relativos desses segmentos, assim como, 16 músculos e 4 ligamentos. Com um total de 11 graus de liberdade, incluindo um novo modelo articular escápulo-torácico, os resultados demonstram que este é capaz de reconstruir de forma precisa e rápida os movimentos escápulo-torácicos e glenoumerais, recorrendo para tal, à cinemática inversa, e à dinâmica inversa e direta. Conta ainda com um método de transformação inovador para determinar, com base nas especificidades dos sujeitos, os locais de inserção muscular. As principais motivações subjacentes ao desenvolvimento desta tese foram contribuir para o aprofundar do atual conhecimento sobre as disfunções do complexo articular do ombro e, simultaneamente, proporcionar à comunidade clínica uma ferramenta biomecânica de livre acesso com o intuito de melhor suportar as decisões clínicas e dessa forma concorrer para uma prática mais efetiva.

(2010) European Journal of Cancer Care Socio-demographic predictors of high support needs in newly diagnosed breast cancer patients This study aimed to identify high support needs and their socio-demographic predictors to improve supportive care for newly diagnosed breast cancer patients. A cross-sectional study measured patients' needs and unsatisfied support needs by the supportive care needs survey (SCNS-34), administered after surgery, chemotherapy or radiotherapy. Socio-demographic, disease and treatment characteristics completed data collection. A total of 308 questionnaires were completed with a response rate of 88%. The most frequent support needs (73.3% of patients) related to information and the highest unsatisfied support needs to the management of emotions and daily life (36.3-39.6% of patients). Younger age predicted high and dissatisfied support needs (P < 0.05). Patients born outside Switzerland or with a lower level of education had more needs in daily living and psychological domains (P < 0.05). Being born outside Switzerland also predicted dissatisfaction with information provided. Being parent was a predictor of significant needs in the daily living domain after adjusting for disease and treatment characteristics (P= 0.01). Therefore, information, psychological and daily living support for newly diagnosed breast cancer patients should be strongly reinforced, particularly in patients being born outside Switzerland, those with children or being younger. For the latter, support in sexuality domain should also be emphasised.

Introduction. Respiratory difficulties in athletes are common, especially in adolescents, even in the absence of exercise-induced bronchoconstriction. Immaturity of the respiratory muscles coupling at high respiratory rates could be a potential mechanism. Whether respiratory muscle training (RMT) can positively influence it is yet unknown. Goal. We investigate the effects of RMT on ventilation and performance parameters in adolescent athletes and hypothesize that RMT will enhance respiratory capacity. Methods. 12 healthy subjects (8 male, 4 female, 17±0.5 years) from a sports/study high school class, competitively involved in various sports (minimum of 10 hours per week) underwent respiratory function testing, maximal minute ventilation (MMV) measurements and a maximal treadmill incremental test with VO2max and ventilatory thresholds (VT1 and VT2) determination. They then underwent one month of RMT (4 times/week) using a eucapnic hyperventilation device, with an incremental training program. The same tests were repeated after RMT. Results. Subjects completed 14.8 sessions of RMT, with an increase in total ventilation per session of 211±29% during training. Borg scale evaluation of the RMT session was unchanged or reduced in all subjects, despite an increase in total respiratory work. No changes (p>0.05) were observed pre/post RMT in VO2max (53.4±7.5 vs 51.6±7.7 ml/kg/min), VT2 (14.4±1.4 vs 14.0±1.1 km/h) or Speed max at end of test (16.1±1.7 vs 15.8±1.7 km/h). MVV increased by 9.2% (176.7±36.9 vs 192.9±32.6 l/min, p<0.001) and FVC by 3.3% (6.70±0.75 vs 4.85±0.76 litres, p<0.05). Subjective evaluation of respiratory sensations during exercise and daily living were also improved. Conclusions. RMT improves MMV and FVC in adolescent athletes, along with important subjective respiratory benefits, although no changes are seen in treadmill maximal performance tests and VO2max measurements. RMT can be easily performed in adolescent without side effects, with a potential for improvement in training capacity and overall well-being.

Older people have the greatest barriers (mostly in terms of ability/opportunity) to participating in exercise, and thus have the highest incidence of decreased ease in carrying out tasks of daily living. In an ageing society, the negative effects of age on skeletal muscle performance would be reduced if compliance to exercise regimens could be encouraged through simple advice on timing, duration, frequency, intensity, as well as appropriate nutritional interventions. Research into the impact of optimal nutrition/diet supplementation in young and/or athletic populations is extensive. Such data are scarce when considering healthy, older populations. We therefore propose to investigate the impact of healthy eating habits with or without supplementation, on exercise responsiveness. This protocol seeks to maximise training benefits on the neural muscular and tendinous complexes.

En el moment actual la presència de la tecnologia de la informació i la comunicació (TIC) es fa imprescindible en les activitats de la vida quotidiana. En l'escola d'una societat moderna està adquirint una presència transcendental i inqüestionable que està canviant les formes d'aprendre i transmetre els coneixements. L'escola millor preparada serà aquella que dispose d'aules autosuficients, és a dir aules en què es troben els recursos tecnològics necessaris per a possibilitar a l'alumnat l'accés a la informació i a la comunicació que ens proporciona la xarxa i els sistemes de presentació multimèdia disponibles en estos temps. La informàtica, Internet, el vídeo, la televisió, i els mitjans per a audicions i projeccions són necessitats que tant l'alumnat com el professorat han de tindre al seu abast en tot moment. Cada vegada més la majoria de les activitats que es realitzen en l'escola necessiten utilitzar estes tecnologies.

BACKGROUND: This study assessed whether breast cancer (BC) patients express similar levels of needs for equivalent severity of symptoms, functioning difficulties, or degrees of satisfaction with care aspects. BC patients who did (or not) report needs in spite of similar difficulties were identified among their sociodemographic or clinical characteristics. PATIENTS AND METHODS: Three hundred and eighty-four (73% response rate) BC patients recruited in ambulatory or surgery hospital services completed the European Organisation for Research and Treatment of Cancer Quality of Life questionnaire (EORTC QLQ)-C30 quality of life [health-related quality of life (HRQOL)], the EORTC IN-PATSAT32 (in-patient) or OUT-PATSAT35 (out-patient) satisfaction with care, and the supportive care needs survey short form 34-item (SCNS-SF34) measures. RESULTS: HRQOL or satisfaction with care scale scores explained 41%, 45%, 40% and 22% of variance in, respectively, psychological, physical/daily living needs, information/health system, and care/support needs (P < 0.001). BC patients' education level, having children, hospital service attendance, and anxiety/depression levels significantly predicted differences in psychological needs relative to corresponding difficulties (adjusted R(2) = 0.11). Medical history and anxiety/depression levels significantly predicted differences in information/health system needs relative to degrees of satisfaction with doctors, nurses, or radiotherapy technicians and general satisfaction (adjusted R(2) = 0.12). Unmet needs were most prevalent in the psychological domains across hospital services. CONCLUSIONS: Assessment of needs, HRQOL, and satisfaction with care highlights the subgroups of BC patients requiring better supportive care targeting.

Three case studies are presented to investigate the possibility of evaluating memory and cognitive capacities of severe intellectual disability with attention given to the ecological environment. Two 22-year-old male patients and a 27-year-old male patient, all three with severe intellectual disability with no verbal communication skills, were evaluated with a new and original paradigm adapted to study cognition in humans from experimental paradigms. We developed a test based on animal models to complement the "home" scale of the Adolescent and Adult Psychoeducational Profile (AAPEP), an assessment instrument designed for adolescents and adults with severe developmental disabilities. Results show that the new instrument is helpful, not only to staff members who can better understand the poor performances of their patients in daily life activities but also in the elaboration of individual acquisition plans. These preliminary results demonstrate the interest in developing a larger controlled study and in publishing our procedure.

Accumulation of physical activity during daily living is a current public health target that is influenced by the layout of the built environment. This study reports how the layout of the environment may influence responsiveness to an intervention. Pedestrian choices (n = 41 717) between stairs and the adjacent escalators were monitored for seven weeks in a train station (Birmingham, UK). After a 3.5 week baseline period, a stair riser banner intervention to increase stair climbing was installed on two staircases adjacent to escalators and monitoring continued for a further 3.5 weeks. Logistic regression analyses revealed that the visibility of the intervention, defined as the area of visibility in the horizontal plane opposite to the direction of travel (termed the isovist) had a major effect on success of the intervention. Only the largest isovist produced an increase in stair climbing (isovist=77.6 m2, OR = 1.10, CIs 1.02-1.19; isovist=40.7 m2, OR = 0.98, CIs 0.91-1.06; isovist=53.2 m2, OR = 1.00, CIs 0.95-1.06). Additionally, stair climbing was more common during the morning rush hour (OR = 1.56, CIs 1.80-2.59) and at higher levels of pedestrian traffic volume (OR = 1.92, CIs 1.68-2.21). The layout of the intervention site can influence responsiveness to point-of-choice interventions. Changes to the design of train stations may maximize the choice of the stairs at the expense of the escalator by pedestrians leaving the station.

Tutkimus kuvaa tarinallisuuden kautta organisaation sisäistä muutosta organisaation toimiessa muuttuvassa toimintaympäristössä. Teoriaosan keskeisinä käsitteinä ovat sosiaalinen identiteetti organisaatiossa sekä muutosjohtaminen. Tutkimus selvittää, miten jatkuva organisatorinen ja toimintaympäristöllinen muutos vaikuttivat organisaation muutoskykyyn ja sen jäseniin, minkälaista muutosjohtamista organisaatio muutostilanteessa tarvitsee ja millaisia vaikutuksia muutoksilla oli organisaation sisällä esiintyviin sosiaalisiin identiteetteihin. Organisaatiomuutos ilmeni organisaation työntekijöissä erilaisina reaktioina: osa menetti uskonsa organisaatioon, toiset turhautuivat ja kaipasivat vahvistusta toiminnalleen ja kolmansilla työn sujuminen nousi keskiöön. Muutospaineiden keskellä organisaatiossa tarvitaan uskottavaa muutosviestintää ja johtavaa identiteettiä, johon kaikki organisaation jäsenet voivat samaistua. Tämän sosiaalisen identiteetin synnyn mahdollistaa organisaation johdon aktiivinen muutosprosessin johtaminen. Myös keskijohdon rooli johdon muutosviestin vahvistamisessa on tärkeä muutosprosessin onnistumiselle ja sisäistämiselle. Muutoksen johtamisessa on oleellista ottaa ihmisten johtaminen osaksi päivittäistä toimintaa. Organisaation jäsenten tunnepohjaisen sitouttamisen kautta saavutetaan mahdollisuus toteuttaa onnistunut muutosprosessi. Keskijohdon tehtävänä on viestiä ylimmän johdon visio työntekijätasolle uskottavasti, jotta muutokseen sitoudutaan. Muutosprosessin onnistumista tulee seurata ja tukea organisaation johdon taholta.

Formal software development processes and well-defined development methodologies are nowadays seen as the definite way to produce high-quality software within time-limits and budgets. The variety of such high-level methodologies is huge ranging from rigorous process frameworks like CMMI and RUP to more lightweight agile methodologies. The need for managing this variety and the fact that practically every software development organization has its own unique set of development processes and methods have created a profession of software process engineers. Different kinds of informal and formal software process modeling languages are essential tools for process engineers. These are used to define processes in a way which allows easy management of processes, for example process dissemination, process tailoring and process enactment. The process modeling languages are usually used as a tool for process engineering where the main focus is on the processes themselves. This dissertation has a different emphasis. The dissertation analyses modern software development process modeling from the software developers’ point of view. The goal of the dissertation is to investigate whether the software process modeling and the software process models aid software developers in their day-to-day work and what are the main mechanisms for this. The focus of the work is on the Software Process Engineering Metamodel (SPEM) framework which is currently one of the most influential process modeling notations in software engineering. The research theme is elaborated through six scientific articles which represent the dissertation research done with process modeling during an approximately five year period. The research follows the classical engineering research discipline where the current situation is analyzed, a potentially better solution is developed and finally its implications are analyzed. The research applies a variety of different research techniques ranging from literature surveys to qualitative studies done amongst software practitioners. The key finding of the dissertation is that software process modeling notations and techniques are usually developed in process engineering terms. As a consequence the connection between the process models and actual development work is loose. In addition, the modeling standards like SPEM are partially incomplete when it comes to pragmatic process modeling needs, like light-weight modeling and combining pre-defined process components. This leads to a situation, where the full potential of process modeling techniques for aiding the daily development activities can not be achieved. Despite these difficulties the dissertation shows that it is possible to use modeling standards like SPEM to aid software developers in their work. The dissertation presents a light-weight modeling technique, which software development teams can use to quickly analyze their work practices in a more objective manner. The dissertation also shows how process modeling can be used to more easily compare different software development situations and to analyze their differences in a systematic way. Models also help to share this knowledge with others. A qualitative study done amongst Finnish software practitioners verifies the conclusions of other studies in the dissertation. Although processes and development methodologies are seen as an essential part of software development, the process modeling techniques are rarely used during the daily development work. However, the potential of these techniques intrigues the practitioners. As a conclusion the dissertation shows that process modeling techniques, most commonly used as tools for process engineers, can also be used as tools for organizing the daily software development work. This work presents theoretical solutions for bringing the process modeling closer to the ground-level software development activities. These theories are proven feasible by presenting several case studies where the modeling techniques are used e.g. to find differences in the work methods of the members of a software team and to share the process knowledge to a wider audience.
