Background: Literature on scoliosis screening is vast, however because of the observational nature of available data and methodological flaws, data interpretation is often complex, leading to incomplete and sometimes, somewhat misleading conclusions. The need to propose a set of methods for critical appraisal of the literature about scoliosis screening, a comprehensive summary and rating of the available evidence appeared essential. METHODS: To address these gaps, the study aims were: i) To propose a framework for the assessment of published studies on scoliosis screening effectiveness; ii) To suggest specific questions to be answered on screening effectiveness instead of trying to reach a global position for or against the programs; iii) To contextualize the knowledge through expert panel consultation and meaningful recommendations. The general methodological approach proceeds through the following steps: Elaboration of the conceptual framework; Formulation of the review questions; Identification of the criteria for the review; Selection of the studies; Critical assessment of the studies; Results synthesis; Formulation and grading of recommendations in response to the questions. This plan follows at best GRADE Group (Grades of Recommendation, Assessment, Development and Evaluation) requirements for systematic reviews, assessing quality of evidence and grading the strength of recommendations. CONCLUSIONS: In this article, the methods developed in support of this work are presented since they may be of some interest for similar reviews in scoliosis and orthopaedic fields.

L’immigration maronite à Montréal date de la fin du XIXe siècle, mais c’est dans les années 1970 et 1990, à la suite des guerres au Liban, qu’il y a eu les plus grandes vagues. De rite syriaque antiochien, mais d’obédience catholique romaine, les immigrants maronites n’avaient pas d’église paroissiale propre. Les autorités diocésaines montréalaises avaient rassemblé les catholiques orientaux dans une même église dont le service était assuré par l’ordre melkite du Saint-Sauveur. En 1969, le père de l’ordre maronite mariamite Élias Najjar fonde avec des Égyptiens maronites la première paroisse. Cette étude se penche sur le processus qui a mené à la naissance de cette paroisse, son évolution à travers le temps et la vie de ses paroissiens. L’approche est narrative et descriptive, elle s’inscrit dans le cadre de l’histoire sociale et religieuse. Les résultats de l’analyse mettent en lumière une église nationale et un nouveau modèle de paroisse diasporique, différent du modèle classique, recelant des diversités infranationale, internationale et interconfessionnelle.

Présentation: Cet article a été publié dans le journal : Computerised medical imaging and graphics (CMIG). Le but de cet article est de recaler les vertèbres extraites à partir d’images RM avec des vertèbres extraites à partir d’images RX pour des patients scoliotiques, en tenant compte des déformations non-rigides due au changement de posture entre ces deux modalités. À ces fins, une méthode de recalage à l’aide d’un modèle articulé est proposée. Cette méthode a été comparée avec un recalage rigide en calculant l’erreur sur des points de repère, ainsi qu’en calculant la différence entre l’angle de Cobb avant et après recalage. Une validation additionelle de la méthode de recalage présentée ici se trouve dans l’annexe A. Ce travail servira de première étape dans la fusion des images RM, RX et TP du tronc complet. Donc, cet article vérifie l’hypothèse 1 décrite dans la section 3.2.1.

A method for the construction of a patient-specific model of a scoliotic torso for surgical planning via inter- patient registration is presented. Magnetic Resonance Images (MRI) of a generic model are registered to surface topography (TP) and X-ray data of a test patient. A partial model is first obtained via thin-plate spline registration between TP and X-ray data of the test patient. The MRIs from the generic model are then fit into the test patient using articulated model registration between the vertebrae of the generic model’s MRIs in prone position and the test patient’s X-rays in standing position. A non-rigid deformation of the soft tissues is performed using a modified thin-plate spline constrained to maintain bone rigidity and to fit in the space between the vertebrae and the surface of the torso. Results show average Dice values of 0.975 ± 0.012 between the MRIs following inter-patient registration and the surface topography of the test patient, which is comparable to the average value of 0.976 ± 0.009 previously obtained following intra-patient registration. The results also show a significant improvement compared to rigid inter-patient registration. Future work includes validating the method on a larger cohort of patients and incorporating soft tissue stiffness constraints. The method developed can be used to obtain a geometric model of a patient including bone structures, soft tissues and the surface of the torso which can be incorporated in a surgical simulator in order to better predict the outcome of scoliosis surgery, even if MRI data cannot be acquired for the patient.

Background This paper presents a method that registers MRIs acquired in prone position, with surface topography (TP) and X-ray reconstructions acquired in standing position, in order to obtain a 3D representation of a human torso incorporating the external surface, bone structures, and soft tissues. Methods TP and X-ray data are registered using landmarks. Bone structures are used to register each MRI slice using an articulated model, and the soft tissue is confined to the volume delimited by the trunk and bone surfaces using a constrained thin-plate spline. Results The method is tested on 3 pre-surgical patients with scoliosis and shows a significant improvement, qualitatively and using the Dice similarity coefficient, in fitting the MRI into the standing patient model when compared to rigid and articulated model registration. The determinant of the Jacobian of the registration deformation shows higher variations in the deformation in areas closer to the surface of the torso. Conclusions The novel, resulting 3D full torso model can provide a more complete representation of patient geometry to be incorporated in surgical simulators under development that aim at predicting the effect of scoliosis surgery on the external appearance of the patient’s torso.

This paper presents a method based on articulated models for the registration of spine data extracted from multimodal medical images of patients with scoliosis. With the ultimate aim being the development of a complete geometrical model of the torso of a scoliotic patient, this work presents a method for the registration of vertebral column data using 3D magnetic resonance images (MRI) acquired in prone position and X-ray data acquired in standing position for five patients with scoliosis. The 3D shape of the vertebrae is estimated from both image modalities for each patient, and an articulated model is used in order to calculate intervertebral transformations required in order to align the vertebrae between both postures. Euclidean distances between anatomical landmarks are calculated in order to assess multimodal registration error. Results show a decrease in the Euclidean distance using the proposed method compared to rigid registration and more physically realistic vertebrae deformations compared to thin-plate-spline (TPS) registration thus improving alignment.

Hors champ (Offscreen) is a collection of short stories and poems that offers rewrites of striking works of contemporary Quebec literature from 1960 to the present. In this collection, the literary legacy is problematized by the bursting of the source of hypotexts. Thus, the rewriting is done by recovering certain formal characteristics of source texts, while putting forward new thematic content. The texts relate to many affiliations, offering not a static and overwhelming posterity, but rather an active heritage, fragmented and drawing from several sources — as Quebec literary heritage does. The stories in the collection do not clearly announce their hypotext but rather begin with an epigraph, as a way to play with the readers’ expectations. The architecture of the collection does not respect the chronological order of hypotexts, and short stories from the same source are not grouped together. Aquin demeure (Aquin remains) compares two contemporary Quebec novels : Ça va aller by Catherine Mavrikakis and Pourquoi Bologne, by Alain Farah. Starting from the observation that the figure of Hubert Aquin exerts a spectral presence on Quebec literature and has bequeathed a problematic and paradoxical legacy to posterity, the project aims to see what readings of this aquinien heritage the works of Mavrikakis and Farah offer. The analysis of Ça va aller begins with a comparative study of the two authorial figures whom are being presented : Hubert Aquin and Robert Laflamme, an avatar of Réjean Ducharme. The study seeks to show how Aquin’s literary legacy is unattainable for the narrator while the analysis of Pourquoi Bologne focuses on the rewrite of Prochain épisode. With Genette’s notions of intertextuality and hypertextuality, the study attempts to determine whether it is possible to consider Pourquoi Bologne a text in the second degree.

Le présent article s’attache à deux des modes de lecture de l’oeuvre aquinienne qui traversent la critique et l’essai québécois contemporains. La lecture biographique, plus volontiers mythifiante, de certains essayistes et biographes aquiniens permettra, d’une part, de réfléchir à la fascination qu’exerce encore aujourd’hui le sacrifice d’Aquin sur ses lecteurs contemporains. D’autre part, il s’agira de mieux cerner l’inscription de l’oeuvre aquinienne dans les dossiers que la revue « Liberté » a fait paraître entre 2006 et 2012. En réactualisant la dimension politique de l’oeuvre d’Aquin, les auteurs de la revue réfléchissent à la fois sur la quasi-disparition de l’engagement politique dans la sphère culturelle et sur le décentrement de la parole littéraire à l’époque contemporaine. D’un point de vue comme de l’autre, Aquin et son oeuvre méritent d’être relus, d’être tirés de la bibliothèque ou du tombeau afin de répondre, ne serait-ce que par la négative, aux angoisses et aux obsessions des contemporains. Nous entendons en somme dépasser certaines idées reçues sur la construction des figures auctoriales, lesquelles viseraient selon certains auteurs à « simplifier [l’oeuvre] pour [la] rendre comestible ». Malgré ses limites évidentes, le mythe offre à l’oeuvre une possible survie dans le concert des médias contemporains et permet de saisir les enjeux d’une certaine représentation, historique sans doute, de la littérature québécoise.

Adolescent idiopathic scoliosis (AIS) is a deformity of the spine manifested by asymmetry and deformities of the external surface of the trunk. Classification of scoliosis deformities according to curve type is used to plan management of scoliosis patients. Currently, scoliosis curve type is determined based on X-ray exam. However, cumulative exposure to X-rays radiation significantly increases the risk for certain cancer. In this paper, we propose a robust system that can classify the scoliosis curve type from non invasive acquisition of 3D trunk surface of the patients. The 3D image of the trunk is divided into patches and local geometric descriptors characterizing the surface of the back are computed from each patch and forming the features. We perform the reduction of the dimensionality by using Principal Component Analysis and 53 components were retained. In this work a multi-class classifier is built with Least-squares support vector machine (LS-SVM) which is a kernel classifier. For this study, a new kernel was designed in order to achieve a robust classifier in comparison with polynomial and Gaussian kernel. The proposed system was validated using data of 103 patients with different scoliosis curve types diagnosed and classified by an orthopedic surgeon from the X-ray images. The average rate of successful classification was 93.3% with a better rate of prediction for the major thoracic and lumbar/thoracolumbar types.

Objective To determine scoliosis curve types using non invasive surface acquisition, without prior knowledge from X-ray data. Methods Classification of scoliosis deformities according to curve type is used in the clinical management of scoliotic patients. In this work, we propose a robust system that can determine the scoliosis curve type from non invasive acquisition of the 3D back surface of the patients. The 3D image of the surface of the trunk is divided into patches and local geometric descriptors characterizing the back surface are computed from each patch and constitute the features. We reduce the dimensionality by using principal component analysis and retain 53 components using an overlap criterion combined with the total variance in the observed variables. In this work, a multi-class classifier is built with least-squares support vector machines (LS-SVM). The original LS-SVM formulation was modified by weighting the positive and negative samples differently and a new kernel was designed in order to achieve a robust classifier. The proposed system is validated using data from 165 patients with different scoliosis curve types. The results of our non invasive classification were compared with those obtained by an expert using X-ray images. Results The average rate of successful classification was computed using a leave-one-out cross-validation procedure. The overall accuracy of the system was 95%. As for the correct classification rates per class, we obtained 96%, 84% and 97% for the thoracic, double major and lumbar/thoracolumbar curve types, respectively. Conclusion This study shows that it is possible to find a relationship between the internal deformity and the back surface deformity in scoliosis with machine learning methods. The proposed system uses non invasive surface acquisition, which is safe for the patient as it involves no radiation. Also, the design of a specific kernel improved classification performance.

Adolescent idiopathic scoliosis (AIS) is a musculoskeletal pathology. It is a complex spinal curvature in a 3-D space that also affects the appearance of the trunk. The clinical follow-up of AIS is decisive for its management. Currently, the Cobb angle, which is measured from full spine radiography, is the most common indicator of the scoliosis progression. However, cumulative exposure to X-rays radiation increases the risk for certain cancers. Thus, a noninvasive method for the identification of the scoliosis progression from trunk shape analysis would be helpful. In this study, a statistical model is built from a set of healthy subjects using independent component analysis and genetic algorithm. Based on this model, a representation of each scoliotic trunk from a set of AIS patients is computed and the difference between two successive acquisitions is used to determine if the scoliosis has progressed or not. This study was conducted on 58 subjects comprising 28 healthy subjects and 30 AIS patients who had trunk surface acquisitions in upright standing posture. The model detects 93% of the progressive cases and 80% of the nonprogressive cases. Thus, the rate of false negatives, representing the proportion of undetected progressions, is very low, only 7%. This study shows that it is possible to perform a scoliotic patient's follow-up using 3-D trunk image analysis, which is based on a noninvasive acquisition technique.

This paper describes a method for analyzing scoliosis trunk deformities using Independent Component Analysis (ICA). Our hypothesis is that ICA can capture the scoliosis deformities visible on the trunk. Unlike Principal Component Analysis (PCA), ICA gives local shape variation and assumes that the data distribution is not normal. 3D torso images of 56 subjects including 28 patients with adolescent idiopathic scoliosis and 28 healthy subjects are analyzed using ICA. First, we remark that the independent components capture the local scoliosis deformities as the shoulder variation, the scapula asymmetry and the waist deformation. Second, we note that the different scoliosis curve types are characterized by different combinations of specific independent components.

The main objective of this letter is to formulate a new approach of learning a Mahalanobis distance metric for nearest neighbor regression from a training sample set. We propose a modified version of the large margin nearest neighbor metric learning method to deal with regression problems. As an application, the prediction of post-operative trunk 3-D shapes in scoliosis surgery using nearest neighbor regression is described. Accuracy of the proposed method is quantitatively evaluated through experiments on real medical data.

One of the major concerns of scoliosis patients undergoing surgical treatment is the aesthetic aspect of the surgery outcome. It would be useful to predict the postoperative appearance of the patient trunk in the course of a surgery planning process in order to take into account the expectations of the patient. In this paper, we propose to use least squares support vector regression for the prediction of the postoperative trunk 3D shape after spine surgery for adolescent idiopathic scoliosis. Five dimensionality reduction techniques used in conjunction with the support vector machine are compared. The methods are evaluated in terms of their accuracy, based on the leave-one-out cross-validation performed on a database of 141 cases. The results indicate that the 3D shape predictions using a dimensionality reduction obtained by simultaneous decomposition of the predictors and response variables have the best accuracy.

One of the major concerns of scoliotic patients undergoing spinal correction surgery is the trunk's external appearance after the surgery. This paper presents a novel incremental approach for simulating postoperative trunk shape in scoliosis surgery. Preoperative and postoperative trunk shapes data were obtained using three-dimensional medical imaging techniques for seven patients with adolescent idiopathic scoliosis. Results of qualitative and quantitative evaluations, based on the comparison of the simulated and actual postoperative trunk surfaces, showed an adequate accuracy of the method. Our approach provides a candidate simulation tool to be used in a clinical environment for the surgery planning process.
