Through a series of experiments, the genotoxic/mutagenic and carcinogenic potential of sewage sludge was assessed. Male Wistar rats were randomly assigned to four groups: Group 1 - negative control; Group 2 - liver carcinogenesis initiated by diethylnitrosamine (DEN; 200 mg/kg i.p.); Group 3 and G4-liver carcinogenesis initiated by DEN and fed 10,000 ppm or 50,000 ppm of sewage sludge. The animals were submitted to a 70% partial hepatectomy at the 3rd week. Livers were processed for routine histological analysis and immunohistochemistry, in order to detect glutathione S-transferase positive altered hepatocyte foci (GST-P+ AHF). Peripheral blood samples for the comet assay were obtained from the periorbital plexus immediately prior to sacrificing. Polychromatic erythrocytes (PCEs) were analyzed in femoral bone-marrow smears, and the frequencies of those micronucleated (MNPCEs) registered. There was no sewage-sludge-induced increase in frequency of either DNA damage in peripheral blood leucocytes, or MNPCEs in the femoral bone marrow. Also, there was no increase in the levels of DNA damage, in the frequency of MNPCEs, and in the development of GST-P AHF when compared with the respective control group.

Background Cost-effectiveness studies have been increasingly part of decision processes for incorporating new vaccines into the Brazilian National Immunisation Program. This study aimed to evaluate the cost-effectiveness of 10-valent pneumococcal conjugate vaccine (PCV10) in the universal childhood immunisation programme in Brazil. Methods A decision-tree analytical model based on the ProVac Initiative pneumococcus model was used, following 25 successive cohorts from birth until 5 years of age. Two strategies were compared: (1) status quo and (2) universal childhood immunisation programme with PCV10. Epidemiological and cost estimates for pneumococcal disease were based on National Health Information Systems and literature. A 'top-down' costing approach was employed. Costs are reported in 2004 Brazilian reals. Costs and benefits were discounted at 3%. Results 25 years after implementing the PCV10 immunisation programme, 10 226 deaths, 360 657 disability-adjusted life years (DALYs), 433 808 hospitalisations and 5 117 109 outpatient visits would be avoided. The cost of the immunisation programme would be R$10 674 478 765, and the expected savings on direct medical costs and family costs would be R$1 036 958 639 and R$209 919 404, respectively. This resulted in an incremental cost-effectiveness ratio of R$778 145/death avoided and R$22 066/DALY avoided from the society perspective. Conclusion The PCV10 universal infant immunisation programme is a cost-effective intervention (1-3 GDP per capita/DALY avoided). Owing to the uncertain burden of disease data, as well as unclear long-term vaccine effects, surveillance systems to monitor the long-term effects of this programme will be essential.
Characterization of PAR1 and FGFR1 expression in invasive breast carcinomas: Prognostic significance

Breast cancer is the most common cause of cancer mortality among women worldwide. Among the several factors associated with breast cancer development, angiogenesis plays an essential role and has currently emerged as a potential diagnostic, prognostic and therapeutic target. Protease-activated receptor 1 (PAR1) and fibroblast growth factor receptor 1 (FGFR1) have important activities in tumor angiogenesis and progression. The aim of this study was to investigate the prognostic significance of these two receptors, hypothesising significant correlations between receptor expression in tumor angiogenesis and clinicopathological parameters customarily used in breast cancer prognosis and prediction. Formalin-fixed and paraffin-embedded samples of ductal invasive breast carcinomas were used to analyze the expression of PAR1 and FGFR1, in the tumor cells as well as in the tumor stroma, and further determine intratumoral microvessel density (iMVD) to quantify intratumoral angiogenesis. Correlations between PAR1 and FGFR1 expression in tumor cells and stroma, iMVD and several clinicopathological parameters and molecular markers used in breast cancer diagnosis have been addressed. The correlation between PAR1 and FGFR1 suggests an association of the two receptors with a more aggressive breast cancer phenotype and, consequently, a potential role during tumor progression. The results reported in the present study also emphasize the importance of microenvironmental factors in tumor progression, while precluding the positive association between iMVD and breast cancer aggressiveness.

BACKGROUND: Alkaline sulfite/anthraquinone (ASA) cooking of Pinus radiata and Pinus caribaea wood chips followed by disk refining was used as a pretreatment for the production of low lignified and high fibrillated pulps. The pulps produced with different delignification degrees and refined at different energy inputs (250, 750 and 1600 Wh) were saccharified with cellulases and fermented to ethanol with Saccharomyces cerevisiae using separated hydrolysis and fermentation (SHF) or semi-simultaneous saccharification and fermentation (SSSF) processes. RESULTS: Delignification of ASA pulps was between 25% and 50%, with low glucans losses. Pulp yield was from 70 to 78% for pulps of P. radiata and 60% for the pulp of P. caribaea. Pulps obtained after refining were evaluated in assays of enzymatic hydrolysis. Glucans-to-glucose conversion varied from 20 to 70%, depending on the degree of delignification and fibrillation of the pulps. The best ASA pulp of P. radiata was used in SHF and SSSF experiments of ethanol production. Such experiments produced maximum ethanol concentration of 20 g L-1, which represented roughly90% of glucose conversion and an estimated amount of 260 L ethanol ton(-1) wood. P. caribaea pulp also presented good performance in the enzymatic hydrolysis and fermentation but, due to the low amount of cellulose present, only 140 L ethanol would be obtained from each ton of wood. CONCLUSION: ASA cooking followed by disk refining was shown to be an efficient pretreatment process, which generated a low lignified and high-fibrillated substrate that allowed the production of ethanol from the softwoods with high conversion yields. The global emergence of vancomycin-resistant Enterococcus faecium (VREfm) has been characterized by a clonal spread of strains belonging to clonal complex 17 (CC17). Genetic features and clonal relationships of 53 VREfm isolated from patients in 2 hospitals in Ribeirao Preto, Sao Paulo, Brazil, during 2005-2010 were determined as a contribution to the Brazilian evolutionary history of these nosocomial pathogens. All isolates were daptomycin susceptible, vancomycin-resistant, and had the vanA gene. The predominant virulence genes were acm and esp. Only 5 VREfm isolated in 2005-2006 had intact Tn1546, while 81% showed Tn1546 with deleted left extremity and insertion of IS1251 between the vanS and vanH genes. Multilocus sequence typing analysis permitted the identification of 9 different sequence types (STs), with 5 being new ones (656, 657, 658, 659, and 660). Predominant STs were ST412 and ST478, all belonging to CC17, except ST658. This is the first report of the ST78 in Brazil. (c) 2012 Elsevier Inc. All rights reserved.

The theoretical framework that underpins this research study is based on the Prospect Theory formulated by Kahneman and Tversky, and Thaler's Mental Accounting Theory. The research aims to evaluate the consumers' behavior when different patterns of discount are offered (in percentage and absolute value and for larger and smaller discounts). Two experiments were conducted to explore these patterns of behavior and the results that were obtained supported the view that the framing effect was a common occurrence. The patterns of choice of individuals in a sample were found to be different due to changes in the ways discounts were offered. This can be explained by the various ways of presenting discount rates that had an impact on the influence of purchase intentions, recommendations and quality perception.

Essa pesquisa-intervenção teve como objetivo cartografar os movimentos instituídos e instituintes presentes no trabalho da Estratégia Saúde da Família, no que tange a composição de suas práticas cuidativas. O referencial teórico metodológico fundamentou-se na análise institucional, linha esquizoanalítica. Foram realizados encontros grupais com uma equipe para discutir o modo como realizavam os cuidados coletivos em ação de educação permanente em saúde. Os sujeitos da pesquisa foram trabalhadores da equipe e estudantes em atividade acadêmica no serviço. A média de participação foi de doze pessoas por encontro, sendo que se desenvolveram oito encontros no período de março a julho de 2010. Os dados foram agrupados em dois estratos imanentes: as relações da equipe e a relação com os usuários. Os estratos apontaram para o atravessamento das instituições de educação, justiça e da divisão técnica e social do trabalho. A reflexão coletiva em grupo mostrou-se potente, para desnaturalizar processos instituídos e interrogar lugares, saberes e práticas.

Estudo qualitativo que objetivou analisar os motivos que levam o doente de tuberculose a abandonar o tratamento, em municípios da região metropolitana de João Pessoa-PB. Como referencial teórico utilizou-se a integralidade do cuidado. Os dados foram coletados no período de outubro a novembro de 2008, mediante entrevista semiestruturada. Participaram nove usuários que abandonaram o tratamento da tuberculose. Na identificação das unidades de significação, utilizou-se a técnica de análise de conteúdo. Os resultados mostram inconsistências relacionadas ao cuidado integral do doente, no que tange à diretriz doutrinária do Sistema Único da Saúde, à consideração do contexto sócio-cultural do usuário com tuberculose e às debilidades na continuidade da atenção, mediante inadequado acompanhamento do mesmo na rede de atenção à saúde. Esses aspectos, ao dificultarem a construção de uma rede de cuidado e apoio ao doente de tuberculose e sua família, contribuem para o abandono do tratamento da tuberculose.

Psychotherapeutic interventions that bring about differentiation, separation, individuation and autonomy in the mother-daughter relationship are recommended as treatment for eating disorders. With this goal in mind, a psychotherapy group for mothers was organized in an outpatient program for adolescents with eating disorders at a public institution, as one of the psychotherapeutic approaches in the multidisciplinary treatment of adolescent patients. Evidence suggests that this approach can be relevant and effective in the treatment of eating disorders.

Este ensaio pretende discutir a aquisição de alimentos para o Programa Nacional de Alimentação Escolar à luz do seu marco legal vigente, na perspectiva de promover hábitos alimentares saudáveis e culturalmente articulados que contribuam para a promoção das condições de saúde da população escolar brasileira e para o desenvolvimento local. Apresentase análise da legislação em vigor do Programa Nacional de Alimentação Escolar, evidenciando a intencionalidade de influenciar o sistema alimentar brasileiro e o padrão alimentar de sua população a partir da alimentação escolar, destacandose as lacunas que desafiam a efetivação de mudanças substanciais na execução do programa. Partindo dessa análise e com base no elevado e crescente consumo de alimentos ultraprocessados no Brasil, e considerando as desvantagens desses alimentos diante de alimentos pouco ou não processados, desenvolvese proposta para orientar a construção de uma pauta de alimentos que seja consistente com o marco legal vigente do programa e com seus objetivos. Argumentase que a predominância de alimentos pouco ou não processados na alimentação escolar pode ser estratégia para o resgate do patrimônio alimentar saudável e para o fortalecimento do desenvolvimento local se resultar da aproximação com a agricultura familiar.

The objective of this study was to evaluate the chemical composition and dry matter in vitro digestibility of stem, leaf, straw, cob and kernel fractions of eleven corn (Zea mays) cultivars, harvested at two cutting heights. The experiment was designed as randomized blocks, with three replicates, in a 2 × 11 factorial arrangement (eleven cultivars and two cutting heights). The corn cultivars evaluated were D 766, D 657, D 1000, P 3021, P 3041, C 805, C 333, AG 5011, FOR 01, CO 9621 and BR 205, harvested at a low cutting height (5 cm above ground) and a high cutting height (5 cm below the first ear insertion). Cutting height influenced the dry matter content of the stem fraction, which was lower (23.95%) in plants harvested at the low, than in plants harvested at the high cutting height (26.28%). The kernel fraction had the highest dry matter in vitro digestibility (85.13%), while cultivars did not differ between each other. Cob and straw were the fractions with the highest level of neutral detergent fiber (80.74 and 79.77%, respectively) and the lowest level of crude protein (3.84% and 3.69%, respectively). The leaf fraction had the highest crude protein content, both for plants of low and high cuttings (15.55% and 16.20%, respectively). The increase in the plant cutting height enhanced the dry matter content and dry matter in vitro digestibility of stem fraction, but did not affect the DM content of the leaf fraction.

In order to inactivate enzymatic deterioration, whole rice bran samples were subjected to two stabilization methods. Changes in nutritional value in terms of, concerning chemical composition, minerals and fatty acid content, were evaluated to supplement existing data and promote the utilization of rice bran in the human diet. The following homemade heat treatments were applied: roasting on a conventional stove or heating in a microwave oven. Based on the results, the different heating methods affected sample composition, since the levels of some nutrients of treated samples showed significant changes (p<0.05) compared to corresponding raw samples. The rice bran treated on a conventional stove produced products with lower moisture (5.14±0.10 g/100 g) and nutrients such as sodium 11.8%; palmitic acid 9.9% and stearic acid 8.1%. The microwave oven procedure resulted in better nutrient preservation, with slightly higher moisture content (6.28±0.10 g/100 g), and appears to be a practical and rapid tool for home heat stabilization of rice bran.
