The inherent analogue nature of medical ultrasound signals in conjunction with the abundant merits provided by digital image acquisition, together with the increasing use of relatively simple front-end circuitries, have created considerable demand for single-bit  beamformers in digital ultrasound imaging systems. Furthermore, the increasing need to design lightweight ultrasound systems with low power consumption and low noise, provide ample justification for development and innovation in the use of single-bit  beamformers in ultrasound imaging systems. The overall aim of this research program is to investigate, establish, develop and confirm through a combination of theoretical analysis and detailed simulations, that utilize raw phantom data sets, suitable techniques for the design of simple-to-implement hardware efficient  digital ultrasound beamformers to address the requirements for 3D scanners with large channel counts, as well as portable and lightweight ultrasound scanners for point-of-care applications and intravascular imaging systems. In addition, the stability boundaries of higher-order High-Pass (HP) and Band-Pass (BP) Σ−Δ modulators for single- and dual- sinusoidal inputs are determined using quasi-linear modeling together with the describing-function method, to more accurately model the  modulator quantizer. The theoretical results are shown to be in good agreement with the simulation results for a variety of input amplitudes, bandwidths, and modulator orders. The proposed mathematical models of the quantizer will immensely help speed up the design of higher order HP and BP Σ−Δ modulators to be applicable for digital ultrasound beamformers. Finally, a user friendly design and performance evaluation tool for LP, BP and HP  modulators is developed. This toolbox, which uses various design methodologies and covers an assortment of  modulators topologies, is intended to accelerate the design process and evaluation of  modulators. This design tool is further developed to enable the design, analysis and evaluation of  beamformer structures including the noise analyses of the final B-scan images. Thus, this tool will allow researchers and practitioners to design and verify different reconstruction filters and analyze the results directly on the B-scan ultrasound images thereby saving considerable time and effort.

Systemic Lupus Erythematosus (SLE) is a chronic inflammatory rheumatic disease which affects the connective tissue. Its etiology is as yet unknown, while its pathogenesis involves the immune system. Both genetic and environmental and hormonal factors play a key role in the impaired immune regulation. A correlation with estrogens is demonstrated by the fact that the greatest incidence is found in young women, when estrogen secretion is at its highest. The disease is also reported to worsen in women taking oral contraceptives. It is therefore believed that the components of oral contraceptives, estrogens (ethinyl estradiol) and progestins, can affect the immune profile. Of the various complications attributed to systemic lupus erythematosus, gastrointestinal disorders are less common but potentially by far the most serious. We report a case of ischemic necrosis with sigma perforation in a patient with SLE. Signs and symptoms of acute abdomen in patients with SLE are rare (0.2%), but serious. Most patients require an exploratory laparotomy, as the causes are often linked with vasculitis.

We present the case of a patient with a double adenocarcinoma of the right colon and sigma associated with a bilateral infiltrating ductal breast carcinoma. Sigma and caecum bowel cancers were diagnosed at colonoscopy, with computerized tomography staging, while breast cancer was found with screening mammography. Following right hemicolectomy , sigmoidectomy and bilateral mastectomy the histology confirmed the presence of colonic adenocarcinoma and infiltrating and in situ lobular cancer. This case report reviews the treatment of synchronous neoplasia.

En la actualidad muchas compañías enfocan sus esfuerzos en la chequera del cliente generando con esto una cortina de humo al interior de la empresa que hace que se descuiden los costos, se conviva con actividades que no agregan valor y se desconozcan las causas de los principales problemas de calidad que afectan al cliente. Este es el caso de la compañía metalmecánica objeto de esta investigación, la cual venía sobrellevando altos niveles de inventarios, reprocesos, problemas de calidad, falta de corresponsabilidad del personal, y otras situaciones que la estaban dejando vulnerable ante la competencia. Este fue el motivo que inspiró esta investigación en aras de validar una metodología de mejora continua para identificar, analizar, proponer, evaluar y reducir los costos de no calidad y sus implicaciones, de una forma ordenada, rigurosa y disciplinada. Posterior a la implementación del plan de acción los resultados no se hicieron esperar y la compañía mejoró su servicio al cliente sin incurrir en excesos de inventario, al igual que disminuyó sus sobrecostos de producción, los cuales eran generados en gran medida por problemas de calidad. Pero lo más significativo es que asimiló que esta metodología es un proceso continuo que no puede parar y que todo el personal de la empresa debe estar implicado.

The existence of genuinely non-geometric backgrounds, i.e. ones without geometric dual, is an important question in string theory. In this paper we examine this question from a sigma model perspective. First we construct a particular class of Courant algebroids as protobialgebroids with all types of geometric and non-geometric fluxes. For such structures we apply the mathematical result that any Courant algebroid gives rise to a 3D topological sigma model of the AKSZ type and we discuss the corresponding 2D field theories. It is found that these models are always geometric, even when both 2-form and 2-vector fields are neither vanishing nor inverse of one another. Taking a further step, we suggest an extended class of 3D sigma models, whose world volume is embedded in phase space, which allow for genuinely non-geometric backgrounds. Adopting the doubled formalism such models can be related to double field theory, albeit from a world sheet perspective.

Target space duality is one of the most profound properties of string theory. However it customarily requires that the background fields satisfy certain invariance conditions in order to perform it consistently; for instance the vector fields along the directions that T-duality is performed have to generate isometries. In the present paper we examine in detail the possibility to perform T-duality along non-isometric directions. In particular, based on a recent work of Kotov and Strobl, we study gauged 2D sigma models where gauge invariance for an extended set of gauge transformations imposes weaker constraints than in the standard case, notably the corresponding vector fields are not Killing. This formulation enables us to follow a procedure analogous to the derivation of the Buscher rules and obtain two dual models, by integrating out once the Lagrange multipliers and once the gauge fields. We show that this construction indeed works in non-trivial cases by examining an explicit class of examples based on step 2 nilmanifolds.

La concepción tradicional del hospital como un lugar donde se acude a recuperar la salud, es cada vez menos utilizada, ya que existe un mejor entendimiento en el sentido que dicho centro de salud viene a ser un conjunto de elementos humanos, materiales y tecnológicos, organizados adecuadamente mediante una buena gestión, para proporcionar servicios de salud (preventiva, curativa y de rehabilitación), en condiciones de máxima eficiencia, confianza y de óptima rentabilidad económica. Es por lo anterior que una gestión apropiada del mantenimiento es esencial para asegurar que el equipamiento del centro hospitalario cumpla con los estándares mínimos requeridos para brindar atención a los pacientes que la requieren, en el momento que la requieren y contribuir a la eficiencia en la prestación de servicios. Como parte de la mejora continua y de la aplicación de un sistema de gestión en el trabajo desarrollado por el departamento de mantenimiento del Hospital San Rafael, se presenta una propuesta utilizando la metodología Seis Sigma, la cual en resumen permitirá diagnosticar las debilidades, medir la situación actual de los procesos clave, analizar las causas de las mismas, diseñar un plan de mejora y controlar los procesos.

As unidades de restauração coletiva dos Serviços de Ação Social (SAS) do Instituto Politécnico de Viana do Castelo (IPVC) têm desenvolvido um esforço para a aplicação de metodologias de Gestão que melhorem a sua eficiência e consequente competitividade. A metodologia 6-Sigma tem como foco a diminuição ou eliminação da incidência de erros ou defeitos num processo. A metodologia Lean tem como objetivo o aumento da produtividade, reduzindo desperdícios e gerando valor. Por conseguinte, o objetivo do presente trabalho consistiu na adoção destas metodologias de trabalho. Aplicou-se a metodologia 6-Sigma ao departamento de receção e aprovisionamento das matérias-primas, de forma a incrementar e agilizar a avaliação de fornecedores, melhorar o desempenho e racionalizar os recursos deste departamento dos SAS da Escola Superior de Tecnologia e Gestão (ESTG) do IPVC. A implementação da filosofia Lean 6-Sigma foi aplicada nas zonas de atendimento e distribuição de refeições prontas, com vista a medir o impacto da flutuação e disponibilidade dos recursos humanos afetos ao serviço de cantinas dos SAS, apoiando o serviço de refeições com qualidade e segurança alimentar, e contribuindo para uma maior eficiência do serviço. A metodologia seguida na investigação e análise deste trabalho segue os objetivos de cada fase do DMAIC (De f i n e, Me a s u r e, Analyse, Improve, Check) segundo a American Society for Quality (ASQ). O estudo permite concluir que as propostas de melhoria verificadas assentam na diminuição de defeitos e desperdícios, logo um aumento da qualidade dos serviços prestados pela organização

Recent and current socio-cultural trends are significant factors impacting on how business is conduced and correspondingly, on how work environments are designed. New communication technology is helping to break physical boundaries and change the way and speed of conducting business. One of the main characteristics of these new workplaces is non-permanency wherein the individual employee has no dedicated personally assigned office, work station, or desk. In this non-territorial, nomadic situation, employees undertake their work tasks in a wide variety of work settings inside and outside the office building. Such environments are understood to be must suitable where there is the need for high interaction with others as well as a high level of concentrated, independent work. This thesis reports on a project designed to develop a deeper understanding of the relationships between people (P) and their built environment (E) in the context of everyday work practice in a nomadic and non-territorial work environment. To achieve this, the study focuses on the experiences of employees as they understand them in relation to their work and the designed/ physical work environment. In this sense, the study is qualitative and grounded in nature. It does not assume any previously established theory nor test any presenting hypothesis. Instead it interviews the participants about their situations at work in their workplace, interprets natural interaction and creates a foundation for the development of theory informing workplace design, particularly theory that recognises the human nature of work and the need, as highlighted by several seminal researchers, for a greater understanding of how people manage and adapt in dynamic work environments.

In the design of tissue engineering scaffolds, design parameters including pore size, shape and interconnectivity, mechanical properties and transport properties should be optimized to maximize successful inducement of bone ingrowth. In this paper we describe a 3D micro-CT and pore partitioning study to derive pore scale parameters including pore radius distribution, accessible radius, throat radius, and connectivity over the pore space of the tissue engineered constructs. These pore scale descriptors are correlated to bone ingrowth into the scaffolds. Quantitative and visual comparisons show a strong correlation between the local accessible pore radius and bone ingrowth; for well connected samples a cutoff accessible pore radius of approximately 100 microM is observed for ingrowth. The elastic properties of different types of scaffolds are simulated and can be described by standard cellular solids theory: (E/E(0))=(rho/rho(s))(n). Hydraulic conductance and diffusive properties are calculated; results are consistent with the concept of a threshold conductance for bone ingrowth. Simple simulations of local flow velocity and local shear stress show no correlation to in vivo bone ingrowth patterns. These results demonstrate a potential for 3D imaging and analysis to define relevant pore scale morphological and physical properties within scaffolds and to provide evidence for correlations between pore scale descriptors, physical properties and bone ingrowth.
