Introdução:Os resultados alcançados pelos transplantes renais nas últimas décadas têm melhorado progressivamente. Objetivo:A fim de determinar a extensão desse progresso, conduzimos uma análise dos resultados obtidos em nosso programa de transplantes através de três períodos diferentes. Métodos:Avaliamos os 600 transplantes renais realizados no HC FMB-UNESP até dezembro de 2011, subdividindo-os em três eras, de acordo com a imunossupressão vigente. Era 1: de 1987 a 2000 (n = 180); associação de ciclosporina e azatioprina. Era 2: de 2001 a 2006 (n = 120); associação de ciclosporina e micofenolato e Era 3: de 2007 a 2011 (n = 300); associação de tacrolimus e micofenolato. Resultados:Os resultados mostram aumento da idade média do receptor, da prevalência de diabetes e do número de transplantes com doador falecido (60%) na terceira era. O uso de terapia de indução foi de 75% era atual contra 46,6% (Era 2) e 3,9% (Era 1), p < 0,0001. Os dados de sobrevida geral por tipo de doador mostram dados semelhantes à literatura. Houve progressivo aumento da sobrevida do enxerto com doadores falecidos em 5 anos, saindo de 13,7% (Era 1) para 81,9% (Era 3). Conclusão:Houve significativas diferenças ao longo do tempo, culminando com aumento do volume de transplantes na atual era (média de 14 transplantes/ano na Era 1 para 75 transplantes/ano na Era 3). Inverteu-se o perfil de transplantes na era atual com predomínio de doador falecido. A melhor sobrevida com doador falecido da atual era foi atribuída a maior experiência do centro e aos esquemas de imunossupressão baseados na combinação de tacrolimus com micofenolato associados a esquema de indução.

Background: Little information on the factors influencing intraoperative cardiac arrest and its outcomes in trauma patients is available. This survey evaluated the associated factors and outcomes of intraoperative cardiac arrest in trauma patients in a Brazilian teaching hospital between 1996 and 2009.Methods: Cardiac arrest during anesthesia in trauma patients was identified from an anesthesia database. The data collected included patient demographics, ASA physical status classification, anesthesia provider information, type of surgery, surgical areas and outcome. All intraoperative cardiac arrests and deaths in trauma patients were reviewed and grouped by associated factors and also analyzed as totally anesthesia-related, partially anesthesia-related, totally surgery-related or totally trauma patient condition-related.Findings: Fifty-one cardiac arrests and 42 deaths occurred during anesthesia in trauma patients. They were associated with male patients (P<0.001) and young adults (18-35 years) (P = 0.04) with ASA physical status IV or V (P<0.001) undergoing gastroenterological or multiclinical surgeries (P<0.001). Motor vehicle crashes and violence were the main causes of trauma (P<0.001). Uncontrolled hemorrhage or head injury were the most significant associated factors of intraoperative cardiac arrest and mortality (P<0.001). All cardiac arrests and deaths reported were totally related to trauma patient condition.Conclusions: Intraoperative cardiac arrest and mortality incidence was highest in male trauma patients at a younger age with poor clinical condition, mainly related to uncontrolled hemorrhage and head injury, resulted from motor vehicle accidents and violence.

Our main purpose in this study was to quantify biological tissue in computed tomography (CT) examinations with the aim of developing a skull and a chest patient equivalent phantom (PEP), both specific to infants, aged between 1 and 5 years old. This type of phantom is widely used in the development of optimization procedures for radiographic techniques, especially in computed radiography (CR) systems. In order to classify and quantify the biological tissue, we used a computational algorithm developed in Matlab (R). The algorithm performed a histogram of each CT slice followed by a Gaussian fitting of each tissue type. The algorithm determined the mean thickness for the biological tissues (bone, soft, fat, and lung) and also converted them into the corresponding thicknesses of the simulator material (aluminum, PMMA, and air). We retrospectively analyzed 148 CT examinations of infant patients, 56 for skull exams and 92 were for chest. The results provided sufficient data to construct a phantom to simulate the infant chest and skull in the posterior anterior or anterior posterior (PA/AP) view. Both patient equivalent phantoms developed in this study can be used to assess physical variables such as noise power spectrum (NPS) and signal to noise ratio (SNR) or perform dosimetric control specific to pediatric protocols.

The demand for health services can be understood as an application needs the user has. The inability to use the outpatient services and limited supply of these make it possible for users to browse sites that focus attention on a greater possibility of entry doors, in which first-aid centers and hospital emergency match this profile, distorting the flow of patients in the network through an inverse demand of the movement hierarchy. Added to this, the burden of care in these services results in overcrowding and poor quality of care. Evaluate the demand of the visits occurred in the Emergency Room of the Hospital of the Medical School of Botucatu / UNESP (PS - HC - FMB / UNESP) during June-July 2010. A transversal, descriptive and retrospective. For data collection sheet was used in the proposed Service unit and the data it was filled out the form with the necessary items for the search. Made an exploratory analysis and frequency distributions for categorical variables of the form. Females predominated (56%) and aged 61 years or older with 30%. 96.5% were owned by DRS VI, and 62.5% of Botucatu. The attendance by the physician on duty and corresponded to 57.7% among the 23 medical specialties, Gastric (7.0%), Cardiology (4.5%), Medical (4.4%), urology (4.2%) and Pulmonology (4.1%) were the ones that stood out. The medical procedures performed that stood out were X-ray (46.4%) and electrocardiogram (ECG) (42.3%) and in most specialty care occurred, only the daily consultation with the patient. It was possible to characterize, so the demand for PS - HC - FMB / UNESP for the period June- July 2010, The predominance of the elderly shows that come along with aging diseases and addictions, causing a greater need for health services. Not only the progress of the radiotherapy, but also the improve of the dose in the patient with adjusted radiotherapy plannings, calculation of shield, radiometric survey, among others, are responsibilities of the medical physic inside of a sector of radiotherapy. This monograph has as objective to above show some of these cited functions of the medical physicis, starting with a theoretical basement and at as a moment to present the calculations and the results found during a visit in the Institute of Radiotherapy of the Valley of the Paraíba, located in the city of São José dos Campos, and in the Regional Hospital of Taubaté in the period of 2006, 2007 and 2008

Health Care Waste (HCW) represents 1%, and it has presently gained a lot of importance. Adequate management is one of the great challenges to be faced by health care centers. It has gained distinction and been widely discussed by members of the sectors involved with sanitation, public health and environmental issues due to waste physical, chemical biological characteristics, which pose potential risk to the environment and public health. The present study aims at evaluating HCW internal management by following all its phases, determining indicators, classifying and quantifying, establishing production rates (kg /patient/day) for the sector and designing materials to disseminate appropriate HCW disposal in the Emergency Room of the UNESP University Hospital in the city of Botucatu according to the guidelines presently in force. : From June to October 2011, the waste flow was observed from its production to final disposal. Four weight measurements were performed on four consecutive days in the month of August by using a properly calibrated (in grams) digital scale at the times scheduled for collection of the produced waste. Hence, the daily and monthly amounts were estimated according to their classification. All the waste packaged in the bags in garbage cans in the Emergency Room for a 24-hour period was considered to be a sample. Separation was not adequately performed in that sector, and waste from Group A was mixed with that from Group D. The amount of infectious waste produced in the sector corresponded to 87.80 %, common waste to 10.93 % and recyclable waste to 1.27%. The mean daily HCW production was of 123.300 kg/day, and the total monthly production was of 3,822 kg/month, which was distributed as follows: Group A 3,355,750 kg/month; Group D 417,570 kg/month and recyclables 48,670 kg/month. With the increasing competitiveness of organizations in search of customers, it is essential that the pursuit of quality becomes a prerequisite for the survival of healthcare organization, is necessary to have working conditions that encourage both personal and professional satisfaction. This study aims to investigate the perception of nurses regarding their satisfaction in the context of work in a State Hospital and contribute to the reflection on the internal customer satisfaction as an important indicator of quality of health services that interfere with quality of care. We used the qualitative method, on phenomenology, with nurses from a state hospital that had at least one year of experience in the institution. Through interviews, it was revealed that satisfaction is related to the literature and is influenced by factors such as human resources, practice nurses, Teamwork, Perception of the patient, professional recognition, physical and material resources and rewarding work, and these themes unfold quality and satisfaction both personally and for organizations

The World Health Organization (WHO) has created a program “Safe Surgery Saves Lives”, focusing on reduce and minimize life threatening or cause serious risk to health for patients submitted to a surgery. This primary goal for this presented study is to apply the checklist from the WHO´s program for children´s surgery in a public university hospital, located in the countryside of São Paulo State, Brazil. Having the responses from professionals surgery´s team, there is an opportunity for evaluation of the features and easiest parts, which are filled in a quickly and in a easy way; focus, supporting and assistance; issues, such as lack of time for filling in, and viability of application of this protocol considering the improvement of the cross functions relationship, and quality of services, providing better assistance and support to the patient
