The management of urban environment, together with the preservation of the natural environment and the creation of a sustainable built environment, is a complex challenge for contemporary societies. In the name of progress, cities are contributing for the degradation of all surrounding ecosystems. Therefore there is an arising demand for developing new strategies and a new urban development paradigm settled in the search for the equilibrium between natural and built environments and efficient use of resources. The objective of this paper is to analyse how the urban expansion of the city of Estarreja took place in relation to the land use, based on the land capability classification maps of the area. Based in the results some sustainable development strategies that might be applied to the city are discussed. The obtained results demonstrate that the city has been growing faster then its population, consuming vast portions of land, since its growth as been occurring in a linear form. Despite this fact, results show that most of this expansion took place towards a territory of lower agricultural potential, when comparing to the location of its original settlement.

We report the case of a 42-year-old female with fatigue on exertion and palpitation consequent to the existence of isolated noncompaction of the myocardium. We discuss clinical and familial findings, diagnostic possibilities, and prognostic and therapeutical implications of this rare disorder of endomyocardial morphogenesis.

PURPOSE: To evaluate 2 left ventricular mass index (LVMI) normality criteria for the prevalence of left ventricular geometric patterns in a hypertensive population ( HT ) . METHODS: 544 essential hypertensive patients, were evaluated by echocardiography, and different left ventricular hypertrophy criteria were applied: 1 - classic : men - 134 g/m² and women - 110 g/m² ; 2- obtained from the 95th percentil of LVMI from a normotensive population (NT). RESULTS: The prevalence of 4 left ventricular geometric patterns, respectively for criteria 1 and 2, were: normal geometry - 47.7% and 39.3%; concentric remodelying - 25.4% and 14.3%; concentric hypertrophy - 18.4% and 27.7% and excentric hypertrophy - 8.8% and 16.7%, which confered abnormal geometry to 52.6% and 60.7% of hypertensive. The comparative analysis between NT and normal geometry hypertensive group according to criteria 1, detected significative stuctural differences,"( *p < 0.05):LVMI- 78.4 ± 1.50 vs 85.9 ±0.95 g/m² *; posterior wall thickness -8.5 ± 0.1 vs 8.9 ± 0.05 mm*; left atrium - 33.3 ± 0.41 vs 34.7 ± 0.30 mm *. With criteria 2, significative structural differences between the 2 groups were not observed. CONCLUSION: The use of a reference population based criteria, increased the abnormal left ventricular geometry prevalence in hypertensive patients and seemed more appropriate for left ventricular hypertrophy detection and risk stratification.

PURPOSE: To evaluate left ventricular mass (LVM) index in hypertensive and normotensive obese individuals. METHODS: Using M mode echocardiography, 544 essential hypertensive and 106 normotensive patients were evaluated, and LVM was indexed for body surface area (LVM/BSA) and for height² (LVM/h²). The 2 indexes were then compared in both populations, in subgroups stratified according to body mass index (BMI): <27; 27-30; > or = 30kg/m². RESULTS: The BSA index does not allow identification of significant differences between BMI subgroups. Indexing by height² provides significantly increased values for high BMI subgroups in normotensive and hypertensive populations. CONCLUSION: Left ventricular hypertrophy (LVH) has been underestimated in the obese with the use of LVM/BSA because this index considers obesity as a physiological variable. Indexing by height² allows differences between BMI subgroups to become apparent and seems to be more appropriate for detecting LVH in obese populations.

PURPOSE - To evaluate diastolic dysfunction (DD) in essential hypertension and the influence of age and cardiac geometry on this parameter. METHODS - Four hundred sixty essential hypertensive patients (HT) underwent Doppler echocardiography to obtain E/A wave ratio (E/A), atrial deceleration time (ADT), and isovolumetric relaxation time (IRT). All patients were grouped according to cardiac geometric patterns (NG - normal geometry; CR - concentric remodeling; CH- concentric hypertrophy; EH - eccentric hypertrophy) and to age (<40; 40 - 60; >60 years). One hundred six normotensives (NT) persons were also evaluated. RESULTS - A worsening of diastolic function in the HT compared with the NT, including HT with NG (E/A: NT - 1.38±0.03 vs HT - 1.27±0.02, p<0.01), was observed. A higher prevalence of DD occurred parallel to age and cardiac geometry also in the prehypertrophic groups (CR). Multiple regression analysis identified age as the most important predictor of DD (r²=0.30, p<0.01). CONCLUSION - DD was prevalent in this hypertensive population, being highly affected by age and less by heart structural parameters. DD is observed in incipient stages of hypertensive heart disease, and thus its early detection may help in the risk stratification of hypertensive patients.

Glossopharyngeal neuralgia is an uncommon condition that has rarely been described in association with syncope. We report here 2 cases of glossopharyngeal neuralgia in elderly patients. Both were male and underwent temporary pacemaker insertion to prevent syncopal episodes. We discuss the clinical and surgical treatment of glossopharyngeal neuralgia, the role of cardiac stimulation, and the possible physiopathological mechanism of the associated cardiac disturbances.

OBJETIVO: Estimar o custo anual do manejo da doença arterial coronária (DAC) em valores do SUS e convênios. MÉTODOS: Estudo de coorte, incluindo pacientes ambulatoriais com DAC comprovada. Considerou-se para estimar custos diretos: consultas, exames, procedimentos, internações e medicamentos. Valores de consultas e exames foram obtidos da tabela SUS e da Lista de Procedimentos Médicos (LPM). Valores de eventos cardiovasculares foram obtidos de internações em hospital público e privado com estas classificações diagnósticas em 2002. O preço dos fármacos utilizado foi o de menor custo no mercado. RESULTADOS: Os 147 pacientes (65±12 anos, 63% homens, 69% hipertensos, 35% diabéticos e 59% com IAM prévio) tiveram acompanhamento médio de 24±8 meses. O custo anual médio estimado por paciente foi de R$ 2.733,00, pelo SUS, e R$ 6.788,00, para convênios. O gasto com medicamentos ($ 1.154,00) representou 80% e 55% dos custos ambulatoriais, e 41% e 17% dos gastos totais, pelo SUS e para convênios, respectivamente. A ocorrência de evento cardiovascular teve grande impacto (R$ 4.626,00 vs. R$ 1.312,00, pelo SUS, e R$ 13.453,00 vs. R$ 1.789,00, para convênios, p<0,01). CONCLUSÃO: O custo médio anual do manejo da DAC foi elevado, sendo o tratamento farmacológico o principal determinante dos custos públicos. Essas estimativas podem subsidiar análises econômicas nesta área, sendo úteis para nortear políticas de saúde pública.

OBJETIVO: Avaliar o tratamento da insuficiência cardíaca (IC) avançada em relação à redução das pressões de enchimento ventricular, com a utilização de doses maiores de vasodilatadores, através da monitorização hemodinâmica invasiva. MÉTODOS: Foram estudados 19 pacientes com IC avançada, nos quais foi instalado o cateter de Swan-Ganz para guiar a administração de diurético intravenoso (IV) e nitroprussiato de sódio, com o objetivo de se reduzir de forma significativa as pressões de enchimento ventricular. Depois de alcançado esse objetivo ou 48 horas, foram introduzidas drogas orais até serem retirados os fármacos venosos, mantendo o benefício hemodinâmico. RESULTADOS: Dos 19 pacientes estudados, 16 (84%) eram do sexo masculino. A idade média foi de 66 ± 11,4 anos; a fração de ejeção média foi de 26 ± 6,3%; 2 pacientes (10,5%) apresentavam classe funcional (CF) III e 17 (89,5%), CF IV. Houve queda da pressão de oclusão da artéria pulmonar de 23 ± 11,50 mmHg para 16 ± 4,05 mmHg (p=0,008), do índice de resistência vascular sistêmica de 3.023 ± 1.153,71 dynes/s/cm-5/m² para 1.834 ± 719,34 dynes/s/cm-5/m² (p=0,0001) e aumento do índice cardíaco de 2,1 ± 0,56 l/min/m² para 2,8 ± 0,73 l/min/m² (p=0,0003). Um subgrupo com hipovolemia foi identificado. CONCLUSÃO: Foi possível reduzir as pressões de enchimento ventricular para valores significativamente menores, obtendo melhora significativa do índice cardíaco, do índice de resistência vascular sistêmica e da pressão média da artéria pulmonar, utilizando-se doses significativamente maiores de vasodilatadores.
