OBJECTIVE: Doppler tissue imaging (DTI) enables the study of the velocity of contraction and relaxation of myocardial segments. We established standards for the peak velocity of the different myocardial segments of the left ventricle in systole and diastole, and correlated them with the electrocardiogram. METHODS: We studied 35 healthy individuals (27 were male) with ages ranging from 12 to 59 years (32.9 ± 10.6). Systolic and diastolic peak velocities were assessed by Doppler tissue imaging in 12 segments of the left ventricle, establishing their mean values and the temporal correlation with the cardiac cycle. RESULTS: The means (and standard deviation) of the peak velocities in the basal, medial, and apical regions (of the septal, anterior, lateral, and posterior left ventricle walls) were respectively, in cm/s, 7.35(1.64), 5.26(1.88), and 3.33(1.58) in systole and 10.56(2.34), 7.92(2.37), and 3.98(1.64) in diastole. The mean time in which systolic peak velocity was recorded was 131.59ms (±19.12ms), and diastolic was 459.18ms (±18.13ms) based on the peak of the R wave of the electrocardiogram. CONCLUSION: In healthy individuals, maximum left ventricle segment velocities decreased from the bases to the ventricular apex, with certain proportionality between contraction and relaxation (P<0.05). The use of Doppler tissue imaging may be very helpful in detecting early alterations in ventricular contraction and relaxation.

OBJECTIVE: The initial site of myocardial infarction (MI) may influence the prevalence of ventricular late potentials (VLP), high-frequency signals, due to the time course of ventricular activation. The prevalence of VLP in a period of more than 2 years after acute MI was assessed focusing on the initially injured wall . METHODS: The prevalence of VLP in a late phase after MI (median of 924 days) in anterior/antero-septal and inferior/infero-dorsal wall lesion was analyzed using signal-averaged electrocardiogram in time domain. The diagnostic performance of the filters employed for analysis on was tested at high-pass cut-off frequencies of 25 Hz, 40 Hz and 80 Hz. RESULTS: The duration of the ventricular activation and its terminal portion were larger in inferior than anterior infarction, at high-pass cut-off frequencies of 40 Hz and 80 Hz. In patients with ventricular tachycardia, these differences were more remarked. The prevalence of ventricular late potentials was three times greater in inferior than anterior infarction. CONCLUSION: Late after myocardial infarction, the prevalence and the duration of ventricular late potentials are greater in lesions of inferior/infero-dorsal than anterior/antero-septal wall confirming their temporal process, reflecting their high-frequency content.

This paper presents a model predictive current control applied to a proposed single-phase five-level active rectifier (FLAR). This current control strategy uses the discrete-time nature of the active rectifier to define its state in each sampling interval. Although the switching frequency is not constant, this current control strategy allows to follow the reference with low total harmonic distortion (THDF). The implementation of the active rectifier that was used to obtain the experimental results is described in detail along the paper, presenting the circuit topology, the principle of operation, the power theory, and the current control strategy. The experimental results confirm the robustness and good performance (with low current THDF and controlled output voltage) of the proposed single-phase FLAR operating with model predictive current control.

OBJECTIVE - To evaluate the Coronary Flow Reserve in the Coronary Sinus through transesophageal Doppler echocardiography in normal subjects. METHODS - We obtained technically adequate flow samples for analysis in 10 healthy volunteers (37±8 years, 5 men) with no history of heart or systemic disease and with mean left ventricular mass index by transthoracic echocardiography of 87±18 g/m². Coronary sinus flow velocity was recorded within the coronary sinus with the patient in a resting condition and during intravenous adenosine infusion at a dose of 140 µg/kg/min for 4 minutes. Recording of coronary sinus blood flow was possible in all cases with measurement of peak systolic, diastolic, and retrograde velocities (PSV, PDV, and PRV, cm/sec), mean systolic and diastolic velocities (MSV and MDV, cm/sec), and systolic and diastolic velocity time integral (VTI S and VTI D, cm/sec). RESULTS - The coronary flow reserve was calculated as the ratio between the blood flow in the basal state and the maximum measured hyperemic blood flow with adenosine infusion. Results are shown as mean and standard deviations. (CFR = PSV + PDV -- PRV/basal PSV): 1st min = 2.2±0.21; 2nd min = 3±0.3; 3rd min = 3.4±0.37; 4th min = 3.6 ± 0.33. CONCLUSION - Although coronary sinus flow had significantly increased in the first minute, higher velocities were seen at third and fourth minutes, indicating that these should be the best times to study coronary sinus flow with intravenous adenosine in continuous infusion.

OBJECTIVE: To assess structural and functional cardiac changes in asymptomatic pregnant women with chronic arterial hypertension (CAH). METHODS: One hundred pregnant women with CAH underwent conventional Doppler echocardiography. The Student t test was used to compare them with 29 normotensive pregnant women (NT) in their third gestational trimester. RESULTS: Systolic (SBP; mmHg) and diastolic (DBP; mmHg) blood pressure values were higher (p<0.001) in the CAH pregnant women (SBP: 139±19 and DBP: 92± 18) as compared with those of the NT group (SBP: 112±10 and DBP: 74±9). A significant enlargement of the left atrium (4.10±0.48 cm vs 3.6±0.3 cm; p<0.001) and of the left ventricular normalized mass (59.6±19.7 g/cm2,7 vs 41.9±3.4 g/cm2,7; p<0.001) was observed. Cardiac output (CO, L/min) and systolic volume (SV, mL) were significantly higher in the CAH group (CO: 6.0±1.54 vs 4.9±2.1, p<0.01; SV: 77.3±19.8 vs 56.5±25.8, p<0.001). CONCLUSION: Chronic hypertensive pregnant women have structural and functional cardiac changes that justify routine cardiologic assessment, even in the absence of cardiopulmonary symptoms.

OBJECTIVE: To assess the usefulness of Doppler tissue imaging (DTI) for evaluating the systolic function of chagasic patients with and without electrocardiographic abnormalities, in comparision with echocardiographic study. METHODS: We studied 77 patients divided into 3 groups as follows: group 1 - control; group 2 - chagasic patients with normal electrocardiographic findings; and group 3 - chagasic patients with abnormal electrocardiographic findings. The following parameters were assessed: left ventricular dimensions and ejection fraction, left atrial dimensions and diastolic function on echocardiography. Systolic velocity and regional isovolumic contraction time (IVCTr) of the septal, anterior, lateral, posterior and inferior left ventricular walls were assessed on DTI. RESULTS: Left ventricular cavitary dimensions, ejection fraction and DTI systolic wave showed significant differences between groups 1 and 3 and between groups 2 and 3, which were not found between groups 1 and 2. IVCTr allowed a statistically significant discrimination among the 3 groups. CONCLUSION: DTI allowed discrimination among the different groups assessed, being superior to echocardiography in identifying early abnormalities of contractility, and, therefore, potentially useful for detecting incipient myocardial alterations in chagasic patients with normal electrocardiographic findings.

OBJECTIVE: To study echocardiographic parameters of left ventricular systolic function and valvar regurgitation under pharmacological influence in mildly symptomatic patients with chronic mitral regurgitation (MR). METHODS: We carried out a double-blind placebo controlled study in 12 patients with MR, mean aged 12.5 years old, who were randomized in 4 phases: A) digoxin; B) enalapril; C) digoxin + enalapril; D) placebo. The medication was administered for 30 days in each phase, and the following variables were analyzed: shortening and ejection fractions, wall stress index of left ventricle, left ventricular meridional end-systolic wall stress, Doppler-derived mean rate of left ventricular pressure rise (mean dP/dt), stroke volume and MR jet area. The clinical variables analysed were heart rate and systemic arterial pressure. RESULTS: No significant variation was observed in the clinical variables analysed. The shortening and ejection fraction, the mean dP/dt and stroke volume significantly increased and the wall stress index of left ventricle, the meridional left ventricular end systolic wall stress and the mitral regurgitation jet area decreased in the phases with medication as compared with that in the placebo phase. CONCLUSION: The parameters of left ventricular systolic function improved significantly and the degree of MR decreased with the isolated administration of digoxin or enalapril in mildly symptomatic patients with chronic MR. The combination of the drugs, however, did not show better results.

OBJECTIVE: To characterize left ventricular regional myocardial function through tissue Doppler echocardiography in healthy adults and to assess the influence of aging in this function. METHODS: In 45 healthy volunteers divided in two groups (< 45 and > 45 years old) we assessed longitudinal and radial regional function (velocities, times intervals and velocity-time integrals). Data were compared in each group and between groups. RESULTS: Systolic function: a) longitudinal: higher velocities and integrals in lateral and inferior walls and in basal segments, with a trend to reduction of these parameters with aging; b) radial: higher basal velocities, no significant change with aging. Diastolic function: a) longitudinal: higher velocities in lateral and inferior walls and in basal segments. With aging e and e/a velocities and integrals decreased, a increased and older individuals showed lower percentage of segments with e/a >1; b) radial: aging was associated with lower e and higher a velocities. CONCLUSION: 1) Tissue Doppler echocardiography detects physiological differences between regional myocardial function of different ventricular segments, in velocities, times intervals and integrals, with physiological heterogeneity and asynchrony; 2) Many of these data are age dependent; 3) Our data contribute to define normal values, and may become useful when compared with data from populations with heart diseases.

OBJECTIVE: To verify the hypothesis that the pulmonary vein pulsatility index is higher in fetuses of diabetic mothers than it is in normal fetuses of nondiabetic mothers. METHODS: Twenty-four fetuses of mothers with either gestational or previous diabetes (cases), and 25 normal fetuses of mothers without systemic disease (control) were examined. Fetuses were examined through prenatal Doppler and color flow mapping. The pulmonary vein pulsatility index was obtained by placing the pulsed Doppler sample volume over the right superior pulmonary vein and applying the formula (systolic velocity - presystolic velocity)/mean velocity. RESULTS: The mean gestational age of the study fetuses was 30.3±2.7 weeks, and gestational age of the controls was 29±3.3 weeks, with no significant difference in gestational age between groups (p=0.14). Fetuses of diabetic mothers had a mean pulmonary vein pulsatility index of 1.6±1, and those of the control group had an index of 0.86±0.27. CONCLUSION: Fetuses of diabetic mothers had pulmonary vein pulsatility indexes (parameter easily obtained through Doppler echocardiography that may be related to fetal diastolic function) higher than those in fetuses of mothers with normal glycemia.

OBJETIVO: Avaliar a acurácia da ultra-sonografia vascular com Doppler colorido em relação à arteriografia digital por subtração de imagens, na identificação das estenoses hemodinamicamente significativas, nas artérias renais. MÉTODOS: Estudados, prospectivamente, pelo exame ultra-sonográfico, 137 artérias renais de 69 pacientes adultos, com suspeita de hipertensão arterial reno-vascular. Os resultados obtidos foram comparados, de maneira duplo-cega, ao estudo arteriográfico digital por subtração de imagens, e calculados a sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e acurácia geral do exame para critérios previamente definidos. RESULTADOS: Na comparação entre os métodos, excluindo-se os laudos inconclusivos (7 artérias), das 130 artérias restantes, 116 (89,2%) artérias tiveram laudos concordantes e 14 (10,8%) discordantes. Os valores de sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e acurácia geral da ultra-sonografia vascular com Doppler colorido foram respectivamente 95,33%, 88,14%, 89,86%, 94,55% e 91,94%. CONCLUSÃO: Existiu boa correlação entre os dois exames na avaliação das estenoses hemodinamicamente significativas das artérias renais, tornando-se a ultra-sonografia vascular com Doppler colorido método não invasivo útil na seleção de pacientes com suspeita de hipertensão reno-vascular.

OBJETIVO: Determinar a correlação entre as velocidades diastólicas do Doppler tissular com a idade em amostra de adultos saudáveis, e correlacionar a idade com as velocidades do fluxo transmitral e de veias pulmonares. MÉTODOS: Estudados, através da ecocardiografia, 51 indivíduos saudáveis, com idades entre 21 e 69 anos e registradas as velocidades miocárdicas diastólicas ao Doppler tissular e determinadas as velocidades dos fluxos transmitral e venoso pulmonar. RESULTADOS: As velocidades miocárdicas diastólicas iniciais septal basal e lateral basal apresentaram correlação inversa com a idade, com r = - 0,40 (p = 0,004) e r = - 0,60 (p = 0,0001) respectivamente. As velocidades atriogênicas do Doppler tissular foram diretamente correlacionadas com a idade, sendo no segmento septal basal r = 0,56 (p = 0,0001) e no segmento lateral basal r = 0,50 (p = 0,0001). As velocidades do fluxo transmitral e do fluxo venoso pulmonar também mostraram correlação com a idade. CONCLUSÃO: Existe correlação entre a idade e as velocidades miocárdicas diastólicas do Doppler tissular e com as velocidades do fluxo transmitral e fluxo venoso pulmonar, demonstrando em indivíduos saudáveis uma variação de parâmetros da função diastólica do ventrículo esquerdo com a evolução natural da idade.

OBJETIVO: Identificar variáveis Doppler-ecocardiográficas associadas ao tipo de cirurgia realizada na regurgitação mitral reumática e determinar a relação entre essas variáveis e resultados da plastia em médio prazo. MÉTODOS: Analisaram-se variáveis Doppler-ecocardiográficas em 68 pacientes com regurgitação mitral reumática grave no dia prévio à cirurgia, que foram divididos, segundo a cirurgia realizada, em grupo-plastia e grupo-troca. Grupo-plastia submeteu-se também a Doppler-ecocardiograma antes da alta hospitalar e seis meses após. Compararam-se variáveis Doppler-ecocardiográficas do exame pré-operatório ao tipo de cirurgia e grau de regurgitação detectado seis meses após. RESULTADOS: Os grupos foram iguais nas características demográficas e função ventricular. O grupo-troca apresentou área mitral menor (p = 0.001). Na análise univariada as variáveis associadas à troca foram restrição da mobilidade dos folhetos anterior (p = 0.01) e posterior (p = 0.01), calcificação do folheto anterior (p = 0.01) e fusão de cordoalha (p = 0.018). Restrição da mobilidade do folheto anterior e área permaneceram como determinantes independentes de implante protético pós-análise multivariada. Dos 7 pacientes com regurgitação mitral maior que discreta, detectada 6 meses após plastia, 6 mostraram antes da cirurgia restrição da mobilidade do folheto anterior e 4, fusão de cordoalha. CONCLUSÃO: Probabilidade de troca valvar é 3,8 vezes maior quando presente restrição da mobilidade do folheto anterior da valva mitral e 2,2 vezes maior para cada 0,5 cm² de redução da área mitral. Restrição da mobilidade do folheto anterior e fusão de cordoalha estão associadas à regurgitação maior que discreta, observada 6 meses após plastia.

OBJETIVO: Quantificar o percentual de contratilidade dos diversos segmentos miocárdicos, em portadores de doença de Chagas, através da medida do strain miocárdico e verificar diferenças na função contrátil radial e longitudinal ventricular entre as formas indeterminada e dilatada da miocardiopatia chagásica crônica quando comparadas a um grupo de indivíduos normais. MÉTODOS: Foram estudados 39 indivíduos, 20 (51,3%) do sexo masculino divididos em quatro grupos: Nl: 17 (43,6%) indivíduos normais; Ind: 7 (17,9%) portadores de doença de Chagas forma indeterminada; C1, 7 (17,9%) portadores de doença de Chagas forma crônica com fração de ejeção < 50% e C2: 8 (20,5%) portadores de doença de Chagas forma crônica com fração de ejeção > 50%. Após realização de ecocardiograma basal foram registradas as imagem em Doppler tecidual para a medida do strain miocárdico nos diversos segmentos aos cortes paraesternais, longitudinal e transversal, e apicais, de duas e quatro câmaras. RESULTADOS E CONCLUSÃO: O percentual de contração dos diversos segmentos miocárdicos, tanto em seu componente radial quanto longitudinal é maior nos indivíduos normais do que em portadores da forma crônica da doença de Chagas e naqueles com a forma indeterminada da doença quando comparado com chagásicos crônicos com FE < 50%. A contratilidade radial do ventrículo esquerdo é maior que a longitudinal nos três grupos (Nl, Ind e Crônico). Os dados apresentados nos permitem propor um caráter progressivo do acometimento miocárdico em portadores da doença de Chagas.
