The Affordable Care Act and the Transformation of US Health Care

The neoliberal period was accompanied by a momentous transformation within the US health care system.  As the result of a number of political and historical dynamics, the healthcare law signed by President Barack Obama in 2010 ‑the Affordable Care Act (ACA)‑ drew less on universal models from abroad than it did on earlier conservative healthcare reform proposals. This was in part the result of the influence of powerful corporate healthcare interests. While the ACA expands healthcare coverage, it does so incompletely and unevenly, with persistent uninsurance and disparities in access based on insurance status. Additionally, the law accommodates an overall shift towards a consumerist model of care characterized by high cost sharing at time of use. Finally, the law encourages the further consolidation of the healthcare sector, for instance into units named “Accountable Care Organizations” that closely resemble the health maintenance organizations favored by managed care advocates. The overall effect has been to maintain a fragmented system that is neither equitable nor efficient. A single payer universal system would, in contrast, help transform healthcare into a social right.

Market reforms and privatisation in the English National Health Service

The article examines developments in the marketisation and privatisation of the English National Health Service, primarily since 1997. It explores the use of competition and contracting out in ancillary services and the levering into public services of private finance for capital developments through the Private Finance Initiative. A substantial part of the article examines the repeated restructuring of the health service as a market in clinical services, initially as an internal market but subsequently as a market increasing opened up to private sector involvement. Some of the implications of market processes for NHS staff and for increased privatisation are discussed. The article examines one episode of popular resistance to these developments, namely the movement of opposition to the 2011 health and social care legislative proposals. The article concludes with a discussion of the implications of these system reforms for the founding principles of the NHS and the sustainability of the service.

Economic crisis and national health system

The text analyzes the impact of the economic crisis in some critical aspects of the National Health System: outcomes, health expenditure, remuneration policy and privatization through Private Public Partnership models. Some health outcomes related to social inequalities are worrying. Reducing public health spending has increased the fragility of the health system, reduced wage income of workers in the sector and increased heterogeneity between regions. Finally, the evidence indicates that privatization does not mean more efficiency and better governance. Deep reforms are needed to strengthen the National Health System.

Commodification of health care systems: The Catalan case

Commodification of the public healthcare system has been a growing process in recent decades, especially in universal healthcare systems and in high-income countries like Spain.  There are substantial differences in the healthcare systems of each autonomous region of Spain, among which Catalonia is characterized by having a mixed healthcare system with complex partnerships and interactions between the public and private healthcare sectors.  Using a narrative review approach, this article addresses various aspects of the Catalan healthcare system, characterizing the privatization and commodification of health processes in Catalonia from a historical perspective with particular attention to recent legislative changes and austerity measures.  The article approximates, the eventual effects that commodification and austerity measures will have on the health of the population and on the structure, accessibility, effectiveness, equity and quality of healthcare services.

Andrienne Pauline Macaire (1796-1855): una artista en los márgenes

Durante el siglo XIX diversos artistas europeos emprendieron el viaje hacia diferentes puntos del continente americano. Entre ellos, mucho se dedicaron a la litografía, logrando en ciertos casos un amplio éxito comercial. Este artículo examina la trayectoria de la artista ginebrina Andrienne Pauline Macaire (1796-1855), dueña junto a su esposo de un temprano emprendimiento litográfico en la ciudad de Buenos Aires. Macaire puede ser considerada la primera artista profesional de renombre activa en esa ciudad. En efecto, es posible diferenciar su trayectoria de las de otras artistas contemporáneas, de las cuales se dispone de menos información y de las que han sobrevivido menos obras. En una primera parte del texto se analiza su inscripción en la literatura artística nacional. A continuación, se describe su formación en Ginebra y se estudia su importante actividad en Buenos Aires, estableciendo conexiones con el panorama artístico local. A lo largo del artículo se presentan obras y fuentes inéditas, o insuficientemente exploradas, a fin de justipreciar su carrera artística y de brindar elementos para futuras investigaciones.

Divisible quantum dynamics satisfies temporal Tsirelson's bound

We show that divisibility of qubit quantum processes implies temporal Tsirelson's bound. We also prove that the classical bound of the temporal Bell's inequality holds for dynamics that can be described by entanglement-breaking channels---a more general class of dynamics than that allowed by classical physics.

Complete framework for efficient characterisation of non-Markovian processes

A non-Markovian process is one that retains `memory' of its past. A systematic understanding of these processes is necessary to fully describe and harness a vast range of complex phenomena; however, no such general characterisation currently exists. This long-standing problem has hindered advances in understanding physical, chemical and biological processes, where often dubious theoretical assumptions are made to render a dynamical description tractable. Moreover, the methods currently available to treat non-Markovian quantum dynamics are plagued with unphysical results, like non-positive dynamics. Here we develop an operational framework to characterise arbitrary non-Markovian quantum processes. We demonstrate the universality of our framework and how the characterisation can be rendered efficient, before formulating a necessary and sufficient condition for quantum Markov processes. Finally, we stress how our framework enables the actual systematic analysis of non-Markovian processes, the understanding of their typicality, and the development of new master equations for the effective description of memory-bearing open-system evolution.

Perfil antropométrico de futebolistas da segunda divisão gaúcha

O objetivo deste estudo é descrever o perfil antropométrico e a composiçao corporal de jogadores de futebol da divisao de acesso profissional do Rio Grande do Sul. O perfil antropométrico apresenta informaçoes comparativas das características do grupo de jogadores e reflete, indiretamente, seu estado de treinamento. Foram avaliados 17 atletas, com idade média de 25,31 ± 6,26. Mediu-se 7 dobras para composiçao corporal e utilizou-se a Equaçao Jackson e Pollock (1978). Os valores médios obtidos foram: massa corporal 77,4 ± 9,52; altura 177,93 ± 6,88; percentual de gordura 9,69 ± 3,09; e densidade corporal 1,076853894 ± 0,007. Na literatura, os valores de percentuais de gordura em atletas de futebol da divisao de acesso, variam entre 6 a 12. No presente estudo, os valores encontram-se dentro da faixa relatada, nao havendo necessidade de controle dietético para a reduçao de gordura corporal ou treinamento específico para este fim. Perfil antropométrico e da composição corporal em jogadores de voleibol : Estudo realizado com universitários da Universidade Federal de Santa Maria- Rio Grande do Sul

O objetivo deste estudo é descrever o perfil antropométrico e a composiçao corporal de jogadores de voleibol da Universidade Federal de Santa Maria- RS. A importância em obter o perfil antropométrico vai além de apenas comparaçoes do grupo de atletas, pois o desempenho e o efeito do treinamento está relacionado. Foram avaliados 14 atletas, com idade média de 22,26 ± 3,43. Mediu-se 7 dobras para composiçao corporal e utilizou-se a Equaçao Jackson e Pollock (1978). Os valores médios obtidos foram: massa corporal 82,28 ± 7,24; estatura 181,9 ± 6,1; percentual de gordura 18,86 ± 3,82; e índica de massa corporal 24,94 ± 2,24. Na literatura, o valor da porcentagem de gordura em atletas de voleibol é de 9,8 ± 2,9 . No presente estudo, os valores encontram-se acima da faixa relatada, havendo uma necessidade da intervençao da nutricionista no controle dietético para a reduçao de gordura corporal e associado o treinamento específico para este fim

Building Jobs in Iowa: New Deal Dams of the Wapsipinicon River Watershed in Northeast Iowa, February 4, 2015

The Wapsipinicon River, affectionately known as the “Wapsi,” flows nearly 300 miles through northeastern Iowa to the Mississippi. Strung throughout its drainage basin is a series of dams that are highly valued by their communities as places of recreation and scenic beauty. The Wapsipinicon has long had a reputation as a fishing stream, known for catfish, northern pike, carp, sunfish, walleye, and smallmouth bass. It is not uncommon to find anglers of all ages gathered at one of the Wapsipinicon’s dams to enjoy a day of fishing and possibly take home a prize catch. As time passes, however, a dam may outlive its original function, impede fish passage, and pose a safety hazard. But the idea of demolishing an outdated dam often rallies the community in an attempt to preserve the landmark.

If not the Virtual university then what? Co-producing e-learning and configuring its users

This paper reflects the changing notion of the Virtual University and its realization. We introduce an approach from the Sociology of Science and Technology (STS) which analyses the construction of the “student as a user” as seen through the “eyes of designers”. We ask how social relations are built into technological artefacts. In showing how socio-technical developments transcend sometimes contradict and various notions of “the student” we discuss difficulties and chances of bridging the gap between designers of e-learning-artefacts and its assumed addresses.
