São apresentados três casos de endocardite por Cândida Parapsilosis que surgiram em crianças com idade entre os sete e os nove anos, após terem sido submetidas a correcção total de Tetralogia de Fallot. As três crianças foram reoperadas, tendo recebido previamente uma delas terapêutica médica com anfotericina B e duas exclusivamente com Ketoconazol oral. Após negativação das hemoculturas foi efectuada remoção cirúrgica das vegetações com substituição do patch septal de dacron. A terapêutica com Ketoconazol prosseguiu durante 24 meses, com follow-up de 30 a 42 meses, não se tendo verificado nem reinfecção nem efeitos secundários da terapêutica. A ecocardiografia bidimensional revelou-se um método eficaz no diagnóstico e seguimento a longo prazo. A terapêutica médico-cirúrgica combinada, com timing cirúrgico baseado em dados clínicos e laboratoriais foi fundamental para os bons resultados, estando as crianças actualmente curadas.

A fasceíte necrosante (FN) é uma doença rara em pediatria. Na maior parte dos casos surge como complicação de varicela, sendo a pele habitualmente a porta de entrada e o Streptococcus pyogenes o agente mais frequente. Os autores descrevem um caso clínico de uma criança de 8 anos, previamente saudável que, no contexto de uma otite média aguda medicada com um anti-inflamatório não esteróide (AINE), desenvolve uma infecção invasiva a Streptococcus 13 hemoliticus do grupo A.

O Síndroma de Cohen é uma doença autossómica recessiva, descrita pela primeira vez em 1973, tendo sido publicados desde essa altura cerca de 100 casos. O diagnóstico é baseado no quadro clínico e fenótipo típico. Em 1994 o gene para o Síndroma de Cohen foi localizado no cromossoma 8. Os autores apresentam o caso clínico de uma criança com características do Síndroma de Cohen, internado por insuficiência cardíaca sem cardiopatia estrutural.

In a liver transplant (LT) center, treatments with Prometheus were evaluated. The main outcome considered was 1 and 6 months survival. Methods. During the study period, 74 patients underwent treatment with Prometheus; 64 were enrolled,with a mean age of 51 13 years; 47men underwent 212 treatments (mean, 3.02 per patient). The parameters evaluated were age, sex, laboratorial (liver enzymes, ammonia) and clinical (model for end-stage liver disease and Child-Turcotte-Pugh score) data. Results. Death was verified in 23 patients (35.9%) during the hospitalization period, 20 patients (31.3%) were submitted to liver transplantation, and 21 were discharged. LT was performed in 4 patients with acute liver failure (ALF, 23.7%), in 7 patients with acute on chronic liver failure (AoCLF, 43.7%), and in 6 patients with liver disease after LT (30%). Seven patients who underwent LT died (35%). In the multivariate analysis, older age (P ¼ .015), higher international normalized ratio (INR) (P ¼ .019), and acute liver failure (P ¼ .039) were independently associated with an adverse 1-month clinical outcome. On the other hand, older age (P ¼ .011) and acute kidney injury (P ¼ .031) at presentation were both related to worse 6-month outcome. For patients with ALF and AoCLF we did not observe the same differences. Conclusions. In this cohort, older age was the most important parameter defining 1- and 6-month survival, although higher INR and presence of ALF were important for 1-month survival and AKI for 6-month survival. No difference was observed between patients who underwent LT or did not have LT.

Objective. To access the incidence of infectious problems after liver transplantation (LT). Design. A retrospective, single-center study. Materials and Methods. Patients undergoing LT from January 2008 to December 2011 were considered. Exclusion criterion was death occurring in the first 48 hours after LT. We determined the site of infection and the bacterial isolates and collected and compared recipient’s variables, graft variables, surgical data, post-LT clinical data. Results. Of the 492 patients who underwent LT and the 463 considered for this study, 190 (Group 1, 41%) developed at least 1 infection, with 298 infections detected. Of these, 189 microorganisms were isolated, 81 (51%) gram-positive bacteria (most frequently Staphylococcus spp). Biliary infections were more frequent (mean time of 160.4 167.7 days after LT); from 3 months after LT, gram-negative bacteria were observed (57%). Patients with infections after LT presented lower aminotransferase levels, but higher requirements in blood transfusions, intraoperative vasopressors, hemodialysis, and hospital stay. Operative and cold ischemia times were similar. Conclusion. We found a 41% incidence of all infections in a 2-year follow-up after LT. Gram-positive bacteria were more frequent isolated; however, negative bacteria were commonly isolated later. Clinical data after LT were more relevant for the development of infections. Donors’ variables should be considered in future analyses.

This is a case report of a 43-year-old Caucasian male with end-stage renal disease being treated with hemodialysis and infective endocarditis in the aortic and tricuspid valves. The clinical presentation was dominated by neurologic impairment with cerebral embolism and hemorrhagic components. A thoracoabdominal computerized tomography scan revealed septic pulmonary embolus. The patient underwent empirical antibiotherapy with ceftriaxone, gentamicin and vancomycin, and the therapy was changed to flucloxacilin and gentamicin after the isolation of S. aureus in blood cultures. The multidisciplinary team determined that the patient should undergo valve replacement after the stabilization of the intracranial hemorrhage; however, on the 8th day of hospitalization, the patient entered cardiac arrest due to a massive septic pulmonary embolism and died. Despite the risk of aggravation of the hemorrhagic cerebral lesion, early surgical intervention should be considered in high-risk patients.

The triatomine species Rhodnius nasutus and Triatoma pseudomaculata were captured on palm trees Orbignya martiana "babaçu ", in the urban zone of Teresina. This kind of palm tree is largely distributed in Piauí State. The predominant species was R. nasutus; the young in stars predominated. The infestation index of palm trees and the infection index of triatomines by flagellates were 96.0 ana 29.1%, respectively. Marsupiais, bats and a rodent were captured in palm trees. The flagellates found in both triatomines ana marsupiais were morphologically and biologically indistinguishable from Trypanosoma cruzi. Forty seven percent (481/1,025) of triatomines were found concentrated in six palm trees where marsupiais circulated. Of the total of 1,025 triatomines 230 (22%) were infected by flagellates and 53.0% (123/230) of these infected triatomines were present in the same six palm trees. No evidence of triatomine domiciliation or human transmission was observed in the houses in the vicinity of palm trees. The results suggest that marsupiais play an important role in the life-cycle of T. cruzi in this region. The natural focus of Chagas' disease, demonstrated in the present study could represent a potential epidemiological threat.

One-hundred and thirty-five bats from 12 species were examined for thepresence of trypomastigotes by means of direct blood examination, xenodiagnosis, and hemoculture. Of those, 44 specimens (32.6%) from 8 species were infected with trypanosomes. Phyllostomus discolor discolor presented the highest rate of infection, being captured only in one locality, while Phyllostomus hastatus hastatus captured in four localities showed high rates. Two species, Anoura geoffroyii and Pteronotus (Phillodia) pamelli rubiginosa, were found infected by T. cruzi-like trypanosomes, apparently described for thefirst time. Flagellates from Artibeus jamaisensis jamaisensisa and A. geoffroyii were able toproduce detectable parasitaemia in young mice. One triatomine bug was found infected in natural conditions, Triatoma brasiliensis was associated with a P.h. hastatus colony, in which six captured bats were also found infected.

The C. cerifera palm tree (carnaúba) is widely distributed in the Northeastem Brazil, including the State of Piauí. This investigation revealed that R. nasutus is the ortly triatomine species captured on that palm tree, in five different localities. 78% of palm trees were infested with triatomines, and 4.0% were infected with flagellates morphologically and biologically indistinguishable from Trypanosoma cruzi. Birds, rodents and marsupials were found as major blood meai sources for R. nasutus.

O Triatoma brasiliensis, Triatoma pseudomaculata e Rhodnius nasutus foram capturados em ecótopos artificiais e naturais em dez localidades situadas na área rural dos municípios de Castelo do Piauí e Pedro II, Estado do Piauí, Brasil. O estádio de ninfa foi predominante e as aves foram a principal fonte alimentar dos triatomíneos. O único triatomíneo capturado no interior dos domicílios investigados foi o T. brasiliensis que albergava flagelados morfo e biologicamente indistinguíveis do Trypanosoma (Schizotrypanum) cruzi. A sorologia específica contra o T. cruzipor imunofluorescência indireta (IFI) revelou 21,7% de positividade entre 566 habitantes examinados. Os resultados sugerem que nessas localidades ocorre a transmissão ativa da doença de Chagas.
