In order to prevent disparities in the management of breast cancer having a direct impact on the prognosis of patients, to promote early detection and optimal treatment while considering the quality of life of patients, Breast Centers are being set up in Switzerland on the basis of existing models in Europe. The centers provide also follow-up of patients and are submitted to certification criteria established by the Swiss Society of Senology and the Swiss Cancer League. These criteria include in particular the expertise of specialists based on a sufficient volume of activity and training, compliance with recommendations of clinical practice, integration of supportive care and timeliness of care. The certification process is voluntary. A database enables the regular assessment of the provided care and of the compliance with standards. The aim and the modalities of the creation of the Breast Centers are discussed.

Fluorescence resonance energy transfer (FRET) allows the user to investigate interactions between fluorescent partners. One crucial issue when calculating sensitized emission FRET is the correction for spectral bleed-throughs (SBTs), which requires to calculate the ratios between the intensities in the FRET and in the donor or acceptor settings, when only the donor or acceptor are present. Theoretically, SBT ratios should be constant. However, experimentally, these ratios can vary as a function of fluorophore intensity, and assuming constant values may hinder precise FRET calculation. One possible cause for such a variation is the use of a microscope set-up with different photomultipliers for the donor and FRET channels, a set-up allowing higher speed acquisitions on very dynamic fluorescent molecules in living cells. Herein, we show that the bias introduced by the differential response of the two PMTs can be circumvented by a simple modeling of the SBT ratios as a function of fluorophore intensity. Another important issue when performing FRET is the localization of FRET within the cell or a population of cells. We hence developed a freely available ImageJ plug-in, called PixFRET, that allows a simple and rapid determination of SBT parameters and the display of normalized FRET images. The usefulness of this modeling and of the plug-in are exemplified by the study of FRET in a system where two interacting nuclear receptors labeled with ECFP and EYFP are coexpressed in living cells.

Background: Methotrexate is a chemotherapeutic agent used to treat a variety of cancers. However, the occurrence of resistance limits its effectiveness. Cytochrome c in its reduced state is less capable of triggering the apoptotic cascade. Thus, we set up to study the relationship among redox state of cytochrome c, apoptosis and the development of resistance to methotrexate in MCF7 human breast cancer cells. Results: Cell incubation with cytochrome c-reducing agents, such as tetramethylphenylenediamine, ascorbate or reduced glutathione, decreased the mortality and apoptosis triggered by methotrexate. Conversely, depletion of glutathione increased the apoptotic action of methotrexate, showing an involvement of cytochrome c redox state in methotrexateinduced apoptosis. Methotrexate-resistant MCF7 cells showed increased levels of endogenous reduced glutathione and a higher capability to reduce exogenous cytochrome c. Using functional genomics we detected the overexpression of GSTM1 and GSTM4 in methotrexate-resistant MCF7 breast cancer cells, and determined that methotrexate was susceptible of glutathionylation by GSTs. The inhibition of these GSTM isoforms caused an increase in methotrexate cytotoxicity in sensitive and resistant cells. Conclusions: We conclude that overexpression of specific GSTMs, GSTM1 and GSTM4, together with increased endogenous reduced glutathione levels help to maintain a more reduced state of cytochrome c which, in turn, would decrease apoptosis, thus contributing to methotrexate resistance in human MCF7 breast cancer cells.

Bridge deck deterioration due to corrosive effect of deicers on reinforcing steel is a major problem facing many agencies. Cathodic protection is one method used to prevent reinforcing steel corrosion. The application of a direct current to the embedded reinforcing steel and a sacrificial anode protects the steel from corrosion. This 1992 project involved placing an Elgard Titanium Anode Mesh Cathodic Protection System on a bridge deck. The anode was fastened to the deck after the Class A repair-work and the overlay was placed using the Iowa Low Slump Dense Concrete System. The system was set up initially at 1 mA/sq ft.

High-speed non-contact laser profilers have become the standard testing equipment for pavement management ride quality testing. The same technology used in the high-speed profilers is now being used in lightweight profilers for construction smoothness testing. The lightweight profilers have many advantages over the California 25-ft profilograph. Despite the many advantages of the lightweight profilers, there is resistance from the contracting industry toward eliminating the 25-ft profilograph for construction ride testing. One way to reduce or overcome the resistance is to evaluate and demonstrate the advantages/disadvantages of the lightweight profiler in actual field use in Iowa. The objective of the study was to purchase a lightweight profiler and to evaluate its suitability for construction smoothness quality verification and quality acceptance on Iowa projects. A lightweight profiler, an Ames Engineering, Inc. LISA single laser unit, was received in February 2003 for the study. Based on the work done during the 2003 construction season, the following conclusions can be made: (1) For hot mix asphalt surfaces, the LISA correlated well with the contractors' profilographs; (2) LISA results are significantly affected by longitudinal tining on portland cement concrete pavements, requiring a laser system upgrade to give accurate results; (3) A significant timesaving was realized by using the LISA; (4) Increasing visibility and reducing time in the construction zone improved safety; (5) One person with limited lifting capabilities could set up and operate the LISA; and (6) With the current Iowa Department of Transportation specification, the LISA cannot totally replace the profilograph, since bridges and short segments with no adjoining pavement would still require a profilograph.

In 2006, a medico-legal consultation service devoted to adult victims of interpersonal violence was set up at the Lausanne University Hospital Centre, Switzerland: the Violence Medical Unit. Patients are received by forensic nurses for support, forensic examination (in order to establish medical report) and community orientation. In 2008, a telephone survey was conducted on patients. The objectives of the survey were to estimate the degree of patients' satisfaction and to document the use of the medical report by six questions. Among the 476 patients admitted to the VMU in 2007, 132 were interviewed. Their overall satisfaction was high with an average mark of 8.7/10. The medical report was used extensively by the interviewed victims (81%) for its primary function - to be produced as evidence. As the consultations are financed by public funds, these results were of interest for advocacy of long-lasting financial support.

At present, there is little fundamental guidance available to assist contractors in choosing when to schedule saw cuts on joints. To conduct pavement finishing and sawing activities effectively, however, contractors need to know when a concrete mixture is going to reach initial set, or when the sawing window will open. Previous research investigated the use of the ultrasonic pulse velocity (UPV) method to predict the saw-cutting window for early entry sawing. The results indicated that the method has the potential to provide effective guidance to contractors as to when to conduct early entry sawing. The aim of this project was to conduct similar work to observe the correlation between initial setting and conventional sawing time. Sixteen construction sites were visited in Minnesota and Missouri over a two-year period. At each site, initial set was determined using a p-wave propagation technique with a commercial device. Calorimetric data were collected using a commercial semi-adiabatic device at a majority of the sites. Concrete samples were collected in front of the paver and tested using both methods with equipment that was set up next to the pavement during paving. The data collected revealed that the UPV method looks promising for early entry and conventional sawing in the field, both early entry and conventional sawing times can be predicted for the range of mixtures tested.

The objective of this work was to evaluate interactions of Lumbricus terrestris juveniles with adults and with inherited burrow systems. An experiment was set up using a two dimensional Evans' boxes microcosm. Adult L. terrestris were added to 16 boxes (one individual per box) and kept in darkness at 17ºC along with eight unoccupied boxes for two months. The adult L. terrestris were removed from eight randomly selected boxes, and L. terrestris juveniles were added (one juvenile per box), composing three treatments with eight replicates: 1, with an adult in an inherited burrow (ABJ); 2, alone in an inherited burrow (BJ); and 3, alone in a previously uninhabited box (J). The proportion of juveniles occupying adult burrows observed was significantly different in treatments ABJ (48%) and BJ (75%). The mean mass of juveniles at experimental termination differed significantly among treatments and was greater in treatment J (4.04±0.39 g) in comparison to the BJ (3.09±0.93 g) and ABJ treatments (2.13±0.64 g). Results suggest a negative influence of both the presence of an adult and its burrow system on juvenile growth. Intraspecific competition partially explained this, but further investigation is required to examine how an inherited environment (i.e. burrow) could negatively affect the growth of juveniles.

OBJECTIVE: To assess the properties of various indicators aimed at monitoring the impact on the activity and patient outcome of a bed closure in a surgical intensive care unit (ICU). DESIGN: Comparison before and after the intervention. SETTING: A surgical ICU at a university hospital. PATIENTS: All patients admitted to the unit over two periods of 10 months. INTERVENTION: Closure of one bed out of 17. MEASUREMENTS AND RESULTS: Activity and outcome indicators in the ICU and the structures upstream from it (emergency department, operative theater, recovery room) and downstream from it (intermediate care units). After the bed closure, the monthly medians of admitted patients and ICU hospital days increased from 107 (interquartile range 94-112) to 113 (106-121, P=0.07) and from 360 (325-443) to 395 (345-436, P=0.48), respectively, along with the linear trend observed in our institution. All indicators of workload, patient severity, and outcome remained stable except for SAPS II score, emergency admissions, and ICU readmissions, which increased not only transiently but also on a mid-term basis (10 months), indicating that the process of patient care delivery was no longer predictable. CONCLUSIONS: Health care systems, including ICUs, are extraordinary flexible, and can adapt to multiple external constraints without altering commonly used activity and outcome indicators. It is therefore necessary to set up multiple indicators to be able to reliably monitor the impact of external interventions and intervene rapidly when the system is no longer under control.

The objective of this work was to set up ideal conditions for conidia mass production of Dicyma pulvinata. Four isolates were compared in terms of their growth and conidia production on various substrates (grains of parboiled rice, common rice, maize and wheat, besides chipped maize and rice husk), temperatures (19, 22, 25, 28 and 31ºC), growth containers (aluminum trays, polypropylene bags and Erlenmeyers) and light regimes (continuous darkness, 6 and 12 hours of light/darkness, and continuous light). Temperature effects on conidia germination capacity were also evaluated. The experiments were done in randomized complete block designs, in factorial arrangements (isolates x treatments - substrates, containers, temperatures and light regimes), with four replicates. In general, parboiled rice and polypropylene bags provided the best development of the fungus. Complete darkness and 6 hours of light increased mycelial growth, whereas continuous light favored sporulation. All tested temperatures favored the cultures of the fungus, except 31ºC. Temperatures between 19 and 25ºC ensure spore germination of more than 76%.

This paper uses a database covering the universe of French firms for the period 1990-2007 to provide a forensic account of the role of individual firms in generating aggregatefluctuations. We set up a simple multi-sector model of heterogeneous firms selling tomultiple markets to motivate a theoretically-founded decomposition of firms' annualsales growth rate into different components. We find that the firm-specific componentcontributes substantially to aggregate sales volatility, mattering about as much as thecomponents capturing shocks that are common across firms within a sector or country. We then decompose the firm-specific component to provide evidence on two mechanismsthat generate aggregate fluctuations from microeconomic shocks highlighted in the recentliterature: (i) when the firm size distribution is fat-tailed, idiosyncratic shocks tolarge firms directly contribute to aggregate fluctuations; and (ii) aggregate fluctuationscan arise from idiosyncratic shocks due to input-output linkages across the economy.Firm linkages are approximately three times as important as the direct effect of firmshocks in driving aggregate fluctuations.

Upper Catfish Creek is located in a 9,300-acre watershed that flows through two significant natural resources, Swiss Valley Park and Swiss Valley Nature Preserves, one of the largest nature preserves in the Midwest. According to DNR’s 2002 305(d) report, that portion of the creek within the park and preserve is classified as a Class B(CW) cold water stream of which a portion has naturally reproducing trout (one of only 30 in the state of Iowa with this capability). Urban sprawl is a real threat to the Upper Catfish Creek Watershed. Currently, 10% of the watershed is residential, but 27% is zoned residential or commercial. The watershed is near Dubuque city limits but the jurisdiction is in the county. Differing criteria for land development between city and county jurisdictions further entices developers to build in outlying areas. County leaders agree there is more that needs to be done and will work with municipalities on uniformity of regulations and follow-up measures. We propose to set up key urban conservation practice models that will address storm water runoff and water quality which can be learned about and viewed by city and county officials, engineers, developers, etc. This would be part of a larger initiative including an educational campaign, inter-jurisdictional planning, the development of a land use GIS database, and agricultural conservation practices. The successes coming out of and learned about this watershed will serve as a model to spread county-wide.
