Esta dissertação descreve o desenvolvimento e avaliação de um procedimento de \Numerical Site Calibration" (NSC) para um Parque Eólico, situado a sul de Portugal, usando Dinâmica de Fluídos Computacional (CFD). O NSC encontra-se baseado no \Site Calibration" (SC), sendo este um método de medição padronizado pela Comissão Electrónica Internacional através da norma IEC 61400. Este método tem a finalidade de quantificar e reduzir os efeitos provocados pelo terreno e por possíveis obstáculos, na medição do desempenho energético das turbinas eólicas. Assim, no SC são realizadas medições em dois pontos, no mastro referência e no local da turbina (mastro temporário). No entanto, em Parques Eólicos já construídos, este método não é aplicável visto ser necessária a instalação de um mastro de medição no local da turbina e, por conseguinte, o procedimento adequado para estas circunstâncias é o NSC. O desenvolvimento deste método é feito por um código CFD, desenvolvido por uma equipa de investigação do Instituto Superior de Engenharia do Porto, designado de WINDIETM, usado extensivamente pela empresa Megajoule Inovação, Lda em aplicações de energia eólica em todo mundo. Este código é uma ferramenta para simulação de escoamentos tridimensionais em terrenos complexos. As simulações do escoamento são realizadas no regime transiente utilizando as equações de Navier-Stokes médias de Reynolds com aproximação de Bussinesq e o modelo de turbulência TKE 1.5. As condições fronteira são provenientes dos resultados de uma simulação realizada com Weather Research and Forecasting, WRF. Estas simulações dividem-se em dois grupos, um dos conjuntos de simulações utiliza o esquema convectivo Upwind e o outro utiliza o esquema convectivo de 4aordem. A análise deste método é realizada a partir da comparação dos dados obtidos nas simulações realizadas no código WINDIETM e a coleta de dados medidos durante o processo SC. Em suma, conclui-se que o WINDIETM e as suas configurações reproduzem bons resultados de calibração, ja que produzem erros globais na ordem de dois pontos percentuais em relação ao SC realizado para o mesmo local em estudo.

For uniformly asymptotically affine (uaa) Markov maps on train tracks, we prove the following type of rigidity result: if a topological conjugacy between them is (uaa) at a point in the train track then the conjugacy is (uaa) everywhere. In particular, our methods apply to the case in which the domains of the Markov maps are Canter sets. We also present similar statements for (uaa:) and C-r Markov families. These results generalize the similar ones of Sullivan and de Faria for C-r expanding circle maps with r > 1 and have useful applications to hyperbolic dynamics on surfaces and laminations.

For diffeomorphisms on surfaces with basic sets, we show the following type of rigidity result: if a topological conjugacy between them is differentiable at a point in the basic set then the conjugacy has a smooth extension to the surface. These results generalize the similar ones of D. Sullivan, E. de Faria and ours for one-dimensional expanding dynamics.

This work describes a novel use for the polymeric film, poly(o-aminophenol) (PAP) that was made responsive to a specific protein. This was achieved through templated electropolymerization of aminophenol (AP) in the presence of protein. The procedure involved adsorbing protein on the electrode surface and thereafter electroploymerizing the aminophenol. Proteins embedded at the outer surface of the polymeric film were digested by proteinase K and then washed away thereby creating vacant sites. The capacity of the template film to specifically rebind protein was tested with myoglobin (Myo), a cardiac biomarker for ischemia. The films acted as biomimetic artificial antibodies and were produced on a gold (Au) screen printed electrode (SPE), as a step towards disposable sensors to enable point-of-care applications. Raman spectroscopy was used to follow the surface modification of the Au-SPE. The ability of the material to rebind Myo was measured by electrochemical techniques, namely electrochemical impedance spectroscopy (EIS) and square wave voltammetry (SWV). The devices displayed linear responses to Myo in EIS and SWV assays down to 4.0 and 3.5 μg/mL, respectively, with detection limits of 1.5 and 0.8 μg/mL. Good selectivity was observed in the presence of troponin T (TnT) and creatine kinase (CKMB) in SWV assays, and accurate results were obtained in applications to spiked serum. The sensor described in this work is a potential tool for screening Myo in point-of-care due to the simplicity of fabrication, disposability, short time response, low cost, good sensitivity and selectivity.

A gold screen printed electrode (Au-SPE) was modified by merging Molecular Imprinting and Self-Assembly Monolayer techniques for fast screening cardiac biomarkers in point-of-care (POC). For this purpose, Myoglobin (Myo) was selected as target analyte and its plastic antibody imprinted over a glutaraldehyde (Glu)/cysteamine (Cys) layer on the gold-surface. The imprinting effect was produced by growing a reticulated polymer of acrylamide (AAM) and N,N′-methylenebisacrylamide (NNMBA) around the Myo template, covalently attached to the biosensing surface. Electrochemical impedance spectroscopy (EIS) and cyclic voltammetry (CV) studies were carried out in all chemical modification steps to confirm the surface changes in the Au-SPE. The analytical features of the resulting biosensor were studied by different electrochemical techniques, including EIS, square wave voltammetry (SWV) and potentiometry. The limits of detection ranged from 0.13 to 8 μg/mL. Only potentiometry assays showed limits of detection including the cut-off Myo levels. Quantitative information was also produced for Myo concentrations ≥0.2 μg/mL. The linear response of the biosensing device showed an anionic slope of ~70 mV per decade molar concentration up to 0.3 μg/mL. The interference of coexisting species was tested and good selectivity was observed. The biosensor was successfully applied to biological fluids.

RESUMO - A preocupação com os conhecimentos, atitudes, crenças e práticas, no que concerne à utilização de radiações ionizantes para fins de diagnóstico, e a sensibilização de todos os agentes envolvidos, médicos, técnicos, físicos, utentes e responsáveis pela Saúde Publica, relativamente aos níveis de radiação emitida nos exames de Tomografia Computorizada (TC), assume particular importância no domínio da Saúde Pública, na medida em que é necessário influenciar o desenvolvimento de práticas que promovam, auditem e garantam a prestação do controlo da qualidade radiológica e dosimétrica nos serviços de Radiologia a nível Nacional. Para tal, e no âmbito da publicação de estudos já realizados ao nível da União Europeia, ―Orientações Europeias dos Critérios de Qualidade para a Tomografia Computorizada (1999) ‖, é proposto estabelecer orientações na realização de estudos que permitam, numa primeira fase, estabelecer a comparação com os resultados obtidos pelos mecanismos de Controlo da Qualidade (CQ), analisar e proceder aos ajustes (se necessário) e, numa segunda fase, implementar uma moldura sistemática de avaliação periódica dos níveis de dose de radiação por exame TC e que permita a monitorização dos dados. Nesse sentido, propõe-se a realização de um Estudo Nacional que envolva a rede hospitalar pública, privada e universitária, partindo da metodologia utilizada em estudos prévios noutros países da Europa, como seja, selecção do equipamento de TC existente na Instituição Hospitalar, onde serão reunidas informações através do preenchimento de questionários relativos ao equipamento a utilizar. Serão recolhidos dados relativos ao utente, ao equipamento e parâmetros de aquisição de imagem, que permitam identificar os níveis de referência de diagnóstico (NRD) em TC, na realidade Portuguesa. Foi efectuado um estudo piloto numa instituição EPE e os valores obtidos não são significativos, nem podem assumir valor preditivo dado o reduzido tamanho da amostra. Apesar disso, sugerem a existência de parâmetros que podem ser alterados e com isso podem fazer variar a dose de radiação utilizada. ENSP/UNL Maria de Fátima Vaz de Carvalho 5 Espera-se obter com este estudo, como foi referido, a base do estabelecimento dos NRD em TC em Portugal. ----------------- ABSTRACT - The purpose of this study, in an empirical point of view, emerges from concern with the knowledge, attitudes, beliefs and practices regarding the use of ionizing radiation for diagnostic purposes and awareness of all actors involved, medical physical, technical, and responsible public health for the development of practices that promote, audited and ensure the provision of radiological quality control and dosing in radiology service at national level. In view of the complexity and characteristics involved in relation to ionizing radiation, all assume their part in protecting the physical integrity of each user and a global perspective, to ensure the safeguarding of public health, while global and globalizing factor. To this end, and in the context of the publication of studies already carried out at European Union level, "European guidelines for quality criteria for computed tomography", it is proposed to establish guidelines in conducting studies to initially establish the comparison with the results obtained by QC and make adjustments if necessary, and subsequently implement a systematic periodic assessment frame that allows monitoring of data. Accordingly, it is proposed to conduct a national study involving the public network, private and University hospitals, that extends from the methodology used in previous studies in other countries of Europe, as is, selection of equipment of existing CT in Hospital Institution, where information will be gathered by filling out questionnaires concerning the equipment to be used. Data will be collected for the wearer, equipment and parameters of image acquisition, identifying diagnostic reference levels (NRD) in CT in Portuguese fact. A pilot study was carried out in an institution EPE and the values obtained are not significant, nor can they take predictive value given the small sample size. Despite this, suggest the existence of parameters that can be changed and this can vary the dose of radiation used It is hoped to get with this study, as mentioned, the basis of the establishment of NRD in CT in Portugal.

The authors present the case of a 16 year old female with 3rd degree burn sequelae. They describe the surgical steps during the three years of treatment with serial and multiple tissue expansion. From the 9 tissue expanders used with reference capacities of 3350cc, 10350 were achieved. The conclusions point out the utility of this technique, using re-expansion and supraexpansion as one, should be associated with careful surgical planning and constant rapport with the patient.

Although the Giemsa-stained thick blood smear (GTS) remains the gold standard for the diagnosis of malaria, molecular methods are more sensitive and specific to detect parasites and can be used at reference centers to evaluate the performance of microscopy. The description of the Plasmodium falciparum, P. vivax, P. malariae and P. ovale ssrRNA gene sequences allowed the development of a polymerase chain reaction (PCR) that had been used to differentiate the four species. The objective of this study was to determine Plasmodium species through PCR in 190 positive smears from patients in order to verify the quality of diagnosis at SUCEN's Malaria Laboratory. Considering only the 131 positive results in both techniques, GTS detected 4.6% of mixed and 3.1% of P. malariae infections whereas PCR identified 19.1% and 13.8%, respectively.

The AIDS epidemic has become a worldwide phenomenon of enormous magnitude and extension, deeply transforming medical practices and public health initiatives. This retrospective survey aimed to analyze clinical and epidemiological characteristics of patients with HIV/AIDS admitted to the Institute of Tropical Diseases Natan Portella, Teresina, Piauí, Brazil, from January, 2001 through December, 2004. Of the 828 patients, 43% were from other states and 71.3% were men. Average patient age was 35.4 ± 11.5 years-old and 85.5% were illiterate or had primary education. The main form of exposure to HIV was heterosexual behavior (54.1%), while injectable drug use was confirmed by only 2.7% of registered cases. The most frequent infectious complications were candidiasis (42.4%) and pneumocystosis (22.2%). Sixty-eight cases (8.2%) of visceral leishmaniasis were registered. Using multivariate analysis, individuals aged over 40 years-old, patients with active tuberculosis, Pneumocystis carinii pneumonia and central nervous system cryptococcosis showed increased risk of death. In this study, young male adults with low educational levels predominated and the most frequent opportunistic infections were candidiasis and pneumocystosis.

Allied to an epidemiological study of population of the Senology Unit of Braga’s Hospital that have been diagnosed with malignant breast cancer, we describe the progression in time of repeated measurements of tumor marker Carcinoembryonic antigen (CEA). Our main purpose is to describe the progression of this tumor marker as a function of possible risk factors and, hence, to understand how these risk factors influences that progression. The response variable, values of CEA, was analyzed making use of longitudinal models, testing for different correlation structures. The same covariates used in a previous survival analysis were considered in the longitudinal model. The reference time used was time from diagnose until death from breast cancer. For diagnostic of the models fitted we have used empirical and theoretical variograms. To evaluate the fixed term of the longitudinal model we have tested for a changing point on the effect of time on the tumor marker progression. A longitudinal model was also fitted only to the subset of patients that died from breast cancer, using the reference time as time from date of death until blood test.

A presente dissertação tem como objetivo analisar se existe relação entre a manipulação de resultados e a qualidade da auditoria, baseado no estudo do comportamento de determinados “accruals” nas empresas portuguesas não cotadas. Nos diversos estudos existentes sobre o tema “Relação da Qualidade da Auditoria e Manipulação de Resultados”, surgem abordados muitos aspetos, nomeadamente no que respeita às motivações, às formas de manipulação e métodos de deteção que se verifica no campo da auditoria e, este trabalho, pretende abordar se o processo da auditoria é, ou não, eficaz na deteção destas práticas efetuadas pelos gestores, pois isso influencia a confiança naqueles que utilizam a informação financeira. Desta forma, o trabalho pretende basear-se nestas abordagens e complementar visões e conclusões. Neste âmbito, surgem perspetivas e informações que alertam para comportamentos de risco, assim como a sua origem, ou seja, as motivações que provocam esta prática, tanto por parte dos gestores como dos administradores. É nesta perspetiva que este trabalho se enquadra, numa sociedade contemporânea que continuadamente dá exemplos reais e concretos destas práticas. Um ponto é comum, que é o facto de a manipulação dos resultados surgir principalmente pelo motivo dos interesses e motivações por parte dos gestores em conseguirem benefícios. Na tese são abordados os incentivos que levam à manipulação no contexto português, que parecem estar relacionados com o contexto económico e fiscal, onde é desenvolvida a atividade dos agentes económicos. Outra abordagem importante no trabalho é a referência às principais metodologias de detenção da manipulação de resultados, nomeadamente os modelos baseados nos accruals e na distribuição de resultados. O modelo empírico deste estudo consiste numa regressão linear múltipla, com o objetivo de explicar a relação, entre a variável accruals discricionários e as variáveis Big4, a dimensão da empresa, o endividamento, o volume de negócios e a rendibilidade. Para complementar este estudo a análise empírica incidiu sobre 4723 empresas portuguesas não cotadas, a amostra usada foi baseada na base de dados SABI, para um período de análise entre 2011 a 2013. Os resultados encontrados sugerem que existe relação entre a qualidade da auditoria e a manipulação dos resultados concluindo que as empresas auditadas pelas Big4 apresentam accruals discricionários inferiores às restantes empresas.

OBJECTIVE: To develop a new method to evaluate the performance of individual ICUs through the calculation and visualisation of risk profiles. METHODS: The study included 102,561 patients consecutively admitted to 77 ICUs in Austria. We customized the function which predicts hospital mortality (using SAPS II) for each ICU. We then compared the risks of hospital mortality resulting from this function with the risks which would be obtained using the original function. The derived risk ratio was then plotted together with point-wise confidence intervals in order to visualise the individual risk profile of each ICU over the whole spectrum of expected hospital mortality. MAIN MEASUREMENTS AND RESULTS: We calculated risk profiles for all ICUs in the ASDI data set according to the proposed method. We show examples how the clinical performance of ICUs may depend on the severity of illness of their patients. Both the distribution of the Hosmer-Lemeshow goodness-of-fit test statistics and the histogram of the corresponding P values demonstrated a good fit of the individual risk models. CONCLUSIONS: Our risk profile model makes it possible to evaluate ICUs on the basis of the specific risk for patients to die compared to a reference sample over the whole spectrum of hospital mortality. Thus, ICUs at different levels of severity of illness can be directly compared, giving a clear advantage over the use of the conventional single point estimate of the overall observed-to-expected mortality ratio.
ESTABLISHING THE REFERENCE RANGE FOR T LYMPHOCYTES SUBPOPULATIONS IN ADULTS AND CHILDREN FROM BRAZIL

SUMMARY In Brazil, the existing reference values for T-lymphocytes subsets are based on data originated in other countries. There is no local information on normal variation for these parameters in Brazilian adults and children. We evaluated the normal variation found in blood donors from five large Brazilian cities, in different regions, and in children living in Salvador, and Rio de Janeiro. All samples were processed by flow cytometry. The results were analyzed according to region, gender, and lifestyle of blood donors. A total of 641 adults (63% males), and 280 children (58% males) were involved in the study. The absolute CD3+, and CD4+ cells count were significantly higher for females (adults and children). Higher CD4+ cell count in adults was associated with smoking, while higher CD8+ count was found among female children. Higher counts, for all T-cells subsets, were detected in blood donors from southeast / south regions while those living in the northern region had the lowest values. Individuals from midwestern and northeastern regions had an intermediate count for all these cells subsets. However, these differences did not reach statistical significance. In Brazil, gender and smoking, were the main determinants of differences in T-lymphocytes reference values.
