The factors that influence decision making in severe aortic stenosis (AS) are unknown. Our aim was to assess, in patients with severe AS, the determinants of management and prognosis in a multicenter registry that enrolled all consecutive adults with severe AS during a 1-month period. One-year follow-up was obtained in all patients and included vital status and aortic valve intervention (aortic valve replacement [AVR] and transcatheter aortic valve implantation [TAVI]). A total of 726 patients were included, mean age was 77.3 ± 10.6 years, and 377 were women (51.8%). The most common management was conservative therapy in 468 (64.5%) followed by AVR in 199 (27.4%) and TAVI in 59 (8.1%). The strongest association with aortic valve intervention was patient management in a tertiary hospital with cardiac surgery (odds ratio 2.7, 95% confidence interval 1.8 to 4.1, p <0.001). The 2 main reasons to choose conservative management were the absence of significant symptoms (136% to 29.1%) and the presence of co-morbidity (128% to 27.4%). During 1-year follow-up, 132 patients died (18.2%). The main causes of death were heart failure (60% to 45.5%) and noncardiac diseases (46% to 34.9%). One-year survival for patients treated conservatively, with TAVI, and with AVR was 76.3%, 94.9%, and 92.5%, respectively, p <0.001. One-year survival of patients treated conservatively in the absence of significant symptoms was 97.1%. In conclusion, most patients with severe AS are treated conservatively. The outcome in asymptomatic patients managed conservatively was acceptable. Management in tertiary hospitals is associated with valve intervention. One-year survival was similar with both interventional strategies.

El trabajo de grado se estructuró en tres capítulos. El primero constituye un marco teórico sobre el cual se fundamentan los siguientes, pues se realiza un análisis de la cláusula penal en el derecho privado, incluidas las prácticas comerciales internacionales. El segundo capítulo aborda el tema de las multas en la contratación estatal, para lo cual se estudió la normatividad y jurisprudencia pertinente; en esa parte del escrito se incluyó el apartado correspondiente al derecho comparado. Finalmente, en el tercer capítulo, se formula una propuesta que permite hacer de esta medida coercitiva provisional una herramienta idónea para lograr el correcto y oportuno cumplimiento del contrato.

La falta disciplinaria será aquella conducta típica, antijurídica y culpable, pues eso se desprende de los principios rectores del derecho disciplinario, los cuales tienen una consagración normativa en el ordenamiento jurídico colombiano, más específicamente en la Ley 734 de 2002, actual Código Disciplinario Único. La Ilicitud Sustancial es el elemento que delimita la antijuridicidad de la responsabilidad disciplinaria, y se debe decir que el alcance de dicho concepto no ha sido definido de manera precisa por parte de la ley, ni de manera unánime por parte de la doctrina, ni muchos menos por parte de la jurisprudencia. Luego de exponer las reflexiones hechas respecto al alcance de la ilicitud sustancial, se argumentará el por qué se debe entender el alcance de ese concepto bajo la siguiente hipótesis: La ilicitud sustancial debe entenderse en términos de antijuridicidad material, lo cual apunta a que la falta, además de ser típica y culpable, debe vulnerar realmente la Función Pública como bien jurídico a proteger por el Derecho Disciplinario, o ponerla en peligro manifiesto, pues ese tipo de derecho sancionador debe ser estrictamente limitado, ya que sus consecuencias son muy gravosas para los derechos de la personas destinatarias de la Ley Disciplinaria.

Tras conocer cuatro años de debate (dentro y fuera de La Habana), los borradores de dos puntos de la agenda y presenciar el crecimiento de bandas criminales y/u organizaciones sucesoras del paramilitarismo, de la mano de la expansión de economías ilegales y una polarización creciente (y preocupante por sus características) en torno a la paz y el conflicto social/armado en el país, el presente trabajo pretende analizar a profundidad el carácter y alcance de los discursos emitidos por los negociadores en La Habana.

Este artigo tem como objetivo investigar o universo poético e imaginário do poeta brasileiro João Manuel Simões. A pesquisa apresenta imagens em que se constata a relação entre poesia e imaginário, tais como o sonho, a inspiração, a imaginação, a memória e a infância, de tal forma que as construções do imaginário tornam-se mais evidentes no plano das construções poéticas.

Sem abandonar a magia da poesia e o compromisso com a linguagem poética, Simões aborda temas que configuram a modernidade lírica como a condição original do ser humano, a arte poética e a sua própria condição de poeta, convertendo o silêncio em palavra e poesia.

Is there a role for prototyping (sketching, pattern making and sampling) in addressing real world problems of sustainability (People, Profit, and Planet), in this case social/healthcare issues, through fashion and textiles research? Skin cancer and related illnesses are a major cause of disfigurement and death in New Zealand and Australia where the rates of Melanoma, a serious form of skin cancer, are four times higher than in the Northern Hemisphere regions of USA, UK and Canada (IARC, 1992). In 2007, AUT University (Auckland University of Technology) Fashion Department and the Health Promotion Department of Cancer Society - Auckland Division (CSA) developed a prototype hat aimed at exploring a barrier type solution to prevent facial and neck skin damage. This is a paradigm shift from the usual medical research model. This paper provides an overview of the project and examines how a fashion prototype has been used to communicate emergent social, environmental, personal, physiological and technological concerns to the trans-disciplinary research team. The authors consider how the design of a product can enhance and support sustainable design practice while contributing a potential solution to an ongoing health issue. Analysis of this case study provides an insight into prototyping in fashion and textiles design, user engagement and the importance of requirements analysis in relation to sustainable development. The analysis and a successful outcome of the final prototype have provided a gateway to future collaborative research and product development.

Background No study relating the changes obtained in the architecture of erector spinae (ES) muscle were registered with ultrasound and different intensities of muscle contraction recorded by surface EMG (electromyography) on the ES muscle was found. The aim of this study was analyse the relationship in the response of the ES muscle during isometric moderate and light lumbar isometric extension considering architecture and functional muscle variables. Methods Cross-sectional study. 46 subjects (52% men) with a group mean age of 30.4 (±7.78). The participants developed isometric lumbar extension while performing moderate and low isometric trunk and hip extension in a sitting position with hips flexed 90 degrees and the lumbar spine in neutral position. During these measurements, electromyography recordings and ultrasound images were taken bilaterally. Bilaterally pennation angle, muscle thickness, torque and muscle activation were measured. This study was developed at the human movement analysis laboratory of the Health Science Faculty of the University of Malaga (Spain). Results Strong and moderate correlations were found at moderate and low intensities contraction between the variable of the same intensity, with correlation values ranging from 0.726 (Torque Moderate – EMG Left Moderate) to 0.923 (Angle Left Light – Angle Right Light) (p < 0.001). This correlation is observed between the variables that describe the same intensity of contraction, showing a poor correlation between variables of different intensities. Conclusion There is a strong relationship between architecture and function variables of ES muscle when describe an isometric lumbar extension at light or moderate intensity.
