OBJECTIVES To evaluate the initial results of a prostate cancer screening program using mobile units in Brazil. METHODS Since 2004, we have conducted a program of prostate cancer screening using mobile units across 231 municipalities from 6 Brazilian states. RESULTS A total of 17 571 men were evaluated by clinical history, digital rectal examination (DRE), and serum free and total prostate-specific antigen (PSA) levels. The recommendations for biopsy were a PSA level of >= 4.0 ng/mL, DRE findings suspicious for cancer, or a PSA level of 2.5-4.0 ng/mL with a percent-free PSA level <15%. The biopsy protocol included 12 biopsy cores from the peripheral zone, 2 from the transition zone, and additional sampling of suspicious areas. The cumulative cancer detection rate was 3.7%. The main indication for biopsy was a PSA level of >= 4.0 ng/mL (51.2%), with a positive predictive value (PPV) of 44.1%. Another 19.7% of biopsied men had suspicious DRE findings with a normal PSA level (PPV 23.5%). A percent-free PSA level of <15% in men with a PSA level of 2.5-4.0 ng/mL and normal DRE findings yielded a PPV of 31.1%. The PPV was greater (70.9%) for the 7.1% of men with both suspicious DRE findings and a PSA level of >4.0 ng/mL. Most cancers were Stage T1-T2 (93.4%), and the percentage of Gleason score of >= 7 was 32.5%. The proportion of insignificant cancers according to Epstein`s criteria was 13.5%. CONCLUSIONS A mobile prostate cancer screening unit enabled an underserved population to gain access to specialized care through the public healthcare system. Objective. The objective of this study was to conduct a cost-effectiveness analysis of a universal rotavirus vaccination program among children : 5 years of age in Brazil. Methods. Considering a hypothetical annual cohort of approximately 3 300 000 newborns followed over 5 years, a decision-tree model was constructed to examine the possible clinical and economic effects of rotavirus infection with and without routine vaccination of children. Probabilities and unit costs were derived from published research and national administrative data. The impact of different estimates for key parameters was studied using sensitivity analysis. The analysis was conducted from both healthcare system and societal perspectives. Results. The vaccination program was estimated to prevent approximately 1735 351 (54%) of the 3 210 361 cases of rotavirus gastroenteritis and 703 (75%) of 933 rotavirus-associated deaths during the 5-year period. At a vaccine price of 18.6 Brazilian reais (R$) per dose, this program would cost R$121 673 966 and would save R$38 536 514 in direct costs to the public healthcare system and R$71 778 377 in direct and indirect costs to society. The program was estimated to cost R$1 028 and R$1 713 per life-years saved (LYS)from the societal and healthcare system perspectives, respectively. Conclusions. Universal rotavirus vaccination was a cost-effective strategy for both perspectives. However, these findings are highly sensitive to diarrhea incidence rate, proportion of severe cases, vaccine coverage, and vaccine price.

Most transportation agencies stipulate that an important planning goal is to provide equitable and just public transport services. However, who is to be served and the type of service that should be provided has been ambiguous. This paper develops a methodology for examining equity in the provision of public transportation services. An approach for identifying areas in need of public transport is developed based upon the use of socio-demographic and economic information. Public transport need is then related to levels of access to service. This approach makes it possible to establish the degree to which public transport services may be considered equitable in relation to need and suitable access. A detailed analysis of the southeast Queensland region of Australia illustrates how this approach may be used to inform public transport decision making.

Fazer prevenção em Saúde Pública implica em conhecimento sistematizado para a proposição de programas e sua avaliação. Quanto à prevenção de Deficiência Mental, que atinge cerca de 10% da população, pouco se conhece no país. Adotando uma metodologia para levantamento de ações preventivas de DM em hospitais e unidades de saúde, junto a gestantes e recém-nascidos, foi possível descrever e analisar a atuação da rede pública de saúde da Grande Vitória/ES, indicando os níveis de prevenção mais atendidos. Foram levantadas as ações de prevenção (AP) de cinco hospitais públicos de grande porte, nove (31%) unidades de saúde e seis secretarias de saúde, entre 1996-97. Os dados de 25 entrevistas mostram que esses locais realizavam 51,5% das 433 AP possíveis (57,4% da prevenção primária e 45,5% da secundária). Particularizando a atuação de cada município e local pesquisado, os dados fornecem subsídios para análises e possíveis mudanças nos indicadores de saúde materno-infantil.

Neste artigo se discute o conceito de subjetividade utilizado nas práticas de saúde. Parte-se da tese de que a concepção de subjetividade predominante no pensamento dos trabalhadores no campo da saúde não está em sintonia com valores e pressupostos de renovação de conceitos e práticas, neste âmbito, na atualidade. Defende-se o caráter processual e coletivo dos processos de produção de subjetividade, contra o caráter individualista, apriorístico e objetificado prevalente nas práticas em psicologia e campos afins. Desdobra-se desta reflexão a defesa das transformações dessas práticas tendo como norte as articulações entre gestão no trabalho e processos de subjetivação, com base nos pressupostos teórico-filosóficos da obra de M. Foucault e na ergologia de linhagem francesa. Nessa direção de análise, afirma-se a importância de se pesquisar as articulações entre organização do trabalho, produção de saúde-doença e processos de subjetivação em curso. O foco das análises é a escola pública.

The State Reform processes combined with the emergence and use of Information and Communication Technology (ICT) originated electronic government policies and initiatives in Brazil. This paper dwells on Brazilian e-government by investigating the institutional design it assumed in the state's public sphere, and how it contributed to outcomes related to e-gov possibilities. The analyses were carried out under an interpretativist perspective by making use of Institutional Theory. From the analyses of interviews with relevant actors in the public sphere, such as state secretaries and presidents of public ICT companies, conclusions point towards low institutionalization of e-gov policies. The institutional design of Brazilian e-gov limits the use of ICT to provide integrated public services, to amplify participation and transparency, and to improve public policies management.

The adoption of faster modes of transportation (mainly the private car) has changed profoundly the spatial organisation of cities. The increase in distance covered due to increased speed of travel and to urban sprawl leads to an increase in energy consumption, being the transportation sector a huge consumer responsible for 61.5% of total world oil consumption and a global final energy consumption of 31.6% in EU-27 (2007). Due to unsustainable transportation conditions, many cities suffer from congestion and various other traffic problems. Such situations get worse with solutions mostly seen in the development of new infrastructure for motorized modes of transportation, and construction of car parking structures. The bicycle, considered the most efficient among all modes of transportation including walking, is a travel mode that can be adopted in most cities contributing for urban sustainability given the associated environmental, economic and social advantages. In many nations a large number of policy initiatives have focused on discouraging the use of private cars, encouraging the use of sustainable modes of transportation, like public transportation and other forms such as bicycling. Given the importance of developing initiatives that favour the use of bicycle as an urban transportation mode, an analysis of city suitability, including distances and slopes of street network, is crucial in order to help decision-makers to plan the city for bicycle. In this research Geographical Information Systems (GIS) technology was used for this purpose and some results are presented concerning the city of Coimbra.

The aim of this paper is to establish some basic guidelines to help draft the information letter sent to individual contributors should it be decided to use this model in the Spanish public pension system. With this end in mind and basing our work on the experiences of the most advanced countries in the field and the pioneering papers by Jackson (2005), Larsson et al. (2008) and Sunden (2009), we look into the concept of “individual pension information” and identify its most relevant characteristics. We then give a detailed description of two models, those in the United States and Sweden, and in particular look at how they are structured, what aspects could be improved and what their limitations are. Finally we make some recommendations of special interest for designing the model for Spain.

O presente estudo, de natureza qualitativa e exploratório, teve como objectivo verificar se a cidade do Porto possui características de uma cidade amiga das pessoas idosas, na perspectiva de idosos residentes neste meio urbano. Uma cidade amiga das pessoas idosas estimula um envelhecimento activo e com dignidade ao optimizar oportunidades para a saúde, participação e segurança. Foram realizados dois focus groups com pessoas idosas habitantes das Freguesias da Vitória e Miragaia, seleccionados a partir de uma amostragem por conveniência, recorrendo-se a um guião de entrevista constituído pelas seguintes categorias: espaços exteriores e edifícios; transportes; habitação; respeito e inclusão social; participação social; participação cívica e emprego; comunicação e informação; apoio comunitário e serviços de saúde. Desta forma, foi possível verificar que, apesar dos participantes identificarem um conjunto de condições que podem ser consideradas amigas das pessoas idosas, a maior parte das características referidas foram encaradas como negativas e com um impacto considerável no seu quotidiano. A participação social, os meios de informação disponíveis e os serviços comunitários são as condições perante as quais os participantes demonstram maior satisfação. Pelo contrário, em relação aos espaços exteriores, referem aspectos, como os grandes declives, as más condições dos pavimentos, os obstáculos nos passeios e a acumulação de lixo, que contribuem para um ambiente desagradável e inseguro. Quanto aos transportes, as modificações na identificação dos veículos, as alterações nos percursos, a pouca consciencialização dos motoristas em relação às necessidades dos mais velhos e as condições das paragens são os principais factores destacados, enquanto as habitações são consideradas antigas e com más condições estruturais e de acesso. De uma forma geral, estes idosos consideram-se pouco reconhecidos e desrespeitados pelos mais jovens e deparam-se com grandes dificuldades no acesso a actividades laborais e de voluntariado.

O papel crucial da escola na sociedade e o exercício da atividade profissional como docente, com um olhar atento sobre o traçar das políticas educativas, motivou a elaboração deste trabalho de investigação, que tem como objeto de estudo os papéis desempenhados pelos diretores das escolas estatais e não estatais e como objetivos específicos estudar o impacto da legislação emanada pela tutela, nas escolas públicas e privadas e analisar as convergências e divergências nas conceções e práticas dos seus diretores. As dimensões analíticas exploradas no estudo abrangem as conceções gestionárias dos diretores quanto aos modelos de gestão, às práticas de autonomia, ao serviço educativo e à prestação de contas. Este trabalho de natureza qualitativa foca o olhar sobre um grupo restrito de atores educativos que foram escolhidos devido ao papel que desempenham na organização educativa e porque a publicação do Decreto- Lei 75/ 2008 de 22 de abril, trouxe alterações à escola pública. A tradição de direção colegial que vigorava nas organizações educativas estatais foi quebrada. O presidente do conselho diretivo é doravante substituído pelo diretor que passa a delegar competências, a designar equipas e a prestar contas à tutela e comunidade educativa à semelhança do diretor da escola privada. O estudo de caso apresentado foi realizado em três escolas públicas e em três colégios privados com recurso a entrevistas semiestruturadas e à análise documental. As conclusões deste trabalho remetem para a existência de muitos pontos de convergência entre a opinião dos diretores da escola pública e privada. As temáticas relativas à autonomia, escolha do pessoal docente e prestação de contas, são olhadas pela mesma perspetiva. A autonomia é vista como “uma miragem”; uma “terra prometida” (Lima e Afonso, 1995). A prestação de contas é exigida aos diretores do ensino estatal e do privado através de instrumentos próximos. As principais divergências situam-se ao nível do menor interesse demonstrado, por parte da direção da escola privada, pela oferta de cursos profissionais e pelo menor investimento em estratégias para a prevenção do abandono escolar, que é considerado pouco significativo na escola não estatal. A defesa da escolha de escola e da modalidade de cheque ensino são outros dos pontos que marcam a divergência entre estes diretores. Abstract: This investigative paper - whose objective is the study of the role of the school directors, both State and non-state, and the impact of legislation on both State and private schools, as well as the analysis of the convergent and divergent conceptions and practices of these directors – is motivated by the crucial role played by schools in our society and by the professional activity of the teacher, with an attentive look at the educational practices. The analytical dimension explored in this study includes the various concepts of management of the school director as models of management, as well as practices in self-sufficiency, budget control and educational service to the community. This study has a qualitative nature and focuses on a small group of individuals who were chosen for the role they play in the whole educational structure, considering that the Decree nº 75/2008, published on April the 22nd, determined alterations to the public school system. The traditional method of control of the public school system has, henceforth, been changed. The headmaster is now substituted by a director who delegates his functions, makes up work teams and elaborates the school budget which is presented to the respective governmental ministry and the community, much like as what happens in private schools. The present study encompasses three public schools and three private schools, the methods of study being semi-structured interviews as well as the consultation of documentation. The conclusions point to many convergent opinions of the school directors of both the public and the private sector. The school directors of both public and private schools used in this study share the same opinion as to the factors involved in the selection of teachers, the elaboration of the school budget and the implementation of self-sufficiency policies. These self-sufficiency policies are seen as a “mirage” or a “promised land” (Lima and Afonso, 1995). The school budget and its management practices are implemented in both public and private schools through similar instruments. The principal differences are noted on smaller, less interesting points, on the part of the direction of the private schools, and result from the elaboration of professional courses and minor investment in the strategies, oriented to the prevention of school drop-outs, which is considered of little significance in the private school sector. The other factors of divergence result from the right to choose the type of school desired and the type of teaching implemented.

OBJECTIVE: To describe the opinion of doctors who participated in the Latin American Study on Cesarean section in Brazil regarding the second opinion strategy when faced with the decision of performing a C-section. METHODS: Seventy-two doctors from the hospitals where the study took place (where the second opinion was routinely sought) and 70 from the control group answered a pre-tested self-administered structured questionnaire. Descriptive tables were prepared based on the frequency of relevant variables on opinion of physicians regarding: effectiveness of the application of the second opinion strategy; on whether they would recommend implementation of this strategy and reasons for not recommending it in private institutions; feasibility of the strategy implementation and reasons for not considering this implementation feasible in private institutions. RESULTS: Half of the doctors from the intervention hospitals (50%) and about two thirds of those in the control group (65%) evaluated the second opinion as being or having the potential of being effective/very effective in their institutions. The great majority of those interviewed from both intervention and control hospitals considered this strategy feasible in public (87% and 95% respectively) but not in private hospitals (64% and 70% respectively), mainly because in the latter the doctors would not accept interference from a colleague in their decision-making process. CONCLUSION: Although the second opinion strategy was perceived as effective in reducing C-section rates, doctors did not regard it feasible outside the public health system in Brazil.

OBJECTIVE: To determine the prevalence of reagent serology for suspected acute toxoplasmosis in pregnant women and to describe clinical, laboratory and therapeutic profiles of mothers and their children. METHODS: A retrospective study was conducted with IgM-anti-Toxoplasma gondii-reagent pregnant women and their children who attended the public health system in the state of Paraná, Southern Brazil, from January 2001 to December 2003. Information were obtained from clinical, laboratory (ELISA IgM/IgG) and ultrasonographic data and from interviews with the mothers. To test the homogeneity of the IgM indices in relation to the treatment used, the Pearson's Chi-square test was applied. Comparisons were considered significant at a 5% level. RESULTS: Two hundred and ninety (1.0%) cases of suspected IgM-reagent infection were documented, with a prevalence of 10.7 IgM-reagent women per 1,000 births. Prenatal care started within the first 12 weeks for 214/290; 146/204 were asymptomatic. Frequent complaints included headaches, visual disturbance and myalgia. Ultrasonography revealed abnormalities in 13 of 204 pregnancies. Chemoprophylaxis was administered to 112/227; a single ELISA test supported most decisions to begin treatment. Pregnant women with IgM indices =2.000 tended to be treated more often. Among exposed children, 44/208 were serologically followed up and all were IgG-reagent, and three IgM-reagent cases showed clinical symptoms. CONCLUSIONS: The existence of pregnant women with laboratorially suspected acute toxoplasmosis who were not properly followed up, and of fetuses that were not adequately monitored, shows that basic aspects of the prenatal care are not being systematically observed. There is need of implementing a surveillance system of pregnant women and their children exposed to T. gondii.
