Fatores de risco e formas de transmissão do Vírus da Imunodeficiência humana (HIV) e o Vírus da hepatite B (VHB) são freqüentemente os mesmos, o qual explica a alta freqüência de co-infecção envolvendo esses dois agentes. O objetivo desta pesquisa foi estimar a prevalência da infecção pelo VHB, analisar possíveis fatores de risco bem como examinar a associação entre a contagem de linfócitos T CD4+, CD8+ e carga viral plasmática em 129 portadores do HIV. Cada participante foi submetido a um questionário específico e tinha uma amostra de sangue testada para os marcadores sorológicos HBsAg, anti-HBc total, anti-HBs, contagem de linfócitos T CD4+, CD8+ e carga viral plasmática. A prevalência total de marcadores para o VHB foi de 46,5%, com 3,1% de soropositividade para o HBsAg, 27,1% para o anti-HBc total e 36,4% para o anti-HBs. Após ajuste por regressão logística, os marcadores sorológicos para hepatite B foram associados com as seguintes variáveis: sexo, preferência sexual e escolaridade. A freqüência de marcadores para hepatite B foi de 39,1% em homens e 16,6% em mulheres. A prevalência de marcadores para hepatite B foi 14,4% em homo/bissexuais e 15,4% em heterossexuais. A menor freqüência de hepatite B (1%) foi encontrada em pessoas com nível superior de escolaridade. Não foram observadas diferenças estatisticamente significante entre contagem de linfócitos CD4+, CD8+ e carga viral plasmática em pacientes co-infectados com VHB comparados com aqueles pacientes infectados somente com HIV.

No presente estudo a soroprevalência da infecção pelo Vírus da hepatite C (VHC) foi investigada em indivíduos portadores da infecção pelo Vírus da imunodeficiência humana 1 (HIV-1) da cidade de Macapá, Estado do Amapá, Brasil. Um total de 120 indivíduos infectados pelo HIV-1 foi testado para a presença de anti-VHC usando um ensaio imunoabsorvente ligado a enzima. Todos os pacientes envolvidos no presente estudo foram testados para a carga viral plasmática do HIV-1 e para os níveis de células T CD4+ no momento do consentimento em fazer parte do estudo. Os pacientes responderam a um questionário epidemiológico no momento da coleta de sangue. Do total de 120 amostras testadas apenas sete (5,83%) foram soropositivas para anti-VHC. Quanto ao gênero, quatro (57,2%) e três (42,8%) eram homens e mulheres, respectivamente. A idade dos indivíduos soropositivos para anti-VHC variou de 21 a 40 anos entre os homens e de 21 a 60 entre as mulheres. Considerando o impacto da coinfecção nos valores de carga viral plasmática do HIV-1 e na contágem de células T CD4+ não foram observadas diferenças significativas (Odds Ratio=2,7368, p=0,3624; Odds Ratio=1,7803, p=0,7764). O uso de drogas injetáveis e de piercing mostrou ser um importante fator de risco para a co-infecção (p<0,05). Os resultados do presente estudo representam os primeiros achados acerca da co-infecção HIV-1/VHC no Estado do Amapá e demonstra a necessidade de um estudo continuado objetivando avaliar a soroprevalência da infecção pelo VHC no Amapá e detectar o real impacto clinico da co-infecção HIV-1/VHC no paciente.

No presente estudo foram investigadas as freqüências das mutações no éxon 1 do gene MBL em um grupo de 128 pacientes com Aids, 116 portadores assintomáticos da infecção pelo HIV-1, 84 mulheres soronegativas profissionais do sexo, com comportamentos de alto risco e 99 indivíduos controles soronegativos, com o objetivo de avaliar a ocorrência de uma possível associação entre os polimorfismos neste gene e a infecção pelo HIV-1. A identificação dos alelos MBL *A, *B, *C e *D foi realizada por meio da reação em cadeia mediada pela polimerase, utilizando sequências de iniciadores específicos e posterior digestão enzimática (RFLP). As análises das frequências alélicas e genotípicas do éxon 1 não mostraram qualquer diferença significativa entre pacientes soropositivos (assintomáticos e Aids) e soronegativos (controle e controle de alto risco) (p>0,05). Não foram observadas associações significativas entre a presença de co-infecções e as variantes alélicas. Entretanto, tuberculose, neurotoxoplasmose, candidíase, neurocriptococose e pneumonia foram as co-infecções com maior prevalência. As associações entre o número de linfócitos TCD4+, a carga viral plasmática e os polimorfismos no éxon 1 do gene MBL nos pacientes com Aids e portadores assintomáticos não foram estatisticamente significante. Desse modo, pode-se sugerir a ausência de associação entre estes polimorfismos e a susceptibilidade à infecção pelo HIV-1, destacando a necessidade de estudos adicionais para determinar se estes polimorfismos apresentam qualquer impacto associado à infecção ou a progressão para a Aids.

A coinfecção do vírus da hepatite C (HCV) em pessoas portadoras do vírus da imunodeficiência humana (HIV) é freqüentemente observada em virtude destes vírus apresentarem similaridade em suas rotas de transmissão, principalmente no que se refere à via parenteral. No Brasil, a prevalência depende da área geográfica considerada, variando de 8,9% a 54%. Nos coinfectados, a progressão da doença pelo HCV é usualmente mais agressiva e apresenta alto nível de viremia, como também, há um risco maior de associação do HCV com a cirrose hepática e/ou hepatocarcinoma. O objetivo do presente estudo foi estimar a prevalência de HCV e fatores de risco associados à coinfecção em pessoas soropositivas para HIV na cidade de Imperatriz Maranhão. Participaram 249 pacientes soropositivos para HIV atendidos no SAE do Programa Municipal de DST/AIDS de Imperatriz do Maranhão. Foi coletado de cada voluntário 10 mL de sangue periférico para realização do teste sorológico, onde foi realizada pesquisa de anticorpos IgG HCV específicos e testes de Biologia Molecular (RT-PCR) para pesquisa do RNA viral e genotipagem. Entre os pacientes observou-se similaridade entre a frequência dos gêneros, 49% masculino e 51% feminino, com média de idade de 40 anos. Foi observado que 98% possuem baixo nível de instrução e 63% possuem renda mensal de até um salário mínimo. A soroprevalência do anti-HCV foi de 2.4% (6/249). Na comparação dos fatores de risco pesquisados entre os pacientes reagentes e não reagentes na pesquisa sorológica de anticorpos HCV específicas demonstraram que a presença de tatuagens e piercing foi o único fator que se mostrou significantes, sendo mais frequente nos reagentes. Esse foi o primeiro estudo que investiga a coinfecção HIV e HCV na cidade de Imperatriz, Maranhão e a identificação de pacientes coinfectados foi de fundamental importância para o serviço que a partir de então irá realizar o acompanhamento destes pacientes.

A co-infecção Leishmania-HIV-Aids é um sério problema de saúde pública em quase todo o mundo. No entanto, os casos de co-infecção ainda são subestimados, uma vez que, a leishmaniose não se constitui doença definidora de Aids. Foi realizado um estudo descritivo transversal de Dezembro de 2011 a Fevereiro de 2012, com o objetivo de investigar a prevalência da co-infecção HIV/Leishmania em pacientes atendidos pelo programa municipal de DST/aids no Centro de Testagem e Aconselhamento (CTA) de Imperatriz-MA. A população de estudo foi constituída por 199 indivíduos. A coleta de dados foi feita por meio de um questionário para a obtenção de dados demográficos, socioeconômicos e epidemiológicos, bem como foi realizado exame de coleta de material biológico (sangue) de todos os pacientes para detecção da infecção por Leishmania sp., por meio de exames laboratoriais (contagem de CD4 e CD8) e pesquisa da PCR. Entre os pacientes observou-se similaridade entre a frequência dos gêneros, 49,2% masculino e 50,8% feminino, com média de idade de 40 anos. Foi observado que 61,8% possuem baixo nível de instrução e 69,3% possuem renda mensal de até um salário mínimo. 2,01% (4/199) dos pacientes analisados apresentaram co-infecção Leishmania/HIV. Sendo, destes, 3 que apresentaram infecção mista por Leishmania (V.) sp e Leishmania (L.) amazonensis, causadores de LTA e um paciente infectado por Leishmania (L.) chagasi, causador de LV. Na comparação dos fatores de risco, comorbidades e complicações entre os pacientes analisados observou-se que a malária foi o único fator que se mostrou significante em torno de 10,05%. Esse foi o primeiro estudo que investiga a coinfecção HIV Leishmaniana cidade de Imperatriz, Maranhão e a identificação de pacientes coinfectados foi de fundamental importância para o serviço que a partir de então poderá realizar o acompanhamento destes pacientes. Este estudo permitiu conhecer a magnitude da prevalência da co-infecção Leishmania/HIV. Assim, sugerimos que o teste anti-Leishmaniaseja realizado em todos os indivíduos com HIV/Aids, e que sejam incrementadas políticas públicas voltadas para essa problemática.

Objective To evaluate the prevalence and risk factors of non-alcoholic fatty liver disease (NAFLD) in postmenopausal women.Methods A cross-sectional study was carried involving 188 women (age >= 45 years and amenorrhea >= 12 months) attending the outpatient unit in south-eastern Brazil. Exclusion criteria were liver disease (hepatitis B and C, cholestatic disease, liver insufficiency), use of drugs that affect liver metabolism; alcoholics; AIDS or cancer history; and morbid obesity. NAFLD was diagnosed by abdominal ultrasound. Clinical, anthropometric (body mass index, waist circumference) and biochemical variables were measured.Results Of the 188 women, 73 (38.8%) had NAFLD. Blood pressure, waist circumference, body mass index, LDL cholesterol, triglycerides and glucose were significantly higher in NAFLD patients when compared with women without NAFLD (control group) (p < 0.05). HOMA-IR values indicated insulin resistance only in the NAFLD group (6.1 +/- 4.6 vs. 2.4 +/- 1.4 in control group, p < 0.05). Metabolic syndrome was detected in 93.1% of the women affected by NAFLD, and 46.1% of the control group (p < 0.05). In multivariate analysis, adjusted for age and weight, the variables considered at risk for the development of NAFLD, were: high waist circumference (odds ratio (OR) 1.07, 95% confidence interval (CI) 1.01-1.13), insulin resistance (OR 3.81, 95% CI 2.01-7.13), and presence of metabolic syndrome (OR 8.68, 95% CI 3.3-24.1).Conclusion NAFLD showed a high prevalence among postmenopausal women. The presence of metabolic syndrome, abdominal obesity and IR were indicators of risk for the development of NAFLD.

Infection with human papilloma virus (HPV) is the most common sexually transmitted disease in the world. Among the 630 million new cases of HPV that occur each year, 30 million develop anogenital warts. Although subclinical infection with HPV is the most common cause, genital warts are also associated with immunosuppression caused by HIV. In view of the high prevalence of HPV/HIV co-infection particularly among men who have sex with men, the objectives of this study were to determine the prevalence of anogenital warts in men with HIV/AIDS and to identify associated factors. A cross-sectional study was conducted on 159 men with HIV/AIDS consecutively selected at a referral service in Botucatu, São Paulo, Brazil, in which the association between sociodemographic, behavioral and clinical variables and the presence of anogenital warts was evaluated. After hierarchical analysis of the data, variables presenting a p value ≤ 0.2 were entered into an unconditional multivariate logistic regression model. Forty-nine (31%) of the HIV-positive patients had anogenital warts. The mean age was 44.6 ± 9.6 years. The main factors associated with the presence of anogenital warts were irregular antiretroviral treatment and genital herpes(HSV). The present study demonstrate that anogenital warts occur in almost one-third of the male population infected with HIV and factors associated with a higher risk of being diagnosed with anogenital warts were irregular cART use and co-infection with HSV, other variables could not be associated.

Verify factors that influence the oral health status of people living with HIV/AIDS (PLWHA) in Brazil. The study was cross-sectional and includes 177 HIV-positive individuals, who answered questionnaire on the sociodemographic conditions, HIV aspects, habits, and satisfaction with the service. The oral health data were collected by means of the decayed, missing, and filled teeth (DMFT) index, use and need of dentures, and the Community Periodontal Index. Average number of the DMFT was 17.64. Most HIV-positive patients presented good periodontal status, 35.0% used dentures, 41.5% needed denture in the maxilla, and 62.0% in the mandible. In the multivariate analysis, older age and dissatisfaction with health care were associated with nonuse of dentures. The abandonment of the use of antiretroviral therapy increased the risk of PLWHA presenting more than three decayed teeth. Poor oral health of the PLWHA was mainly influenced by sociodemographic factors and use and satisfaction with service.

This Prevention Center Paper (No. 22) describes the HIV/AIDS related knowledge, attitudes and practices of a random sample of 1240 Nebraska adolescents in grades 9-12. The data were gathered in 1989. Data were gathered by staff of Health Education, Inc., a Nebraska-based nonprofit research and development corporation, as part of a contract with the Nebraska Department of Education. The Nebraska Department of Education has a major HIV /AIDS cooperative agreement with the U.S. Centers for Disease Control (CDC) in Atlanta, Georgia. Schools were selected at random from each of the six classifications of Nebraska schools established by the Nebraska Department of Education. Two to three classrooms for each grade 9-12 were then randomly selected within each sampled school. All students in the classes on the day of the survey voluntarily completed CDC's HIV / AIDS adolescent survey. All responses were anonymous. Classroom teachers and school administrators 'were not involved in the data collection in any way. A data collection protocol was followed to ensure validity in this self-report survey. This report is divided into four parts: Part 1 deals with students' acceptance of HIV/AIDS instruction and of people with HIV / AIDS. Part 2 describes students' access to HIV / AIDS information: Part 3 is about students ' knowledge of HIV / AIDS, and Part 4 discusses Nebraska adolescents' practices that increase the risk of HIV/AIDS.

Animals present an enormous variety of behavioural defensive mechanisms, which increase their survival, but often at a cost. Several animal taxa reduce their chances of being detected and/or recognized as prey items by freezing (remaining completely motionless) in the presence of a predator. We studied costs and benefits of freezing in immature Eumesosoma roeweri (Opiliones, Sclerosomatidae). Preliminary observations showed that these individuals often freeze in the presence of the syntopic predatory spider Schizocosa ocreata (Araneae, Lycosidae). We verified that harvestmen paired with predators spent more time freezing than when alone or when paired with a conspecific. Then. we determined that predator chemical cues alone did not elicit freezing behaviour. Next, we examined predator behaviour towards moving/non-moving prey and found that spiders attacked moving prey significantly more, suggesting an advantage of freezing in the presence of a predator. Finally, as measure of the foraging costs of freezing, we found that individuals paired with a predator for 2 h gained significantly less weight than individuals paired with a conspecific or left alone. Taken together, our results suggest that freezing may protect E. roeweri harvestmen from predatory attacks by wolf spiders, but at the cost of reduced food and/or water intake. (C) 2009 Elsevier B.V. Monitoring the extent of and trends in multidrug-resistant tuberculosis (MDR-TB) is a priority of the Brazilian National Tuberculosis Control Programme. The current study aimed to estimate the incidence of MDR-TB, describe the profile of TB drug resistance in risk groups and examine whether screening for MDR-TB adhered to the recommended guidelines. A descriptive study that examined diagnosed cases of pulmonary TB was conducted in the city of Santos, Brazil, between 2000-2004. Of the 2,176 pulmonary TB cases studied, 671 (30.8%) met the criteria for drug sensitivity testing and, of these cases, 31.7% (213/671) were tested. Among the tested cases, 9.4% were resistant to one anti-TB drug and 15% were MDR. MDR was observed in 11.6% of 86 new TB cases and 17.3% of 127 previously treated cases. The average annual incidence of MDR-TB was 1.9 per 100,000 inhabitants-years. The extent of known MDR-TB in the city of Santos is high, though likely to be underestimated. Our study therefore indicates an inadequate adherence to the guidelines for MDR-TB screening and suggests the necessity of alternative strategies of MDR-TB surveillance.

Objectives: To describe the epidemiological profile, risk behaviors, and the prior history of sexually transmitted diseases (STDs) in women living with acquired immunodeficiency syndrome (AIDS). Methods: Cross-sectional study, performed at the Centro de Referencia e Treinamento em DST/AIDS of Sao Paulo. The social, demographic, behavioral, and clinical data such as age, schooling, marital status, age at first sexual intercourse, number of sexual partners, parity, use of drugs, time of HIV diagnosis, CD4 count, and viral load determination were abstracted from the medical records of women living with AIDS who had gynecological consultation scheduled in the period from June 2008 to May 2009. Results: Out of 710 women who were scheduled to a gynecological consultation during the period of the study, 598 were included. Previous STD was documented for 364 (60.9%; 95% CI: 56.9%-64.8%) women. The associated factors with previous STDs and their respective risks were: human development index (HDI) <0.50 (ORaj = 5.5; 95% CI: 2.8-11.0); non-white race (ORaj = 5.2; 95% CI: 2.5-11.0); first sexual intercourse at or before 15 years of age (ORaj = 4.4; 95% CI: 2.3-8.3); HIV infection follow-up time of nine years or more (ORaj = 4.2; 95% CI: 2.3-7.8)]; number of sexual partners during the entire life between three and five partners (ORaj = 2.2; 95% CI: 1.1-4.6), and six or more sexual partners (ORaj = 3.9; 95% CI: 1.9-8.0%); being a sex worker (ORaj = 1.9; 95% CI: 1.1-3.1). Conclusions: A high prevalence of a prior history of STDs in the studied population was found. It is essential to find better ways to access HIV infection prevention, so that effective interventions can be more widely implemented. (C) 2012 Elsevier Editora Ltda. All rights reserved.

The purpose of this study is to estimate the survival probability of patients following their first admission for the treatment of AIDS to an infectious disease reference hospital in Belo Horizonte, Brazil, during 2005. Study subjects were monitored during a 12-month period to identify factors associated with survival probability. Late diagnosis was recorded among many of the 250 study subjects: almost half (44.8%) were diagnosed less than 30 days prior to or during their hospitalization. A high mortality rate was also detected: 39.6% of the subjects died during the 12 months of monitoring. The cumulative survival probability of the cohort group was estimated at 68.0% after 3 months and at 61.2% after 12 months. However, certain patient subgroups analyzed had even lower cumulative survival probabilities after 12 months of monitoring: if diagnosed during hospitalization, it was estimated at only 48.0% and those with no record of antiretroviral treatment had a 48.5% cumulative survival probability. Patients with severe anemia had the lowest survival probability, similar among the two lymphocyte count groups (<1000 mm(3) and >= 1000 mm(3)), the former with a 45.5% survival probability and the latter with a 46.7% one. The proportional death risk was 2.5-fold higher for men residing in other area than the capital city of the State of Minas Gerais and greater metropolitan region when compared with women residing there. The findings of this study highlight the importance of early diagnosis for predicting patient survival and reinforce the necessity off acilitating HIV diagnosis.

This study investigates the influence of neighbourhood socioeconomic conditions on women's likelihood of experiencing intimate partner violence (IPV) in Sao Paulo, Brazil. Data from 940 women who were interviewed as part of the WHO multi-country study on women's health and domestic violence against women, and census data for Sao Paulo City, were analyzed using multilevel regression techniques. A neighbourhood socioeconomic-level scale was created, and proxies for the socioeconomic positions of the couple were included. Other individual level variables included factors related to partner's behaviour and women's experiences and attitudes. Women's risk of IPV did not vary across neighbourhoods in Sao Paulo nor was it influenced by her individual socioeconomic characteristics. However, women in the middle range of the socioeconomic scale were significantly more likely to report having experienced violence by a partner. Partner behaviours such as excessive alcohol use, controlling behaviour and multiple sexual partnerships were important predictors of IPV. A women's likelihood of IPV also increased if either her mother had experienced IPV or if she used alcohol excessively. These findings suggest that although the characteristics of people living in deprived neighbourhoods may influence the probability that a woman will experience IPV, higher-order contextual dynamics do not seem to affect this risk. While poverty reduction will improve the lives of individuals in many ways, strategies to reduce IPV should prioritize shifting norms that reinforce certain negative male behaviours. Objective: to identify the different types of morphological alterations from lipodystrophy syndrome (LS) in outpatients and relate them to the therapeutic regimen used. Method: a cross-sectional study which recruited 60 patients with HIV and LS and 79 without LS, who consented to interview and data collection from their medical notes. Results: the region most affected by lipoatrophy was the face; by lipohypertrophy, the abdomen, and by the mixed form, the alterations to the abdomen, face, and upper and lower limbs. Conclusion: among the therapeutic regimens, that comprised of zidovudine, lamivudine and efavirenz seemed to protect against LS. Nursing can act in the early identification of the changes, as well as providing guidance and support for patients affected by the changes in their body image.
