Tämä insinöörityö on dokumentti projektista, joka on tehty SF-Design Oy:n nimissä asiakasprojektina. Työssä käydään läpi turva- ja opastevalojen ryhmävalvontayksikön elektro-niikkasuunnittelu ja siihen tarvittavan mikrotietokoneen ohjaamiseen tarvittavan ohjelman suunnittelu ja kirjoittaminen. Työ jakautuu karkeasti jaoteltuna neljään osa-alueeseen. Ensiksi käydään läpi laitteiston elektroniikkasuunnittelu ja siihen liittyvää teoriaa. Seuraavaksi työssä käsitellään mikrotietokoneiden ohjelmistokehitystä ja käydään läpi työssä suunniteltu ja laadittu ohjelmisto STMicroelectronicsin ST62T10C-prosessorille. Lopuksi ennen suunnittelu- ja kehitysympäristöjen esittelyä käsitellään suunnitellun laitteiston testaus-, käyttö- ja asennustoimenpiteet. Aivan aluksi esitellään projektissa toteutettuun laitesuunnitteluun vaikuttaneet asiakkaan antama spesifikaatio ja 13.2.2003 voimaan astunut Euroopan parlamentin ja neuvoston asettama RoHS-direktiivi. Direktiivi vaikutti elektroniikkasuunnitteluun komponenttien valinnan osalta. Direktiivin mukaan sen voimaanastumispäivän jälkeen ei saa käyttää komponentteja, jotka sisältävät ympäristölle ja ihmisille vaarallisia aineita. Elektroniikkasuunnitteluosiossa suunnittelu on jaettuna toiminnallisiin osiin. Osiossa käy-dään läpi virtamuuntajien kalibrointi sekä jännitevahvistimen suunnittelu ja simulointi. Ky-seisessä osiossa käsitellään vielä prosessorin vaatimat erityiskytkennät, vikailmoitus-led-valojen ohjaukseen tarvittava kytkentä sekä ennen laitteiston vaatimien käyttöjännitteiden suunnittelua laitteistoon tarvittavien potentiovapaitten lähtöjen toteutus. Ohjelmistokehi-tysosiossa pohditaan mikrotietokoneen ohjelmoinnin perusteita, ennen kuin projektissa laadittua ohjelmaa käydään läpi. Projektissa laadittua koodia ei sen salaisuuden takia käydä yksityiskohtaisesti läpi tässä työssä. Projektissa käytettiin elektroniikkasuunnittelussa PADS-ohjelmistoja PowerLogic ja PowerPCB. Lisäksi käytettiin vahvistinkytkennän simulointiin OrCADin Capture CIS -ohjelmaa. Projektin työläimpään vaiheeseen, ohjelman suunnitteluun, kehitykseen ja kirjoittamiseen käytettiin Raisonancen STMicroelectronicsin mikrotietokoneille kehittämää ohjelmistokehitysympäristöä RIDE -ohjelmistoa. Elektroniikkasuunnittelussa käytetyt oh-jelmistot ovat hyvin yleisesti teollisuuden käytössä.

The objective of this study was to compare the quality of life of women who survived breast cancer and women who have not had the disease, and to identify associated factors. It is cross-sectional study in which the comparison group includes survivors of breast cancer aged between 40 and 69 years. One hundred and fifty-four women were included, 70 with breast cancer and 84 without the disease. The SF-36 was used to evaluate quality of life. The chi-square test and multivariate linear models were used to compare the groups. The estimated mean physical and mental components were significantly better for the group who survived the disease (51.10 and 52.25, respectively) compared to the group without cancer (47.26 and 47.93, respectively). The study indicates that survivors of breast cancer had a better quality of life compared to women without the disease.
Qualidade de vida de pacientes com diabetes mellitus antes e após participação em programa educativo

Estudo quase-experimental com objetivo de avaliar a qualidade de vida relacionada à saúde de pessoas com diabetes mellitus, antes e após participação em um programa educativo de cinco meses. Participaram 51 sujeitos, com predomínio do sexo feminino (56,9%), média e desvio-padrão de 57,65 ± 11,44 anos de idade, em um serviço de atenção primária no interior paulista, em 2008. Para obtenção dos dados utilizou-se o questionário SF-36. O instrumento mostrou-se confiável para a população estudada, sendo os valores de alpha de Cronbach para os componentes físico e mental 0,83 e 0,89, respectivamente. Os resultados mostraram melhora discreta em quase todos os domínios, embora apenas o estado geral de saúde antes (63,96 ± 19,03) e após (70,59 ± 17,82) o programa educativo tenha apresentado diferença estatisticamente significativa t(50) = -2,16, p < 0,05. A participação dos sujeitos no programa educativo em diabetes mellitus também contribuiu para a melhoria da percepção acerca de seu estado geral de saúde.

Este estudo estimou a validade de constructo pelo teste de grupos conhecidos do Instrumento para Mensuração do Impacto da Doença no Cotidiano do Valvopata (IDCV) quanto a sinais e sintomas, função ventricular sistólica, fração de ejeção do ventrículo esquerdo (FEVE) e qualidade de vida relacionada à saúde (QVRS) em 153 coronariopatas em seguimento ambulatorial. Os dados foram obtidos pela aplicação do IDCV e das versões brasileiras do The Medical Study 36-item Short Form Health Survey – SF-36 e MacNew Heart Disease Health-related Quality of Life Questionnair e. Foi utilizado o teste de Mann-Whitney para verificar a capacidade do IDCV em discriminar o impacto quanto a sinais e sintomas, FEVE e disfunção sistólica ventricular, bem como o teste de Kruskal-Wallis para verificar seu poder de discriminação em relação à QVRS. Constatou-se que o IDCV discriminou o impacto entre aqueles que pontuaram nos quartis (≤Q1, Q1-Q3, ≥Q3) de QVRS. Os achados deste estudo contribuem para o refinamento do IDCV na mensuração do impacto da doença entre coronariopatas.

Objective To analyze pelvic floor muscle strength (PFMS), urinary continence and quality of life related to urinary incontinence (UI) of women in the first trimester of pregnancy. Method Cross-sectional study with a sample of 500 women who started prenatal care in a complementary healthcare facility in Guarulhos, state of São Paulo, from 2012 and 2013. Pelvic floor muscle strength was evaluated through perineometry. The pregnant women who presented UI answered the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF). Results It was found that maternal age (OR=1.06; CI95% 1.02-1.11) and prior UI (OR=15.12; 95%CI 8.19-27.92) are the variables that, in tandem, best explain the occurrence of UI at the beginning of pregnancy. The mean score on the ICIQ-SF was 8.2 (SD=3.9), considered a moderate impact on quality of life. Conclusion Older pregnant women with prior UI are more likely to have UI in the first trimester of pregnancy.

OBJECTIVE: The effect of minor orthopaedic day surgery (MiODS) on patient's mood. METHODS: A prospective population-based cohort study of 148 consecutive patients with age above 18 and less than 65, an American Society of Anaesthesiology (ASA) score of 1, and the requirement of general anaesthesia (GA) were included. The Medical Outcomes Study - Short Form 36 (SF-36), Beck Anxiety Inventory (BAI) and Beck Depression Inventory (BDI) were used pre- and post-operatively. RESULTS: The mean physical component score of SF-36 before surgery was 45.3 (SD=+/-10.1) and 8 weeks following surgery was 44.9 (SD=+/-11.04) [n=148, p=0.51, 95% CI=(-1.03 to 1.52)]. For the measurement of the changes in mood using BDI, BAI and SF-36, latent construct modelling was employed to increase validity. The covariance between mood pre- and post-operatively (cov=69.44) corresponded to a correlation coefficient, r=0.88 indicating that patients suffering a greater number of mood symptoms before surgery continue to have a greater number of symptoms following surgery. When the latent mood constructs were permitted to have different means the model fitted well with chi(2) (df=1)=0.86 for which p=0.77, thus the null hypothesis that MiODS has no effect on patient mood was rejected. CONCLUSIONS: MiODS affects patient mood which deteriorates at 8 weeks post-operatively regardless of the pre-operative patient mood state. More importantly patients suffering a greater number of mood symptoms before MiODS continue to have a greater number of symptoms following surgery.

OBJETIVE to create a reduced version of the QASCI, which is structurally equivalent to the long one and meets the criteria of reliability and validity. METHOD Through secondary data from previous studies, the participants were divided into two samples, one for the development of reduced version and the second for study of the factorial validity. Participants responded to QASCI, the SF 36, the ADHS and demographic questions. RESULTS A reduced version of 14 items showed adequate psychometric properties of validity and internal consistency, adapted to a heptadimensional structure that assesses positive and negative aspects of care. CONCLUSION Confirmatory factor analysis revealed a good fit with the advocated theoretical model.

O estudo da espiritualidade e a sua relação com a saúde mental e geral, sobretudo, associada ao envelhecimento, continua ainda muito escasso a nível nacional. Assim, o presente estudo tem como objetivo avaliar a relação entre a espiritualidade e a saúde mental e geral nos idosos institucionalizados. Trata-se de uma investigação quantitativa, do tipo observacional, descritivo e correlacional, transversal e entre-sujeitos. A amostra foi composta por 55 idosos, utentes da Santa Casa de Misericórdia de Braga (SCMB), de ambos os géneros, com idades compreendidas entre os 55 e os 99 anos. Os instrumentos utilizados foram a Escala de Avaliação da Espiritualidade (EE), o Brief Symptom Inventory (BSI), o Short-Form Health Survey (MOS SF-36v2) e um Questionário sociodemográfico complementar. Os resultados evidenciam que estamos perante uma amostra de sujeitos com elevada espiritualidade, sem problemas psicopatológicos e saúde física satisfatória, sendo as dimensões menos elevadas o desenvolvimento emocional e funcionamento físico. Para além disso, constatamos uma associação estatisticamente significativa negativa entre as dimensões da espiritualidade e as do BSI e uma associação significativa positiva entre as dimensões da espiritualidade e o funcionamento social do SF-36 v2. Por fim, relativamente à variação dos resultados da espiritualidade em função das variáveis sociodemográficas, verificamos que as mulheres apresentam resultados significativamente superiores nas duas dimensões, as pessoas mais velhas apresentam resultados significativamente superiores na dimensão crenças e os idosos que não foram à escola mas sabem ler/escrever também apresentam resultados superiores nessa dimensão. Não se verificaram diferenças em função do estado civil. Uma vez que esta é uma temática pouco estudada em Portugal, esperamos ter contribuído para o aprofundamento do conhecimento nesta área e que novos estudos continuem a incidir sobre a mesma, de forma a conseguirmos desenvolver programas de intervenção cada vez mais eficazes e direcionados para a promoção do bem-estar nos idosos e de um processo de envelhecimento saudável.

Plutonium and (90)Sr are considered to be among the most radiotoxic nuclides produced by the nuclear fission process. In spite of numerous studies on mammals and humans there is still no general agreement on the retention half time of both radionuclides in the skeleton in the general population. Here we determined plutonium and (90)Sr in human vertebrae in individuals deceased between 1960 and 2004 in Switzerland. Plutonium was measured by sensitive SF-ICP-MS techniques and (90)Sr by radiometric methods. We compared our results to the ones obtained for other environmental compartments to reveal the retention half time of NBT fallout (239)Pu and (90)Sr in trabecular bones of the Swiss population. Results show that plutonium has a retention half time of 40+/-14 years. In contrast (90)Sr has a shorter retention half time of 13.5+/-1.0 years. Moreover (90)Sr retention half time in vertebrae is shown to be linked to the retention half time in food and other environmental compartments. These findings demonstrate that the renewal of the vertebrae through calcium homeostatic control is faster for (90)Sr excretion than for plutonium excretion. The precise determination of the retention half time of plutonium in the skeleton will improve the biokinetic model of plutonium metabolism in humans.

During free walking, gait is automatically adjusted to provide optimal mechanical output and minimal energy expenditure; gait parameters, such as cadence, fluctuate from one stride to the next around average values. It was described that this fluctuation exhibited long-range correlations and fractal-like patterns. In addition, it was suggested that these long-range correlations disappeared if the participant followed the beep of metronome to regulate his or her pace. Until now, these fractal fluctuations were only observed for stride interval, because no technique existed to adequately analyze an extended time of free walking. The aim of the present study was to measure walking speed (WS), step frequency (SF) and step length (SL) with high accuracy (<1 cm) satellite positioning method (global positioning system or GPS) in order to detect long-range correlations in the stride-to-stride fluctuations. Eight participants walked 30 min under free and constrained (metronome) conditions. Under free walking conditions, DFA (detrended fluctuation analysis) and surrogate data tests showed that the fluctuation of WS, SL and SF exhibited a fractal pattern (i.e., scaling exponent alpha: 0.5 < alpha < 1) in a large majority of participants (7/8). Under constrained conditions (metronome), SF fluctuations became significantly anti-correlated (alpha < 0.5) in all participants. However, the scaling exponent of SL and WS was not modified. We conclude that, when the walking pace is controlled by an auditory signal, the feedback loop between the planned movement (at supraspinal level) and the sensory inputs induces a continual shifting of SF around the mean (persistent anti-correlation), but with no effect on the fluctuation dynamics of the other parameters (SL, WS).

El objetivo fue determinar la variación batimétrica y temporal de la composición funcional por gremios tróficos del grupo Poliquetos. Se colectaron muestras de macrobentos e información oceanográfica en tres estaciones ubicadas en la bahía (BH, 35 m), plataforma interna (PI, 65 m) y plataforma externa (PE, 117 m), de febrero 2003 a octubre 2008. Los resultados indicaron que la variabilidad estacional está sujeta a cambios de mayor escala, como el incremento de la producción primaria en primavera y verano, posibilitando un aumento de los depositívoros superficiales (SDF) en la BH durante primavera, por efecto de la acumulación de materia orgánica fitoplanctónica que llega al fondo. Asimismo, la disminución de los depositívoros superficiales (SDF) y el aumento de los suspensívoros (SF) en verano, en la BH y PI, se pueden explicar debido al incremento de la descarga de partículas terrígenas de origen vegetal, que afectan físicamente al sustrato. En tanto, los consumidores de interfaz (IF) que dominaron en verano/otoño en la PE, donde la influencia continental es menor, pueden haber sido favorecidos por la mayor calidad de flujo acumulado de fitodetritus en los sedimentos y a un menor contenido de OD, lo cual disminuyó quizás la presión por predación y competencia.

PURPOSE: Health-related quality of life (HRQoL) is considered a representative outcome in the evaluation of chronic disease management initiatives emphasizing patient-centered care. We evaluated the association between receipt of processes-of-care (PoC) for diabetes and HRQoL. METHODS: This cross-sectional study used self-reported data from non-institutionalized adults with diabetes in a Swiss canton. Outcomes were the physical/mental composites of the short form health survey 12 (SF-12) physical composite score, mental composite score (PCS, MCS) and the Audit of Diabetes-Dependent Quality of Life (ADDQoL). Main exposure variables were receipt of six PoC for diabetes in the past 12 months, and the Patient Assessment of Chronic Illness Care (PACIC) score. We performed linear regressions to examine the association between PoC, PACIC and the three composites of HRQoL. RESULTS: Mean age of the 519 patients was 64.5 years (SD 11.3); 60% were male, 87% reported type 2 or undetermined diabetes and 48% had diabetes for over 10 years. Mean HRQoL scores were SF-12 PCS: 43.4 (SD 10.5), SF-12 MCS: 47.0 (SD 11.2) and ADDQoL: -1.6 (SD 1.6). In adjusted models including all six PoC simultaneously, receipt of influenza vaccine was associated with lower ADDQoL (β=-0.4, p≤0.01) and foot examination was negatively associated with SF-12 PCS (β=-1.8, p≤0.05). There was no association or trend towards a negative association when these PoC were reported as combined measures. PACIC score was associated only with the SF-12 MCS (β=1.6, p≤0.05). CONCLUSIONS: PoC for diabetes did not show a consistent association with HRQoL in a cross-sectional analysis. This may represent an effect lag time between time of process received and health-related quality of life. Further research is needed to study this complex phenomenon.

OBJECTIVE: We investigated whether the INTERMED, a generic instrument for assessing biopsychosocial case complexity and direct care, identifies organ transplant patients at risk of unfavourable post-transplant development by comparing it to the Transplant Evaluation Rating Scale (TERS), the established measure for pretransplant psychosocial evaluation. METHOD: One hundred nineteen kidney, liver, and heart transplant candidates were evaluated using the INTERMED, TERS, SF-36, EuroQol, Montgomery-Åsberg Depression Rating Scale (MADRS), and Hospital Anxiety & Depression Scale (HADS). RESULTS: We found significant relationships between the INTERMED and the TERS scores. The INTERMED highly correlated with the HADS,MADRS, and mental and physical health scores of the SF-36 Health Survey. CONCLUSIONS: The results demonstrate the validity and usefulness of the INTERMED instrument for pretransplant evaluation. Furthermore, our findings demonstrate the different qualities of INTERMED and TERS in clinical practice. The advantages of the psychiatric focus of the TERS and the biopsychosocial perspective of the INTERMED are discussed in the context of current literature on integrated care.

INTRODUCTION: The influence of specific health problems on health-related quality of life (HRQoL) in childhood cancer survivors is unknown. We compared HRQoL between survivors of childhood cancer and their siblings, determined factors associated with HRQoL, and investigated the influence of chronic health problems on HRQoL. METHODS: Within the Swiss Childhood Cancer Survivor Study, we sent a questionnaire to all survivors (≥16 years) registered in the Swiss Childhood Cancer Registry, who survived >5 years and were diagnosed 1976-2005 aged <16 years. Siblings received similar questionnaires. We assessed HRQoL using Short Form-36 (SF-36). Health problems from a standard questionnaire were classified into overweight, vision impairment, hearing, memory, digestive, musculoskeletal or neurological, and thyroid problems. RESULTS: The sample included 1,593 survivors and 695 siblings. Survivors scored significantly lower than siblings in physical function, role limitation, general health, and the Physical Component Summary (PCS). Lower score in PCS was associated with a diagnosis of central nervous system tumor, retinoblastoma or bone tumor, having had surgery, cranio-spinal irradiation, or bone marrow transplantation. Lower score in Mental Component Summary was associated with older age. All health problems decreased HRQoL in all scales. Most affected were survivors reporting memory problems and musculoskeletal or neurological problems. Health problems had the biggest impact on physical functioning, general health, and energy and vitality. CONCLUSIONS: In this study, we showed the negative impact of specific chronic health problems on survivors' HRQoL. IMPLICATIONS FOR CANCER SURVIVORS: Therapeutic preventive measures, risk-targeted follow-up, and interventions might help decrease health problems and, consequently, improve survivors' quality of life.
