Environmental contamination with Mycobacterium tuberculosis complex (MTC) has been considered crucial for bovine tuberculosis persistence in multi-host-pathogen systems. However, MTC contamination has been difficult to detect due to methodological issues. In an attempt to overcome this limitation we developed an improved protocol for the detection of MTC DNA. MTC DNA concentration was estimated by the Most Probable Number (MPN) method. Making use of this protocol we showed that MTC contamination is widespread in different types of environmental samples from the Iberian Peninsula, which supports indirect transmission as a contributing mechanism for the maintenance of bovine tuberculosis in this multi-host-pathogen system. The proportion of MTC DNA positive samples was higher in the bovine tuberculosis-infected than in presumed negative area (0.32 and 0.18, respectively). Detection varied with the type of environmental sample and was more frequent in sediment from dams and less frequent in water also from dams (0.22 and 0.05, respectively). The proportion of MTC-positive samples was significantly higher in spring (p<0.001), but MTC DNA concentration per sample was higher in autumn and lower in summer. The average MTC DNA concentration in positive samples was 0.82 MPN/g (CI95 0.70-0.98 MPN/g). We were further able to amplify a DNA sequence specific of Mycobacterium bovis/caprae in 4 environmental samples from the bTB-infected area.

BACKGROUND: The zoonosis bovine tuberculosis (TB) is known to be responsible for a considerable proportion of extrapulmonary TB. In Mozambique, bovine TB is a recognised problem in cattle, but little has been done to evaluate how Mycobacterium bovis has contributed to human TB. We here explore the public health risk for bovine TB in Maputo, by characterizing the isolates from tuberculous lymphadenitis (TBLN) cases, a common manifestation of bovine TB in humans, in the Pathology Service of Maputo Central Hospital, in Mozambique, during one year. RESULTS: Among 110 patients suspected of having TBLN, 49 had a positive culture result. Of those, 48 (98 %) were positive for Mycobacterium tuberculosis complex and one for nontuberculous mycobacteria. Of the 45 isolates analysed by spoligotyping and Mycobacterial Interspersed Repetitive Unit - Variable Number Tandem Repeat (MIRU-VNTR), all were M. tuberculosis. No M. bovis was found. Cervical TBLN, corresponding to 39 (86.7 %) cases, was the main cause of TBLN and 66.7 % of those where from HIV positive patients. We found that TBLN in Maputo was caused by a variety of M. tuberculosis strains. The most prevalent lineage was the EAI (n?=?19; 43.2 %). Particular common spoligotypes were SIT 48 (EAI1_SOM sublineage), SIT 42 (LAM 9), SIT 1 (Beijing) and SIT53 (T1), similar to findings among pulmonary cases. CONCLUSIONS: M. tuberculosis was the main etiological agent of TBLN in Maputo. M. tuberculosis genotypes were similar to the ones causing pulmonary TB, suggesting that in Maputo, cases of TBLN arise from the same source as pulmonary TB, rather than from an external zoonotic source. Further research is needed on other forms of extrapulmonary TB and in rural areas where there is high prevalence of bovine TB in cattle, to evaluate the risk of transmission of M. bovis from cattle to humans.

OBJETIVO: Investigar a presença de transtornos depressivos em crianças portadoras de leucemia linfoide aguda (LLA) e insuficiência renal crônica terminal (IRCT) atendidas no IMIP. MÉTODO: Estudo descritivo do tipo série de casos, composto por 52 crianças entre 8 e 15 anos portadoras de LLA e de IRCT. RESULTADOS: Três (5,8%) casos preenchiam os critérios para episódio depressivo maior (EDM), sendo dois portadores de IRCT e um portador de LLA. Oito (15,4%) preenchiam os critérios para transtorno distímico (TD), todos eles portadores de IRCT. A associação entre faixa etária e EDM não foi significativa (p=0,327). Entretanto, a faixa etária foi significante em relação ao TD (p=0,014), todos os seus portadores tinham entre 12 e 15 anos de idade. A associação entre os transtornos depressivos e o tempo de evolução da doença de base não foi significante. Contudo, observou-se uma tendência a quanto maior o tempo de evolução da doença de base, maior a associação com o TD. CONCLUSÃO: A frequência de EDM ficou dentro da faixa encontrada na literatura para escolares saudáveis, entretanto, a de TD foi mais alta. Não foram encontradas diferenças significantes entre as faixas etárias no diagnóstico de EDM. Porém, corroborando a literatura, a faixa etária maior prevaleceu em relação ao TD.

Renal cell tumors (RCTs) are the most lethal of the common urological cancers. The widespread use of imaging entailed an increased detection of small renal masses, emphasizing the need for accurate distinction between benign and malignant RCTs, which is critical for adequate therapeutic management. Histone methylation has been implicated in renal tumorigenesis, but its potential clinical value as RCT biomarker remains mostly unexplored. Hence, the main goal of this study was to identify differentially expressed histone methyltransferases (HMTs) and histone demethylases (HDMs) that might prove useful for RCT diagnosis and prognostication, emphasizing the discrimination between oncocytoma (a benign tumor) and renal cell carcinoma (RCC), especially the chromophobe subtype (chRCC). We found that the expression levels of three genes-SMYD2, SETD3, and NO66-was significantly altered in a set of RCTs, which was further validated in a large independent cohort. Higher expression levels were found in RCTs compared to normal renal tissues (RNTs) and in chRCCs comparatively to oncocytomas. SMYD2 and SETD3 mRNA levels correlated with protein expression assessed by immunohistochemistry. SMYD2 transcript levels discriminated RCTs from RNT, with 82.1% sensitivity and 100% specificity (AUC=0.959), and distinguished chRCCs from oncocytomas, with 71.0% sensitivity and 73.3% specificity (AUC: 0.784). Low expression levels of SMYD2, SETD3, and NO66 were significantly associated with shorter disease-specific and disease-free survival, especially in patients with non-organ confined tumors. We conclude that expression of selected HMTs and HDMs might constitute novel biomarkers to assist in RCT diagnosis and assessment of tumor aggressiveness.

Objetivos Comparar a ocorrência de sintomas depressivos entre dois grupos homogêneos de pacientes em diálise, um em hemodiálise (HD) e outro em diálise peritoneal (DP), verificando o possível papel preditor do método, assim como avaliar a influência de variáveis sociodemográficas e clínicas no diagnóstico deste transtorno. Métodos Amostra envolveu dois grupos homogêneos de pacientes em TRS, 30 em HD e 30 em DP. Aplicou-se o Beck Depression Inventory (BDI), escala que avalia a presença e intensidade de sintomas depressivos. Resultados Os grupos apresentavam características sociodemográficas e clínicas semelhantes, exceto quanto à escolaridade. A média de pontos do BDI no grupo HD foi maior que no grupo DP: 12,53 versus 11,13 (p = 0,352). A presença de sintomas depressivos no grupo em HD ocorreu em 36,7% dos pacientes contra apenas 23,3% do grupo DP (OR 1,9 [IC 0,61 – 5,86]). Não houve diferenças quanto às variáveis, exceto escolaridade. Dos pacientes com ensino fundamental I, 52,9% apresentaram sintomas. Nos pacientes com ensino médio ou superior, a ocorrência de sintomas depressivos foi 20% (p = 0,051). Conclusão Houve tendência à ocorrência de sintomas depressivos em pacientes em HD quando comparados aos pacientes em DP. O risco dessa ocorrência foi quase duas vezes maior nos em HD. Menor escolaridade associou-se à ocorrência de sintomas. As médias da pontuação do BDI nos dois grupos não foram significantes.

The aim of this study was to determine if mycobacterial lineages affect infection risk, clustering, and disease progression among Mycobacterium tuberculosis cases in The Netherlands. Multivariate negative binomial regression models adjusted for patient-related factors and stratified by patient ethnicity were used to determine the association between phylogenetic lineages and infectivity (mean number of positive contacts around each patient) and clustering (as defined by number of secondary cases within 2 years after diagnosis of an index case sharing the same fingerprint) indices. An estimate of progression to disease by each risk factor was calculated as a bootstrapped risk ratio of the clustering index by the infectivity index. Compared to the Euro-American reference, Mycobacterium africanum showed significantly lower infectivity and clustering indices in the foreign-born population, while Mycobacterium bovis showed significantly lower infectivity and clustering indices in the native population. Significantly lower infectivity was also observed for the East African Indian lineage in the foreign-born population. Smear positivity was a significant risk factor for increased infectivity and increased clustering. Estimates of progression to disease were significantly associated with age, sputum-smear status, and behavioral risk factors, such as alcohol and intravenous drug abuse, but not with phylogenetic lineages. In conclusion, we found evidence of a bacteriological factor influencing indicators of a strain's transmissibility, namely, a decreased ability to infect and a lower clustering index in ancient phylogenetic lineages compared to their modern counterparts. Confirmation of these findings via follow-up studies using tuberculin skin test conversion data should have important implications on M. tuberculosis control efforts.

OBJECTIVE: To analyze the impact of acute renal failure (ARF) on the evolution of infants undergoing cardiac surgery. METHODS: We assessed 15 infants undergoing cardiac surgery who developed (ARF). Their demographic, clinical and surgical data, and evolution were analyzed. RESULTS: Their mean age was 4.4±4.0 months (8 days to 24 months). Twelve infants were males, and 4 patients already had ARF at surgery. The primary cause of ARF was immediate acute cardiac dysfunction in 10 infants, cardiac dysfunction associated with sepsis in 2 infants, and isolated sepsis in 3 infants. All children depended on mechanical ventilation during their postoperative period, 14 infants used vasoactive drugs, and 11 had an infectious process associated with ARF. Thirteen infants required dialytic treatment. Eleven infants developed oluguric ARF, and all had to undergo peritoneal dialysis; of the 4 patients with non-oliguric, 2 required dialysis, the main indication being hypervolemia. Of these 13 dialyzed infants, 4 died in the first 24 hours because of the severity of the underlying cardiac disease (mean urea level of 49±20 mg/dl). The mortality rate for the entire group was 60% , and it was higher among the patients with oliguria ARF (73% vs 25%, p<0.001). The cause of death was acute cardiac dysfunction in 6 infants (early type-1ARF) and sepsis in the 3 remaining infants (late type-2 ARF). CONCLUSION: The mortality rate of ARF associated with cardiac surgery in infants was hight, being higher among children with oliguria; peritoneal dialysis was indicated due to clinically uncontrolled hypervolemia and not to the uremic hypercatabolic state.

OBJECTIVE: To assess the results of surgical myocardial revascularization in renal transplant patients. METHODS: From 1991 to 2000, 11 renal transplant patients, whose ages ranged from 36 to 59 (47.5±8) years, 8 males and 3 females, underwent myocardial revascularization. The time interval between renal transplantation and myocardial revascularization ranged from 25 to 120 (mean of 63.8±32.7) months. RESULTS: The in-hospital mortality rate was 9%. One patient died on the 4th postoperative day from septicemia and respiratory failure. The mean graft/patient ratio was 2.7±0.8. Only 1 patient required slow hemodialysis during 24 hours in the postoperative period, and no patient had a definitive renal lesion or lost the transplanted kidney. The actuarial survival curves after 1, 2, and 3 years were, respectively, 90.9%, 56.8%, and 56.8%. CONCLUSION: Renal transplant patients may undergo myocardial revascularization with no lesion in or loss of the transplanted kidney.

OBJECTIVE: To assess the acute effects of high glucose concentrations on vascular reactivity in the isolated non diabetic rabbit kidney. METHODS: Rabbits were anaesthetized for isolation of the kidneys. Renal arteries and veins were cannulated for perfusion with Krebs-Henselleit solution and measurement of perfusion pressure. After 3 hours of perfusion with glucose 5,5 mM (control ) and 15 mM, the circulation was submitted to sub maximal precontraction (80% of maximal response) trough continuous infusion of noradrenaline 10 mM. Vascular reactivity was then assessed trough dose-responses curves with endothelium-dependent (acetylcholine) and independent (sodium nitroprusside) vasodilators. The influence of hyperosmolarity was analyzed with perfusion with mannitol 15mM. RESULTS: A significant reduction in the endothelium-dependent vasodilation in glucose 15mM group was observed compared to that in control, but there was no difference in endothelium-independent vasodilation. After perfusion with mannitol 15 mM, a less expressive reduction in endothelium-dependent vasodilation was observed, only reaching significance in regard to the greatest dose of acetylcholine. CONCLUSION: High levels of glucose similar to those found in diabetic patients in the postprandial period can cause significant acute changes in renal vascular reactivity rabbits. In diabetic patients these effects may also occur and contribute to diabetes vascular disease.

OBJETIVO: Identificar a incidência, fatores de risco e mortalidade de insuficiência renal aguda (IRA), em pacientes submetidos à cirurgia para revascularização miocárdica com circulação extracorpórea. MÉTODOS: Estudados prospectivamente todos os pacientes submetidos a cirurgia de revascularização miocárdica e as variáveis clínicas e laboratoriais analisadas através de métodos uni e multivariado (regressão logística). RESULTADOS: Insuficiência renal aguda ocorreu em 16,1% dos 223 pacientes estudados, diálise foi necessária em 4,9% dos pacientes. Os fatores de risco associados à IRA na análise univariada foram: idade > 63 anos OR 3,6 (95% IC=1,6 a 8,3), creatinina sérica pré-operatória > 1,2 mg/dl OR 5,9 (95% IC=2,4 a 14,6), duração da circulação extracorpórea > 90 min OR 2,1 (95% IC=1,0 a 4,4), uso de balão intra-aórtico OR 2,6 (95% IC=1,2 a 5,5); necessidade de drogas inotrópicas OR 4,4 (95% IC=1,9 a 10,2) e, na análise multivariada, foram fatores independentes associados à IRA idade > 63 anos OR 3,0 (95% IC=1,3 a 7,2), creatinina sérica pré-operatória > 1,2 mg/dl OR 4,3 (95% IC=1,6 a 11,4), necessidade de drogas inotrópicas OR 3,2 (95% IC=1,3 a 8,0). A mortalidade nos pacientes com IRA foi de 25,0 % em comparação com 1,1% entre os sem IRA e 63,6% entre os que necessitaram de diálise. CONCLUSÃO: Insuficiência renal aguda em cirurgia de revascularização miocárdica é uma complicação freqüente e está associada à alta mortalidade. Sendo fatores de risco independentes: idade, insuficiência renal prévia e necessidade de drogas inotrópicas.

OBJETIVO: Determinar a prevalência de doença cardiovascular (DCV) e de fatores de risco tradicionais em portadores de insuficiência renal crônica em avaliação para inclusão em lista para transplante renal. MÉTODOS: Foram submetidos à avaliação clínica e exames complementares 195 pacientes com insuficiência renal crônica dialítica e comparados a grupo de 334 hipertensos pareados por idade. As equações de Framingham foram usadas para o cálculo do risco absoluto (RA); o risco relativo (RR) foi calculado tendo como referência o risco absoluto da coorte de baixo risco de Framingham. RESULTADOS: Do total, 37% apresentaram algum tipo de doença cardiovascular na avaliação inicial, sendo que arteriopatia obstrutiva (23%) foi a mais prevalente. Excluídos os pacientes com doença cardiovascular, em relação aos fatores de risco tradicionais, houve diferença significativa quanto à pressão arterial sistólica e colesterol total (maiores no grupo de hipertensos) e às prevalências de homens, diabetes e tabagismo, maiores no grupo de insuficiência renal crônica, que apresentou maior grau de hipertrofia ventricular esquerda, menor pressão arterial diastólica e menor prevalência de história familiar de doença cardiovascular e obesidade. O risco relativo para doença cardiovascular dos pacientes com insuficiência renal crônica foi mais elevado em relação à população controle de Framingham porém não diferiu da observada no grupo de hipertensos. CONCLUSÃO: Em candidatos a transplante renal é significativa a prevalência de doença cardiovascular e de fatores de risco tradicionais; as equações de Framingham não quantificam adequadamente o risco cardiovascular real e outros fatores de risco específicos desta população devem contribuir para o maior risco cardiovascular.

Mulher de 60 anos com quadro de insuficiência cardíaca classe funcional III/IV (NYHA) de etiologia imuno-mediada (miocardite autoimune). Após tentativas terapêuticas sem sucesso, foi utilizada imunoglobulina intravenosa, deteriorando a função renal, complicação rara desta terapia. Após hemodiálise a paciente recuperou a função renal e ocorreu melhora da insuficiência cardíaca crônica para classe funcional I.

A endomiocardiofibrose (EMF) é uma cardiomiopatia restritiva, caracterizada por envolvimento fibrótico do endocárdio e miocárdio adjacente, e disfunção diastólica, causada por alteração da distensibilidade, dificultando o enchimento ventricular adequado, estando a função sistólica preservada. Clinicamente, apresenta-se como insuficiência cardíaca, mas, para o diagnóstico etiológico correto, são necessárias perspicácia semiológica, suspeita e investigação clínica. Relatamos o caso de uma paciente portadora de endomiocardiofibrose biventricular associada à amiloidose renal.
