A 36 year old male was admitted in December 1997 to hospital with afternoon fever, malaise and hepatosplenomegaly. He also had a dry cough, dyspnoea and anaemia. Pneumonia caused by Pneumocystis carinii and human immunodeficiency virus (HIV) infection were documented. The HIV infection was confirmed in 1997 with 290,000 virus copies. The patient had been in the Mexican State of Chiapas which is known to be endemic for visceral leishmaniosis (VL) and localized cutaneous leishmaniosis (LCL). The visceral symptoms were diagnosed as VL and the causal agent was identified as Leishmania (L.) mexicana. Identification of Leishmania was carried out by the analysis of amplified DNA with specific primers belonging to the Leishmania subgenus and by dot blot positive hybridisation of these polymerase chain reaction derived products with kDNA from the L. (L.) mexicana MC strain used as probe. This is the first case in Mexico of VL caused by a species of Leishmania that typically produces a cutaneous disease form.

The aim of this paper is to measure the returns to human capital. We use a unique data set consisting of matched employer-employee information. Data on individuals' human capital include a set of 26 competences that capture the utilization of workers' skills in a very detailed way. Thus, we can expand the concept of human capital and discuss the type of skills that are more productive in the workplace and, hence, generate a higher payoff for the workers. The rich information on firm's and workplace characteristics allows us to introduce a broad range of controls and to improve previous research in this field. This paper gives evidence that the returns to generic competences differ depending on the position of the worker in the firm. Only numeracy skills are reward independent of the occupational status of the worker. The level of technology used by the firm in the production process does not directly increase workers’ pay, but it influences the pay-off to some of the competences. JEL Classification: J24, J31

Syrian dry areas have been for several millennia a place of interaction between human populations and the environment. If environmental constraints and heterogeneity condition the human occupation and exploitation of resources, socio-political, economic and historical elements play a fundamental role. Since the late 1980s, Syrian dry areas are viewed as suffering a serious water crisis, due to groundwater overdraft. The Syrian administration and international development agencies believe that groundwater overexploitation is also leading to a decline of agricultural activities and to poverty increase. Action is thus required to address these problems.However, the overexploitation diagnosis needs to be reviewed. The overexploitation discourse appears in the context of Syria's opening to international organizations and to the market economy. It echoes the international discourse of "global water crisis". The diagnosis is based on national indicators recycling old Soviet data that has not been updated. In the post-Soviet era, the Syrian national water policy seems to abandon large surface water irrigation projects in favor of a strategy of water use rationalization and groundwater conservation in crisis regions, especially in the district of Salamieh.This groundwater conservation policy has a number of inconsistencies. It is justified for the administration and also probably for international donors, since it responds to an indisputable environmental emergency. However, efforts to conserve water are anecdotal or even counterproductive. The water conservation policy appears a posteriori as an extension of the national policy of food self-sufficiency. The dominant interpretation of overexploitation, and more generally of the water crisis, prevents any controversary approach of the status of resources and of the agricultural system in general and thus destroys any attempt to discuss alternatives with respect to groundwater management, allocation, and their inclusion in development programs.A revisited diagnosis of the situation needs to take into account spatial and temporal dimensions of the groundwater exploitation and to analyze the co-evolution of hydrogeological and agricultural systems. It should highlight the adjustments adopted to cope with environmental and economic variability, changes of water availability and regulatory measures enforcements. These elements play an important role for water availability and for the spatial, temporal, sectoral allocation of water resource. The groundwater exploitation in the last century has obviously had an impact on the environment, but the changes are not necessarily catastrophic.The current groundwater use in central Syria increases the uncertainty by reducing the ability of aquifers to buffer climatic changes. However, the climatic factor is not the only source of uncertainty. The high volatility of commodity prices, fuel, land and water, depending on the market but also on the will (and capacity) of the Syrian State to preserve social peace is a strong source of uncertainty. The research should consider the whole range of possibilities and propose alternatives that take into consideration the risks they imply for the water users, the political will to support or not the local access to water - thus involving a redefinition of the economic and social objectives - and finally the ability of international organizations to reconsider pre-established diagnoses.

This TIP, Substance Abuse Treatment for Persons With Co-Occurring Disorders, revises TIP 9, Assessment and Treatment of Patients With Coexisting Mental Illness and Alcohol and Other Drug Abuse. The revised TIP provides information about new developments in the rapidly growing field of co-occurring substance use and mental disorders and captures the state-of-the-art in the treatment of people with co-occurring disorders. The TIP focuses on what the substance abuse treatment clinician needs to know and provides that information in an accessible manner. Résumé : Cette étude a concerné un groupe de patients séjournant pour la première fois en milieu psychiatrique à l'âge avancé. La moitié étaient âgés de quatre-vingt ans ou plus et deux tiers étaient des femmes. Deux tiers provenaient de leur domicile et un quart étaient sous protection légale. L'admission a été faite pour une dépression dans la moitié des cas, pour une démence dans un tiers des cas, pour une symptomatologie anxieuse dans un quart des cas. Les diagnostics psychiatriques à la sortie de l'hôpital étaient différents de ceux annoncés à l'admission. Le syndrome démentiel concernait la moitié des patients et la maladie d'Alzheimer prédominait chez la femme, alors que les démences vasculaires et celles associées à d'autres maladies étaient deux fois plus fréquentes chez l'homme. Les troubles de l'humeur ne concernaient qu'un cinquième des cas et les troubles de l'adaptation un sixième des cas. Presque une moitié des patients présentaient deux ou trois diagnostics psychiatriques concomitants. La co-morbidité somatique était dominée par les maladies cardio-vasculaires, notamment chez l'homme. Un tiers des femmes et autant d'hommes présentaient au moins quatre diagnostics somatiques concomitants. La durée moyenne d'hospitalisation de quatre-vingt-cinq jours a été prolongée ou raccourcie par certains facteurs: le placement en Etablissement Médico-Social prolonge de plus de deux mois l'hospitalisation. Les personnes mariées restent presque deux mois de moins à l'hopital. Le trouble de la personnalité et du comportement et le trouble dépressif récurrent prolongent de presque six mois, respectivement trois mois, la durée de l'hospitalisation. Plus de la moitié des- patients sont placés en Etablissement Médico-Social. Certains facteurs augmentent le risque de placement: la démence d'Alzheimer accroît le risque de quatre fois ; avec chaque année en plus, le risque d'être placé en institution augmente de 20% ; il en va de même pour les hospitalisations prolongées. Les patients qui rentrent à domicile (moins de la moitié, surtout des femmes) ont besoin d'aide : suivi médico-infirmier, aide .aux tâches ménagères. Le suivi ambulatoire a été assuré par l'hôpital de jour avant tout, la Consultation ambulatoire en collaboration avec les centres médico-sociaux jouant un rôle important. Les ré-hospitalisations à l'Hôpital de Psychiatrie de l'Age Avancé dans l'année suivant la sortie ont concerné un quart de femmes et un septième d'hommes, avec une durée de séjour plus courte que celle lors de la première hospitalisation.

A natural case of co-infection by Leishmania and Trypanosoma is reported in a dog (Canis familiaris) in south- western state of Mato Grosso do Sul, Brazil. Both amastigote and trypomastigote forms were observed after Giemsa staining of cytological preparations of the dog's bone marrow aspirate. No parasite was detected using medium culture inoculation of the sample. DNA obtained from the bone marrow aspirate sample and from the blood buffy coat was submitted to polymerase chain reaction (PCR) with a set of rDNA-based primers S4/S12. The nucleotide sequence of the PCR product was identical to that of Trypanosoma (Trypanozoon) evansi. The S4/S12 PCR was then used as template in a nested-PCR using a specific Leishmania set S17/S18 as primers, to explain the amastigote forms. The nucleotide sequence of the new PCR product was identical to that of Leishmania (Leishmania) chagasi. This case, as far as we know, is the first report of a dog co-infected with these parasites, suggesting that besides L. (L.) chagasi, the natural transmission of T. (T.) evansi occurs in the area under study.

Herein we have focused attention on major phenotypic features of peripheral blood eosinophils from chronic Schistosoma mansoni-infected patients. For this purpose, detailed immunophenotypic profiles of a range of cell surface markers were performed, including activation markers (CD23/CD69/CD25/HLA-DR), co-stimulatory molecules (CD28/CD80/CD86), chemokine receptors (CXCR1/CXCR2/CCR3/CCR5) besides L-selectin-CD62L and adhesion molecules (CD18/CD54). Our major findings pointed out increased frequency of CD23+-cells, besides decreased percentages of CD69+-eosinophils, suggesting a chronic activation status with low frequency of early activated eosinophils in chronic S. mansoni-infected patients (INT) in comparison to non-infected individuals (NI). Moreover, a dichotomic expression of beta-chemokine receptors was observed during human schistosomiasis mansoni with higher CCR5 and lower levels of CCR3 observed between groups. Enhanced expression of co-stimulatory receptors (CD28/CD86) and adhesion molecules (CD54/CD18), besides striking lower frequency of L-selectin+ were reported for eosinophils from INT group as compared to NI. Interestingly, the frequency of CD62L+-eosinophils and a range of cell activation related molecules pointed out an opposite pattern of association in NI and INT, where only INT patients that display lower frequency of CD62L+-eosinophils (first CD62L tertile) kept the unusual relationship between the expression of L-selectin and the CD23 activation marker. These findings suggest that distinct dynamic of activation markers expressed by eosinophils may occur during chronic S. mansoni infection.

A partir de la creació i aplicació didàctica d'una WebQuest en un curs de 4t de Primària s'analitza com i de uina manera pot influir positivament en el rendiment personal i acadèmic del grup d'alumnes, grup que presenta unes característiques acusades de dificultats d'atenció generalitzada, forts desnivells de coneixements, i en el qual es troben alumnes amb necessitats educatives especials (n.e.e.). Les webquest poden ser un bon recurs didàctic per a motivar el treball cooperatiu i la interacció entre alumnes, professors i les tecnologies de la informació i la comunicació.

Toxoplasmosis and ascaridiasis evoke polar Th-1 and Th-2 host immune responses, respectively. A study to investigate the specific cytokine profile production by in vitro cultures of peripheral blood mononuclear cells from individuals living under precarious sanitary conditions in a highly endemic area for the parasites Toxoplasma gondii and Ascaris lumbricoides was conducted. High levels of both IFN-³ (Th-1) and IL-13 (Th-2) were observed in groups of co-infected individuals presenting toxoplasmic ocular lesions. Significantly lower IL-10 and TGF-² levels were produced by co-infected individuals in comparison with groups of individuals not infected with A. lumbricoides and either positive or negative for T. gondii living under good sanitary conditions (control groups). The possible influence of co-parasitism on the clinical presentation of ocular toxoplasmosis is discussed.

The study was undertaken in eight endemic districts of Orissa, India, to find the members of the species complexes of Anopheles culicifacies and Anopheles fluviatilis and their distribution patterns. The study area included six forested districts (Keonjhar, Angul, Dhenkanal, Ganjam, Nayagarh and Khurda) and two non-forested coastal districts (Puri and Jagatsingpur) studied over a period of two years (June 2007-May 2009). An. culicifacies A, B, C and D and An. fluviatilis S and T sibling species were reported. The prevalence of An. culicifacies A ranged from 4.2-8.41%, B from 54.96-76.92%, C from 23.08-33.62% and D from 1.85-5.94% (D was reported for the first time in Orissa, except for occurrences in the Khurda and Nayagarh districts). The anthropophilic indices (AI) were 3.2-4.8%, 0.5-1.7%, 0.7-1.37% and 0.91-1.35% for A, B, C and D, respectively, whereas the sporozoite rates (SR) were 0.49-0.54%, 0%, 0.28-0.37% and 0.41-0.46% for A, B, C and D, respectively. An. fluviatilis showed a similarly varied distribution pattern in which S was predominant (84.3% overall); its AI and SR values ranged from 60.7-90.4% and 1.2-2.32%, respectively. The study observed that the co-existence of potential vector sibling species of An. culicifacies (A, C and D) and An. fluviatilis S (> 50%) was responsible for the high endemicity of malaria in forested districts such as Dhenkanal, Keonjhar, Angul, Ganjam, Nayagarh and Khurda (> 5% slide positivity rate). Thus, the epidemiological scenario for malaria is dependent on the distribution of the vector sibling species and their vectorial capacity.

Inhibitory receptors mediate CD8 T-cell hyporesponsiveness against cancer and infectious diseases. PD-1 and CTLA-4 have been extensively studied, and blocking antibodies have already shown clinical benefit for cancer patients. Only little is known on extended co-expression of inhibitory receptors and their ligands. Here we analyzed the expression of eight inhibitory receptors by tumor-antigen specific CD8 T-cells. We found that the majority of effector T-cells simultaneously expressed four or more of the inhibitory receptors BTLA, TIM-3, LAG-3, KRLG-1, 2B4, CD160, PD-1 and CTLA-4. There were major differences depending on antigen-specificity, differentiation and anatomical localization of T-cells. On the other hand, naive T-cells were only single or double positive for BTLA and TIM-3. Extended co-expression is likely relevant for effector T-cells, as we found expression of multiple ligands in metastatic lesions of melanoma patients. Together, our data suggest that naive T-cells are primarily regulated by BTLA and TIM-3, whereas effector cells interact via larger numbers of inhibitory receptors. Blocking multiple inhibitory receptors simultaneously or sequentially may improve T-cell based therapies, but further studies are necessary to clarify the role of each receptor-ligand pair.

La tecnología actual ha permitido que en los últimos años la nanociencia y la nanotecnología sean puntos críticos en el desarrollo del conocimiento. En estos momentos se desarrollan sistemas de dimensiones nanométricas que son interesantes debido a sus potenciales aplicaciones en diferentes ámbitos como en química, física, biología, materiales, medicina, cosmética... Dentro de estos sistemas nanoscópicos se encuentran las nanopartículas, estructuras con un tamaño inferior a los 100nm de longitud. En esta clasificación existen a su vez diferentes categorías, como las nanopartículas metálicas, semiconductoras, magnéticas, etc. y es exactamente en esta última tipoogía donde se centra este estudio. Este proyecto de investigación desarrolla la síntesis de magnetita (Fe3O4), ferrita de cobalto (CoFe2O4) y ferrita de cobre (CuFe2O4) con la finalidad de utilizarlas como dopante en superconductores. El método sintético utilizado es del tipo solvotérmico y se lleva a cabo en trietilenglicol, el cual actúa a la vez como disolvente y como estabilizante de las nanopartículas. Las partículas así obtenidas son dispersables en medios polares como el etanol absoluto. Los precursores de este método sintético son los respectivos acetilacetonatos metálicos debido a que el ligando orgánico descompone en productos volátiles. Existen diferentes factores que afectan a la síntesis, tales como la velocidad de ascenso de la temperatura, la agitación, la presencia de agua, la temperatura de descomposición de los precursores, etc. Algunos de estos factores han sido estudiados con detalle y aplicados con tal de optimizar el método experimental. Las nanopartículas sintetizadas han sido analizadas mediante diversas técnicas físicas con tal de establecer diferentes parámetros, tales como su composición fnal, su pureza, su estructura, sus propiedades magnéticas, etc. Estas técnicas son diversas: desde la espectroscopia infrarroja hasta medidas mediante SQUID, pasando por rayos X, microscopía electrónica y termogravimetría. Los resultados han sido favorables en la síntesis de la magnetita y también en la ferrita de cobalto, ya que las nanopartículas obtenidas son homogéneas, fácilmente dispersables en algoholes, estables por largos períodos de teimpo, rápidas de sintetizar, etc. El único problema observado ha sido la síntesis de ferrita de cobre la cual se ha de optimizar, ya que el producto final ha resultado ser una mezcla de tres compuesto diferentes.

Esta memoria de investigación es producto del trabajo experimental que se realizó con resinas de intercambio iónico para la síntesis intermatricial de nanopartículas metálicas de Co y Pd. Los estudios que se realizaron correspondieron a la iniciación en esta modalidad de síntesis de nanopartículas. En ellos se contemplaron las propiedades intrínsecas de la resina como: la capacidad de intercambio iónico, y funcionalidad química, tanto antes como después de la síntesis. Uno de los objetivos es que la resina no pierda ninguna de sus propiedades originales, sino que gane más versatilidad. La importancia que tienen en la actualidad los materiales nanométricos inspira a su modificación y nuevas formas de producción. Con esta investigación se inicia el trabajo de cara a los estudios doctorales. Aquí se estudia la aplicación de las nanopartículas de paladio y cobalto soportadas en resinas de intercambio iónico. Las nanopartículas de paladio tienen aplicación en el área de catálisis, y en su síntesis intermatricial, se consigue un catalizador heterogéneo, que brinda gran facilidad de recuperarlo y proceder con varios ciclos catalíticos. Así mismo, con el cobalto se pretendió otorgar propiedades ferromagnéticas, de manera que el proceso de separación de la resina de los medios de reacción, sea tan fácil como la aplicación de un campo magnético o pasar un imán sobre el lugar en cuestión.
