Transcripts similar to those that encode the nonstructural (NS) proteins NS3 and NS5 from flaviviruses were found in a salivary gland (SG) complementary DNA (cDNA) library from the cattle tick Rhipicephalus microplus.Tick extracts were cultured with cells to enable the isolation of viruses capable of replicating in cultured invertebrate and vertebrate cells. Deep sequencing of the viral RNA isolated from culture supernatants provided the complete coding sequences for the NS3 and NS5 proteins and their molecular characterisation confirmed similarity with the NS3 and NS5 sequences from other flaviviruses. Despite this similarity, phylogenetic analyses revealed that this potentially novel virus may be a highly divergent member of the genus Flavivirus. Interestingly, we detected the divergent NS3 and NS5 sequences in ticks collected from several dairy farms widely distributed throughout three regions of Brazil. This is the first report of flavivirus-like transcripts inR. microplus ticks. This novel virus is a potential arbovirus because it replicated in arthropod and mammalian cells; furthermore, it was detected in a cDNA library from tick SGs and therefore may be present in tick saliva. It is important to determine whether and by what means this potential virus is transmissible and to monitor the virus as a potential emerging tick-borne zoonotic pathogen.

Cerebral malaria (CM) is a life-threatening complication of Plasmodium falciparum malaria that continues to be a major global health problem. Brain vascular dysfunction is a main factor underlying the pathogenesis of CM and can be a target for the development of adjuvant therapies for the disease. Vascular occlusion by parasitised red blood cells and vasoconstriction/vascular dysfunction results in impaired cerebral blood flow, ischaemia, hypoxia, acidosis and death. In this review, we discuss the mechanisms of vascular dysfunction in CM and the roles of low nitric oxide bioavailability, high levels of endothelin-1 and dysfunction of the angiopoietin-Tie2 axis. We also discuss the usefulness and relevance of the murine experimental model of CM by Plasmodium berghei ANKA to identify mechanisms of disease and to screen potential therapeutic interventions.

Helicobacter pylori infection is one of the most common infections worldwide and is associated with gastric diseases. Virulence factors such as VacA and CagA have been shown to increase the risk of these diseases. Studies have suggested a causal role of CagA EPIYA-C in gastric carcinogenesis and this factor has been shown to be geographically diverse. We investigated the number of CagA EPIYA motifs and the vacA i genotypes in H. pylori strains from asymptomatic children. We included samples from 40 infected children (18 females and 22 males), extracted DNA directly from the gastric mucus/juice (obtained using the string procedure) and analysed the DNA using polymerase chain reaction and DNA sequencing. The vacA i1 genotype was present in 30 (75%) samples, the i2 allele was present in nine (22.5%) samples and both alleles were present in one (2.5%) sample. The cagA-positive samples showed distinct patterns in the 3’ variable region of cagA and 18 of the 30 (60%) strains contained 1 EPIYA-C motif, whereas 12 (40%) strains contained two EPIYA-C motifs. We confirmed that the studied population was colonised early by the most virulent H. pylori strains, as demonstrated by the high frequency of the vacA i1 allele and the high number of EPIYA-C motifs. Therefore, asymptomatic children from an urban community in Fortaleza in northeastern Brazil are frequently colonised with the most virulent H. pylori strains.

Transmission of Trypanosoma cruziduring pregnancy is estimated to occur in less than 20% of infected mothers; however, the etiopathogenesis is not completely understood. The Centre for Studies on Chagas Disease provides confirmation of T. cruziinfection for individuals living in central Brazil. In this retrospective hospital-based study, all requests for diagnosis of T. cruziinfection in individuals less than 21 years old from 1994-2014 were searched. We end with 1,211 individuals and their respective infected mothers. Congenital transmission of infection was confirmed in 24 individuals (2%) in central Brazil, an area where the main T. cruzi lineage circulating in humans is TcII. This low prevalence of congenital Chagas disease is discussed in relation to recent findings in the south region of Brazil, where TcV is the main lineage and congenital transmission has a higher prevalence (approximately 5%), similar to frequencies reported in Argentina, Paraguay and Bolivia. This is the first report to show geographical differences in the rates of congenital transmission of T. cruziand the relationship between the prevalence of congenital transmission and the type of Tc prevalent in each region.

Chagas disease, caused by Trypanosoma cruzi infection, is a zoonosis of humans, wild and domestic mammals, including dogs. In Panama, the main T. cruzi vector is Rhodnius pallescens, a triatomine bug whose main natural habitat is the royal palm, Attalea butyracea. In this paper, we present results from three T. cruzi serological tests (immunochromatographic dipstick, indirect immunofluorescence and ELISA) performed in 51 dogs from 24 houses in Trinidad de Las Minas, western Panama. We found that nine dogs were seropositive (17.6% prevalence). Dogs were 1.6 times more likely to become T. cruziseropositive with each year of age and 11.6 times if royal palms where present in the peridomiciliary area of the dog’s household or its two nearest neighbours. Mouse-baited-adhesive traps were employed to evaluate 12 peridomestic royal palms. All palms were found infested with R. pallescens with an average of 25.50 triatomines captured per palm. Of 35 adult bugs analysed, 88.6% showed protozoa flagellates in their intestinal contents. In addition, dogs were five times more likely to be infected by the presence of an additional domestic animal species in the dog’s peridomiciliary environment. Our results suggest that interventions focused on royal palms might reduce the exposure to T. cruzi infection.

This study evaluated the antifungal susceptibility profile and the production of potential virulence attributes in a clinical strain of Candida nivariensis for the first time in Brazil, as identified by sequencing the internal transcribed spacer (ITS)1-5.8S-ITS2 region and D1/D2 domains of the 28S of the rDNA. For comparative purposes, tests were also performed with reference strains. All strains presented low planktonic minimal inhibitory concentrations (PMICs) to amphotericin B (AMB), caspofungin (CAS), and voriconazole. However, our strain showed elevated planktonic MICs to posaconazole (POS) and itraconazole, in addition to fluconazole resistance. Adherence to inert surfaces was conducted onto glass and polystyrene. The biofilm formation and antifungal susceptibility on biofilm-growing cells were evaluated by crystal violet staining and a XTT reduction assay. All fungal strains were able to bind both tested surfaces and form biofilm, with a binding preference to polystyrene (p < 0.001). AMB promoted significant reductions (≈50%) in biofilm production by our C. nivariensis strain using both methodologies. This reduction was also observed for CAS and POS, but only in the XTT assay. All strains were excellent protease producers and moderate phytase producers, but lipases were not detected. This study reinforces the pathogenic potential of C. nivariensis and its possible resistance profile to the azolic drugs generally used for candidiasis management.

OBJECTIVE Serum levels of soluble TNF-like weak inducer of apoptosis (sTWEAK) and its scavenger receptor CD163 (sCD163) have been linked to insulin resistance. We analysed the usefulness of these cytokines as biomarkers of type 2 diabetes in a Spanish cohort, together with their relationship to food consumption in the setting of the Di@bet.es study. RESEARCH DESIGN AND METHODS This is a cross-sectional, matched case-control study of 514 type 2 diabetes subjects and 517 controls with a Normal Oral Glucose Tolerance Test (NOGTT), using data from the Di@bet.es study. Study variables included clinical and demographic structured survey, food frequency questionnaire and physical examination. Serum concentrations of sTWEAK and sCD163 were measured by ELISA. Linear regression analysis determined which variables were related to sTWEAK and sCD163 levels. Logistic regression analysis was used to estimate odd ratios of presenting type 2 diabetes. RESULTS sCD163 concentrations and sCD163/sTWEAK ratio were 11.0% and 15.0% higher, respectively, (P<0.001) in type 2 diabetes than in controls. Following adjustment for various confounders, the OR for presenting type 2 diabetes in subjects in the highest vs the lowest tertile of sCD163 was [(OR), 2,01 (95%CI, 1,46-2,97); P for trend <0.001]. Coffee and red wine consumption was negatively associated with serum levels of sCD163 (P = 0.0001 and; P = 0.002 for coffee and red wine intake, respectively). CONCLUSIONS High circulating levels of sCD163 are associated with type 2 diabetes in the Spanish population. The association between coffee and red wine intake and these biomarkers deserves further study to confirm its potential role in type 2 diabetes.

BACKGROUND In 1997, 18.5% of the cases of Meningococcal Disease caused b serogroup C in Andalusia were children between 2 and 4 years of age; ages where the initial immune response and the duration of the capsular A + C meningococcal polysaccharide vaccine is less than in older age groups. Research was designed in order to measure the immune response produced by this vaccine in children from 2 to 6 years of age and to compare it with the natural immunity present in unvaccinated children. METHODS I. Dual monitoring study: a) groups of children vaccinated previously and control groups, b) groups of children who were going to be vaccinated, for pre and post-vaccination (1, 6 and 12 months) analysis and a control group. II. The bactericidal activity was measured according to the standardised protocol of the CDC with regard to the strain of N. meningitidis C-11. The sera with bactericidal activity (TAB) > 1:8 were considered to be protective. RESULTS 1 and 2 months following vaccination, the proportion of TAB > 1:8 was significantly higher than that of the control group (65.6% and 73% in comparison to 2.2% and 12%). In the pre-vaccine and post-vaccine (after 6, 7, 12 and 13 months) verification, no significant difference between vaccinated individuals and controls was observed. CONCLUSIONS The differences between vaccinated and unvaccinated individuals 1 and 2 months following vaccination indicate seroconversion in the vaccinated individuals. For the age group of between 2 to 6 years of age, the bactericidal activity acquired decline quickly, as, after 6 months, differences between this group and the control group are no longer observed.

Diante da vivência em tratar de indivíduos que apresentam lesão medular, nos deparamos com a problemática envolvida no cotidiano dessas pessoas após uma situação de trauma súbito e inesperado. Com a intenção de prestar cuidados efetivos a estas pessoas nos preocupamos, neste estudo, em identificar a situação pela qual elas estão passando, se houve mudanças e quais foram elas, tanto em nível psíquico como físico e social. Para tanto, optou-se pela realização de urna entrevista individual semi-estruturada, utilizando-se o recurso da gravação, o instrumento de coleta de dados consta de dados de identificação e dados a respeito da vida do entrevistado. O discurso livre com gravação e posterior transcrição caracteriza um estudo de caso de natureza, sobretudo, exploratória. Os dados obtidos receberam uma análise temática utilizando concepções sobre qualidade de vida segundo FORATTINI (1991). Neste estudo foram identificadas uma deficiência física e uma série de incapacidades. A deambulação é a primeira perda notada pelo paciente após a lesão e a sua primeira questão ao profissional mais próximo dele, é que se estabeleça um canal de comunicação que permita esta expressão. Mas devemos lembrar do processo possível de reabilitação que não aborda somente a busca pela locomoção, permeando o reaprendizado nesta nova situação de vida. Constatamos modificações em todas as esferas (física, social, psicológica), sendo que as mais afetadas, segundo os dados colhidos, foram a psicológica e a social devido às alterações físicas. A principal sensação de perda refere-se ao papel social, sendo que esta é caracterizada, principalmente, pela impossibilidade de retornar ao trabalho. As mudanças nas concepções de auto-imagem e auto-estima também foram identificadas, em menor grau, pela sensação de incapacidade. A perda física mais importante e compreensivelmente mais relatada refere-se a perda de sensibilidade e capacidade motora dos membros.

O presente estudo teve por objetivos: 1) verificar a incidência de ocorrências adversas (OAs) com medicação relacionadas ao tempo de infusão das soluções hidroeletrolíticas e ao número de doses de antibióticos prescritos e administrados aos pacientes; 2) caracterizar a natureza dessas ocorrências. A investigação foi realizada em duas UTIs de um hospital geral do Município de São Paulo. Fizeram parte do estudo os dados contidos nos prontuários de 51 pacientes que estiveram internados naquelas Unidades no mês de Agosto de 1996. Quanto à caracterização dos pacientes, 60% tinham idade acima de 60 anos, 58,8% eram mulheres, 49,1% permaneceram na UTI entre 1 e 5 dias e, ao saírem dela, 41,2 % foram para a Unidade de Cuidados Semi-Intensivos. Referente à incidência de 0As relacionadas ao tempo de administração das soluções hidroeletrolíticas e ao número de doses de antibióticos, constatou-se o não cumprimento das prescrições médicas dos pacientes, em 76,3% e 38,6%, respectivamente. A maior freqüência de irregularidades quanto aos soros (60,2%) foi referente a administração em tempo menor que o prescrito (adiantamentos) e, no caso dos antibióticos, a redução do número de doses (85%). Considerando-se tais irregularidades como ocorrências indesejáveis na prática da enfermagem na UTI, há que se investir na busca de medidas preventivas dessas ocorrências.

O homem, ao se defrontar com a cirurgia e a anestesia, tem alterações hormonais provocadas pela ansiedade, moduladas pela avaliação cognitiva deste enfrentamento e acompanhadas de alterações fisiológicas perceptíveis. Os objetivos do presente estudo foram verificar a correlação entre o traço e o estado de ansiedade no período pré-operatóro, e a influência da ansiedade, na resposta do paciente, no período pós-operatório imediato, em sala de recuperação anestésica. A amostra foi composta por 33 pacientes cirúrgicos. Para a coleta de dados foram utilizados, além dos inventários auto-aplicáveis de traço-estado de ansiedade do paciente no período pré-operatório imediato, dois (2) formulários contendo parâmetros de avaliação clínica dos pacientes. Os dados foram coletados nos períodos pré-operatório imediato e pós-operatório imediato. Os resultados mostraram uma correlação estatisticamente significante e positiva entre o traço e o estado de ansiedade, no período pré-operatório ou seja, os pacientes ao enfrentar o ato anestésico-cirúrgico, têm seu estado de ansiedade alterado, quando correlacionado ao seu traço de ansiedade e uma correlação não significante entre o estado de ansiedade, no pré-operatório imediato com os parâmetros clínicos avaliados no período pós-operatório imediato, isto é, os pacientes não apresentaram alterações decorrentes do estado de ansiedade, no período pós-operatório imediato.

O estudo objetivou compreender o significado de estar com AIDS para mães portadoras do HIV/AIDS, identificar as emoções e os modos de enfrentamento decorrentes da relação mãe-filho sadio. Realizado à luz da teoria das representações sociais, teve como resultante "ser mãe" e "estar com AIDS" representado como um processo que não se esgota na doença, mas mostra-se vinculado à polaridade do bem e do mal. Traz ainda a sexualidade como algo que contém essas polaridades e a AIDS, vinculada ao aspecto mal. As mulheres deste estudo mostraram se julgar culpadas de algo e merecem o castigo imposto pela doença.

Artigo de revisão bibliográfica que aborda o tema Coping na perspectiva do Modelo Interacionista Cognitivo, e apresenta como objetivos traçar um paralelo entre esse referencial da psicologia com o processo saúde-doença, e pontuar a relevância do assunto e os caminhos possíveis de serem trabalhados pelo enfermeiro, entendendo ser este um referencial importante que o orienta no desenvolvimento de suas ações assistenciais.
