America’s low-income families struggle to protect their children from multiple threats to their health and growth. Many research and advocacy groups explore the health and educational effects of food insecurity, but less is known about these effects on very young children. Children’s HealthWatch, a group of pediatric clinicians and public health researchers, has continuously collected data on the effects of food insecurity alone and in conjunction with other household hardships since 1998. The group’s peer reviewed research has shown that a number of economic risks at the household level, including food, housing and energy insecurity, tend to be correlated. These insecurities alone or in conjunction increase the risk that a young child will suffer various negative health consequences, including increases in lifetime hospitalizations, parental report of fair or poor health,1 or risk for developmental delays.2 Child food insecurity is an incremental risk indicator above and beyond the risk imposed by household-level food insecurity. The Children’sHealthwatch research also suggests public benefits programs modify some of these effects for families experiencing hardships. This empirical evidence is presented in a variety of public venues outside the usual scientific settings, such as congressional hearings, to support the needs of America’s most vulnerable population through policy change. Children’s HealthWatch research supports legislative solutions to food insecurity, including sustained funding for public programs and re-evaluation of the use of the Thrifty Food Plan as the basis of SNAP benefits calculations. Children’s HealthWatch is one of many models to support the American Academy of Pediatrics’ call to “stand up, speak up, and step up for children.”3 No isolated group or single intervention will solve child poverty or multiple hardships. INTRODUCTION The new ATS/ERS consensus report recommends in vitro validation of multiple-breath inert gas washout (MBW) equipment based on a lung model with simulated physiologic conditions. We aimed to assess accuracy of two MBW setups for infants and young children using this model, and to compare functional residual capacity (FRC) from helium MBW (FRCMBW ) with FRC from plethysmography (FRCpleth ) in vivo. METHODS The MBW setups were based on ultrasonic flow meter technology. Sulfur hexafluoride and helium were used as tracer gases. We measured FRC in vitro for specific model settings with and without carbon dioxide and calculated differences of measured to generated FRC. For in vivo evaluation, difference between FRCMBW and FRCpleth was calculated in 20 healthy children, median age 6.1 years. Coefficient of variation (CV) was calculated per FRC. RESULTS In the infant model (51 runs, FRC 80-300 ml), mean (SD) relative difference between generated and measured FRCs was 0.7 (4.7) %, median CV was 4.4% for measured FRCs. In the young child model, one setting (8 runs, FRC 400 ml) showed a relative difference of up to 13%. For the remaining FRCs (42 runs, FRC 600-1,400 ml), mean (SD) relative difference was -2.0 (3.4) %; median CV was 1.4% for measured FRCs. In vivo FRCpleth exceeded FRCMBW values by 37% on average. CONCLUSIONS Both setups measure lung volumes in the intended age group reliably and reproducibly. Characteristics of different techniques should be considered when measuring lung volumes in vivo. PURPOSE To describe the clinical outcome of corneal cross-linking (CXL) in a young child with keratoconus. METHODS This is a case report of a young girl with keratoconus with ophthalmologic findings and 3-year follow-up. Follow-up visits included visual acuity measurement, retinoscopy, corneal tomography, and topography. RESULTS A girl with Down syndrome was diagnosed with bilateral keratoconus and relative amblyopia at the age of 4 years. The best-corrected near visual acuity was 20/100 binocularly. Corneal tomography showed the following parameters: OD K(max) 47.2 diopters (D), thinnest location 442 μm; OS K(max) 49.6 D, thinnest location 432 μm. Three months later, the keratoconus in the left eye progressed (K(max) 50.2 D, thinnest location 424 μm), and CXL was performed. One year later, CXL was necessary also in the right eye because of progression. The girl was most recently reexamined at the age of 7 years. The corrected near visual acuity was 20/80 in both eyes. The corneal curvature slightly flattened, and the corneal thickness stabilized (OD K(max) 46.8 D, thinnest location 389 μm; OS K(max) 49.4 D, thinnest location 360 μm). CONCLUSIONS Onset of keratoconus can occur in early childhood, especially in patients with Down syndrome. In this case, CXL was performed at 4 and 5 years of age without complications and stopped further keratoconus progression.

Background. The parents of a sick child likely experience situational anxiety due to their young child being unexpectedly hospitalized. The emotional upheaval may be great enough that their anxiety inhibits them in providing positive support to their hospitalized child. Because anxiety affects psychological distress as well as behavioral distress, identifying parental distress helps parents improving their coping mechanisms. ^ Purpose. The study compared situational anxiety levels between Taiwanese fathers and mothers and focused on differences between parental anxiety levels at the beginning of the child's unplanned hospitalization and at time of discharge. The study also identified factors related to the parents' distress and use of coping mechanisms. ^ Methods. A descriptive, comparative research design was used to determine the difference between the anxiety levels of 62 Taiwanese father-mother dyads during the situational crisis of their child's unexpected hospitalization. The Mandarin version (M) of Visual Analog Scale (VAS-M), State-Trait Anxiety Inventory (STAI-M), and the Index of Parent Participation/Hospitalized Child (IPP/HC-M) were used to differentiate maternal and paternal anxiety levels and identify factors related to the parents' distress. Questionnaires were completed by parents within 24-36 hours of the child's hospital admission and within 24 hours prior to discharge. A paired t-test, two sample t-test, and linear mixed regression model were used to test and support the study hypothesis. ^ Results. The findings reveal that the mothers' anxiety levels did not significantly differ from the fathers' anxiety level when their child had a sudden admission to the hospital. In particular, parental state anxiety levels did not decrease during the child's hospital stay and subsequent discharge. Moreover, anxiety levels did not differ between parents regardless of whether the child's disease was acute or chronic. The most effective factor related to parental situational anxiety was parental perception of the severity of the child's illness. ^ Conclusions. Parental anxiety was found to be significantly related to changes in their perception of the severity of their child's illness. However, the study was not able to illustrate how parental involvement in the child's hospital care was related to parental perception of the severity of their child's illness. Future studies, using a qualitative approach to gamer more information as to what variables influence parental anxiety during a situational crisis, may provide a richer database from which to modify key variables as well as the instruments used to improve the quality of the data obtained. ^

The South Asia Infant Feeding Research Network (SAIFRN) was established in 2007 to foster and coordinate a research partnership among South Asian and international research groups interested in infant and young child feeding. SAIFRN has brought together a mix of researchers and program managers from Bangladesh, India, Nepal, Pakistan, and Sri Lanka together with international partners from Australia. As the first activity, SAIFRN conducted a series of analyses using Demographic and Health Surveys of Bangladesh, Nepal, and Sri Lanka and the National Family Health Survey of India. The results highlight that most indicators of infant and young child feeding in these four countries have not reached the targeted levels. The rates vary considerably by country, and the factors associated with poor feeding practices were not always consistent across countries. Driven by the ultimate goal of improved child survival in the region, SAIFRN wishes to expand its partnerships with governmental and nongovernmental organizations that share common interests both within and outside the South Asia region. In the future, SAIFRN hopes to provide more opportunities to researchers in the region to improve their skills by participating in capacity-building programs in collaboration with international partner institutions, and looks forward to liaising with potential donors to support such activities.

Education might be conceptualized as a swarm of signs. Deleuze, in Proust and Signs (1964/2000) suggests that “Everything that teaches us something emits signs” (p. 4). Such conceptualizations regard education as fluid, multiple and temporal; a young child can display great skill in decoding some signs but not others. Regarding education as temporal and complex operates at some distance to the sociocultural concepts suggested by Vygotsky (1978) which focus on linear sequences of gaining managed, culturally-loaded knowledge from more experienced others. Despite differing theorizations around apprenticeship, during early years education a child becomes sensitive to signs that collectively prioritize conventionalized knowledge acquisition and communication practices. Drawing for learning and communicating exemplifies apprenticeship as a creative process rather than as sequential or culturally driven, and serves to exemplify Deleuzian concepts around the relationships between time and learning, rather than age or development stage and learning.

This chapter focuses on ‘intergenerational collaborative drawing’, a particular process of drawing whereby adults and children draw at the same time on a blank paper space. Such drawings can be produced for a range of purposes, and based on different curriculum or stimulus subjects. Children of all ages, and with a range of physical and intellectual abilities are able to draw with parents, carers and teachers. Intergenerational collaborative drawing is a highly potent method for drawing in early childhood contexts because it brings adults and children together in the process of thinking and theorizing in order to create visual imagery and this exposes in deep ways to adults and children, the ideas and concepts being learned about. For adults, this exposure to a child’s thinking is a far more effective assessment tool than when they are presented with a finished drawing they know little about. This chapter focuses on drawings to examine wider issues of learning independence and how in drawing, preferred schema in the form of hand-out worksheets, the suggestive drawings provided by adults, and visual material seen in everyday life all serve to co-opt a young child into making particular schematic choices. I suggest that intergenerational collaborative drawing therefore serves to work as a small act of resistance to that co-opting, in that it helps adults and children to collectively challenge popular creativity and learning discourses.

O fenômeno social que se desenhou na esfera da educação infantil nos últimos anos, provocado muito pelo aumento do número de crianças em unidades educacionais, traz a emergência de repensar o fazer pedagógico e a atuação do profissional da educação infantil, buscando compreender a criança pequena como um ator social. Embora o debate sobre a finalidade da educação infantil tenha se intensificado quanto a ser sua tarefa ensinar e reproduzir o modelo de currículo por disciplinas, de outra parte, acentua-se a defesa por uma educação infantil comprometida com a brincadeira e a cultura infantil, como expresso nas atuais Diretrizes Curriculares Nacionais para Educação Infantil, de 2009. Neste sentido, com a contribuição de teóricos das áreas da educação, psicologia e sociologia e a partir da análise documental de parte da produção do conhecimento produzida na Creche UFF, este trabalho busca identificar e compreender como o brincar se apresenta em 35 produções elaboradas a partir e por essa unidade de educação infantil. Para tanto, tem como metodologia a análise documental, visando a pesquisa de natureza qualitativa, à luz do paradigma interpretativo. Esta pesquisa contribui com o reconhecimento da identidade dessa unidade de educação infantil, quando articula atividade de ensino, pesquisa e extensão para alunos dos cursos de graduação e pós-graduação e professores pesquisadores e, consequentemente, contribui para o diálogo e formação dos profissionais das variadas áreas do conhecimento que se dedicam a estudos da infância. Também com propostas curriculares na perspectiva de um trabalho pedagógico com as crianças no sentido de valorizar a brincadeira como algo próprio da cultura infantil. Além disso, busca proporcionar para estudiosos um repensar a respeito da importância dessa Unidade Universitária Federal de Educação Infantil como um campo de formação profissional e produção do conhecimento sobre a infância

Los beneficios que aporta la musicoterapia en alumnos con Trastorno del Espectro Autista, han sido demostrados profusamente por los distintos autores, si bien carecemos de literatura suficiente sobre su utilización en las Aulas Abiertas Especializadas en colegios ordinarios (Aulas TEA). En este sentido, el objetivo del trabajo, ha consistido en analizar qué mejoras aporta la musicoterapia al desarrollo de la comunicación en los alumnos con Trastorno del Espectro Autista dentro de las Aulas Abiertas de los CEIPs de Castilla-La Mancha y la Comunidad Autónoma de Madrid. Para ello, se ha realizado una amplia revisión documental de fuentes de referencia y se ha entrevistado a los docentes responsables de las Aulas Abiertas Especializadas que utilizan actividades de musicoterapia como recurso en el aula. Se concluye el artículo manifestando, en primer lugar, la escasa integración de la musicoterapia en las aulas TEA (menos del 20% de los centros). En aquellas aulas que sí se programa con actividades de musicoterapia, los beneficios que ésta aporta se ven reflejados en un incremento claro de la intención comunicativa en los alumnos. Además, a la hora de planificar las actividades se tiene muy en cuenta conocer las preferencias y la historia musical del niño. No obstante, existen factores que impiden el aprovechamiento total de las posibilidades terapéuticas de la musicoterapia debido, especialmente a: a) una escasa formación del profesorado y b) un espacio inadecuado para poner en práctica una sesión de musicoterapia.

La transmission mère-enfant (TME) du virus de l’hépatite C (VHC) est la première cause d’acquisition de l’infection chez les enfants des pays développés. Celle-ci prend place dans <10% des cas. Toutefois, dans le cas d’une coinfection maternelle avec le virus de l’immunodéficience de type 1 (VIH-1), ce taux est accru alors qu’il n’existe aucune intervention préventive de la TME du VHC. Le VHC arbore une diversité importante qui est le résultat d’une réplication exempte de mécanisme de correction. Il est donc retrouvé chez son hôte sous la forme d’un spectre de virions génétiquement apparentés mais différents qu’on appelle quasiespèce. Lorsque le VHC est transmis entre adultes, seulement un nombre limité de variantes sont responsables de l’infection, c’est ce qu’on appelle un goulot d’étranglement génétique. L’existence d’un tel profil de transmission lors de la TME du VHC restait, jusqu’à maintenant, à confirmer. En se basant sur la détection par RT-PCR de la virémie à la naissance, la TME du VHC est réputée prendre place in utero et peripartum, une dynamique de transmission qui reste à démontrer. Ici, nous rapportons une analyse longitudinale de la TME du VHC par séquençage de nouvelle génération chez 5 paires mère-enfant dont 3 mères sont également coinfectées avec le VIH-1. L’analyse de l’identité des variantes virales basée sur la séquence nucléotidique des régions hypervariables 1-2 de la glycoprotéine E2 (positions 1491-1787 de l’isolat H77) révèle qu’un nombre limité de variantes virales sont transmises de la mère à l’enfant lorsque la mère est seulement infectée par le VHC (n = 1-4 variantes transmises). Dans le cas de la coinfection maternelle avec le VIH-1, ce nombre est toutefois drastiquement plus important (n = 111-118). La détection de variantes retrouvées chez la mère au deuxième trimestre et l’enfant mais non détectées subséquemment chez la mère témoigne que la TME du VHC peut prendre place aussi tôt que lors du deuxième trimestre de grossesse. Finalement, nous montrons que la dynamique d’infection chez l’enfant implique une augmentation transitoire de la virémie concomitante avec une perte de diversité de la quasiespèce. Dans l’ensemble ces résultats sont les premiers à démontrer directement l’existence d’un goulot d’étranglement lors de la TME du VHC. Celui-ci serait moins restringent dans le cas de la coinfection maternelle avec le VIH-1. Cette transmission peut prendre place aussi tôt que lors du deuxième trimestre de grossesse et il semblerait qu’un spectre limité de variantes soit responsable pour l’établissement de l’essentiel de la production virale chez le jeune enfant.

Objective: This study offers a description of the kinds of linguistic aids offered by adults while giving explanations to young children about different issues. During the midst of these conversations children have several opportunities to learn, not only from objects and events, but also from the utterances used to refer to them, learning how realities are named after, and in many cases, how to participate in them. Methodology: Through the analysis of eleven minutes of interaction between young child and his maternal grandparents. Results: Adults offers young child different discursive ways for helping him to add information, organize and clarify the topic that he is talking about. Conclusions: The results allow to identify different ways that the adults use for helping young child to improve his explanations.

Antecedentes El dolor en neonatos ha sido un problema poco explorado. Se ha propuesto el uso de las terapias no farmacológicas para su tratamiento, sin embargo existen pocas aproximaciones sistemáticas para la evaluación de su eficacia. Objetivos Determinar la eficacia de las terapias no farmacológicas en el manejo del dolor en neonatos pretérmino a través de una revisión sistemática. Metodología Se realizó una revisión sistemática de la literatura para evaluar la eficacia de las terapias no farmacológicas en el manejo del dolor en el recién nacido petérmino. La búsqueda se realizó a través de las bases de datos Embase, Cochrane, Bireme y Embase. Se identificaron estudios publicados inglés y español. Se realizó un análisis cualitativo y cuantitativo. Resultados Se incluyeron 10 ensayos clínicos. La solución de sacarosa administrada por vía oral mostró reducir la intensidad del dolor en el recién nacido. La intubación y toma de muestras facilitada por el cuidador mostró también reducir la intensidad del dolor. Conclusión Se recomienda la administración solución de sacarosa y acompañamiento del cuidador durante los procedimientos como medidas para reducir el dolor en el recién nacido pretérmino.

O presente estudo propôs-se estudar a temática da aprendizagem de um aluno com deficiência intelectual e desenvolvimental em contexto de sala de aula e as implicações no desenvolvimento de processo educativo. Foi realizado com um jovem que atingiu este ano a maioridade e que esteve a frequentar o 9º ano de escolaridade. Este jovem apresenta características que o definem como portador de Deficiência Intelectual e Desenvolvimental (DID). Realizou-se a análise documental através das informações que constam do processo educativo do jovem, das entrevistas efectuadas aos professores que o acompanharam no seu percurso escolar e dos registos de observação de aulas. Procedeu-se à discussão dos resultados, onde se aferiu que as escolas regulares apresentam, actualmente condições para responder às necessidades académicas, pessoais e sociais destes jovens, contudo o processo de aprendizagem destes alunos deveria passar por uma preparação para a vida ativa através do estabelecimento de protocolos com Entidades formadoras, que visam formação a nível técnico e que proporcionam a inclusão em mercado laboral. É imperioso compreender que o processo de aprendizagem destes jovens com Necessidades Edicativas Especiais (NEE) implica uma multiplicidade de aspetos, sendo o direito à igualdade de oportunidades primordial, considerando que todos têm direito a ser diferentes quando a igualdade os descaracteriza.
