Considering that the number of day-care centers for pre-school-age children has expanded rapidly in developing countries, and that these institutions presenting conditions that facilitate the transmission of many enteric agents, a parasitological survey was carried out in three municipal day-cares from Botucatu: two in the urban area (one in downtown area and the other one in the city periphery area) and the third in the rural area. Three separate stool specimens were collected from 147 children ranging from 0 to 72 months old and 20 staff members. Each stool specimen was processed by Lutz and zinc sulfate flotation methods. The frequency of giardiasis observed among children of downtown, periphery and rural day-cares was 69.6%, 52.7% and 69.6%, respectively. Only one employee was positive for G. lamblia. The examination of three stool specimens increased the positivity for G. lamblia: from the ninety three final positive examinations, 24 (25.5%) and 8 (8.5%) were positives only after examination of the second and third samples, respectively. Others intestinal organisms like Ascaris lumbricoides (20.4%), Trichuris trichiura (19.0%). Hymenolepis nana (8.8%), Entamoeba coli (22.4%) and Blastocystis hominis (32.0%) were frequently found in the children. There was no significant association among localization of the day-cares, sex of the children and the levels of G. lamblia infection. According to the age, G. lamblia was found mainly in children between 12 to 47 months old.

Pinworm infection was prospectively studied during one year in 469 children attending three day care centers. Each child was examined at six months intervals using up to three perianal swabs with adhesive tape. Those found infected were treated with mebendazole. At the beginning of the study we found a prevalence of 28% that dropped to 13% and 12% in the following study periods. The reinfection rate was twice the incidence rate in both study periods. We also found a small percentage (10%) of the children reinfected in most or all study periods. There was a high correlation between reinfection and perianal itching. Our results add further knowledge to the epidemiology of intestinal parasites in day care centers.

The prevalence of antibodies against cytomegalovirus (CMV) and the incidence of CMV infection were tested in 98 children aged 5 to 36 months who attended the day-care center of a University hospital in São Paulo. At the beginning of the study the overall prevalence of anti-CMV IgG antibodies was 44% (43/98). Saliva and/or urine samples were obtained from 38 of the 43 children that were seropositive at the beginning of the study for isolation of the virus, and 52.6% of these children were found to excrete CMV in one of the two materials. Among the 37 children that were initially seronegative from whom it was possible to obtain a new blood sample 6 to 12 months later, 22 (59.5%) presented seroconversion. The rate of viral excretion through urine or saliva from the children that seroconverted was 50%. These results indicate that CMV infection is frequent and occurs early among the children who attend this day-care center. However, controlled studies using molecular epidemiology techniques are needed to define more precisely the role of day-care centers in CMV dissemination.

RESUMO: Várias intervenções psiquiátricas e psicossociais têm demonstrado ser eficazes na redução da depressão e da ansiedade em indivíduos que sofreram um acidente coronário agudo. A possiblidade de modificarem a evolução da doença coronária e de reduzirem a mortalidade cardíaca continua, no entanto, por comprovar. Este estudo teve como principal objectivo avaliar a efectividade de uma intervenção de Psiquiatria de Ligação (PL) em doentes com cardiopatia isquémica aguda. Métodos: um grupo inicial de 129 doentes consecutivos, internados por Enfarte Agudo do Miocárdio (EAM) ou Angina Instável, numa unidade de cuidados intensivos foi avaliado com a Hospital Anxiety e Depression Scale (HADS). Os doentes que apresentaram uma pontuação ≥8 nas subescalas da Depressão ou da Ansiedade (n=72) foram aleatoriamente distribuídos por grupo de intervenção (GI) (n=37) e grupo de controlo (GC) (n=35). O GI foi sujeito a uma intervenção de PL, realizada durante o período hospitalar, que se iniciou nos primeiros dias de internamento e consistiu em pelo menos 3 sessões individuais (incluindo avaliação psiquiátrica, psicoterapia de suporte, intervenção psicoeducativa e medicação, quando necessário). A sessão pré-alta envolveu o cônjuge e abordou a modificação de comportamentos de risco, a adesão terapêutica e o regresso ao trabalho. O GC recebeu os cuidados habituais da unidade de internamento. Todos os doentes completaram uma entrevista inicial para avaliação do estado cognitivo (Mini Mental State Examination – MMSE), do ajustamento social (Social Problems Questionnaire – SPQ) e de aspectos sociodemográfi cos e clínicos. Os doentes foram reavaliados antes da alta, aos 45 dias, 3 e 6 meses com a HADS, o SPQ e ainda com o Nottingham Health Profi le (NHP) para avaliação da qualidade de vida. No follow-up de 6 meses foi colhida informação sobre sobrevivência, número e duração de reinternamentos, número de dias de baixa e regresso ao trabalho. Resultados: na amostra de 129 doentes avaliados no início do internamento, 20,9% apresentavam níveis de depressão ≥8 na subescala da Depressão (HADS), 53,5% níveis de ansiedade ≥8 na subescala da Ansiedade (HADS) e 9,3% perturbações cognitivas (MMSE). A avaliação longitudinal desta amostra mostrou que os níveis de depressão, inicialmente baixos, aumentaram nos 45 dias após o internamento, para depois diminuírem até ao fi nal do follow-up. Os níveis de ansiedade, que eram inicialmente altos, aumentaram nos 45 dias seguintes e antiveram- se estáveis, mas altos, até ao fi m do estudo. O GI apresentou uma pontuação média na subescala da depressão signifi cativamente inferior à do GC no follow-up de 6 meses (5,8±4,1 no GI vs. 7,9±4,3 no GC, p=0,04). O número de doentes deprimidos foi signifi cativamente menor no GI nas avaliações realizadas aos 3 meses (11 vs. 18 no GC, p=0,04) e aos 6 meses (12 vs. 18 no GC, p= 0,05). O mesmo aconteceu com o número de doentes ansiosos aos 3 meses (15 no GI vs. 23 no GC, p=0,01). As dimensões do NHP “Isolamento social” aos 45 dias e “Reacção emocional” aos 45 dias e aos 3 meses, bem como a qualidade de vida geral (NHP 2ª parte) aos 3 meses, mostraram melhoria signifi cativa no grupo de intervenção. Embora a intervenção tenha reduzido o nível médio da ansiedade nas várias avaliações após a alta, esta redução não atingiu signifi cância estatística. A intervenção realizada não teve impacto na mortalidade ou nas variáveis relacionadas com a evolução da doença cardíaca no período do follow-up. Conclusões: Os resultados do presente estudo mostram a alta prevalência de depressão e de ansiedade após um acidente coronário agudo e a manutenção de níveis altos de ansiedade nos 6 meses seguintes. Os resultados comprovam também a efectividade de uma intervenção em PL no tratamento da depressão e da ansiedade em doentes que sofreram um acidente coronário agudo. Estes resultados apontam para a necessidade de desenvolvimento de programas de PL para este tipo de doentes, tanto no hospital geral como nos cuidados de saúde primários. Sugerem ainda a necessidade de desenvolvimento de investigação que permita estabelecer o impacto específi co dos diversos tipos de intervenção, assim como compreender os mecanismos subjacentes à associação da depressão e da ansiedade com a doença coronária.----------ABSTRACT:Different types of psychiatric and psychochosocial interventions have proven effi cacy in decreasing anxiety and depression in coronary heart disease. There is, however, an ongoing discussion about the impact these interventions may have on the clinical outcome and on cardiac mortality. The main objective of the current study was to evaluate the effectiveness of a consultation liaison psychiatry (CL) intervention on a group of patients admitted with Myocardial Infarction or Unstable Angina, to a Coronary Care Unit. Methods: The study had a prospective, randomised, controlled design, with a 6-month follow-up. One hundred and twenty-nine consecutive patients were assessed during the first 48 hours of admission with the Hospital Anxiety and Depression Scale (HADS). Those with a score of ≥8 on the Depression or the Anxiety subscales (n=72) were randomly allocated to intervention (n=37) and usual care (n=35). The CL intervention, started during the fi rst days of admission, had a minimum of 3 (60 minutes) sessions, and included a psychiatric evaluation, supportive psychotherapy, a psychoeducational intervention, when necessary, psychotropic drugs. The last session, shortly before discharge, included the spouse and was focused on compliance, modifi cation of behavioral risk factors, and possible diffi culties upon returning to work. Cognitive status (Mini-Mental State Examination - MMSE), social adjustment (Social Problems Questionnaire - SPQ), and demographic and clinical characteristics were also assessed at baseline. Patients were reassessed before discharge, and at 45 days, 3 and 6 months after admission with HADS, SPQ, and with Nottingham Health Profile (NHP) for quality of life. Survival, number of readmissions and days of readmission, number of sickleave days and return to work were assessed at six months. Results: The initial sample of 129 patients, presented a 20.9% prevalence of depressive symptoms, 53.5% of anxiety symptoms, and 9.3% of cognitive disorders. The longitudinal evaluation of this sample showed that the initially low levels of depression were increased 45 days later, and slowlly decreased afterwards till the 6-month follow-up. Initially high anxiety levels, somewhat decreased before discharge, had increased 45 days later, and stayed stable and high till the end of the study. The intervention group showed a signifi cantly lower depression mean score at 6 months (5.8±4.1 vs. 7.9±4.3 in the controls, p=0.04). The number of patients considered depressed was lower in the intervention group at 3(11 vs. 18 controls, p=0.04) and 6 months (12vs. 18 controls, p=0,05). The number of anxious patients was also lower in the intervention.

An individual experiences double coverage when he bene ts from more than one health insurance plan at the same time. This paper examines the impact of such supplementary insurance on the demand for health care services. Its novelty is that within the context of count data modelling and without imposing restrictive parametric assumptions, the analysis is carried out for di¤erent points of the conditional distribution, not only for its mean location. Results indicate that moral hazard is present across the whole outcome distribution for both public and private second layers of health insurance coverage but with greater magnitude in the latter group. By looking at di¤erent points we unveil that stronger double coverage e¤ects are smaller for high levels of usage. We use data for Portugal, taking advantage of particular features of the public and private protection schemes on top of the statutory National Health Service. Nosocomial infection among male patients in a public psychiatric hospital was studied and the definitions for use in long-term care facilities were employed for diagnosis. The overall nosocomial infection rate was 6.7 per 1,000 day inpatients; 55.6% of these infections were identified in the respiratory tract, 50% of them being respiratory viral diseases; 38.9% of the nosocomial infections involved the eyes, ears, nose, throat and mouth, and 5.6% involved the skin and soft tissues. The epidemiological characteristics and the main clinical alterations of these inpatients were also identified.

Cytomegalovirus (CMV) infection is the most common congenital infection, affecting 0.4% to 2.3% newborns. Most of them are asymptomatic at birth, but later 10% develop handicaps, mainly neurological disturbances. Our aim was to determine the prevalence of CMV shed in urine of newborns from a neonatal intensive care unit using the polymerase chain reaction (PCR) and correlate positive cases to some perinatal aspects. Urine samples obtained at first week of life were processed according to a PCR protocol. Perinatal data were collected retrospectively from medical records. Twenty of the 292 cases (6.8%) were CMV-DNA positive. There was no statistical difference between newborns with and without CMV congenital infection concerning birth weight (p=0.11), gestational age (p=0.11), Apgar scores in the first and fifth minutes of life (p=0.99 and 0.16), mother's age (p=0.67) and gestational history. Moreover, CMV congenital infection was neither related to gender (p=0.55) nor to low weight (<2,500g) at birth (p=0.13). This high prevalence of CMV congenital infection (6.8%) could be due to the high sensitivity of PCR technique, the low socioeconomic level of studied population or the severe clinical status of these newborns.

This study evaluates the transmission of CMV infection in 120 children aged 1 to 15 years with Down syndrome who attended a day-care center for handicapped children in São Paulo, Brazil. A blood sample was obtained from each children at the beginning of the study for detection of IgG and IgM cytomegalovirus (CMV) antibodies by an immunofluorescence assay. Samples of saliva and urine were obtained every 3 months from the children with CMV antibodies to detect shedding of the virus by culture in human foreskin fibroblasts, by detection of pp65 CMV-antigen and by a nested PCR assay. The prevalence of anti CMV-IgG antibodies was 76.6% (92/120), and IgM anti-CMV antibodies were detected in 13% (12/92) of the seropositive children. During the first viral evaluation, CMV was detected in the urine and/or saliva in 39/90 (43.3%) of the seropositive children. In the second and third evaluations, CMV was detected in 41/89 (46%) and in 35/89 (39.3%) children, respectively. Detection of CMV was shown both in urine and saliva in 28/39 (71.8%), 19/41(46.3%) and 20/35 (57.1%) of the children excreting the virus, respectively. Additionally, in 33/49 (67.4%) of the excreters CMV could be demonstrated in urine or saliva in at least two out of the three virological evaluations carried out sequentially in a six month period. Of the 28 initially seronegative children, 26 were re-examined for anti-CMV IgG antibodies about 18 months after the negative sample; seroconversion was found in 10/26 (38.5%). Taking all 536 samples of urine or saliva examined by virus culture and pp65 antigen detection during the study into account, 159 (29.6%) were positive by virus culture and 59 (11%) gave a positive result with the pp65 assay. These data demonstrate the high prevalence of CMV shedding and the high risk of CMV infection in children with Down syndrome attending a day-care center for mentally handicapped patients. The virus culture was more sensitive than the pp65 CMV antigen assay for CMV detection in both urine and saliva samples.

Whereas child care quality has been extensively studied in the U.S., there is much less information about the quality of child care in other countries.With one of the highest maternal employment rates in Europe, it is important to examine child care in Portugal. Thirty toddler classrooms in child care centers were observed. The purpose of this studywas to determine whether structural features account for overall toddler child care quality. Results showed younger and better-paid teachers provided better toddler child care quality. Space available per childwas not a statistically significant predictor of toddler child care quality.Overall quality results suggest some issues to be addressed by early education policy makers and indicate the need to promote quality in Portuguese toddler child care programs.

A total of 730 children aged less than 7 years, attending 8 day-care centers (DCCs) in Belém, Brazil were followed-up from January to December 1997 to investigate the occurrence of human-herpes virus 6 (HHV-6) infection in these institutional settings. Between October and December 1997 there have been outbreaks of a febrile- and -exanthematous disease, affecting at least 15-20% of children in each of the DCCs. Both serum- and- plasma samples were obtained from 401 (55%) of the 730 participating children for the detection of HHV-6 antibodies by enzyme-linked immunosorbent assay (ELISA), and viral DNA amplification through the nested-PCR. Recent HHV-6 infection was diagnosed in 63.8% (256/401) of them, as defined by the presence of both IgM and IgG-specific antibodies (IgM+/IgG+); of these, 114 (44.5%) were symptomatic and 142 (55.5%) had no symptoms (p = 0.03). A subgroup of 123 (30.7%) children were found to be IgM-/IgG+, whereas the remaining 22 (5.5%) children had neither IgM nor IgG HHV-6- antibodies (IgM-/IgG-). Of the 118 children reacting strongly IgM-positive ( > or = 30 PANBIO units), 26 (22.0%) were found to harbour the HHV-6 DNA, as demonstrated by nested-PCR. Taken the ELISA-IgM- and- nested PCR-positive results together, HHV-6 infection was shown to have occurred in 5 of the 8 DCCs under follow-up. Serological evidence of recent infections by Epstein-Barr virus (EBV) and parvovirus B19 were identified in 2.0% (8/401) and 1.5% (6/401) of the children, respectively. Our data provide strong evidence that HHV-6 is a common cause of outbreaks of febrile/exanthematous diseases among children attending DCCs in the Belém area.

The importance of hands in the transmission of nosocomial infection has been world wide admitted. However, it is difficult to induce this behavior in health-care workers. The aim of the present work was to point out the importance of hand bacteria colonization, the influence of hand washing and of patient physical examination. One hundred health-care workers were randomly divided in two groups: Group A without hand washing previous to patient physical examination or handling (PPE); group B with hand washing previous to PPE. Direct fingerprint samples in Columbia agar before and after PPE were obtained. The colonies were counted and identified by conventional techniques, and antibiograms according to NCCLS were performed. Before PPE group A participants showed a high number of bacteria regarding group B participants (73.9 Vs 20.7; p < 0.001); 44 out of 50 participants were carriers of potentially pathogen bacteria. No group B participants were carriers of potential pathogen bacteria before PPE. The latter group showed an increase in number of bacteria after PPE (20.7 CFU (before) Vs 115.9 CFU (after); p < 0.001). Sixteen group B participants were contaminated after PPE with potential pathogens such as S. aureus (50% of them meticillin resistant); Escherichia coli, Pseudomonas aeruginosa and Enterococcus faecalis, half of them multiresistant. We can conclude on the importance of these results to implement educational programs and to provide the health-care workers with the proper commodities to fulfill this practice.

Evidence indicates that exposure to high levels of noise adversely affects human health, and these effects are dependent upon various factors. In hospitals, there are many sources of noise, and high levels exert an impact on patients and staff, increasing both recovery time and stress, respectively. The goal of this pilot study was to develop, implement and evaluate the effectiveness of a training program (TP) on noise reduction in a Neonatal Intensive Care Units (NICU) by comparing the noise levels before and after the implementation of the program. A total of 79 health professionals participated in the study. The measurements of sound pressure levels took into account the layout of the unit and location of the main sources of noise. General results indicated that LAeq levels before implementation of the training program were often excessive, ranging from 48.7 ± 2.94 dBA to 71.7 ± 4.74 dBA, exceeding international guidelines. Similarly following implementation of the training program noise levels remained unchanged (54.5 ± 0.49 dBA to 63.9 ± 4.37 dBA), despite a decrease in some locations. There was no significant difference before and after the implementation of TP. However a significant difference was found for Lp, Cpeak, before and after training staff, suggesting greater care by healthcare professionals performing their tasks. Even recognizing that a TP is quite important to change behaviors, this needs to be considered in a broader context to effectively control noise in the NICU.
