This cross-sectional study investigates whether calcium intakes from dairy and non-dairy sources, and absolute intakes of various dairy products, are associated with periodontitis. The calcium intake (mg/day) of 135 older Danish adults was estimated by a diet history interview and divided into dairy and non-dairy calcium. Dairy food intake (g/day) was classified into four groups: milk, cheese, fermented foods and other foods. Periodontitis was defined as the number of teeth with attachment loss ≥3 mm. Intakes of total dairy calcium (Incidence-rate ratio (IRR) = 0.97; p = 0.021), calcium from milk (IRR = 0.97; p = 0.025) and fermented foods (IRR = 0.96; p = 0.03) were inversely and significantly associated with periodontitis after adjustment for age, gender, education, sucrose intake, alcohol consumption, smoking, physical activity, vitamin D intake, heart disease, visits to the dentist, use of dental floss and bleeding on probing, but non-dairy calcium, calcium from cheese and other types of dairy food intakes were not. Total dairy foods (IRR = 0.96; p = 0.003), milk (IRR = 0.96; p = 0.028) and fermented foods intakes (IRR = 0.97; p = 0.029) were associated with reduced risk of periodontitis, but cheese and other dairy foods intakes were not. These results suggest that dairy calcium, particularly from milk and fermented products, may protect against periodontitis. Prospective studies are required to confirm these findings.

A series of studies in schizophrenic patients report a decrease of glutathione (GSH) in prefrontal cortex (PFC) and cerebrospinal fluid, a decrease in mRNA levels for two GSH synthesizing enzymes and a deficit in parvalbumin (PV) expression in a subclass of GABA neurons in PFC. GSH is an important redox regulator, and its deficit could be responsible for cortical anomalies, particularly in regions rich in dopamine innervation. We tested in an animal model if redox imbalance (GSH deficit and excess extracellular dopamine) during postnatal development would affect PV-expressing neurons. Three populations of interneurons immunolabeled for calcium-binding proteins were analyzed quantitatively in 16-day-old rat brain sections. Treated rats showed specific reduction in parvalbumin immunoreactivity in the anterior cingulate cortex, but not for calbindin and calretinin. These results provide experimental evidence for the critical role of redox regulation in cortical development and validate this animal model used in schizophrenia research.

Major outputs of the neocortex are conveyed by corticothalamic axons (CTAs), which form reciprocal connections with thalamocortical axons, and corticosubcerebral axons (CSAs) headed to more caudal parts of the nervous system. Previous findings establish that transcriptional programs define cortical neuron identity and suggest that CTAs and thalamic axons may guide each other, but the mechanisms governing CTA versus CSA pathfinding remain elusive. Here, we show that thalamocortical axons are required to guide pioneer CTAs away from a default CSA-like trajectory. This process relies on a hold in the progression of cortical axons, or waiting period, during which thalamic projections navigate toward cortical axons. At the molecular level, Sema3E/PlexinD1 signaling in pioneer cortical neurons mediates a "waiting signal" required to orchestrate the mandatory meeting with reciprocal thalamic axons. Our study reveals that temporal control of axonal progression contributes to spatial pathfinding of cortical projections and opens perspectives on brain wiring.

A 98-year-old woman was referred to our hospital because of myoclonia. The concentration of calcium and vitamin D in the serum was low. In this context, we concluded of neuromuscular irritability secondary to hypocalcaemia. The symptoms disappeared after a treatment of intravenous calcium. This case shows how important it is to investigate electrolytes in case of neuromuscular irritability symptoms in elderly people.

Extracellular calcium participates in several key physiological functions, such as control of blood coagulation, bone calcification or muscle contraction. Calcium homeostasis in humans is regulated in part by genetic factors, as illustrated by rare monogenic diseases characterized by hypo or hypercalcaemia. Both serum calcium and urinary calcium excretion are heritable continuous traits in humans. Serum calcium levels are tightly regulated by two main hormonal systems, i.e. parathyroid hormone and vitamin D, which are themselves also influenced by genetic factors. Recent technological advances in molecular biology allow for the screening of the human genome at an unprecedented level of detail and using hypothesis-free approaches, such as genome-wide association studies (GWAS). GWAS identified novel loci for calcium-related phenotypes (i.e. serum calcium and 25-OH vitamin D) that shed new light on the biology of calcium in humans. The substantial overlap (i.e. CYP24A1, CASR, GATA3; CYP2R1) between genes involved in rare monogenic diseases and genes located within loci identified in GWAS suggests a genetic and phenotypic continuum between monogenic diseases of calcium homeostasis and slight disturbances of calcium homeostasis in the general population. Future studies using whole-exome and whole-genome sequencing will further advance our understanding of the genetic architecture of calcium homeostasis in humans. These findings will likely provide new insight into the complex mechanisms involved in calcium homeostasis and hopefully lead to novel preventive and therapeutic approaches. A nationwide survey was conducted in Switzerland to assess the quality level of osteoporosis management in patients aged 50 years or older presenting with a fragility fracture to the emergency ward of the participating hospitals. Eight centres recruited 4966 consecutive patients who presented with one or more fractures between 2004 and 2006. Of these, 3667 (2797 women, 73.8 years old and 870 men, 73.0 years old in average) were considered as having a fragility fracture and included in the survey. Included patients presented with a fracture of the upper limbs (30.7%), lower limbs (26.4%), axial skeleton (19.5%) or another localisation, including malleolar fractures (23.4%). Thirty-two percent reported one or more previous fractures during adulthood. Of the 2941 (80.2%) hospitalised women and men, only half returned home after discharge. During diagnostic workup, dual x-ray absorptiometry (DXA) measurement was performed in 31.4% of the patients only. Of those 46.0% had a T-score < or =-2.5 SD and 81.1% < or =-1.0 SD. Osteoporosis treatment rate increased from 26.3% before fracture to 46.9% after fracture in women and from 13.0% to 30.3% in men. However, only 24.0% of the women and 13.8% of the men were finally adequately treated with a bone active substance, generally an oral bisphosphonate, with or without calcium / vitamin D supplements. A positive history of previous fracture vs none increased the likelihood of getting treatment with a bone active substance (36.6 vs 17.9%, ? 18.7%, 95% CI 15.1 to 22.3, and 22.6 vs 9.9%, ? 12.7%, CI 7.3 to 18.5, in women and men, respectively). In Switzerland, osteoporosis remains underdiagnosed and undertreated in patients aged 50 years and older presenting with a fragility fracture.

ABSTRACT Introduction The purpose of this study was to assess specific osteoporosis-related health behaviours and physiological outcomes including daily calcium intake, physical activity levels, bone strength, as assessed by quantitative ultrasound, and bone turnover among women between the ages of 18 and 25. Respective differences on relevant study variables, based on dietary restraint and oral contraceptive use were also examined. Methods One hundred women (20.6 ± 0.2 years of age) volunteered to participate in the study. Informed written consent was obtained by all subjects prior to participation. The study and all related procedures were approved by the Brock University Research Ethics Board. Body mass, height, relative body fat, as well as chest, waist and hip circumferences were measured using standard procedures. The 10-item restrained eating subscale of the Dutch Eating Behaviour Questionnaire (DEBQ) was used to assess dietary restraint (van Strien et al., 1986). Daily calcium intake was assessed by the Rapid Assessment Method (RAM) (Hertzler & Frary 1994). Weekly physical activity was documented by the 4-item Godin Leisure-Time Exercise Questionnaire (Godin & Shephard 1985). Bone strength was determined from the speed of sound (SOS) as measured by QUS (Sunlight 7000S). SOS measurements (m/s) were taken of the dominant and non-dominant sides of the distal one third of the radius and the mid-shaft of the tibia. Resting blood samples were collected from all subjects between 9am and 12pm, in order to evaluate the impact of lifestyle factors on biochemical markers of bone turnover. Blood was collected during the early follicular phase of the menstrual cycle (approximately days 1-5) for all subjects. Samples were centrifliged and the serum or plasma was aliquoted into separate tubes and stored at -80°C until analysis. The bone formation markers measured were Osteocalcin (OC), bone specific alkaline phosphatase (BAP) and 25-OH vitamin D. The bone resorption markers measured were the carboxy (CTx) and amino (NTx) terminal telopeptides of type-I collagen crosslinks. All markers were assessed by ELISA. Subjects were divided into high (HDR) and low dietary restrainers (LDR) based on the median DEBQ score, and also into users (BC) and non-users (nBC) of oral contraceptives. A series of multiple one way ANOVA's were then conducted to identify differences between each set of groups for all relevant variables. A two-way ANOVA analysis was used to explore significant interactions between dietary restraint and use of oral contraceptives while a univariate follow-up analysis was also performed when appropriate. Pearson Product Moment Correlations were used to determine relationships among study variables. Results HDR had significantly higher BMI, %BF and circumference measures but lower daily calcium intake than LDR. There were no significant differences in physical activity levels between HDR and LDR. No significant differences were found between BC and nBC in body composition, calcium intake and physical activity. HDR had significantly lower tibial SOS scores than LDR in both the dominant and non-dominant sites. The post-hoc analysis showed that within the non-birth control group, the HDR had significantly lower tibial SOS scores of bone strength when compared to the LDR but Aere were no significant differences found between the two dietary restraint groups for those currently on birth control. HDR had significantly lower levels of OC than LDR and the BC group had lower levels of BAP than the nBC group. Consistently, the follow-up analysis revealed that within those not on birth control, subjects who were classified as HDR had significantly (f*<0.05) lower levels of OC when compared with LDR but no significant differences were observed in bone turnover between the two dietary restraint groups for those currently on birth control. Physical activity was not correlated with SOS scores and bone turnover markers possibly due to the low physical activity variability in this group of women. Conclusion This is the first study to examine the effects of dietary restraint on bone strength and turnover among this population of women. The most important finding of this study was that bone strength and turnover are negatively influenced by dietary restraint independent of relative body fat. In general, the results of the present thesis suggest that dietary restraint, oral contraceptive use, as well as low daily calcium intake and low physical activity levels were widespread behaviours among this population of college-aged women. The young women who were using dietary restraint as a strategy to lose weight, and thus were in the HDR group, despite their higher relative body fat and weight, had lower scores of bone strength and lower levels of markers of bone turnover compared to the low dietary restrainers. Additionally, bone turnover seemed to be negatively affected by oral contraceptives, while bone strength, as assessed by QUS, seemed unaffected by their use in this population of young women. Physical activity (weekly energy expenditure), on the other hand, was not associated with either bone strength or bone tiimover possibly due to the low variability of this variable in this population of young Canadian women.

The purpose of this study was to compare bone speed of sound (SOS) measured by quantitative ultrasound, circulating levels of IGF- 1 and biochemical markers of bone turnover in pre- (Pr) and post-menarcheal (Po) synchronized swimmers (SS) and controls (NS). Seventy participants were recruited: 8 PrSS, 22 PoSS, 20 PrNS, and 20 PoNS. Anthropometric measures of height, weight, skeletal maturity and percent body fat were taken, and dietary intake evaluated using 24-hour recall. Bone SOS was measured at the distal radius and mid-tibia and blood samples analyzed for IGF-1, osteocalcin, NTx, and 25-OH vitamin D. Results demonstrated maturational effects on bone SOS, IGF-1 and bone turnover (p<0.05), with no differences observed between SS and NS. Main effects were observed for a reduced caloric intake in SS compared to NS (p<0.05). Therefore, SS does not offer additive affects on bone strength but imparts no adverse affects to skeletal health in these athletes.

Over the last two decades, the prevalence of obesity in the general population has been steadily increasing. Obesity is a major issue in scientific research because it is associated with many health problems, one of which is bone quality. In adult females, adiposity is associated with increased bone mineral density, suggesting that there is a protective effect of fat on bone. However, the association between adiposity and bone strength during childhood is not clear. Thus, the purpose of this study was to compare bone strength, as reflected by speed of sound (SOS), of overweight and obese girls and adolescents with normal-weight age-matched controls. Data from 75 females included normal-weight girls (G-NW; body fat:::; 25%; n = 21), overweight and obese girls (GOW; body fat ~ 28%; n = 19), normal-weight adolescents (A-NW, body fat:::; 25%; n = 13) and overweight and obese adolescents (A-OW; body fat ~ 28%; n = 22). Nutrition was assessed with a 24-hour recall questionnaire and habitual physical activity was measured for one week using accelerometry. Using quantitative ultrasound (QUS; Sunlight Omnisense™), bone SOS was measured at the distal radius and mid-tibia. No differences were found between groups in daily total energy, calcium or vitamin D intake. However, all groups were below the recommended daily calcium intake of 1300 mg (Osteoporosis Canada, 2008). Adolescents were significantly less active than girls (14.7 ± 0.6 vs. 6.3 ± 0.6% active for G and A, respectively). OW accumulated significantly less minutes of moderate-to-very vigorous physical activity per day (MVPA) than NW in both age groups (114 ± 6 vs. 57 ± 5 min/day for NW and OW, i respectively). Girls had significantly lower radial SOS (3794 ± 87 vs. 3964 ± 64 mls for G-NW and A-NW, respectively), and tibial SOS (3678 ± 86 vs. 3878 ± 52 mls for G-NW and A-NW, respectively) than adolescents. Radial SOS was similar in the two adiposity groups within each age group. However, tibial SOS was lower in the two overweight groups (3601 ± 75 mls vs. 3739 ± 134 mls for G-OW and A-OW, respectively) compared with the age-matched normal-weight controls. Body fat percentage negatively correlated with tibial SOS in the study sample as a whole (r = -0.30). However, when split into groups, percent bo~y fat correlated with tibial SOS only in the A-OW group (r = -0.53). MVPA correlated with tibial SOS (r = 0.40), once age was partialed out. In conclusion, in contrast withthe higher bone strength characteristic of obese adult women, overweight and obese girls and adolescents are characterized by low tibial bone strength, as assessed with QUS. The differences between adiposity groups in tibial SOS may be at least partially due to the reduced weight-bearing physical activity levels in the overweight girls and adolescents. However, other factors, such as hormonal influences associated with high body fat may also playa role in reducing bone strength in overweight girls. Further research is required to reveal the mechanisms causing low bone strength in overweight and obese children and adolescents.

This study examined the bone mineral content (BMC) in young women with Adolescent Idiopathic Scoliosis (AIS), treated with a brace (27.9 ±21.6 months, for 18.0±5.4 h/d) during adolescence (AIS-B, n = 15, 25.6 ±5.8 yrs), versus women with AIS but no treatment (AIS-NB, n = 15, 24.0 ±4.0 yrs), and women without AIS (C, n = 19, 23.5 ±3.8 yrs). After controlling for lean body mass, calcium and vitamin D daily intake, and strenuous physical activity, femoral neck BMC was lower in the AIS-B compared with AIS-NB and C (all p’s < .05). In summary, women with AIS, braced during their growing years are characterized by low lower limb BMC. However, the lack of a relationship between brace treatment duration and BMC, suggests that bracing was not the likely mechanism.

Diet has an important role in the maintenance of oral health, but the relationship between diet and clinical outcomes following sanative therapy (ST) has not been investigated. Due to their antioxidant and anti-inflammatory properties, we hypothesized that periodontal patients with higher intakes of vitamin C, vitamin D, eicosapentaenoic acid (EPA) or docosahexaenoic acid (DHA) would have greater reductions in probing depth (PD) after ST. Patients completed the Block food frequency questionnaire, a supplement use questionnaire and had their serum 25-hydroxyvitamin D measured. There were no significant associations between intakes of vitamin C, vitamin D, EPA, DHA and PD. There were, however, negative associations between intakes of linoleic acid, α- linolenic acid or total vegetable intake and PD, as well as a positive association between the total omega-6/omega-3 ratio and PD (p < 0.05). Therefore, dietary intakes of essential fatty acids and vegetables may be important modulators of periodontal outcomes following ST.

Purpose: Adolescent idiopathic scoliosis (AIS) is often associated with low bone mineral content and density (BMC, BMD). Bracing, used to manage spine curvature, may interfere with the growth-related BMC accrual, resulting in reduced bone strength into adulthood. The purpose of this study was to assess the effects of brace treatment on BMC in adult women, diagnosed with AIS and braced in early adolescence. Methods: Participants included women with AIS who: (i) underwent brace treatment (AIS-B, n = 15, 25.6 ± 5.8 yrs), (ii) underwent no treatment (AIS, n = 15, 24.0 ± 4.0 yrs), and (iii) a healthy comparison group (CON, n = 19, 23.5 ± 3.8 yrs). BMC and body composition were assessed using dual-energy X-ray absorptiometry. Differences between groups were examined using a oneway ANOVA or ANCOVA, as appropriate. Results: AIS-B underwent brace treatment 27.9 ± 21.6 months, for 18.0 ± 5.4 h/d. Femoral neck BMC was lower (p = 0.06) in AIS-B (4.54 ± 0.10 g) compared with AIS (4.89 ± 0.61 g) and CON (5.07 ± 0.58 g). Controlling for lean body mass, calcium and vitamin D daily intake, and strenuous physical activity, femoral neck BMC was statistically different (p = 0.02) between groups. A similar pattern was observed at other lower extremity sites (p < 0.05), but not in the spine or upper extremities. BMC and BMD did not correlate with duration of brace treatment, duration of daily brace wear, or overall physical activity. Conclusion: Young women with AIS, especially those who were treated with a brace, have significantly lower BMC in their lower limbs compared to women without AIS. However, the lack of a relationship between brace treatment duration during adolescence and BMC during young adulthood, suggests that the brace treatment is not the likely mechanism of the low BMC.

PHEX est une protéine importante dans le processus de minéralisation osseuse. Des mutations ou la délétion d’une partie de ce gène causent l’hypophosphatémie liée au chromosome X (XLH). Cette maladie est caractérisée par une hypophosphatémie, accompagnée de défauts de minéralisation, de rachitisme et de lésions ostéomalaciques. Avec l’hypophosphatémie, les taux circulants de vitamine D devraient être augmentés, ce qui n’est pas le cas d’où une régulation anormale de la production de vitamine D a lieu. Cependant, malgré le fait que cette protéine soit une peptidase, aucun substrat physiologique n’a encore été répertorié pour PHEX. PHEX est une protéine membranaire de type II de la famille M13 des métalloendopeptidases à zinc possédant un court domaine N-terminal cytosolique, un segment transmembrannaire d’environ 20 acides aminés et une large portion C-terminale extracellulaire où se trouve le site actif de l’enzyme. PHEX est exprimée de façon majoritaire dans les os et dans les dents et elle apparaît à l’initiation de la minéralisation. Les patients souffrant de XLH et la souris Hyp, qui est un modèle animal de la maladie humaine, montrent des quantités importantes de la protéine FGF23. De plus, FGF23 est impliqué dans une autre maladie reliée au métabolisme du phosphate, l’hypophosphatémie rachitique autosomale dominante (ADHR) où des mutations de FGF23 causent sensiblement les mêmes symptômes que XLH. FGF23 est produit principalement par les ostéoblastes et les ostéocytes. FGF23 cause une hypophosphatémie par la diminution de l’expression du cotransporteur NaPi de type II, responsable de la réabsorption du phosphate rénal. L’hypothèse proposée dans la littérature serait que PHEX activerait ou inactiverait des peptides importants pour la minéralisation osseuse. Plus spécifiquement, l’activation ou l’inactivation de ces peptides aurait pour rôle de réguler les quantités de FGF23. Selon l’hypothèse mentionnée précédemment, la régulation de PHEX pourrait donc avoir un effet sur la minéralisation. Une quantité croissante de données sur la régulation de PHEX sont maintenant disponibles. Par exemple, la vitamine D diminue l’expression de PHEX tandis que les glucocorticoïdes et l’hormone de croissance augmentent son expression. Dans une première étude, nous avons voulu déterminer si un peptide relié à la minéralisation osseuse, le PTHrP1-34, pouvait réguler l’expression de PHEX. Nous avons déterminé que le PTHrP1-34 peut réguler de façon négative l’expression de PHEX dans les cellules UMR-106, une lignée cellulaire ostéoblastique. Cette régulation passe par la voie de l’AMPc/protéine kinase A. De plus, cette diminution d’expression est également observée au jour 7 dans des cultures primaires d’ostéoblastes de rat en minéralisation. Par la suite, nous avons étudié un mutant de PHEX, le mutant E4Q retrouvé chez un patient souffrant de XLH, où la mutation se retrouve dans le domaine cytosolique de PHEX. Cette mutation n’interfère pas avec le site catalytique de l’enzyme puisque ce mutant de PHEX peut tout aussi bien cliver un substrat synthétique que la protéine sauvage. Il a été déterminé que cette mutation annule un motif di-acide. Nous avons démontré que ce motif di-acide est responsable de la liaison de PHEX à COPII, responsable de la formation de vésicules de sécrétion. De plus, il semblerait que ce motif soit important, probablement par son interaction avec COPII, à l’incorporation de PHEX dans des vésicules de calcification, lesdites vésicules étant importantes dans le processus de minéralisation. Finalement, des essais de compétitions ont démontré que la minéralisation pouvait être perturbée lorsque l’on surexprimait la queue cytosolique sauvage de PHEX, contrairement à la queue mutée. Ceci suggère possiblement que l’interaction avec COPII menant à l’incorporation de PHEX dans les vésicules de calcification ou d’autres protéines comprenant de tels motifs pourrait être importante pour la minéralisation. Finalement, la dernière étude porte sur la protéine FGF23. Nous avons démontré, par la surexpression de FGF23 dans la lignée MC3T3 d’ostéoblastes de souris, que cette surexpression a un effet sur la sénescence de ces cellules. En effet, des essais de sénescence ont montré l’augmentation de celle-ci lorsque FGF23 est surexprimé. Par contre, la prolifération n’est pas altérée. De plus, il semblerait que la différenciation soit plus rapide, tel qu’observé par une minéralisation survenant plus tôt, mais n’étant pas plus importante. Bref, la surexpression de FGF23 semblerait faire en sorte que les ostéoblastes se différencient plus rapidement et passent donc à un état de sénescence prématuré comparativement aux cellules sauvages. Ceci est en accord avec la littérature où KLOTHO, un cofacteur de FGF23 permettant sa liaison avec une plus grande affinité sur son récepteur, lorsqu’inactivé démontre un phénotype similaire au vieillissement incluant un phénotype de sénescence.

L’insuffisance rénale chronique (IRC) est associée à une réduction du métabolisme de plusieurs médicaments, due à une diminution du cytochrome P450 (CYP450) hépatique. Nos études précédentes ont montré que l’IRC affecte l’activité in vivo et in vitro, de même que l’expression protéique et génique des différents isoformes du CYP450, via la présence du sérum urémique et de l’hormone parathyroïdienne (PTH). Ce projet de doctorat se divise en quatre parties. Premièrement, nous avons développé une méthode d’analyse de l’activité du CYP450, à l’aide de la production du 3-hydroxy-5,5-dimethyl-4-[4-(methylsulfonyl)phenyl] furan-2(5H)-one (DFH) à partir du 3-[(3,4-difluorobenzyl)oxy]-5,5-dimethyl-4-[4-methylsulfonyl)phenyl] furan-2(5H)-one (DFB). Cette méthode nous a permis de mieux quantifier l’activité dans les études subséquentes. Deuxièmement, l’activité du CYP450 3A est diminuée chez les patients atteints d’IRC. De plus, il a déjà été démontré que des toxines urémiques dialysables seraient impliquées puisque l’hémodialyse prévient cette inhibition du CYP450. Par contre, le mécanisme expliquant l’amélioration transitoire la composition du sérum de patients atteints d’IRC par l’hémodialyse n’est pas connu. L’objectif du projet est d’évaluer l’effet de l’hémodialyse sur l’expression protéique et génique, de même que sur l’activité du CYP450 3A2 dans un modèle d’hépatocytes de rat en culture. Troisièmement, la déficience en calcidiol est fréquente dans les cas d’IRC et l’étiologie est peu connue. Nous avons récemment montré que l’IRC est associée à une diminution du métabolisme des médicaments par le foie suite à une réduction des différents isoformes du CYP450 en partie médiée par l’hormone parathyroïdienne (PTH). La 25-hydroxylation de la vitamine D, au niveau du foie, permet la formation du calcidiol par différents isoformes du CYP450 (CYP2C11, 27A1, 2R1, 3A2 et 2J3) et pourrait être ainsi altérée en présence d’IRC. Les objectifs de cette étude sont de a) confirmer la diminution de synthèse de calcidiol en présence d’IRC et b) évaluer le rôle de la PTH dans la déficience en calcidiol. Finalement, afin de mieux comprendre les inhibitions du CYP450, nous avons étudié les voies de signalisation impliquées dans la régulation du CYP450 en présence d’IRC et avec la PTH puisque les mécanismes d’action demeurent imprécis. La contribution des facteurs de transcription et des récepteurs nucléaires suivants est étudiée ; le récepteur pregnane X (PXR), le récepteur constitutif androstane (CAR) et le facteur nucléaire kappa B (NF-κB), puisqu’ils sont potentiellement activés par le récepteur de la PTH et ces molécules ont été précédemment impliqués dans la régulation du CYP450. Les résultats obtenus montrent que l’hémodialyse des patients atteints d’IRC améliore transitoirement l’expression du CYP450 lorsque des hépatocytes sont mis en culture avec du sérum provenant de ces patients. Aussi, la 25-hydroxylation de la vitamine D est affectée par l’IRC. Les voies de signalisation du NF-κB et les facteurs nucléaires PXR et CAR sont impliqués dans l’inhibition du CYP450. En conclusion, l’IRC affecte, non seulement le métabolisme des médicaments mais aussi l’hydroxylation de la vitamine D, un des rôles endogènes effectués par le CYP450. Ces études nous permettent de mieux comprendre les effets de l’IRC afin de mieux cibler les traitements de choix pour les patients qui en sont atteints.
