El tema de la autonomía en Bolivia tiene varias aristas, mientras para unos es la lucha por la autodeterminación de los pueblos originarios, para otros es la búsqueda de una mayor descentralización política y económica. Es un tema arraigado históricamente entre profundas negaciones y lucha por el poder. Los medios de comunicación son actores clave en éste proceso, los posicionamientos discursivos particularmente en las editoriales expresan de que lado están, por tanto, estudiar estas representaciones de la realidad, analizar las ideologías, subyacentes, abren el camino para de alguna manera conocer si el trabajo que realizan apoya o no a la democracia o más bien acentúa los conflictos sociales

BACKGROUND: Most patients with glioblastoma are older than 60 years, but treatment guidelines are based on trials in patients aged only up to 70 years. We did a randomised trial to assess the optimum palliative treatment in patients aged 60 years and older with glioblastoma. METHODS: Patients with newly diagnosed glioblastoma were recruited from Austria, Denmark, France, Norway, Sweden, Switzerland, and Turkey. They were assigned by a computer-generated randomisation schedule, stratified by centre, to receive temozolomide (200 mg/m(2) on days 1-5 of every 28 days for up to six cycles), hypofractionated radiotherapy (34·0 Gy administered in 3·4 Gy fractions over 2 weeks), or standard radiotherapy (60·0 Gy administered in 2·0 Gy fractions over 6 weeks). Patients and study staff were aware of treatment assignment. The primary endpoint was overall survival. Analyses were done by intention to treat. This trial is registered, number ISRCTN81470623. FINDINGS: 342 patients were enrolled, of whom 291 were randomised across three treatment groups (temozolomide n=93, hypofractionated radiotherapy n=98, standard radiotherapy n=100) and 51 of whom were randomised across only two groups (temozolomide n=26, hypofractionated radiotherapy n=25). In the three-group randomisation, in comparison with standard radiotherapy, median overall survival was significantly longer with temozolomide (8·3 months [95% CI 7·1-9·5; n=93] vs 6·0 months [95% CI 5·1-6·8; n=100], hazard ratio [HR] 0·70; 95% CI 0·52-0·93, p=0·01), but not with hypofractionated radiotherapy (7·5 months [6·5-8·6; n=98], HR 0·85 [0·64-1·12], p=0·24). For all patients who received temozolomide or hypofractionated radiotherapy (n=242) overall survival was similar (8·4 months [7·3-9·4; n=119] vs 7·4 months [6·4-8·4; n=123]; HR 0·82, 95% CI 0·63-1·06; p=0·12). For age older than 70 years, survival was better with temozolomide and with hypofractionated radiotherapy than with standard radiotherapy (HR for temozolomide vs standard radiotherapy 0·35 [0·21-0·56], p<0·0001; HR for hypofractionated vs standard radiotherapy 0·59 [95% CI 0·37-0·93], p=0·02). Patients treated with temozolomide who had tumour MGMT promoter methylation had significantly longer survival than those without MGMT promoter methylation (9·7 months [95% CI 8·0-11·4] vs 6·8 months [5·9-7·7]; HR 0·56 [95% CI 0·34-0·93], p=0·02), but no difference was noted between those with methylated and unmethylated MGMT promoter treated with radiotherapy (HR 0·97 [95% CI 0·69-1·38]; p=0·81). As expected, the most common grade 3-4 adverse events in the temozolomide group were neutropenia (n=12) and thrombocytopenia (n=18). Grade 3-5 infections in all randomisation groups were reported in 18 patients. Two patients had fatal infections (one in the temozolomide group and one in the standard radiotherapy group) and one in the temozolomide group with grade 2 thrombocytopenia died from complications after surgery for a gastrointestinal bleed. INTERPRETATION: Standard radiotherapy was associated with poor outcomes, especially in patients older than 70 years. Both temozolomide and hypofractionated radiotherapy should be considered as standard treatment options in elderly patients with glioblastoma. MGMT promoter methylation status might be a useful predictive marker for benefit from temozolomide. Despite the fact that mineralocorticoid receptor (MR) antagonist drugs such as spironolactone and eplerenone reduce the mortality in heart failure patients, there is, thus far, no unambiguous demonstration of a functional role of MR in cardiac cells. The aim of this work was to investigate the activation pathway(s) mediating corticosteroid-induced up-regulation of cardiac calcium current (ICa). In this study, using neonatal cardiomyocytes from MR or glucocorticoid receptor (GR) knockout (KO) mice, we show that MR is essential for corticosteroid-induced up-regulation of ICa. This study provides the first direct and unequivocal evidence for MR function in the heart.

Platelet P2YI2 receptor inhibition with clopidogrel, prasugrel or ticagrelor plays a key role to prevent recurrent ischaemic events after percutaneous coronary intervention in acute coronary syndromes or elective settings. The degree of platelet inhibition depends on the antiplatelet medication used and is influenced by clinical and genetic factors. A concept of therapeutic window exists. On one side, efficient anti-aggregation is required in order to reduce cardio-vascular events. On the other side, an excessive platelet inhibition represents a risk of bleeding complications. This article describes the current knowledge about some platelet function tests and genetic tests and summarises their role in the clinical practice.

It is known that post-movement beta synchronization (PMBS) is involved both in active inhibition and in sensory reafferences processes. The aim of this study was examine the temporal and spatial dynamics of the PMBS involved during multi-limb coordination task. We investigated post-switching beta synchronization (assigned PMBS) using time-frequency and source estimations analyzes. Participants (n = 17) initiated an auditory-paced bimanual tapping. After a 1500 ms preparatory period, an imperative stimulus required to either selectively stop the left while maintaining the right unimanual tapping (Switch condition: SWIT) or to continue the bimanual tapping (Continue condition: CONT). PMBS significantly increased in SWIT compared to CONT with maximal difference within right central region in broad-band 14âeuro"30 Hz and within left central region in restricted-band 22âeuro"26 Hz. Source estimations localized these effects within right pre-frontal cortex and left parietal cortex, respectively. A negative correlation showed that participants with a low percentage of errors in SWIT had a large PMBS amplitude within right parietal and frontal cortices. This study shows for the first time simultaneous PMBS with distinct functions in different brain regions and frequency ranges. The left parietal PMBS restricted to 22âeuro"26 Hz could reflect the sensory reafferences of the right hand tapping disrupted by the switching. In contrast, the right pre-frontal PMBS in a broad-band 14âeuro"30 Hz is likely reflecting the active inhibition of the left hand stopped. Finally, correlations between behavioral performance and the magnitude of the PMBS suggest that beta oscillations can be viewed as a marker of successful active inhibition.

Le scandale de l'amiante, l'épidémie de suicides au travail ou bien encore le "karôshi" sont autant de faits qui invitent à une réflexion renouvelée sur les conditions de travail. Cette thèse part d'une volonté de revisiter, dans un monde professionnel bouleversé par les progrès constants de la technique et les pressions économiques, les liens entre santé et sécurité, telles que visées aux art. 328 CO, 6 LTr et 82 LAA. Il s'agit d'examiner l'articulation entre ces dispositions, sur la base d'une analyse historique et de l'apport actuel du système légal, et la pertinence de repenser celui-ci. En effet, le pluralisme des sources en la matière, hétérogènes de par leur nature (de droit public ou privé) et/ou de par leur objet (la santé ou la sécurité), et le manque d'harmonie entre elles ne sont pas sans créer difficultés d'application et doubles emplois. La présente thèse commence par quelques repères chronologiques sur la mise en place progressive et pragmatique de la législation sociale suisse. Suit un survol des lois protectrices et autres normativités extra-juridiques, qui laisse conclure à un foisonnement et à une hétérogénéité des sources applicables. Puis, les trois dispositions générales topiques, à savoir les articles 328 CO, 6 LTr et 82 LAA, font successivement l'objet d'un examen détaillé, en vue d'évaluer leur bien-fondé respectif et la pertinence d'une approche compartimentée de la santé et sécurité au travail. Et donc de conclure par une réflexion sur l'intérêt d'un changement, notamment par la suppression du dualisme LTr / LAA.

Sedentary lifestyle in children is increasing at an alarming rate. Now, promotion of physical activity by health professionals is a promising way. To support childhood specialists in this role, a transdisciplinary training is being developped.

O objetivo deste trabalho foi avaliar o efeito de dietas, com diferentes teores de fósforo, no perfil eletrolítico sanguíneo e no ganho de peso corporal em suínos. Foram utilizados 60 suínos híbridos comerciais, machos castrados, com peso corporal médio inicial de 61,07±0,86 kg, com aproximadamente 110 dias de idade. Os animais foram distribuídos em blocos ao acaso, com 5 tratamentos e 12 repetições cada um. Os tratamentos foram constituídos de dietas com 0,135, 0,200, 0,265, 0,330 e 0,395% de fósforo disponível. No início e no final do período experimental, foi determinado o peso corporal e foram coletadas amostras de sangue, sem jejum dos animais, para a mensuração de sódio, potássio, cloreto, magnésio total, fósforo e cálcio ionizado. Observou-se resposta quadrática do peso corporal, de acordo com os tratamentos, e das concentrações séricas de fósforo, magnésio total e cálcio iônico em decorrência dos tratamentos. O ganho de peso corporal foi maior nos animais alimentados com suplementação de fósforo em teores acima de 0,135%. O melhor resultado foi obtido com a dieta de 0,330% de fósforo, porém, o aumento a partir de 0,200% na dieta ocasionou elevação na concentração sérica de fósforo.

O objetivo deste trabalho foi caracterizar o óleo-resina da copaíba (Copaifera reticulata) e estimar, por meio de marcadores microssatélites, a variabilidade genética da espécie na Floresta Nacional do Tapajós, PA. A amostragem foi realizada em duas áreas, distanciadas de 5 km, em 136 árvores. A diversidade genética foi avaliada com seis marcadores microssatélites derivados de C. langsdorffii, e o óleo obtido de 30 árvores (15 de cada área) foi caracterizado em termos físicos e químicos. O óleo C. reticulata apresenta aspecto líquido, fino, odor fraco e de coloração amarelo-dourada (73,3% das plantas), com viscosidade muito variável (18 a 187 Pa-s) e densidade média de 0,975±0,049 g cm-3. O índice de acidez variou de 9,62 a 10,17 mg g-1 de KOH e o de saponificação de 100,63 a 109,84 mg g-1. A análise molecular identificou 78 alelos, com média de 13 por loco. A heterozigosidade esperada variou 0,59 a 0,85 (média de 0,75), com nível de endogamia de 0,375 a 0,419. Houve pouca diferenciação genética entre as populações das diferentes áreas de coleta (F ST = 0,030), mas a variabilidade foi maior entre os grupos genéticos detectados pelo programa Structure (F ST = 0,070). Essa maior variabilidade indica que não há ameaças à conservação genética da copaíba, em médio prazo.

OBJECTIVE: Critically ill patients are at high risk of malnutrition. Insufficient nutritional support still remains a widespread problem despite guidelines. The aim of this study was to measure the clinical impact of a two-step interdisciplinary quality nutrition program. DESIGN: Prospective interventional study over three periods (A, baseline; B and C, intervention periods). SETTING: Mixed intensive care unit within a university hospital. PATIENTS: Five hundred seventy-two patients (age 59 ± 17 yrs) requiring >72 hrs of intensive care unit treatment. INTERVENTION: Two-step quality program: 1) bottom-up implementation of feeding guideline; and 2) additional presence of an intensive care unit dietitian. The nutrition protocol was based on the European guidelines. MEASUREMENTS AND MAIN RESULTS: Anthropometric data, intensive care unit severity scores, energy delivery, and cumulated energy balance (daily, day 7, and discharge), feeding route (enteral, parenteral, combined, none-oral), length of intensive care unit and hospital stay, and mortality were collected. Altogether 5800 intensive care unit days were analyzed. Patients in period A were healthier with lower Simplified Acute Physiologic Scale and proportion of "rapidly fatal" McCabe scores. Energy delivery and balance increased gradually: impact was particularly marked on cumulated energy deficit on day 7 which improved from -5870 kcal to -3950 kcal (p < .001). Feeding technique changed significantly with progressive increase of days with nutrition therapy (A: 59% days, B: 69%, C: 71%, p < .001), use of enteral nutrition increased from A to B (stable in C), and days on combined and parenteral nutrition increased progressively. Oral energy intakes were low (mean: 385 kcal*day, 6 kcal*kg*day ). Hospital mortality increased with severity of condition in periods B and C. CONCLUSION: A bottom-up protocol improved nutritional support. The presence of the intensive care unit dietitian provided significant additional progression, which were related to early introduction and route of feeding, and which achieved overall better early energy balance.
