INTRODUÇÃO: O Programa HIPERDIA foi implantado em 2001 na atenção básica devido à gravidade epidemiológica da Hipertensão Arterial Sistêmica (HAS) e o Diabetes Mellitus (DM), afecções que podem acarretar sérias complicações incutindo limitações e sofrimento na vida de seus portadores e suas famílias. Nesse contexto, ressalte-se a importância da prevenção primária dessas afecções e de suas complicações. Passado mais de uma década de sua implementação, cabe refletir acerca do impacto nas condições de saúde que o Programa tem gerado entre seus usuários. OBJETIVOS: Elucidar os princípios, a filosofia e a política norteadora do Programa HIPERDIA do Ministério da Saúde (MS); descrever como funciona o atendimento do usuário pela equipe de saúde de um Programa HIPERDIA; explorar os comportamentos de vida e saúde demonstrados pelos usuários do HIPERDIA após sua inserção no Programa. DESCRIÇÃO METODOLÓGICA: Trata-se de um Estudo de Caso segundo Yin (2010) cujo objeto de estudo foi um Programa HIPERDIA executado numa Unidade Básica de Saúde da periferia de Belém-PA. Os dados foram obtidos por meio de entrevista com usuários, equipe e gestores do Programa, além da observação direta do campo, consulta de prontuários e da documentação oficial do MS. A análise dos dados obtidos foi feita por meio da estratégia analítica “Contando com Proposições Teóricas” e da técnica analítica “Combinação de Padrão”. RESULTADOS: A precarização da gestão do Programa HIPERDIA ilustrada por deficiências na infraestrutura, insumos, medicamentos e fragilidades na rede de referência e contrarreferência, a demanda espontânea excessiva que sobrecarrega a equipe de saúde alocada em número insuficiente e o contexto de pobreza e violência urbana em que os usuários vivem contribuem para a baixa adesão ao regime terapêutico e limitam a equipe em sua atuação interdisciplinar e integral. Ademais, o modelo de atenção vigente é pautado no tradicional, baseado em consultas e prescrições e que não tem se demonstrado suficiente para atender integralmente às necessidades de atenção ao portador de condição crônica, que requer cuidados prolongados com adoção de estilos de vida mais condizentes e saudáveis. CONSIDERAÇÕES FINAIS: Grandes desafios se desenham diante dos dados emergidos deste estudo e envolvem ações macrogovernamentais e intersetoriais que promovam melhorias nas condições de vida da população o que requer vontade política para investimento dos recursos necessários. Não obstante a isso, a enfermagem pode dar sua contribuição promovendo cuidado cultural e ações de autocuidado, personalizando o plano de ação e trazendo a corresponsabilidade do usuário e sua família na melhoria de sua condição de saúde.

A exploração de maciços rochosos com uso de explosivos provoca inevitáveis impactos ambientais e desconforto para as populações do entorno, as quais estão expostas cotidianamente aos seus efeitos. Os principais efeitos ambientais se fazem sentir através do ultralançamento de fragmentos, da geração de vibrações no terreno, de sobrepressão atmosférica, da emissão de materiais particulados (poeira) na atmosfera, do aumento dos níveis de ruído, do assoreamento de áreas e/ou de drenagens adjacentes às minerações, além da alteração visual e paisagística. Esses problemas podem ser reduzidos a um nível aceitável pela comunidade afetada, se a lavra for executada de modo a preservar as condições de saúde, segurança e bem estar da população afetada, adotando-se técnicas modernas de extração e beneficiamento, e pelo monitoramento continuado dos parâmetros ambientais envolvidos, mantendo-os, no mínimo, dentro dos limites estabelecidos pelos organismos governamentais fiscalizadores.

The increase in elderly population is a worldwide phenomenon and has different consequences not only in terms of health, but also in economic terms. One of those consequences is the institutionalization. The project, running in two elderly care institutions (hereafter IRLPi) located in Marília – SP aims to: a) Train multidisciplinary team to work in IRLPi’s; b) Develop and promote activities that improve life quality focusing on the preservation of mental health; c) study general and mental health conditions of patients under elderly care institution programs. The project was implemented in three phases: a) data collection and elaboration of plan of activities; b) Application of activities; and c) assessment/ analysis of results. Regarding Phase 1, data were collected from 3 forms, through which it was possible to verify: Institutional Data; Social Data and General Health. The data related to health were collected from the medication of continuous use. All data were launched into the database (Access) and subjected to the descriptive statistical analysis. Data related to mental health were confirmed from the application of clinical trials (Geriatric Depression Scale - EGD, and MMSE-T).

To understand the elderly's perception of their current condition. Methodology. Study undertaken in 2012 using the qualitative method of Minayo and the thematic analysis according to Bardin's suggestions. Data were collected through semi-structured interviews that took place in the homes of the elderly people. The guiding question was: At this point in your life, how do you feel? Tell me. Results. The elderly who were satisfied stated that this was due to the good relationship with their family, spouse, to the fact of having autonomy and respect from the society. Those who were shown to be dissatisfied reported lack of family support, physical limitations imposed by age and the presence of illnesses as the main causes. Conclusion. The adult population requires the use of care technologies that cover all the stages of life, including old age. Nursing professionals should be prepared for the increasing care demand of these people.

Epidemiological data on the conditions of oral health are essential for professionals in the health care plan, execute and evaluate dental programs. In Brazil, most oral health programs directs its resources to middle schools, leaving the preschool children in a secondary plan. The objective of this study was to identify the prevalence of dental caries in preschool children from 4-6 years of age from schools of child education (EMEIs) of the city of Araçatuba-SP. The study was conducted on a sample of 275 children of both genders, representative of the population of preschool children in the city of Araçatuba-SP. In order to assess the prevalence and severity of dental caries indices we used the dmft and DMFT indexes, according to the codes and standards recommended by the World Health Organization (WHO).The average dmft of 1.88 was found, comprising 78% of decayed teeth, 21% of restaured teeth and 1% with indicated extraction teeth. Regarding the permanent dentition, the average DMFT was 0.08, consisting of 71% of decayed teeth, 29% restored and 0% of missing teeth. Based on results found, there was a high caries prevalence in deciduous and permanent teeth, demonstrating the need for more dental care to the group studied.

Introduction: To correctly attend the health needs of the elderly population, it is necessary to invest in training of human resources. Thus, it becomes indispensable to insert the geriatric dentistry content into graduation courses. Objective: To describe the experiences and the results obtained during 10 years of an university extension project focused on oral health care of institutionalized elderly. Methodology: This is a documental and descriptive work, which was analyzed the institutional process of ten years of the University Extension Program, “Always Smiling - Dental Care for the Elderly Institutionalized of Araçatuba, “ developed at the Araçatuba Dental School, FOA/ UNESP. It were collected quantitative and qualitative data, and these were classified and interpreted according to the technique of categorizing the content. Results: In ten years of implementation, the project “Always Smiling” involved undergraduate students, professors and post-graduate students, who together have developed practical activities such as health education, hygiene supervision, recreational and educational activities, epidemiological surveys of oral health conditions and consequently, dental care according to the need of the treatment. Conclusion: The project “Always Smiling” serves as a working model for elderly health care, because it acts involves many aspects, from the prevention of diseases, even to the training of elderly caregivers, benefiting the institutionalized elderly and health professionals.

Enteric organisms, pseudomonads and other opportunistic microorganisms in the oral microbiota have been linked to serious infections in patients hospitalized in intensive care units (ICU). The present study evaluated the presence of family Enterobacteriaceae, Pseudomonas aeruginosa, and Acinetobacter baumannii in the mouth of patients in ICU, correlating it with oral and systemic conditions. Data on health, socioeconomic status, medication use, drug addiction, medical and family histories of patients held for more than 72 hours in the ICU with a diagnosis of severe infection or that developed this condition after entry in said unit were obtained. Fifty patients provided clinical samples of supragingival and subgingival biofilms, saliva and oral mucous membranes were collected, as well as respiratory secretions from patients with pneumonia, blood and urine for sepsis. The presence of target microorganisms was carried out by polymerase chain reaction (PCR) and by culture using selective media. The Chi-square and Mann-Whitney tests were used for statistical analysis, and the significance level was 5%. The intraoral clinical conditions of the patients were poor. The family Enterobacteriaceae was the most prevalent, affecting 39.5% of the supragingival biofilm samples of patients attended in ICU and 18.6% of patients in the control group, besides the rods were the only group found in extraoral samples.

Drug addiction is one of the biggest public health problems worldwide, not only by the dimensions of the problem, but also by the severity of the damage, creating favorable conditions for opportunistic microorganisms such as class Mollicutes. This study aims to evaluate the presence of the main species and genera of this group in the subgingival biofilm of drug addiction patients, comparing them with non-dependent subjects. For this purpose, data on systemic health conditions, socioeconomic characteristics, drug addiction from 72 patients with drug addiction kept in rehab clinics and 100 non-dependent patients who formed the control group were obtained. Intra and extraoral clinical examinations were performed and samples of subgingival plaque were collected through sterile absorbent paper cones. The presence of different genera and species of the class Mollicutes was evaluated by PCR using the specific primers and conditions for each microorganism. The statistical analysis was performed using the Chi-square test for comparisons of three or more variables and the Mann-Whitney test, with significance level of 5%. Out of species and genera evaluated, Mycoplasma salivarium showed correlation with gingival inflammation in both patient groups and was more frequently detected among drug addiction patients

The relationship between the occurrence of enterococci in the oral microbiota and serious infections in patients hospitalized in intensive care units (ICU) has been established. This study evaluated the presence of Enterococcus faecalis and other species of this genus in the mouths of patients on ICU, correlating it with oral and systemic conditions. Data on health and socioeconomic, medication use, medical and family history of patients maintained for 72 hours in the ICU, diagnosed with severe infection or who have developed this condition after the entry to the unit were obtained. Fifty patients provided intraoral and extraoral clinical samples for analysis (above and subgingival biofilm, saliva and buccal mucosa, followed by obtaining samples of respiratory secretions for patients with pneumonia, and blood and urine for sepsis). The presence of target microorganisms was performed by polymerase chain reaction (PCR) and culture using selective media. The chi-square and Mann-Whitney tests were used for statistical analysis, and the significance level was 5%. The intraoral clinical conditions of the patients showed poor. E. faecalis was significantly more frequent microorganism, followed by E. faecium. The use of broadspectrum antimicrobial action was associated with the presence of these opportunistic microorganisms. These bacteria were more frequent in patients with periodontitis or gingivitis. The results showed that enterococci associated with serious infectious processes may originate from resident microbiota of patients and its prevalence is not elevated in healthy individuals.

Drug addiction won dramatic aspects in terms of its dimensions and the effects that it imposes. These chemical agents are able to reduce the immune reactivity and tissue repair, and enhance microbial aggression, aggravating the destruction of the periodontium and other side effects. This study aimed to evaluate the presence of key periodontal pathogens in the mouth of drug addiction patients, comparing it with individuals who do not exhibit this dependence, as well as assess the influence of oral conditions on the occurrence of such microorganisms. For this purpose, data on systemic health conditions, socioeconomic, patterns of licit or illicit drug consumption of 100 patients with chemical dependency kept in rehabilitation clinics and an equal number of non-dependent patients, who formed the control group were obtained. Intra and extraoral clinical examinations were performed and samples of supragingival and subgingival biofilm, saliva and mucous membranes were collected. The presence of the targeted microorganism was assessed by polymerase chain reaction (PCR). It was found that Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola showed close correlation with bone loss and gingival bleeding in drug addiction dependents and control group, but the oral mucous membranes and saliva of addicts showed higher occurrence of these pathogens.

Objective. To identify reasons why women look for early health care attention at the end of pregnancy. Methodology. Qualitative study based in socio-existential phenomenology proposed by Alfred Schütz. Nine pregnant women who consulted to the maternity service without being in real labor were interviewed. Results. The following analysis categories emerged: Having different symptoms or signals from the ones they had felt or been informed about, not being able to identify real labor, fear to fetal complications for being close to the probable labor date, feeling calmer when being seen by professionals of the institution, lack of problem solving by other services of the health institution, fearing out of hospital delivery, imitating real labor, looking for tranquility in the idealization of labor and modifying behaviors to guarantee attention. Conclusion. Women are insecure and fear because of a fragmented health care, where they do not perceive themselves linked to the prenatal control program anymore, and at the same time they don’t find the health care assistance they want.
