Através de meios de cultura, foi pesquisada a posição de colônias de Staphylococcus aureus em colchões, visando avaliar a eficácia do procedimento de limpeza e desinfecção dos leitos do Hospital Escola da Faculdade de Medicina do Triângulo Mineiro (Uberaba). Foram analisadas amostras de 50 colchões no período de 22 de outubro de 2000 a 16 de janeiro de 2001. As amostras foram coletadas e semeadas, pela técnica de esgotamento, em dois meios de cultivo (ágar sangue e manitol) com posterior realização de provas de catalase e coagulase . Na análise estatística, foram utilizados os testes não paramétricos Mann-Whitney, Kruswkal- Wallis e Wilcoxon Matched Pairs Test com nível de significância p < 0,05. Foram utilizadas 600 placas de meio de cultivo. Houve crescimento em 94 (15,6%), sendo 82 (87,2%) antes e 12 (12,8%) após a limpeza e desinfecção. Em relação à posição no leito, as amostras semeadas no meio de cultivo com manitol mostraram que não houve redução significativa na posição inferior do leito (p>0,05). Os resultados apontam e alertam para falhas no procedimento de limpeza e desinfecção dos leitos hospitalares por nós estudados.

Este estudo avaliou a resistência antimicrobiana associada de Pseudomonas aeruginosa e Staphylococcus aureus a um agente antimicrobiano com outras drogas. A resistência antimicrobiana associada foi calculada através do risco relativo. Houve uma relação óbvia entre resistência à oxacilina e a outros agentes antimicrobianos entre os isolados de Staphylococcus aureus resistentes à oxacilina (68,5%) superior a 32%, com exceção da linezolida (6,7%). Resistência associada pronunciada entre drogas foi observada para isolados de Pseudomonas aeruginosa, particularmente entre ciprofloxacina e os carbapenens (59,6% a 60,7%), entre aminoglicosídeos e carbapenens (66,3% a 67,7%) e os demais β-lactâmicos (52,3% a 85,8%). O presente trabalho enfatiza a importância da cultura diagnóstica e do teste de suscetibilidade na seleção de um correto agente antimicrobiano com relação ao impacto clínico no aumento da multirresistência e na seleção de resistência antimicrobiana associada.

O objetivo desse estudo foi descrever um caso de pneumonia necrotizante por Staphylococcus aureus resistente a meticilina. A amostra foi isolada em hemocultura coletada menos de 48 horas da admissão hospitalar. A paciente era previamente hígida quando do início do processo infeccioso. O isolado possuía o gene mecA, com "staphylococcal cassette chromosome mec" tipo IVa". A presença de Staphylococcus aureus carreando esse determinante genético em nosso meio deve ser considerada em pneumonias comunitárias graves.

Staphylococcus aureus is an important opportunistic pathogen that can cause a wide variety of diseases from mild to life-threatening conditions. S. aureus can colonize many parts of the human body but the anterior nares are the primary ecological niche. Its clinical importance is due to its ability to resist almost all classes of antibiotics available together with its large number of virulence factores. MRSA (Methicillin-Resistant S. aureus) strains are particularly important in the hospital settings, being the major cause of nosocomial infections worldwide. MRSA resistance to β-lactam antibiotics involves the acquisition of the exogenous mecA gene, part of the SCCmec cassette. Fast and reliable diagnostic techniques are needed to reduce the mortality and morbidity associated with MRSA infections, through the early identification of MRSA strains. The current identification techniques are time-consuming as they usually involves culturing steps, taking up to five days to determine the antibiotic resistance profile. Several amplification-based techniques have been developed to accelerate the diagnosis. The aim of this project was to develop an even faster methodology that bypasses the DNA amplification step. Gold-nanoprobes were developed and used to detect the presence of mecA gene in S. aureus genome, associated with resistance traits, for colorimetric assays based on non-crosslinking method. Our results showed that the mecA and mecA_V2 gold-nanoprobes were sensitive enough to discriminate the presence of mecA gene in PCR products and genomic DNA (gDNA) samples for target concentrations of 10 ng/μL and 20 ng/μL, respectively. As our main objective was to avoid the amplification step, we concluded that the best strategy for the early identification of MRSA infection relies on colorimetric assays based on non-crosslinking method with gDNA samples that can be extracted directly from blood samples.

La producción de productos lácteos conforma un complejo productivo de larga trayectoria en Argentina, con grandes transformaciones en el sector lechero en los últimos años. La provincia de Córdoba es una de las principales áreas de producción lechera, participando del 34.5 por ciento de la producción nacional. Esta provincia cuenta con un alto número de industrias, principalmente localizadas en la cuenca de Villa María, y concentra el 35 por ciento de los establecimientos lecheros del país. La Mastitis bovina (MB) es la principal causa de pérdidas económicas para el productor y la industria láctea a nivel mundial y regional, por lo cual se plantea la necesidad de mejorar la calidad higiénica y sanitaria de la leche a través de un mayor control de la mastitis. La MB es una inflamación de la glándula mamaria (GM) asociada a una infección bacteriana. Es la enfermedad más común y de mayor incidencia en el ganado lechero, siendo la principal causa de pérdidas económicas para la industria láctea a nivel mundial. Las medidas actuales de control de MB están basadas en prácticas de higiene apropiadas, reducción de la exposición ambiental al patógeno y terapia antibiótica del ganado, las cuales no son totalmente efectivas en el control de la infección. La defensa de la GM contra los patógenos causantes de MB depende de factores anatómicos, celulares y solubles, siendo la eficiencia de esos mecanismos la que determina la resistencia a nuevas infecciones. Los mediadores inmunes innatos y adquiridos de los tejidos y secreciones de la GM actúan en forma coordinada en la protección de la glándula contra enfermedades contagiosas. Staphylococcus aureus es el agente etiológico más importante en la mastitis. Esta bacteria evade la respuesta inmune inflamatoria mediante la inducción de mecanismos inmunosupresores, llevando a la patología a un curso crónico. S. aureus además puede colonizar el tejido epitelial y formar películas bacterianas conocidas como biofilms. Así, S. aureus adquiere más resistencia a la terapia antibiótica y a la acción del sistema inmunológico determinando la persistencia de la enfermedad. Nuestra hipótesis es que una respuesta inmune desarrollada en un microambiente particular de citoquinas, quimioquinas y células, podría contribuir a la evasión del patógeno y al desarrollo de una infección crónica. El polisacárido Quitosano (Q) presenta un efecto antibacteriano e inmunoestimulante en cultivos de células inmunes de ganado bovino, un efecto protectivo en modelos murinos de mastitis y actividad anti-biofilms de distintas cepas bacterianas. Por sus propiedades intrínsecas, este polisacárido es un candidato ideal en la regulación de las respuestas inmunológicas. El objetivo de este proyecto es caracterizar el microambiente local y sistémico, las señales inducidas por el microorganismo que promueven una falla inmunológica y caracterizar el efecto de Q sobre las respuestas generadas en la GM, mediante la realización de numerosos estudios in vivo e in vitro. Las estrategias para controlar la MB por S. aureus y disminuir el impacto de esta patología en la industria láctea, podrían orientarse a la manipulación del sistema inmunológico de la GM bovina a fin de incrementar los mecanismos de defensa naturales del huésped. Los resultados que se desprendan del proyecto permitirán adquirir conocimiento para el desarrollo de nuevas estrategias de inmunointervención a fin de controlar la MB por S. aureus y disminuir el impacto de esta patología en la industria láctea. Siendo la MB una patología relevante no solo en lo que hace a status sanitario animal, línea prioritaria definida en el Plan Estratégico Nacional de Ciencia, Tecnología e Innovación "Bicentenario 2006-2010", sino también a las implicancias económicas de esta problemática, y dado el desarrollo de la actividad lechera en la región y en el país, los resultados obtenidos podrían tener un importante impacto socioeconómico.

Staphylococcus aureus harbors redundant adhesins mediating tissue colonization and infection. To evaluate their intrinsic role outside of the staphylococcal background, a system was designed to express them in Lactococcus lactis subsp. cremoris 1363. This bacterium is devoid of virulence factors and has a known genetic background. A new Escherichia coli-L. lactis shuttle and expression vector was constructed for this purpose. First, the high-copy-number lactococcal plasmid pIL253 was equipped with the oriColE1 origin, generating pOri253 that could replicate in E. coli. Second, the lactococcal promoters P23 or P59 were inserted at one end of the pOri253 multicloning site. Gene expression was assessed by a luciferase reporter system. The plasmid carrying P23 (named pOri23) expressed luciferase constitutively at a level 10,000 times greater than did the P59-containing plasmid. Transcription was absent in E. coli. The staphylococcal clumping factor A (clfA) gene was cloned into pOri23 and used as a model system. Lactococci carrying pOri23-clfA produced an unaltered and functional 130-kDa ClfA protein attached to their cell walls. This was indicated both by the presence of the protein in Western blots of solubilized cell walls and by the ability of ClfA-positive lactococci to clump in the presence of plasma. ClfA-positive lactococci had clumping titers (titer of 4,112) similar to those of S. aureus Newman in soluble fibrinogen and bound equally well to solid-phase fibrinogen. These experiments provide a new way to study individual staphylococcal pathogenic factors and might complement both classical knockout mutagenesis and modern in vivo expression technology and signature tag mutagenesis.

Animal models of infective endocarditis (IE) induced by high-grade bacteremia revealed the pathogenic roles of Staphylococcus aureus surface adhesins and platelet aggregation in the infection process. In humans, however, S. aureus IE possibly occurs through repeated bouts of low-grade bacteremia from a colonized site or intravenous device. Here we used a rat model of IE induced by continuous low-grade bacteremia to explore further the contributions of S. aureus virulence factors to the initiation of IE. Rats with aortic vegetations were inoculated by continuous intravenous infusion (0.0017 ml/min over 10 h) with 10(6) CFU of Lactococcus lactis pIL253 or a recombinant L. lactis strain expressing an individual S. aureus surface protein (ClfA, FnbpA, BCD, or SdrE) conferring a particular adhesive or platelet aggregation property. Vegetation infection was assessed 24 h later. Plasma was collected at 0, 2, and 6 h postinoculation to quantify the expression of tumor necrosis factor (TNF), interleukin 1α (IL-1α), IL-1β, IL-6, and IL-10. The percentage of vegetation infection relative to that with strain pIL253 (11%) increased when binding to fibrinogen was conferred on L. lactis (ClfA strain) (52%; P = 0.007) and increased further with adhesion to fibronectin (FnbpA strain) (75%; P < 0.001). Expression of fibronectin binding alone was not sufficient to induce IE (BCD strain) (10% of infection). Platelet aggregation increased the risk of vegetation infection (SdrE strain) (30%). Conferring adhesion to fibrinogen and fibronectin favored IL-1β and IL-6 production. Our results, with a model of IE induced by low-grade bacteremia, resembling human disease, extend the essential role of fibrinogen binding in the initiation of S. aureus IE. Triggering of platelet aggregation or an inflammatory response may contribute to or promote the development of IE.

Functional characterization of transformed or natively present bacterial virulence proteins can be achieved employing various model systems. A prerequisite is to verify the correct expression of the transformed protein or the presence of the native protein in the microbe. Traditionally, antibodies are raised against the protein or a peptide thereof, followed by Western blot analysis or by fluorescence-activated cell sorting. Alternatively, the protein-coding gene can be fused with a downstream reporter gene, the expression of which reports the simultaneous expression of the upstream recombinant protein. Although being powerful, these methods are time consuming, especially when multiple proteins must be assessed. Here we describe a novel way to validate the expression of Gram-positive surface proteins covalently attached to the peptidoglycan. Eighteen out of the 21 known LPXTG-motif carrying cell wall-associated proteins of Staphylococcus aureus were cloned in Lactoccocus lactis either alone, in combinations or as truncated forms, and their correct expression was assessed by liquid chromatography coupled to mass spectrometry (LC-MS). The method is rapid, sensitive and precise. It can identify multiple proteins in transformed constructs without the time and cost needed for raising and testing multiple sets of antibodies.

The efficacy of trovafloxacin against Staphylococcus aureus and viridans group streptococci was investigated in vitro and in an experimental model of endocarditis. The MICs at which trovafloxacin and ciprofloxacin inhibited 90% of clinical isolates of such bacteria (MIC90s) were (i) 0.03 and 2 mg/liter, respectively, for 30 ciprofloxacin-susceptible S. aureus isolates, (ii) 32 and 128 mg/liter, respectively, for 20 ciprofloxacin-resistant S. aureus isolates, and (iii) 0.25 and 8 mg/liter, respectively, for 28 viridans group streptococci. Rats with aortic vegetations were infected with either of two ciprofloxacin-susceptible but methicillin-resistant S. aureus strains (strains COL and P8), one penicillin-susceptible Streptococcus sanguis strain, or one penicillin-resistant Streptococcus mitis strain. Rats were treated for 3 or 5 days with doses that resulted in kinetics that simulated those achieved in humans with trovafloxacin (200 mg orally once a day), ciprofloxacin (750 mg orally twice a day), vancomycin (1 g intravenously twice a day), or ceftriaxone (2 g intravenously once a day). Against the staphylococci, the activities of both trovafloxacin and ciprofloxacin were equivalent to that of vancomycin, and treatment of endocarditis with these drugs was successful (P &lt; 0.05). However, ciprofloxacin selected for resistant derivatives in vitro and in vivo, whereas trovafloxacin was 10 to 100 times less prone than ciprofloxacin to select for resistance in vitro and did not select for resistance in vivo. Against the two streptococcal isolates, trovafloxacin significantly (P &lt; 0.05) decreased bacterial counts in the vegetations but was less effective than the control drug, ceftriaxone. Thus, a simulated oral dose of trovafloxacin (200 mg per day) was effective against ciprofloxacin-susceptible staphylococci and was less likely than ciprofloxacin to select for resistance. The simulated oral dose of trovafloxacin also had some activity against streptococcal endocarditis, but optimal treatment of infections caused by such organisms might require higher doses of the drug.
