A pneumonia nosocomial é a principal causa de infecção nosocomial em unidades de tratamento intensivo e possui alta morbi/mortalidade. A incidência cumulativa varia, conforme os autores, entre limites amplos desde 8% até 51%, dependendo do tipo de paciente e do uso de instrumentação das vias respiratórias. Nos casos específicos das pneumonias de má resolução e da pneumonia associada à ventilação mecânica, o diagnóstico é problemático devido à ausência de uma padronização definitiva, decorrente não só da grande variabilidade fisiopatológica como também da pouca acurácia dos critérios clínicos, microbiológicos e radiológicos. Estes fatos ensejaram a utilização progressiva de técnicas invasivas sofisticadas para coleta de amostras respiratórias endobrônquicas. Entretanto, a validação dessas técnicas para uso sistemático depende ainda de estudos que avaliem não só o seu custo/benefício em cenários clínicos diversos como também a comparação de seu desempenho para o diagnóstico com um padrão-ouro histopatológico. Além disso, o rendimento das técnicas invasivas é freqüentemente limitado quando são aplicadas em pacientes sob antibioticoterapia, que constituem a maioria dos casos em unidades de cuidados intensivos. A otimização desses testes, discutindo suas indicações e avaliando sua capacidade técnica para a confirmação ou exclusão de pneumonia, é justificada pela necessidade da instituição precoce e correta do tratamento, pois disto resulta menor mortalidade, redução de custos e permanência no hospital. Entre os testes que podem auxiliar no diagnóstico encontra-se o exame direto do lavado broncoalveolar, que proporciona resultados precoces e úteis para a tomada de decisão, mas não suficientemente testados em diferentes situações clínicas ou experimentais. Com o objetivo de avaliar o rendimento diagnóstico do exame direto precoce e das culturas quantitativas obtido por lavado broncoalveolar, estudou-se sua utilização em um modelo experimental de pneumonia provocada através de inoculação bacteriana intrabrônquica em ratos. O estudo comparou a acurácia do exame direto e dos exames culturais em três grupos de animais: Grupo A com pneumonia pneumocócica (37 animais); Grupo P com pneumonia por P. aeruginosa (26 animais) e Grupo B controle (10 animais), utilizando a histopatologia dos pulmões como teste de referência para o diagnóstico. Os Grupos A e P foram ainda randomizados em dois subgrupos cada, para tratamento ou não com antibióticos, usando penicilina no grupo pneumococo e amicacina no grupo Pseudomonas. Como resultado, observou-se que nos animais com pneumonia e ausência de antibióticos a pesquisa de bactéria fagocitada (BIC) no exame direto mostrou um rendimento elevado para o diagnóstico, sendo superior ao das culturas quantitativas. No grupo com pneumonia pneumocócica a BIC mostrou: S:94,4% a 100%, E:100%, VPP:100% e VPN:100%; e as culturas quantitativas mostraram: S:77,8%, E:100%, VPP:100%, VPN:40%. Nos com pneumonia por Pseudomonas a BIC obteve: S: 69%, E:100%; VPP:100% e VPN:71,4%); e as culturas quantitativas mostraram valores muito baixos: S:28,6%, E:100%, VPP:100% e VPN:50%). Nos animais com pneumonia pneumocócica sob tratamento antibiótico havia uma queda acentuada de sensibilidade das culturas quantitativas (S:21%) e, em menor grau da BIC (S:57,9%), mas sem perda da especificidade de ambos os exames. Ao contrário, nos animais com pneumonias por Pseudomonas sob tratamento não havia alteração no rendimento dos dois exames, cujos resultados foram semelhantes aos dos animais sem tratamento. Não havia diferenças de leitura da BIC para o diagnóstico, contando a sua positividade em macrófagos ou em neutrófilos infectados. A avaliação global dos casos estudados, reunindo todos os grupos (tratados e não-tratados) em ambos os modelos de pneumonia, mostrou que a acurácia do exame direto, representado pela pesquisa da BIC, foi superior (66%) ao das culturas quantitativas (53%). As conclusões principais do estudo foram: 1) o exame direto do lavado broncoalveolar (LBA) mostrou-se um teste útil e de alto rendimento para o diagnóstico de pneumonia quando realizado na ausência de antibióticos; 2) o LBA feito na vigência de antibióticos efetivos para a pneumonia perde mais de 50% de sua acurácia, mas não é afetado quando o antibiótico for ineficaz ou houver resistência ao mesmo; 3) a pesquisa de BIC no LBA é um exame de resultado precoce, de alta especificidade e com melhor rendimento do que as culturas quantitativas.

O enfoque fenomenológico de Alonso Fernandes foi a base deste estudo. O autor tenta provar que houve uma evolução qualitativa na forma de descarga da tensão excessiva, um desembocar somático da angústia. Melhor definindo, nos dias de hoje, não há mais lugar para a histeria demonstrativa, teatral, necessitando palco para bem resultar. A vida atual, impessoal, ativa e materialista tornou obsoleto o mecanismo transformador da angústia reprimida em símbolo corporal. O endeusamento do jovem, a falta de espaço, o nível de aspiração muito elevado e o estresse em alta doses torna compreensível que seja sobre este mesmo corpo que ele lance todo o excesso de tensão e angústia que é ensejado pela vida urbana. O presente estudo indaga a existência de uma patologia especifica para os indivíduos que reagem desta forma. Para tal verificação usou-se 3 grupos de 30 indivíduos subdivididos em: doentes de "fundo nervoso", doentes "orgânicos" e sadios. Dois tipos de instrumento foram aplicados com o objetivo de estudar o perfil de personalidade da amostra: O Inventário de Personalidade MHPI e um questionário que abrangesse o tipo de vida, costumes, hábitos, renda, escolaridade, etc do grupo em estudo. A hipótese básica é que não há uma patologia mental específica nos pacientes com doenças de "fundo nervoso" e que fatores tais como a exiguidade do espaço vital, o nível de aspiração, a preocupação com a autoimagem e o nível de estresse influenciam predominantemente a personalidade dos sujeitos pertencentes ao grupo das doenças psicossomáticas. Comprovou-se com boa margem de certeza a ideia de que a fuga para o corpo não é um quadro patológico delimitado mas sim um mecanismo de defesa, uma ajuda ao indivíduo obrigado a sobreviver as pressões de nossos dias. Além disto, a existência de respostas relativas a um nível de aspiração exagerado no grupo de doentes de "fundo nervoso" comprova ser tal variável uma das grandes vertentes propiciadoras da escolha da via somática como forma de descarga da angústia.

The Bologna Process introduced some changes in the curriculum of higher education institutions (HEIs) and defined that academic learning should consider the needs of the labour market. HEIs and employers agree that personal skills are the most important set of competence of graduates (Pavlin, Akkuyunlu, Kovacic, & Svetlik, 2009). The goals of this work were to explore how the work experienced by teams of students in HEIs might help them improve their personal skills, namely empirically explore the perception of teamwork and personality into two groups of students. The study was based on the theoretical model of Team Evolution and Maturation (TEAM, Fransen, 2012). The sample consisted of 99 students of the 3rd year of the degree (1st cycle) in Computer Science (49 students) and the 2nd year of the Bachelor's Degree (1st cycle) in Psychology (50 students), from the University of Madeira, Portugal. Areas of personality and team collaboration were evaluated with a Pre- and Post-test. Findings show that the perception of the teamwork collaboration of students in Computer Science and Psychology majors seems to be influenced by their scientific area, by gender, by the selection method of the time-organiser, the self-perceived personality of the time-organiser, the self perceived personality of the non-time-organiser, and the size of the team. It is expected that this data will contribute to further theoretical and practical reflection on the teamwork among college students and their performance in the labour market.

Individual lifestyle includes health and risk behaviors that can altar health status. Excess weight is a public health problem of modern civilization and there is an estimated mean prevalence of 45% in European countries. In Spain, the Murcia Region is an area of high morbidity and mortality from cardiovascular disorders. In this study we assess the differences in health and risk behaviors in ove/weight and normal weight undergraduates at the Universidad Católica San Antonio de Murcia (UCAM). Methods: Transversal design of parallel groups (overweight - cases and normal weight - control) , formed using the anthropometric technique. A questionnaire applied to a sample of 471 undergraduates of either sex, between the ages of 18 and 29 years, enrolled in 4 bachelor degree courses (ADE, CA, PER, PUB) at UCAM. We performed a standardized measurement of body mass (weight in kg), height (in meters) using a Seca® scale with calibrated stadiometer, waist and hip circumferences (in cm) with an inelastic tape and skinfolds thickness (triceps and subscapular in mm) with a Holtain® caliper, to calculate body mass index (BMI), waist-to-hip ratio (WHR) and the sum of skinfolds (SSF). We applied a lifestyle questionnaire about alcohol and tobacco consumption, knowledge and behaviors related to health indicators (arterial pressure and cholesterol), diet and physical activity. The information was collected in April and May, 2001 at the UCAM laboratory of Applied Nutrition. Statistical analysis: analysis of independent groups, contingency tables that reveal which qualitativa variables show differences and associations between the groups, Pearson's chi-square,and a significance levei of p < 0.05 followed by a residual analysis (1.96). Descriptive statistics (mean and standard deviation) were used to establish the two groups: case and contrai with 65 men and 26 women each who had BMI < 25 kg/m2. Results: A total of 65 of the men assessed (14%) and 26 (6%) of the women were overweight. Mean body mass index of the case group was 27. 78 ±: 2.83 kg/m2 in the men and 26.26 ± 1.37 kg/m2 in the women, while contrai group men had mean BMI of 22.36 ± 1.72 kg/m2, while for the women it was 20.76 ±: 2.13 kg/m2. The self-declared values of weight and height were underestimated, but with high accuracy, sensitivity and specificity. Thus, these can be used to calculate the BMI of overweight Spanish undergraduates. Regular vigorous physical activity was observed only in normal weight men. The analysis showed the following significant differences for the qualitativa variables of the two groups. The contrai group was interested in arterial hypertension, believed that they were not overweight, that they had no abdominal fat, and had not considered controlling 'fatty food consumption. Those who thought of controlling it sometimes, did so without professional help. However, part of the overweight group believed that they were overweight and had abdominal fat between average and considerable, had often or always considered controlling fatty foods and had often or always tried to control consumption with the help of professionals. They had always thought of engaging in physical activities, unlike the normal weight individuals. Nearly all (95%) of the overweight undergraduates and most (75%) of the normal weight group reported that they sometimes or always controlled fatty food ingestion. Mean physical activity was nearly twice as high in the summer than in the winter. Conclusions: The overweight undergraduates in this sample displayed a lifestyle with a greater number of healthy behaviors when compared to normal weight individuals

Exposure to intermittent episodes of social defeat stress can increase drug seeking and leads to intense drug taking in rats.This study investigated the consequences of repeated, intermittent social defeat stress on patterns of drug self-administration in rats with access to heroin, cocaine, or a heroin-cocaine combination (speedball).Male Long-Evans rats were either handled (controls) or subjected to 25-min social defeat stress episodes on days 1, 4, 7, and 10 during confrontations with an aggressive resident. Ten days following the last defeat, rats were assessed for locomotor cross-sensitization in response to heroin or cocaine. Animals were then prepared with intrajugular catheters for drug self-administration. Separate groups of controls and defeated rats were examined for self-administration of heroin (experiment 1), a heroin-cocaine combination (experiment 2), or cocaine (experiment 3). Drug self-administration patterns were evaluated using fixed or progressive ratio schedules of reinforcement during limited access sessions or a 24-h unlimited access binge.Rats with a history of intermittent social defeat stress showed sensitized locomotor behavior when challenged with heroin or cocaine relative to controls. During the 24-h binge session, defeated rats escalated cocaine-taking behavior (ca. 110 mg/kg vs. 66 mg/kg in controls), persisted in self-administering cocaine or the heroin-cocaine mixture for more hours, and showed a tendency for increased heroin-cocaine intake, but no effects on heroin taking.A history of social defeat stress seems to preferentially promote escalated intake of cocaine but not heroin, unless a heroin-cocaine combination is available.

Human cooperation is a hallmark of this species due to its wide extension to genetically unrelated individuals and complex division of labor. It is considered an evolutionary puzzle, because the theory of evolution by natural selection predicts that self-interested individuals tend to be selected. Different theories have been proposed to explain the evolution of cooperation, which the most important are kin selection and reciprocal altruism. Considering the evolutionary continuity between species, humans and other primates have several common traits that help to promote cooperation between individuals of these species. Two features, however, seem to be particularly humans: inequality aversion and preferences in relation to others. Although human cooperation is not necessarily related to morality, cooperative traits are the basis for moral tendencies. The development of human morality is a combination of early prosocial tendencies, cooperative skills displayed at different ages, social learning and cultural transmission of norms. The social stimulus seems to be particularly important in promoting cooperative behavior in children and adults. In order to study the influence of social stimuli, as verbal feedback, on children cooperation, a study was conducted with children in a public goods game. 407 children from public schools in Natal / RN, divided into 21 groups, between six and nine years, participated in eight rounds of this game. After each round, seven groups received praise for larger donations, seven groups have been criticized by smaller donations, and the other seven received no comment. Children cooperated more when criticized, without significant differences between sexes, although young children have cooperated more negative than older children. The results are likely related to the anticipation and avoidance of punishment associated with the feedback (although this did not occur), and greater sensitivity to the authority in younger children. Nevertheless, the cooperation decreased in all groups until the last day of play. The results suggest an early sensitivity to moral punishment, whose role in the maintenance of social relations must have been important in the evolution of cooperation in humans

Objectives: the purpose of this study is to employ optical microscopy to measure the thickness of the hybrid layer and the penetration (tags) of an aggressive self-etching adhesive system into sound dentin.Methods: occtusat cavities were prepared in 40 extracted human posterior teeth. The prepared teeth were randomly assigned to four experimental groups with 10 specimens each. The self-etching adhesive system Adper Prompt L-Pop was applied to the dentin surface as follows: Group 1: cavosurface enamel was etched for 60 s and dentin for 20 s with 35% phosphoric acid get, immediately followed by application of the self -etching adhesive with a brush to the entire cavity for 15 s; Groups 2, 3, and 4: no pre-etching was performed, and the self -etching adhesive was applied to both enamel and dentin for 15, 30 and 45 s, respectively. After curing, the cavities were fitted with composite resin Fittek Z250. Afterwards, the teeth were decalcified and the restorations were carefully removed for later embedding in paraffin. The specimens were serially sectioned at 6 mu m of thickness and sequentially mounted in glass slides. These sections were stained with Brown and Brenn staining for posterior analysis and measurement of the hybrid layer and resin tags on a tight microscope with a micrometric ocular 40/075. The results were submitted to analysis of variance at the 5% level.Results: whenever there was significance, the Tukey test was applied at the 5% level. The specimens receiving application of acid etching before the selfetching. adhesive displayed a larger thickness of the hybrid layer; on the other hand, specimens receiving only application of the self -etching adhesive on dentin for 15, 30 and 45 s exhibited similar thickness of the hybrid layer. As regards the resin tags, no statistically significant differences could be found between the study groups.Conclusions: it could be concluded that the increase in the time of application of the self-etching adhesive Adper Prompt L-Pop did not significantly influence the formation and thickness of hybrid layer, as well as its penetration into the sound dentin surface. The aim of this study was to evaluate the response of human pulps capped with a calcium hydroxide hard-setting cement or with two-step self-etch adhesive systems. Pulp exposures were performed on the occlusal floor, and the bleeding control was performed with saline solution. The exposed pulp tissue was capped with Clearfil LB 2V (2V) or Clearfil SE Bond (SE) and restored with a composite resin. In control group, the pulpal wound was capped with Ca(OH)(2) cement and restored with Clearfil LB 2V or Clearfil SE Bond + composite resin. After 30 and 90 days, the teeth were extracted, processed for hematoxylin and eosin, and categorized in a histological score system. The pulpal response was worse for groups capped with the self-etch adhesive systems (2V and SE) in both periods of evaluation, when compared to their respective control groups at 90 days (p < 0.05). For both self-etch systems evaluated, the pulp tissue exhibited moderate to severe inflammatory cell infiltrate involving the coronal pulp with chronic abscesses. Dentin bridging was observed in a few specimens. For the calcium hydroxide groups, almost all specimens showed dentin bridge formation, with few scattered inflammatory cells and normal tissue below the pulp exposure site. Calcium hydroxide should be used as the material of choice for pulp capping, and the use of two-step self-etch adhesives for human pulp capping is contraindicated.

The purpose of this study was to investigate the bond strength of fiber post previously laser treated root canals. Forty single-rooted bovine teeth were endodontically treated, randomly and equally divided into two main groups according to the type of pretreatment: G1: 2.5% NaOCl (control group); and G2: Er,Cr:YSGG laser. Each group was further subdivided into 2 groups based on the category of adhesive systems/ luting materials used: a: an etch-and-rinse resin cement (Single Bond/RelyX ARC; 3M ESPE), and b: a self-adhesive resin cement (Rely X Unicem; 3M ESPE). Three 1.5 mm thick slabs were obtained per root and the push-out test was performed at a crosshead speed of 0.5 mm/min until post dislodgement occurred. Data were analyzed by ANOVA and post-hoc Tukey's test at a pre-set alpha of 0.05. Analysis of variance showed no statistically significant difference (p > 0.05) among the groups G1a (25.44 ± 2.35) and G1b (23.62 ± 3.48), G2a (11.77 ± 2.67) and G2b (9.93 ± 3.37). Fractures were observed at the interface between the dentin and the resin in all groups. The Er,Cr:YSGG laser irradiation did not influence on the bond strength of the resin cements and the etch-and-rinse resin cement had better results on bond strength than self-adhesive resin cement.
