Websites are, nowadays, the face of institutions, but they are often neglected, especially when it comes to contents. In the present paper, we put forth an investigation work whose final goal is the development of a model for the measurement of data quality in institutional websites for health units. To that end, we have carried out a bibliographic review of the available approaches for the evaluation of website content quality, in order to identify the most recurrent dimensions and the attributes, and we are currently carrying out a Delphi Method process, presently in its second stage, with the purpose of reaching an adequate set of attributes for the measurement of content quality.

Many public organisations have been under great pressure in recent years to increase the efficiency and transparency of outputs, to rationalise the use of public resources, and to increase the quality of service delivery. In this context, public organisations were encouraged to introduce the New Public Management reforms with the goal of improving the efficiency and effectiveness of the performance organisation through a new public management model. This new public management model is based on measurement by outputs and outcomes, a clear definition of responsibilities, the transparency and accountability of governmental activities, and on a greater value for citizens. What type of performance measurement systems are used in police services? Based on the literature, we see that multidimensional models, such as the Balanced Scorecard, are important in many public organisations, like municipalities, universities, and hospitals. Police services are characterised by complex, diverse objectives and stakeholders. Therefore, performance measurement of these public services calls for a specific analysis. Based on a nationwide survey of all police chiefs of the Portuguese police force, we find that employee performance measurement is the main form of measurement. Also, we propose a strategic map for the Portuguese police service.

A espondilopatia cervical uma patologia de etiologia multifactorial, que leva a um estreitamento do canal vertebral e consequentemente compressão da medula espinhal cervical caudal e das raízes nervosas, devido a alterações anatómicas e posicionais ao nível das vértebras cervicais (C5, C6 e C7). Os animais afectados, mais frequentemente raças grandes (Dobermen e Dogue Alemão) podem apresentar ataxia dos membros pélvicos, com ou sem dor cervical, que pode progredir para défices neurológicos dos membros anteriores e em última instância quadriplegia. O diagnóstico desta patologia é feito com base na sintomatologia clínica e exames de imagem (mielografia, tomografia axial computorizada e/ou ressonância magnética). O tratamento pode ser médico ou cirúrgico de acordo com o grau de afecção sendo também o prognóstico variável.

In order to obtain evidence on the size of the impact of the Hong Kong/68 variant of influenza A2 virus on the population of São Paulo, Brazil, serum samples taken in 1967 before this variant appeared and during successive years after it appeared were examined for their antibody content. Haemagglutination-inhibition tests performed on a total of 2726 serum samples from adults showed a sharp decrease in 1969 of the proportion of sera without antibody to the Hong Kong/68 variant and a corresponding mercase in the proportion with high titres. It was concluded that about three-quarters of the adult population became infected at some time after the variant appeared, the majority in the first year of prevalence.

Foi efetuada revisão dos aspectos epidemiológicos do câncer cervical, um dos mais freqüentes em mulheres de países em desenvolvimento. No Brasil a incidência varia de 23,7/100.000, em Porto Alegre, a 83,2/100.000, em Recife. Nos Estados Unidos a incidência em 1978 foi de 6,8/100.000 entre as mulheres brancas e de 14,7/100.000 entre as negras. Várias observações sugerem a hipótese de que o câncer cervical esteja relacionado com algum aspecto da atividade sexual, possivelmente algum agente transmitido por via venérea. As evidências têm implicado o papilomavirus humano (HPV) como o principal agente etiológico deste câncer. Vários trabalhos foram analisados quanto à validade desta hipótese etiológica, mostrando que há uma relação entre HPV e o câncer cervical. Foram analisados os fatores de risco mais conhecidos, tais como o comportamento sexual, o tabagismo e a contracepção, diante das várias possibilidades etiológicas existentes.

The importance of Social Responsibility (SR) is higher if this business variable is related with other ones of strategic nature in business activity (competitive success that the company achieved, performance that the firms develop and innovations that they carries out). The hypothesis is that organizations that focus on SR are those who get higher outputs and innovate more, achieving greater competitive success. A scale for measuring the orientation to SR has defined in order to determine the degree of relationship between above elements. This instrument is original because previous scales do not exist in the literature which could measure, on the one hand, the three classics sub-constructs theoretically accepted that SR is made up and, on the other hand, the relationship between SR and the other variables. As a result of causal relationships analysis we conclude with a scale of 21 indicators, validated scale with a sample of firms belonging to the Autonomous Community of Extremadura and it is the first empirical validation of these dimensions we know so far, in this context.

INTRODUCTION: In Mexico, breast cancer (BC) is one of the main causes of cancer deaths in women, with increasing incidence and mortality in recent years. Therefore, the aim of the study is identify possible risk factors related to BC. METHODS: An epidemiological study of hospital cases of BC and controls with cervical uterine cancer (CUCA) was carried out at eight third level concentration hospitals in Mexico City. The total of 353 incident cases of BC and 630 controls with CUCA were identified among women younger than 75 years who had been residents of the metropolitan area of Mexico City for at least one year. Diagnosis was confirmed histologically in both groups. Variables were analyzed according to biological and statistical plausibility criteria. Univariate, bivariate and multivariate analyses were carried out. Cases and controls were stratified according to the menopausal hormonal status (pre and post menopause). RESULTS: The factors associated with BC were: higher socioeconomic level (OR= 2.77; 95%CI = 1.77 - 4.35); early menarche (OR= 1.32; 95%CI= 0.88 - 2.00); old age at first pregnancy (>31 years: OR= 5.49; 95%CI= 2.16 - 13.98) and a family history of BC (OR= 4.76; 95% CI= 2.10 - 10.79). In contrast, an increase in the duration of the breastfeeding period was a protective factor (>25 months: OR= 0.38; 95%CI= 0.20 - 0.70). CONCLUSIONS: This study contributes to the identification of risk factors for BC described in the international literature, in the population of Mexican women. Breastfeeding appears to play an important role in protecting women from BC. Because of changes in women`s lifestyles, lactation is decreasing in Mexico, and young women tend not to breastfeed or to shorten the duration of lactation.

O presente trabalho teve como objectivos avaliar a influência de diversas grandezas e parâmetros de ensaio no índice de fluidez de termoplásticos e calcular a incerteza associada às determinações. Numa primeira fase, procedeu-se à identificação dos principais parâmetros que influenciam a determinação do índice de fluidez, tendo sido seleccionados a temperatura do plastómetro, o peso de carga, o diâmetro da fieira, o comprimento da medição, o tipo de corte e o número de provetes. Para avaliar a influência destes parâmetros na medição do índice de fluidez, optou-se pela realização de um planeamento de experiências, o qual foi dividido em três etapas. Para o tratamento dos resultados obtidos utilizou-se como ferramenta a análise de variância. Após a completa análise dos desenhos factoriais, verificou-se que os efeitos dos factores temperatura do plastómetro, peso de carga e diâmetro da fieira apresentam um importante significado estatístico na medição do índice de fluidez. Na segunda fase, procedeu-se ao cálculo da incerteza associada às medições. Para tal seleccionou-se um dos métodos mais usuais, referido no Guia para a Expressão da Incerteza da Medição, conhecido como método GUM, e pela utilização da abordagem “passo a passo”. Inicialmente, foi necessária a construção de um modelo matemático para a medição do índice de fluidez que relacionasse os diferentes parâmetros utilizados. Foi estudado o comportamento de cada um dos parâmetros através da utilização de duas funções, recorrendo-se novamente à análise de variância. Através da lei de propagação das incertezas foi possível determinar a incerteza padrão combinada,e após estimativa do número de graus de liberdade, foi possível determinar o valor do coeficiente de expansão. Finalmente determinou-se a incerteza expandida da medição, relativa à determinação do índice de fluidez em volume.

OBJETIVO: Identificar y evaluar los factores que predisponen a la utilización de la prueba de Papanicolaou en la población que usa el servicio del Programa Nacional de Detección Oportuna de Cáncer en México. MÉTODOS: Se realizó un estudio del tipo transversal de enero de 1997 a diciembre de 1998, en la Ciudad de México; se incluyeron a 2.107 mujeres en edad reproductiva que acudieron a un servicio de planificación familiar, las cuáles respondieron a un cuestionario con preguntas sobre características: sociodemográficas, factores de riesgo reproductivo asociados a cáncer cervical, historia de vida sexual, uso de métodos anticonceptivos, conocimiento de la utilidad y utilización del Papanicolaou. Para el análisis de los datos se construyeron modelos multivariados de regresión logística no condicional. RESULTADOS: Los predisponentes de utilización del programa de detección oportuna de cáncer en esta población fueron: el conocimiento de la utilidad del Papanicolaou incrementó en seis veces más la posibilidad de utilización (IC 95% 4,70-7,67); el antecedente de utilización de dos o más métodos de planificación familiar (OR=2,38; IC 95% 1,75-3,24); el antecedente de historia de infección vaginal (OR=2,18; IC 95% 1,73-2,75), y la aceptación del esposo para la realización de exploraciones ginecológicas (OR=1,56; IC 95% 1,07-2,29). CONCLUSIONES: La implementación de programas educativos en la prevención de cáncer, deberán incluir la utilidad de las pruebas de detección. En México, en mujeres en edad reproductiva utilización de la prueba de Papanicolaou se ofrece predominantemente en forma oportunista, por lo que el antecedente de utilización de los Servicios de Salud es un determinante para la utilización del Programa de Detección Oportuna de Cáncer Cervical. Estos resultados muestran la necesidad de ampliar la promoción a la salud de este programa a las mujeres de alto riesgo, incluyendo a sus parejas sexuales.

A dor cervical é cada vez mais um problema comum e incapacitante nos países industrializados. O objectivo do estudo foi determinar a prevalência de dor cervical nos últimos seis meses, intensidade da dor e incapacidade funcional em indivíduos com dor cervical. O estudo do tipo observacional, descritivo e transversal. Amostra constituída por alunos, docentes, administrativos e auxiliares da ESTSP (n=285). Os instrumentos utilizados foram: questionário de caracterização da amostra, escala visual analógica e Neck Disability Index. A prevalência de dor cervical foi elevada e a dor classificada como moderada. A incapacidade funcional variou entre ligeira e moderada.

A crucial method for investigating patients with coronary artery disease (CAD) is the calculation of the left ventricular ejection fraction (LVEF). It is, consequently, imperative to precisely estimate the value of LVEF--a process that can be done with myocardial perfusion scintigraphy. Therefore, the present study aimed to establish and compare the estimation performance of the quantitative parameters of the reconstruction methods filtered backprojection (FBP) and ordered-subset expectation maximization (OSEM). Methods: A beating-heart phantom with known values of end-diastolic volume, end-systolic volume, and LVEF was used. Quantitative gated SPECT/quantitative perfusion SPECT software was used to obtain these quantitative parameters in a semiautomatic mode. The Butterworth filter was used in FBP, with the cutoff frequencies between 0.2 and 0.8 cycles per pixel combined with the orders of 5, 10, 15, and 20. Sixty-three reconstructions were performed using 2, 4, 6, 8, 10, 12, and 16 OSEM subsets, combined with several iterations: 2, 4, 6, 8, 10, 12, 16, 32, and 64. Results: With FBP, the values of end-diastolic, end-systolic, and the stroke volumes rise as the cutoff frequency increases, whereas the value of LVEF diminishes. This same pattern is verified with the OSEM reconstruction. However, with OSEM there is a more precise estimation of the quantitative parameters, especially with the combinations 2 iterations × 10 subsets and 2 iterations × 12 subsets. Conclusion: The OSEM reconstruction presents better estimations of the quantitative parameters than does FBP. This study recommends the use of 2 iterations with 10 or 12 subsets for OSEM and a cutoff frequency of 0.5 cycles per pixel with the orders 5, 10, or 15 for FBP as the best estimations for the left ventricular volumes and ejection fraction quantification in myocardial perfusion scintigraphy.

Neonatal anthropometry is an inexpensive, noninvasive and convenient tool for bedside evaluation, especially in sick and fragile neonates. Anthropometry can be used in neonates as a tool for several purposes: diagnosis of foetal malnutrition and prediction of early postnatal complications; postnatal assessment of growth, body composition and nutritional status; prediction of long-term complications including metabolic syndrome; assessment of dysmorphology; and estimation of body surface. However, in this age group anthropometry has been notorious for its inaccuracy and the main concern is to make validated indices available. Direct measurements, such as body weight, length and body circumferences are the most commonly used measurements for nutritional assessment in clinical practice and in field studies. Body weight is the most reliable anthropometric measurement and therefore is often used alone in the assessment of the nutritional status, despite not reflecting body composition. Derived indices from direct measurements have been proposed to improve the accuracy of anthropometry. Equations based on body weight and length, mid-arm circumference/head circumference ratio, and upper-arm cross-sectional areas are among the most used derived indices to assess nutritional status and body proportionality, even though these indices require further validation for the estimation of body composition in neonates.

OBJECTIVE: To examine the effects of the length and timing of nighttime naps on performance and physiological functions, an experimental study was carried out under simulated night shift schedules. METHODS: Six students were recruited for this study that was composed of 5 experiments. Each experiment involved 3 consecutive days with one night shift (22:00-8:00) followed by daytime sleep and night sleep. The experiments had 5 conditions in which the length and timing of naps were manipulated: 0:00-1:00 (E60), 0:00-2:00 (E120), 4:00-5:00 (L60), 4:00-6:00 (L120), and no nap (No-nap). During the night shifts, participants underwent performance tests. A questionnaire on subjective fatigue and a critical flicker fusion frequency test were administered after the performance tests. Heart rate variability and rectal temperature were recorded continuously during the experiments. Polysomnography was also recorded during the nap. RESULTS: Sleep latency was shorter and sleep efficiency was higher in the nap in L60 and L120 than that in E60 and E120. Slow wave sleep in the naps in E120 and L120 was longer than that in E60 and L60. The mean reaction time in L60 became longer after the nap, and faster in E60 and E120. Earlier naps serve to counteract the decrement in performance and physiological functions during night shifts. Performance was somewhat improved by taking a 2-hour nap later in the shift, but deteriorated after a one-hour nap. CONCLUSIONS: Naps in the latter half of the night shift were superior to earlier naps in terms of sleep quality. However performance declined after a 1-hour nap taken later in the night shift due to sleep inertia. This study suggests that appropriate timing of a short nap must be carefully considered, such as a 60-min nap during the night shift.
