Non-steroidal anti-inflammatory drugs (NSAIDs) are the drugs most frequently involved in hypersensitivity drug reactions. Histamine is released in the allergic response to NSAIDs and is responsible for some of the clinical symptoms. The aim of this study is to analyze clinical association of functional polymorphisms in the genes coding for enzymes involved in histamine homeostasis with hypersensitivity response to NSAIDs. We studied a cohort of 442 unrelated Caucasian patients with hypersensitivity to NSAIDs. Patients who experienced three or more episodes with two or more different NSAIDs were included. If this requirement was not met diagnosis was established by challenge. A total of 414 healthy unrelated controls ethnically matched with patients and from the same geographic area were recruited. Analyses of the SNPs rs17740607, rs2073440, rs1801105, rs2052129, rs10156191, rs1049742 and rs1049793 in the HDC, HNMT and DAO genes were carried out by means of TaqMan assays. The detrimental DAO 16 Met allele (rs10156191), which causes decreased metabolic capacity, is overrepresented among patients with crossed-hypersensitivity to NSAIDs with an OR = 1.7 (95% CI = 1.3-2.1; Pc = 0.0003) with a gene-dose effect (P = 0.0001). The association was replicated in two populations from different geographic areas (Pc = 0.008 and Pc = 0.004, respectively). CONCLUSIONS AND IMPLICATIONS: The DAO polymorphism rs10156191 which causes impaired metabolism of circulating histamine is associated with the clinical response in crossed-hypersensitivity to NSAIDs and could be used as a biomarker of response.

BACKGROUND A prospective study was performed to compare the prevalence of morphometric vertebral fractures (MVF) between patients with inflammatory bowel disease (IBD) and healthy subjects and to identify predictive factors of fracture. METHODS A total of 107 patients with IBD (53 with Crohn's disease and 54 with ulcerative colitis) and 51 healthy subjects participated in the study. Information about anthropometric parameters, toxins, previous fractures, and parameters related to this disease were evaluated. The index of vertebral deformity, bone mass density (BMD), and biochemical parameters were calculated. RESULTS A total of 72 fractures were detected in 38.32% of patients with IBD, and 10 fractures were detected in 13.73% of healthy subjects; the risk of fracture in patients with IBD was higher than that in control subjects (OR, 4.03; 95% CI, 1.652-9.847; p < 0.002). We found no correlation between fracture and BMD in patients with IBD (lumbar spine, r = -0.103, p = 0.17 and femoral neck, r = -0.138, p = 0.07). Corticosteroid treatment was not associated with prevalent vertebral fractures nor with taking corticosteroids (r = 0.135, p = 0.14) or the duration for which they were taken (r = 0.08, p = 0.38), whereas this relationship was present in the controls (r = -0.365, p = 0.01). In the multivariate analysis, none of the measured parameters were significantly predictive of fracture, only to manifested IBD. Hypovitaminosis D was observed in 55.14% of patients with IBD. CONCLUSIONS The prevalence of morphometric vertebral fractures is higher in patients with IBD than in the healthy population, without association with BMD or corticoid treatment. Simply having IBD was proven to be a predictive factor of fracture. We observed a high incidence of hypovitaminosis D in patients with IBD.

BACKGROUND Observational studies implicate higher dietary energy density (DED) as a potential risk factor for weight gain and obesity. It has been hypothesized that DED may also be associated with risk of type 2 diabetes (T2D), but limited evidence exists. Therefore, we investigated the association between DED and risk of T2D in a large prospective study with heterogeneity of dietary intake. METHODOLOGY/PRINCIPAL FINDINGS A case-cohort study was nested within the European Prospective Investigation into Cancer (EPIC) study of 340,234 participants contributing 3.99 million person years of follow-up, identifying 12,403 incident diabetes cases and a random subcohort of 16,835 individuals from 8 European countries. DED was calculated as energy (kcal) from foods (except beverages) divided by the weight (gram) of foods estimated from dietary questionnaires. Prentice-weighted Cox proportional hazard regression models were fitted by country. Risk estimates were pooled by random effects meta-analysis and heterogeneity was evaluated. Estimated mean (sd) DED was 1.5 (0.3) kcal/g among cases and subcohort members, varying across countries (range 1.4-1.7 kcal/g). After adjustment for age, sex, smoking, physical activity, alcohol intake, energy intake from beverages and misreporting of dietary intake, no association was observed between DED and T2D (HR 1.02 (95% CI: 0.93-1.13), which was consistent across countries (I(2) = 2.9%). CONCLUSIONS/SIGNIFICANCE In this large European case-cohort study no association between DED of solid and semi-solid foods and risk of T2D was observed. However, despite the fact that there currently is no conclusive evidence for an association between DED and T2DM risk, choosing low energy dense foods should be promoted as they support current WHO recommendations to prevent chronic diseases.

Los problemas relacionados con el peso y la alimentación, incluyendo los trastornos de la conducta alimentaria (TCA) y la obesidad, se asocian con una amplia gama de trastornos físicos y mentales. La tasa de obesidad entre adultos y adolescentes se ha duplicado en los últimos 20 años, mientras que, paralelamente, también se ha registrado un aumento en la prevalencia de TCA. A pesar de que la obesidad y los TCA se tratan y estudian como patologías diferentes, diversos estudios han identificado factores de riesgo y protectores que son comunes entre ambas afectaciones. Sin embargo, pocos son los estudios que analizan conjuntamente factores neuropsicológicos, biológicos y ambientales, de manera a determinar las interacciones existentes entre estos aspectos.Objetivos:1) Explorar las diferencias clínicas (conducta alimentaria, actividad y sueño y personalidad), neuropsicológicas (funciones ejecutivas y de atención) y del sistema olfativo-sensorial entre los dos diferentes subtipos de TCA y la obesidad, al ser comparado entre los grupos; 2) Explorar la asociación entre funcionamiento neuropsicológico y características clínicas (conducta alimentaria y perfil de personalidad), IMC y sistema olfativo-sensorial entre los distintos grupos. Método: Estudio transversal de casos y controles. La muestra será dividida en 4 grupos: 2 de casos (30 bajo peso con TCA y 30 Obesas con TCA), formados por pacientes admitidos, tratados en la Unidad de Trastornos Alimentarios y diagnosticados según el DSM-IV; y 2 de controles (30 bajo peso sin TCA y 30 Obesas sin TCA). CRITERIOS DE INCLUSION: Ser mujer, tener entre 18-45 años, Cumplir criterios diagnósticos DSM-IV (en caso TCA). Instrumentos: 1)Neuropsicologicos: TCI-R, WCST, SCWT; 2)Antropométricos: TANITA MC-180 Multifrecuencia; 3)Olfato-sensoriales: SSOT, Taste Strips; 4)Actividad: ACTIWATCH 7; 5)Calidad del sueño: PSQI ,Escala EPWORTH.Variables socio-demográficas también serán recogidas durante la entrevista clínica. Todos los instrumentos serán aplicados en la misma unidad en una única entrevista exceptuando el ACTIWATCH que los sujetos deberán llevar durante 6 días.

OBJECTIVE Endogenous uveitis is a major cause of visual loss mediated by the immune system. The protein tyrosine phosphatase non-receptor type 22 (PTPN22) gene encodes a lymphoid-specific phosphatase that plays a key role in T-cell receptor (TCR) signaling. Two independent functional missense single nucleotide polymorphisms (SNPs) located within the PTPN22 gene (R263Q and R620W) have been associated with different autoimmune disorders. We aimed to analyze for the first time the influence of these PTPN22 genetic variants on endogenous non-anterior uveitis susceptibility. METHODS We performed a case-control study of 217 patients with endogenous non-anterior uveitis and 718 healthy controls from a Spanish population. The PTPN22 polymorphisms (rs33996649 and rs2476601) were genotyped using TaqMan allelic discrimination assays. The allele, genotype, carriers, and allelic combination frequencies were compared between cases and controls with χ(2) analysis or Fisher's exact test. RESULTS Our results showed no influence of the studied SNPs in the global susceptibility analysis (rs33996649: allelic P- value=0.92, odds ratio=0.97, 95% confidence interval=0.54-1.75; rs2476601: allelic P- value=0.86, odds ratio=1.04, 95% confidence interval=0.68-1.59). Similarly, the allelic combination analysis did not provide additional information. CONCLUSIONS Our results suggest that the studied polymorphisms of the PTPN22 gene do not play an important role in the pathophysiology of endogenous non-anterior uveitis.

OBJECTIVE Delusional disorder has been traditionally considered a psychotic syndrome that does not evolve to cognitive deterioration. However, to date, very little empirical research has been done to explore cognitive executive components and memory processes in Delusional Disorder patients. This study will investigate whether patients with delusional disorder are intact in both executive function components (such as flexibility, impulsivity and updating components) and memory processes (such as immediate, short term and long term recall, learning and recognition). METHODS A large sample of patients with delusional disorder (n = 86) and a group of healthy controls (n = 343) were compared with regard to their performance in a broad battery of neuropsychological tests including Trail Making Test, Wisconsin Card Sorting Test, Colour-Word Stroop Test, and Complutense Verbal Learning Test (TAVEC). RESULTS When compared to controls, cases of delusional disorder showed a significantly poorer performance in most cognitive tests. Thus, we demonstrate deficits in flexibility, impulsivity and updating components of executive functions as well as in memory processes. These findings held significant after taking into account sex, age, educational level and premorbid IQ. CONCLUSIONS Our results do not support the traditional notion of patients with delusional disorder being cognitively intact.

INTRODUCTION The relationship between circulating prolactin and invasive breast cancer has been investigated previously, but the association between prolactin levels and in situ breast cancer risk has received less attention. METHODS We analysed the relationship between pre-diagnostic prolactin levels and the risk of in situ breast cancer overall, and by menopausal status and use of postmenopausal hormone therapy (HT) at blood donation. Conditional logistic regression was used to assess this association in a case-control study nested within the European Prospective Investigation into Cancer and Nutrition (EPIC) cohort, including 307 in situ breast cancer cases and their matched control subjects. RESULTS We found a significant positive association between higher circulating prolactin levels and risk of in situ breast cancer among all women [pre-and postmenopausal combined, ORlog2 = 1.35 (95%CI 1.04-1.76), Ptrend = 0.03]. No statistically significant heterogeneity was found between prolactin levels and in situ cancer risk by menopausal status (Phet = 0.98) or baseline HT use (Phet = 0.20), although the observed association was more pronounced among postmenopausal women using HT compared to non-users (Ptrend = 0.06 vs Ptrend = 0.35). In subgroup analyses, the observed positive association was strongest in women diagnosed with in situ breast tumors <4 years compared to ≥4 years after blood donation (Ptrend = 0.01 vs Ptrend = 0.63; Phet = 0.04) and among nulliparous women compared to parous women (Ptrend = 0.03 vs Ptrend = 0.15; Phet = 0.07). CONCLUSIONS Our data extends prior research linking prolactin and invasive breast cancer to the outcome of in situ breast tumours and shows that higher circulating prolactin is associated with increased risk of in situ breast cancer.

Estudos relativos às atividades da Controladoria que se inter-relacionam com outras ciências, como Psicologia e Sociologia, favorecem conhecimentos mais holísticos sobre essa área organizacional. Assim, neste estudo objetiva-se verificar os impactos da implantação das normas internacionais de contabilidade na controladoria à luz da Teoria da Estruturação. O pressuposto é que o processo de implantação das normas internacionais vem contribuir para a produção e a reprodução da ordem social da área organizacional controladoria. A Teoria da Estruturação de Giddens (1979; 1996; 2003) sustenta esta pesquisa exploratória, com abordagem qualitativa, realizada por meio de um estudo de caso em uma indústria têxtil. Nela, analisaram-se impactos da implantação das normas internacionais de contabilidade na controladoria, no período de 2008 a 2010, que abrange a fase de implantação dos CPCs na empresa, emitidos pelo Comitê de Pronunciamentos Contábeis (CPC). A coleta de dados deu-se por meio de observação, análise documental, entrevistas individuais e grupo focal. A análise das entrevistas individuais e do grupo focal foi realizada com o auxílio do software Atlas.ti. Os resultados apontaram que o processo de implantação das normas internacionais de contabilidade impactou a estrutura da área organizacional controladoria, por meio de novas atividades, discussões e mudanças nos processos e controles, além da interação dos agentes, por meio de comunicação e capacidades, permitindo a produção e a reprodução dessa área. Conclui-se que a dualidade da estrutura preconizada por Giddens (1979; 1996; 2003) ocorreu na empresa no período analisado, pois a estrutura impactou a interação e vice-versa.

BACKGROUND Obesity is positively associated with colorectal cancer. Recently, body size subtypes categorised by the prevalence of hyperinsulinaemia have been defined, and metabolically healthy overweight/obese individuals (without hyperinsulinaemia) have been suggested to be at lower risk of cardiovascular disease than their metabolically unhealthy (hyperinsulinaemic) overweight/obese counterparts. Whether similarly variable relationships exist for metabolically defined body size phenotypes and colorectal cancer risk is unknown. METHODS AND FINDINGS The association of metabolically defined body size phenotypes with colorectal cancer was investigated in a case-control study nested within the European Prospective Investigation into Cancer and Nutrition (EPIC) study. Metabolic health/body size phenotypes were defined according to hyperinsulinaemia status using serum concentrations of C-peptide, a marker of insulin secretion. A total of 737 incident colorectal cancer cases and 737 matched controls were divided into tertiles based on the distribution of C-peptide concentration amongst the control population, and participants were classified as metabolically healthy if below the first tertile of C-peptide and metabolically unhealthy if above the first tertile. These metabolic health definitions were then combined with body mass index (BMI) measurements to create four metabolic health/body size phenotype categories: (1) metabolically healthy/normal weight (BMI < 25 kg/m2), (2) metabolically healthy/overweight (BMI ≥ 25 kg/m2), (3) metabolically unhealthy/normal weight (BMI < 25 kg/m2), and (4) metabolically unhealthy/overweight (BMI ≥ 25 kg/m2). Additionally, in separate models, waist circumference measurements (using the International Diabetes Federation cut-points [≥80 cm for women and ≥94 cm for men]) were used (instead of BMI) to create the four metabolic health/body size phenotype categories. Statistical tests used in the analysis were all two-sided, and a p-value of <0.05 was considered statistically significant. In multivariable-adjusted conditional logistic regression models with BMI used to define adiposity, compared with metabolically healthy/normal weight individuals, we observed a higher colorectal cancer risk among metabolically unhealthy/normal weight (odds ratio [OR] = 1.59, 95% CI 1.10-2.28) and metabolically unhealthy/overweight (OR = 1.40, 95% CI 1.01-1.94) participants, but not among metabolically healthy/overweight individuals (OR = 0.96, 95% CI 0.65-1.42). Among the overweight individuals, lower colorectal cancer risk was observed for metabolically healthy/overweight individuals compared with metabolically unhealthy/overweight individuals (OR = 0.69, 95% CI 0.49-0.96). These associations were generally consistent when waist circumference was used as the measure of adiposity. To our knowledge, there is no universally accepted clinical definition for using C-peptide level as an indication of hyperinsulinaemia. Therefore, a possible limitation of our analysis was that the classification of individuals as being hyperinsulinaemic-based on their C-peptide level-was arbitrary. However, when we used quartiles or the median of C-peptide, instead of tertiles, as the cut-point of hyperinsulinaemia, a similar pattern of associations was observed. CONCLUSIONS These results support the idea that individuals with the metabolically healthy/overweight phenotype (with normal insulin levels) are at lower colorectal cancer risk than those with hyperinsulinaemia. The combination of anthropometric measures with metabolic parameters, such as C-peptide, may be useful for defining strata of the population at greater risk of colorectal cancer.

O estudo elaborou critérios de avaliação dos componentes da paramentação cirúrgica de hospitais do município de São Paulo. A maior dificuldade nessa elaboração deveu-se à inexistência de normas e consenso sobre a porosidade dos tecidos como barreira microbiológica e de contato com sangue e outros fluidos. Contudo, foi possível reconhecer aspectos positivos e problemáticos. Observou-se que existe semelhanças dos componentes entre os hospitais, no que se refere a modelo e tipo de tecido. As luvas foram os componentes com maior índice de adequação e os propés, os de maior índice de inadequação. O aspecto mais problemático referiu-se aos controles de aquisição e de reprocessamento dos componentes re-utilizáveis.

Ocorrências iatrogênicas com medicação na UTI são eventos indesejáveis que exigem pronta intervenção do enfermeiro. Opresente estudo foi realizado com os seguintes objetivos:- verificar a conduta dos enfermeiros diante de uma ocorrência commedicação, identificar os sentimentos vividos nessas situações e caracterizar os fatores relacionados a esses eventos. Pormeio de um questionário respondido por 148 enfermeiros de UTI (76,7%) de 7 hospitais do Município de São Paulo, nosanos de 1997 e 1998, os dados foram obtidos e analisados segundo freqüência absoluta e percentual. Os resultados permitiramconcluir que as condutas mais citadas incluíram: comunicar o fato ao médico (31,7%), intensificar os controles (26,5%) ecomunicar a chefia de enfermagem (13,5%). Ansiedade, impotência e culpa foram sentimentos mais apontados com38,1%, 14,6% e 12,3%, respectivamente. O fator relacionado às ocorrências mais freqüente foi a displicência do funcionário(23,1%). Quanto à vivência desse tipo de ocorrência, a maior porcentagem (43,2%) referiu ter vivido situação semelhanteraras vezes.

A matrícula de alunos, nas Escolas Municipais de Educação Infantil de São Paulo, está vinculada à apresentação da caderneta de vacinação, para incentivar os pais a manterem atualizado o calendário de imunizações das crianças. Não há, porém, controles periódicos para verificar se as vacinações estão atualizadas. Para sanar tal deficiência, foi elaborado um projeto com objetivo de facilitar o controle da imunização infantil, por meio da criação de um programa de computador, para realizar esse controle, testado em uma Escola Municipal de Educação Infantil, durante três meses. Foram cadastrados nesse sistema 286 alunos, dos quais, 236 (82,5%) receberam notificações por estar com seu quadro de vacinação incompleto. Dentre os alunos que receberam as notificações, 21,2% atualizaram a vacinação, 2,5% devolveram suas cadernetas inalteradas e os demais ainda estavam providenciando a atualização. O programa foi capaz de identificar falhas, reduzindo as chances da propagação de doenças transmissíveis no ambiente escolar.

O estudo buscou identificar as variáveis de posicionamento e pega, durante a amamentação, relacionadas aos traumas mamilares. Estudo caso-controle que investigou o aparecimento do trauma mamilar entre mulheres internadas em um hospital Universitário de São Paulo, em 2004 e 2005. Os casos foram puérperas com diagnóstico de trauma mamilar uni ou bilateral. Para a análise dos dados, foram aplicados os testes qui-quadrado, t de Student, razão de chances (IC= 95%) e análise de correspondência. Foram estudadas 146 puérperas e seus recém-nascidos, sendo 73 casos e 73 controles. As variáveis de posicionamento e pega, estatisticamente significativas para a ocorrência da lesão, foram: criança com pescoço torcido, queixo longe da mama e lábio inferior virado para dentro. A prevenção do trauma, no início da amamentação, é decisiva para a continuidade desta prática. O acompanhamento do posicionamento adequado é determinante para o estabelecimento da amamentação efetiva e prolongada.

O objetivo deste estudo foi comparar a carga de trabalho de enfermagem requerida pelos pacientes adultos, idosos e muito idosos, incluindo nas análises as intervenções realizadas e a evolução da carga de trabalho entre admissão e alta. Estudo prospectivo, longitudinal, realizado em UTIs gerais de quatro hospitais do Município de São Paulo. Os resultados apontaram que, independente da idade, houve similaridade da carga de trabalho de enfermagem na admissão, bem como na evolução das demandas de cuidados dos pacientes. Diferenças entre os grupos foram observadas no NAS na alta da UTI e nos seguintes itens desse instrumento: Monitorização e controles e Procedimentos de higiene, na admissão, Suporte respiratório e Hiperalimentação intravenosa na alta e Mobilização e posicionamento tanto na alta como na admissão. Concluiu-se, portanto, que a idade interferiu somente em aspectos específicos da carga de trabalho de enfermagem requeridas por pacientes internados em UTIs.

Introducción y objetivos. Se ha señalado que, en la miocardiopatía hipertrófica (MCH), la desorganización de las fibras regionales da lugar a segmentos en los que la deformación es nula o está gravemente reducida, y que estos segmentos tienen una distribución no uniforme en el ventrículo izquierdo (VI). Esto contrasta con lo observado en otros tipos de hipertrofia como en el corazón de atleta o la hipertrofia ventricular izquierda hipertensiva (HVI-HT), en los que puede haber una deformación cardiaca anormal, pero nunca tan reducida como para que se observe ausencia de deformación. Así pues, proponemos el empleo de la distribución de los valores de strain para estudiar la deformación en la MCH. Métodos. Con el empleo de resonancia magnética marcada (tagged), reconstruimos la deformación sistólica del VI de 12 sujetos de control, 10 atletas, 12 pacientes con MCH y 10 pacientes con HVI-HT. La deformación se cuantificó con un algoritmo de registro no rígido y determinando los valores de strain sistólico máximo radial y circunferencial en 16 segmentos del VI. Resultados. Los pacientes con MCH presentaron unos valores medios de strain significativamente inferiores a los de los demás grupos. Sin embargo, aunque la deformación observada en los individuos sanos y en los pacientes con HVI-HT se concentraba alrededor del valor medio, en la MCH coexistían segmentos con contracción normal y segmentos con una deformación nula o significativamente reducida, con lo que se producía una mayor heterogeneidad de los valores de strain. Se observaron también algunos segmentos sin deformación incluso en ausencia de fibrosis o hipertrofia. Conclusiones. La distribución de strain caracteriza los patrones específicos de deformación miocárdica en pacientes con diferentes etiologías de la HVI. Los pacientes con MCH presentaron un valor medio de strain significativamente inferior, así como una mayor heterogeneidad de strain (en comparación con los controles, los atletas y los pacientes con HVI-HT), y tenían regiones sin deformación.
