The number of patients treated by haemodialysis (HD) is continuously increasing. The complications associated with vascular accesses represent the first cause of hospitalisation in these patients. Since 2001 nephrologists, surgeons, angiologists and radiologists at the CHUV are working to develop a multidisciplinary model that includes planning and monitoring of HD accesses. In this setting the echo-Doppler represents an important tool of investigation. Every patient is discussed and decisions are taken during a weekly multidisciplinary meeting. A network has been created with nephrologists of peripheral centres and other specialists. This model allows to centralize investigational information and coordinate patient care while keeping and even developing some investigational activities and treatment in peripheral centres.

AIM: Alpha1-adrenergic receptors (alpha1-ARs) are classified into three subtypes: alpha1A-AR, alpha1B-AR, and alpha1D-AR. Triple disruption of alpha1A-AR, alpha1B-AR, and alpha1D-AR genes results in hypotension and produces no contractile response of the thoracic aorta to noradrenalin. Presently, we characterized vascular contractility against other vasoconstrictors, such as potassium, prostaglandin F2alpha (PGF(2alpha)) and 5-hydroxytryptamine (5-HT), in alpha1A-AR, alpha1B-AR, and alpha1D-AR triple knockout (alpha1-AR triple KO) mice. MAIN METHODS: The contractile responses to the stimulation with vasoconstrictors were studied using isolated thoracic aorta. KEY FINDINGS: As a result, the phasic and tonic contraction induced by a high concentration of potassium (20 mM) was enhanced in the isolated thoracic aorta of alpha1-AR triple KO mice compared with that of wild-type (WT) mice. In addition, vascular responses to PGF(2alpha) and 5-HT were also enhanced in the isolated thoracic aorta of alpha1-AR triple KO mice compared with WT mice. Similar to in vitro findings with isolated thoracic aorta, in vivo pressor responses to PGF(2alpha) were enhanced in alpha1-AR triple KO mice. Real-time reverse transcription-polymerase chain reaction analysis and western blot analysis indicate that gene expression of the 5-hydroxytryptamine 2A (5-HT(2A)) receptor was up-regulated in the thoracic aorta of alpha1-AR triple KO mice while the prostaglandin F2alpha receptor (FP) was unchanged. SIGNIFICANCE: These results suggest that loss of alpha1-ARs can lead to enhancement of vascular responsiveness to the vasoconstrictors and may imply that alpha1-ARs and the subsequent signaling regulate the vascular responsiveness to other stimulations such as depolarization, 5-HT and PGF(2alpha).

Les anomalies vasculaires constituent des affections rares présentes dès la naissance. Elles sont classées selon leur histologie en tumeurs ou malformations vasculaires. La connaissance de ces anomalies par le médecin de famille est importante, permettant ainsi leur diagnostic, qui repose essentiellement sur la clinique. Les caractéristiques anatomiques et hémodynamiques de l'anomalie sont précisées par l'angiologue grâce à l'écho-Doppler. Cet examen permet un suivi de la lésion et le diagnostic d'éventuelles complications. Un bilan radiologique peut compléter cette évaluation avant discussion multidisciplinaire en vue d'un traitement parfois difficile. Dans cet article, un bref aperçu des malformations vasculaires et de leur prise en charge multidisciplinaire est discuté, en particulier dans le cas du syndrome de Klippel-Trenaunay. Vascular anomalies are rare conditions that could be observed at all ages. They are classified, according to their histology, in vascular tumors or vascular malformations. The general practitioner plays a significant role in diagnosis and patient management, diagnosis being suspected on clinical history. In case of vascular anomaly, ultrasound-Doppler assessment is helpful to characterize morphologic and hemodynamic changes of the lesion and permits to monitor the evolution and to detect complications. Further investigations are often necessary prior to multidisciplinary management. In this article, a brief overview of vascular anomalies, their multidisciplinary management and the exemple of Klippel-Trenaunay syndrome are presented.

O presente estudo foi desenvolvido com o objetivo de avaliar a incidência e a severidade da malformação floral em diferentes cultivares de mangueira (Mangifera indica L.), em condições de clima semi-árido, no município de Santa Maria da Vitória, Estado da Bahia. O experimento foi conduzido no mês de julho de 2001, em um cultivo comercial formado por diversas cultivares divididas em talhões. Os tratamentos foram compostos por seis cultivares assim distribuídas: T1- Rosa; T2- Haden; T3- Bourbon; T4- Palmer; T5- Tommy Atkins; T6-Van Dyke. Nas condições em que o trabalho foi desenvolvido, os menores índices de incidência e severidade da malformação floral foram obtidos pela cultivar Rosa que não apresentou sintomas, seguida pela 'Bourbon'. A cultivar Haden apresentou os maiores índices da doença.

A malformação da mangueira causada pelo fungo Fusarium subglutinans Wollenweb & Reinking é provavelmente a doença que mais causa prejuízos à produção de manga no Brasil e em outros países produtores. Esse fungo foi isolado de uma planta-matriz da cultivar Tommy Atkins com avançados sintomas da doença, purificado e preparado para ser inoculado em 15 cultivares de manga nacionais e importadas. Inicialmente, foram inoculadas, em julho de 2000, 10 cultivares: Bourbon IAC - 100, Coração-de-Boi, Keitt, Parvin, Primor de Amoreira, Sensation, Smith, Surpresa, Tommy Atkins e Van Dyke. O segundo grupo foi inoculado em dezembro de 2000, com as cultivares: Adams, Bhadauran, Palmer, Princesa e Zill, e repetiu-se a inoculação com outras mudas das cultivares Primor de Amoreira, Sensation e Tommy Atkins, com a finalidade de se compararem as duas épocas de inoculação. Através dos dados obtidos com as plantas avaliadas nas duas épocas de inoculação, após 11 meses de observações, realizou-se o teste de médias Z para comparar a proporção de plantas doentes entre as cultivares, podendo-se concluir, com os resultados obtidos, que as cultivares Bhadauran, Palmer, Parvin, Sensation, Surpresa, Van Dyke e Zill apresentam menor porcentagem de plantas com sintomas de malformação ou menor progressão de sintomas em relação às outras cultivares inoculadas, para as condições de ambiente protegido em que foram realizados os ensaios.

O gênero Fusarium é responsável por doenças em diversas plantas economicamente importantes. Entre estas doenças, destaca-se a malformação da mangueira, causada pelo fungo Fusarium subglutinans. O objetivo deste trabalho foi desenvolver oligonucleotídeos iniciadores para reação em cadeia da polimerase (PCR), específicos para o fungo F. subglutinans da mangueira. A amplificação de DNA de oito Fusarium spp. de diferentes hospedeiros, usando o oligonucleotídeo randômico UBC-41 (TTAACCGGGG), produziu um fragmento de aproximadamente 1.300 pb somente para o fungo da mangueira. Tendo em vista que padrões de bandeamento por RAPD não são considerados confiáveis devido à baixa reprodutibilidade dos resultados, o fragmento diferencial foi eluído do gel de agarose, purificado, clonado e seqüenciado. As seqüências nucleotídicas foram utilizadas para identificar e sintetizar quatro pares de oligonucleotídeos específicos, denominados Fs 5, Fs 13, Fs 14 e Fs 15. DNAs de Fusarium spp. de outros hospedeiros (alho, amendoim, cana-de-açúcar, ciclâmen, ervilha, melão e trigo), da planta de mangueira cv. Tommy Atkins sadia e de outros cinco isolados de F. subglutinans de mangueira sintomática, foram submetidos à amplificação com os pares de oligonucleotídeos. Fragmentos amplificados foram visualizados somente para F. subglutinans de mangueira, demonstrando assim a especificidade dos oligonucleotídeos SCAR desenhados.

BACKGROUND: Remodeling of quiescent vessels with increases in permeability, vasodilatation, and edema are hallmarks of inflammatory disorders. Factors involved in this type of remodeling represent potential therapeutic targets. OBJECTIVES: We investigated whether the nuclear hormone receptor peroxisome proliferator-activated receptor (PPAR) β/δ, a regulator of metabolism, fibrosis, and skin homeostasis, is involved in regulation of this type of remodeling. METHODS: Wild-type and various Pparb/d mutant mice were used to monitor dermal acute vascular hyperpermeability (AVH) and passive systemic anaphylaxis-induced hypothermia and edema. PPARβ/δ-dependent kinase activation and remodeling of endothelial cell-cell junctions were addressed by using human endothelial cells. RESULTS: AVH and dilatation of dermal microvessels stimulated by vascular endothelial growth factor A, histamine, and thrombin are severely compromised in PPARβ/δ-deficient mice. Selective deletion of the Pparb/d-encoding gene in endothelial cells in vivo similarly limits dermal AVH and vasodilatation, providing evidence that endothelial PPARβ/δ is the major player in regulating acute dermal microvessel remodeling. Furthermore, endothelial PPARβ/δ regulatory functions are not restricted to the skin vasculature because its deletion in the endothelium, but not in smooth muscle cells, also leads to reduced systemic anaphylaxis, the most severe form of allergic reaction, in which an acute vascular response plays a key role. PPARβ/δ-dependent AVH activation likely involves the activation of mitogen-activated protein kinase and Akt pathways and leads to downstream destabilization of endothelial cell-cell junctions. CONCLUSION: These results unveil not only a novel function of PPARβ/δ as a direct regulator of acute vessel permeability and dilatation but also provide evidence that antagonizing PPARβ/δ represents an important strategy to consider for moderating diseases with altered endothelial integrity, such as acute inflammatory and allergic disorders.

Elevated serum phosphorus, calcium, and fibroblast growth factor 23 (FGF23) levels are associated with cardiovascular disease in chronic renal disease. This study evaluated the effects of sucroferric oxyhydroxide (PA21), a new iron-based phosphate binder, versus lanthanum carbonate (La) and sevelamer carbonate (Se), on serum FGF23, phosphorus, calcium, and intact parathyroid hormone (iPTH) concentrations, and the development of vascular calcification in adenine-induced chronic renal failure (CRF) rats. After induction of CRF, renal function was significantly impaired in all groups: uremic rats developed severe hyperphosphatemia, and serum iPTH increased significantly. All uremic rats (except controls) then received phosphate binders for 4 weeks. Hyperphosphatemia and increased serum iPTH were controlled to a similar extent in all phosphate binder-treatment groups. Only sucroferric oxyhydroxide was associated with significantly decreased FGF23. Vascular calcifications of the thoracic aorta were decreased by all three phosphate binders. Calcifications were better prevented at the superior part of the thoracic and abdominal aorta in the PA21 treated rats. In adenine-induced CRF rats, sucroferric oxyhydroxide was as effective as La and Se in controlling hyperphosphatemia, secondary hyperparathyroidism, and vascular calcifications. The role of FGF23 in calcification remains to be confirmed.
