Relationship between psychological factors and symptoms of TMD in university undergraduate students.

Temporomandibular disorders is a collective term used to describe a number of related disorders involving the temporomandibular joints, masticatory muscles and occlusion with common symptoms such as pain, restricted movement, muscle tenderness and intermittent joint sounds. The multifactorial TMD etiology is related to emotional tension, occlusal interferences, tooth loss, postural deviation, masticatory muscular dysfunction, internal and external changes in TMJ structure and the various associations of these factors. The aim of this study was to evaluate the prevalence of the relationship between signs of psychological distress and temporomandibular disorder in university students. A total 150 volunteers participated in this study. They attended different courses in the field of human science at one public university and four private universities. TMD was assessed by the Research Diagnostic Criteria (RDC) questionnaire. Anxiety was measured by means of a self-evaluative questionnaire, Spielberger's Trait-State anxiety inventory, to evaluate students'state and trait anxiety. The results of the two questionnaires were compared to determine the relationship between anxiety levels and severity degrees of chronic TMD pain by means of the chi-square test. The significance level was set at 5%. The statistical analysis showed that the TMD degree has a positive association with state-anxiety (p = 0.008; p < 0.05) and negative with trait-anxiety (p = 0.619; p < 0.05). Moreover a high TMD rate was observed among the students (40%). This study concluded that there is a positive association between TMD and anxiety.

Purpose: This study evaluated the relationship between anxiety levels and severity degrees of chronic orofacial pain of temporomandibular disorder in brazilian university students. Methods: 150 volunteers (117 men and 33 women), with age ranging from 17 to 30 years, were subjects to this study. Spielberger's trait-state anxiety inventory was used to evaluate trait and state anxiety of the students, while examination for chronic orofacial pain was performed in accordance with the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Association between anxiety levels and severity degrees of chronic orofacial pain was tested using the Chi-square test. The significance level was set at p<.05. Results: The results showed that 32.7% (n = 49) of the subjects exhibited TMD, and the intensity of chronic orofacial pain was classified as degree 1 in 85.7% (n = 42) and as degree 2 in 14.3% (n = 7) of them. Based on the results of the Spielberger's trait-state anxiety inventory, the majority of the students had moderate anxiety (48.6% and 48.1%, respectively). The correlation between trait-anxiety levels and chronic orofacial pain degrees was significant and positive (p<.05). However, no significant correlation was found to state-anxiety levels and chronic orofacial pain degrees (p>.05). Conclusion: It was concluded that chronic orofacial pain of TMD could be present in university students and anxiety may be related. © 2010 Japan Prosthodontic Society. Published by Elsevier Ireland. Neurocysticercosis (NCC) is an infection of the central nervous system (CNS) caused by the metacestode larval form of the parasite Taenia sp. Many factors can contribute to the endemic nature of cysticercosis. The inflammatory process that occurs in the tissue surrounding the parasite and/or distal from it can result from several associated mechanisms and may be disproportionate with the number of cysts. This discrepancy may lead to difficulty with the proper diagnosis in people from low endemic regions or regions that lack laboratory resources. In the CNS, the cysticerci have two basic forms, isolated cysts (Cysticercus cellulosae = CC) and racemose cysts (Cysticercus racemosus = CR), and may be meningeal, parenchymal, or ventricular or have a mixed location. The clinical manifestations are based on two fundamental syndromes that may occur in isolation or be associated: epilepsy and intracranial hypertension. They may be asymptomatic, symptomatic or fatal; have an acute, sub-acute or chronic picture; or may be in remission or exacerbated. The cerebrospinal fluid (CSF) may be normal, even in patients with viable cysticerci, until the patients begin to exhibit the classical syndrome of NCC in the CSF, or show changes in one or more routine analysed parameters. Computed tomography (CT) and magnetic resonance imaging (MRI) have allowed non-invasive diagnoses, but can lead to false negatives. Treatment is a highly controversial issue and is characterised by individualised therapy sessions. Two drugs are commonly used, praziquantel (PZQ) and albendazole (ABZ). The choice of anti-inflammatory drugs includes steroids and dextrochlorpheniramine (DCP). Hydrocephalus is a common secondary effect of NCC. Surgical cases of hydrocephalus must be submitted to ventricle-peritoneal shunt (VPS) immediately before cysticidal treatment, and surgical extirpation of the cyst may lead to an absence of the surrounding inflammatory process. The progression of NCC may be simple or complicated, have remission with or without treatment and may exhibit symptoms that can disappear for long periods of time or persist until death. Unknown, neglected and controversial aspects of NCC, such as the impaired fourth ventricle syndrome, the presence of chronic brain oedema and psychic complaints, in addition to the lack of detectable glucose in the CSF and re-infection are discussed. © 2011 Bentham Science Publishers.

Aim. The aim of this paper was to investigate the relationship between the accumulation of different anthropometric indicators and the prevalence of some chronic diseases in women over 40 years of age. Methods. The sample was comprised of 562 women between 40 and 95 years of age (64.5 ± 11.4) attended by the research projects were carried out in two cities in southeastern Brazil. Anthropometric measurements were taken: weight, height, waist circumference, hip circumference, and the values of BMI and WHR were later calculated. The referenced morbidity questionnaire was also applied, based on the Standard Health Questionnaire (SHQ), which analyzes the presence of degenerative chronic diseases in the adult population. For the statistical treatment, the chi-square and binary logistic regression tests were performed, with significance set at 5%. Results. The relationship between three changes in the anthropometric indicators and the greater incidence of diseases continued significant for hypertension (OR=3.77 [95% CI: 2.14-6.65], =P=0.001), and for endocrine and metabolic diseases (OR=2.59 [95%: 1:47 to 4:32], =P=0.001), regardless of the effects of age and physical activities. Conclusion. The simultaneity of body fat indicators is more strongly associated with the prevalence of some chronic diseases (hypertension, endocrine, and metabolic), relative to the individualized use of anthropometric indicators.

O diabetes mellitus é uma doença crônica de etiologia múltipla, cujo tratamento inclui mudanças no estilo de vida, onde a adoção de hábitos alimentares saudáveis é de suma importância para o controle da doença. No entanto, a adesão ao plano alimentar é um dos aspectos de maior desafio para o tratamento desta patologia. Esta pesquisa teve como objetivo comparar a eficácia do uso de procedimentos de automonitoração sobre o comportamento de adesão a dois tipos de regras nutricionais (Plano Alimentar – PA e Contagem Total de Carboidratos - CTC) em adultos com diabetes Tipo 2. Participaram quatro adultos inscritos no Programa HiperDia de uma Unidade Municipal de Saúde, na cidade de Belém, apresentando dificuldades de adesão à dieta. A coleta de dados aconteceu no laboratório de Patologia da Nutrição e em ambiente domiciliar dos participantes. O procedimento constou de: (1) Composição da amostra e entrevista para confirmação dos critérios de inclusão; (2) Caracterização da linha de base (LB) do comportamento alimentar; (3) Intervenção: Treino de Automonitoração (AM) com PA para dois participantes da Condição A (CTa) e com CTC para dois participantes da Condição B (CTb); (4) Reversão das condições de Treino; (5) Follow-up e (6) Entrevista Final. O Treino em AM incluiu a verificação da correspondência entre o registro do comportamento alimentar e as regras nutricionais; a análise dos custos e benefícios da emissão dos comportamentos de seguimento das regras estabelecidas; análises funcionais da emissão ou não do comportamento de seguir as regras e o planejamento das ações de adesão. O participante foi solicitado a registrar todas as refeições realizadas em intervalos de dois em dois dias a cada visita domiciliar da pesquisadora. Calculou-se o Índice de Adesão à Dieta (IAD) por dia de registro. Nos resultados, observou-se que, na linha de base os IADs de todos os participantes estavam abaixo de 50%. No entanto, após o Treino em AM, a média dos IADs obtidos pelos participantes quando submetidos ao PA foi igual a 62,49%, enquanto a obtida em CTC foi de 75,50%. Os valores absolutos dos IADs encontrados nos participantes da CTa foram maiores do que os IADs dos participantes da CTb. Todos os participantes, independente da condição, apresentaram aumento nos IADs ao serem comparados com a LB, com declínio após a suspensão da AM. Ao término da AM, dois participantes escolheram PA e dois a CTC para seguir o tratamento; entretanto, observou-se que todos mantiveram o seguimento do PA no follow-up. Inferiu-se que a CTC parece ser mais eficaz em instalar comportamentos alimentares adequados e o PA parece obter bons resultados na manutenção dos comportamentos instalados. Todos os participantes se encontravam com excesso de peso na LB, dois participantes reduziram o peso após a intervenção, mas todos mantiveram o diagnóstico nutricional da LB. Quanto à hemoglobina glicada, todos os participantes apresentavam valores acima de 6% durante a LB e, após a intervenção, três participantes reduziram esse valor. Discute-se a importância de nutricionistas auxiliarem o paciente a ficar sob o controle das mudanças observadas em seu repertório por meio da análise de contingências, prescrevendo um tratamento individualizado e para além do foco na doença.

O diabetes Mellitus é uma doença crônica que nos últimos anos tem contribuído para o aumento da mortalidade no mundo. O enfermeiro destaca-se como ator primordial na assistência ao paciente diabético por desempenhar assistência de enfermagem nos diversos níveis de atenção à saúde. O estudo se propôs a descrever a compreensão dos diabéticos sobre as complicações com os pés; identificar as informações transmitidas pelos enfermeiros e analisar fatores que contribuem para a incidência de complicações nos pés que implicam no autocuidado. Pesquisa descritiva de natureza qualitativa, desenvolvida nos meses de maio a julho de 2013, com trinta diabéticos internados em um Hospital Universitário de Belém-Pará. Os dados foram coletados por meio de entrevista semi-estruturada e submetidos à análise de conteúdo temática de acordo com o modelo proposto por Bardin. Todos os preceitos legais foram rigorosamente considerados. Participaram do estudo dezessete mulheres e treze homens com média de idade de sessenta anos. A maioria dos informantes apresentava baixo nível de escolaridade, desconhecimento e desinformação sobre o diabetes que implicou na realização do autocuidado. A análise dos dados obtidos permitiu apreender quatro categorias centrais: descoberta e controle do diabetes; complicações do diabetes e o cuidado com os pés; evidências que interferem no autocuidado dos pés; contribuições da equipe de saúde para a prática do autocuidado com os pés. A discussão foi realizada com base no referencial teórico que deu sustentação a pesquisa e relacionados à Teoria do Autocuidado de Orem. Esta pesquisa procurou enfatizar o ambiente hospitalar de internação, por ser um ambiente característico para o atendimento curativo, contudo acreditamos que possa ser também um local propício a ações educativas relacionadas ao autocuidado, e preventivas quanto às complicações com os pés de pacientes diabéticos.

Anemia is a prevalent comorbidity and marker of a poorer prognosis in patients with heart failure (HF). Its clinical relevance, as well as its pathophysiology and the clinical management of these patients are important subjects in the specialized literature. In the present review, we describe the current concepts on the pathophysiology of anemia in HF, its diagnostic criteria, and the recommendations for iron supplementation. Also, we make a critical analysis of the major studies showing evidences on the benefits of this supplementation. The four main components of anemia are addressed: chronic disease, dilutional, renal and malabsorption. In patients with HF, the diagnostic criteria are the same as those used in the general population: serum ferritin levels lower than 30 mcg/L in patients without kidney diseases and lower than 100 mcg/L or serum ferritin levels between 100-299 mcg/L with transferring saturation lower than 20% in patients with chronic kidney diseases. Finally, the therapeutic possibilities for anemia in this specific patient population are discussed.

This study evaluates social inequalities in health according to level of schooling in the male population. This was a cross-sectional, population-based study with a sample of 449 men ranging from 20 to 59 years of age and living in Campinas, Sao Paulo State, Brazil. The chi-square test was used to verify associations, and a Poisson regression model was used to estimate crude and adjusted prevalence ratios. Men with less schooling showed higher rates of alcohol consumption and dependence, smoking, sedentary lifestyle during leisure time, and less healthy eating habits, in addition to higher prevalence of bad or very bad self-rated health, at least one chronic disease, hypertension, and other health problems. No differences were detected between the two schooling strata in terms of use of health services, except for dental services. The findings point to social inequality in health-related behaviors and in some health status indicators. However, possible equity was observed in the use of nearly all types of health services.

Aim. The aim of this study was to evaluate the internal reliability and validity of the BrazilianPortuguese version of Duke Anticoagulation Satisfaction Scale (DASS) among cardiovascular patients. Background. Oral anticoagulation is widely used to prevent and treat thromboembolic events in several conditions, especially in cardiovascular diseases; however, this therapy can induce dissatisfaction and reduce the quality of life. Design. Methodological and cross-sectional research design. Methods. The cultural adaptation of the DASS included the translation and back-translation, discussions with healthcare professionals and patients to ensure conceptual equivalence, semantic evaluation and instrument pretest. The BrazilianPortuguese version of the DASS was tested among subjects followed in a university hospital anticoagulation outpatient clinic. The psychometric properties were assessed by construct validity (convergent, known groups and dimensionality) and internal consistency/reliability (Cronbachs alpha). Results. A total of 180 subjects under oral anticoagulation formed the baseline validation population. DASS total score and SF-36 domain correlations were moderate for General health (r = -0.47, p < 0.01), Vitality (r = -0.44, p < 0.01) and Mental health (r = -0.42, p < 0.01) (convergent). Age and length on oral anticoagulation therapy (in years) were weakly correlated with total DASS score and most of the subscales, except Limitation (r = -0.375, p < 0.01) (Known groups). The Cronbachs alpha coefficient was 0.79 for the total scale, and it ranged from 0.76 (hassles and burdens)0.46 (psychological impact) among the domains, confirming the internal consistency reliability. Conclusions. The BrazilianPortuguese version of the DASS has shown levels of reliability and validity comparable with the original English version. Relevance to clinical practice. Healthcare practitioners and researchers need internationally validated measurement tools to compare outcomes of interventions in clinical management and research tools in oral anticoagulation therapy.

Chagas disease is now an active disease in the urban centers of countries of nonendemicity and endemicity because of congenital and blood and/or organ transplantation transmissions and the reactivation of the chronic disease in smaller scale than vectorial transmission, reported as controlled in countries of endemicity. Oral transmission of Chagas disease has emerged in unpredictable situations in the Amazon region and, more rarely, in areas of nonendemicity where the domiciliary triatomine cycle was under control because of exposition of the food to infected triatomine and contaminated secretions of reservoir hosts. Oral transmission of Chagas disease is considered when >1 acute case of febrile disease without other causes is linked to a suspected food and should be confirmed by the presence of the parasite after direct microscopic examination of the blood or other biological fluid sample from the patient.

As Doenças Crônicas Não Transmissíveis representam a maior carga de morbimortalidade no Brasil. Em 2011, o Ministério da Saúde lançou seu Plano de Ações Estratégicas para o Enfrentamento das Doenças Crônicas Não Transmissíveis, enfatizando ações populacionais para controlar as doenças cardiovasculares, diabetes, câncer e doença respiratória crônica, predominantemente pelo controle do fumo, inatividade física, alimentação inadequada e uso prejudicial de álcool. Apesar da produção científica significativa sobre essas doenças e seus fatores de risco no Brasil, poucos são os estudos de coorte nessa temática. Nesse contexto, o Estudo Longitudinal da Saúde do Adulto (ELSA-Brasil) acompanha 15.105 servidores públicos do País. Seus dados espelham a realidade brasileira de altas prevalências de diabetes e hipertensão e dos fatores de risco. A diversidade das informações produzidas permitirá aprofundar o entendimento causal dessas doenças e subsidiar políticas públicas para seu enfrentamento.
