OBJECTIVE: To assess the association between cardiovascular risk factors and acute myocardial infarction as the first manifestation of ischemic heart disease, correlating them with coronary angiographic findings. METHODS: We carried out a cross-sectional study of 104 patients with previous acute myocardial infarction, who were divided into 2 groups according to the presence or absence of angina prior to acute myocardial infarction. We assessed the presence of angina preceding acute myocardial infarction and risk factors, such as age >55 years, male sex, smoking, systemic arterial hypertension, lipid profile, diabetes mellitus, obesity, sedentary lifestyle, and familial history of ischemic heart disease. On coronary angiography, the severity of coronary heart disease and presence of left ventricular hypertrophy were assessed. RESULTS: Of the 104 patients studied, 72.1% were males, 90.4% were white, 73.1% were older than 55 years, and 53.8% were hypertensive. Acute myocardial infarction was the first manifestation of ischemic heart disease in 49% of the patients. The associated risk factors were systemic arterial hypertension (RR=0.19; 95% CI=0.06-0.59; P=0.04) and left ventricular hypertrophy (RR=0.27; 95% CI=0,.8-0.88; P=0.03). The remaining risk factors were not statistically significant. CONCLUSION: Prevalence of acute myocardial infarction as the first manifestation of ischemic heart disease is high, approximately 50%. Hypertensive individuals more frequently have symptoms preceding acute myocardial infarction, probably due to ventricular hypertrophy associated with high blood pressure levels.

La thèse porte sur la relation entre l’environnement familial et le comportement sexuel des adolescents vivant dans un contexte de socialisation. L’environnement familial est mesuré par le type de famille de résidence, le type d’union du chef de ménage, la survie des parents, la cohabitation avec les grands-parents et le nombre de personnes de moins de 20 ans par adulte dans le ménage. Le comportement sexuel est opérationnalisé par l’entrée en sexualité prémaritale, l’utilisation du condom au premier rapport sexuel, le nombre de partenaires sexuels, le recours au partenaire sexuel occasionnel et l’utilisation systématique du condom au cours des douze derniers mois. Les données proviennent d’enquêtes transversales. Elles ont servi à établir des associations entre les indicateurs des deux concepts. Les résultats sont présentés dans trois articles (Chapitres III à V). Une analyse descriptive de l’environnement familial au Burkina Faso montre qu’une majorité des enfants de 0 à 14 ans (78,4% en 1993 et 77,6% en 2003) et des adolescents de 12 à 19 ans (61,1% en 2004) vit auprès des deux parents, en union monogame ou polygame. Cependant, certains enfants et adolescents résident avec les parents dans des ménages dirigés par d’autres personnes. Le décès des parents (7,7% en 1993 et 7,3% en 2003 pour les enfants; 16,5% en 2004 pour les adolescents), la pratique du confiage (10,4% en 1993 et 8,9% en 2003 pour les enfants; 26,9% en 2004 pour les adolescents) et la monoparentalité (11,2% en 1993 et 13,6% en 2003 pour les enfants; 12% en 2004 pour les adolescents) affectent amplement l’environnement familial. C’est dans l’adolescence que les individus sont plus nombreux à être privés de la présence des deux parents. Il existe une association statistique significative entre l’environnement familial et le comportement sexuel des adolescents du Burkina Faso. Cette relation varie en fonction de l’indicateur de comportement à l’étude et du sexe des adolescents ciblés. Par exemple, l’absence des deux parents dans le ménage n’est pas systématiquement associée au comportement sexuel à risque. S’agissant des indicateurs de comportement sexuel, l’âge au premier rapport sexuel est associé significativement aux autres indicateurs des garçons et des filles. Une entrée précoce en sexualité (avant 14 ans) est associée à une plus grande probabilité d’adoption de comportement sexuel à risque. Toutefois, elle est moins susceptible d’être associée au recours à plusieurs partenaires sexuels. Les résultats ont conduit à des recommandations pour les politiques et les programmes de santé sexuelle et reproductive. Au Burkina Faso, la priorité des futures actions devrait viser la sensibilisation des parents, des tuteurs et des adultes du ménage pour l’éducation, le soutien et le suivi de tous les adolescents, sans exception de sexe et de statut dans la sexualité. Les institutions extrafamiliales, comme l’école, devraient être mises à contribution pour appuyer l’effort des membres de la famille. Une stratégie combinée de promotion du report de l’entrée en sexualité et de l’éducation sexuelle pourrait être un moyen plus sûr et plus sécuritaire pour protéger la santé dans l’adolescence.

The aim of this study was to determine whether any differences in the GH-IGF-I axis in juvenile calves were predictive of fertility problems as adult cows. Endogenous metabolic hormone profiles before and after feeding and the response to a GH-releasing factor (GRF) challenge were measured in prepubertal (6 month) dairy calves. These metabolic parameters were subsequently related to physical characteristics at puberty and to ovarian function during the first lactation. Milk progesterone analysis was used to categorize the animals into those with normal progesterone profiles following calving (n = 17) and those that developed delayed ovulation (DOV1, n = 9) or persistent corpus luteum (PCL1, n = 6) profiles. There were associations between prepubertal GH parameters, glucose and non-esterified fatty acid (NEFA) concentrations and the body condition score at which the animals attained puberty. The calves which subsequently developed DOV1 profiles as cows tended to have a higher GH pulse amplitude during fasting than normal profile animals, they did not show the anticipated decrease in circulating glucose concentrations following a post-prandial rise in insulin and they also had the lowest IGF-I concentrations. The calves that later developed PCL1 had a significantly larger GH pulse amplitude and pulse area than normal profile animals in the fed period and had the highest IGF-I concentrations. There were no differences in prepubertal insulin or NEFA concentrations or in the GH response to a GRF challenge between the different progesterone profile categories. Plasma IGF-I concentrations in prepubertal animals were positively correlated with their post-calving concentrations, whereas glucose concentrations had a negative correlation between these time-periods. These results suggested that the different juvenile endocrine profiles of the DOV1 cows may predispose them to a higher rate of tissue mobilization during lactation and a consequent reduction in fertility, while altered GH and IGF-I levels in PCL1 cows may later contribute to the maintenance of the persistent corpus luteum. Therefore metabolic differences in prepubertal calves were later reflected by altered reproductive function during the first lactation.

In the year 2007 a General Observation Period (GOP) has been performed within the German Priority Program on Quantitative Precipitation Forecasting (PQP). By optimizing the use of existing instrumentation a large data set of in-situ and remote sensing instruments with special focus on water cycle variables was gathered over the full year cycle. The area of interest covered central Europe with increasing focus towards the Black Forest where the Convective and Orographically-induced Precipitation Study (COPS) took place from June to August 2007. Thus the GOP includes a variety of precipitation systems in order to relate the COPS results to a larger spatial scale. For a timely use of the data, forecasts of the numerical weather prediction models COSMO-EU and COSMO-DE of the German Meteorological Service were tailored to match the observations and perform model evaluation in a near real-time environment. The ultimate goal is to identify and distinguish between different kinds of model deficits and to improve process understanding.

Objective Genetic sonography following first-trimester combined screening appears to increase substantially detection rates for Down syndrome but it relies on the unproved assumption of independence between these tests. In this study we aimed to investigate the relationship between first-trimester nuchal translucency (NT) and a series of second-trimester soft markers and structural defects in unaffected pregnancies. Methods NT measurement in the first trimester was followed by second-trimester scan (18 to 23 + 6 weeks) including examination for three categorical markers (intracardiac echogenic foci, hyperechogenic bowel and structural defects) and measurement of nasal bone length, nuchal-fold thickness, femur length, humerus length, renal pelvis diameter and prenasal thickness. All continuous variables were expressed in multiples of the median (MoM) for gestation and correlation coefficients between log-transformed NT and second-trimester variables were calculated. In addition, frequencies of soft markers and structural defects in cases with increased NT were compared to those with normal NT, using MoM cut-offs. Results In a dataset of 1970 cases, NT was significantly correlated (P < 0.05) with all second-trimester continuous variables, the correlation being strongest for nuchal-fold thickness (r = 0.10). There was a higher frequency of cases with second-trimester nuchal-fold thickness above the 97.5th centile (10.7 vs. 2.2%) and hyperechogenic bowel (2.4 vs. 0.1%) in cases with increased NT. Conclusions Straightforward reassessment of risk using likelihood ratios derived from the second-trimester genetic sonogram might lead to inaccurate estimates. Multivariate models using continuous second-trimester variables might be preferable in sequential screening strategies. Copyright. 2012 ISUOG. Published by John Wiley & Sons, Ltd.

Drive for thinness (DT) and social body comparison (SBC) have been highly correlated with body dissatisfaction, a robust risk factor for eating disorders; however, there is little understanding of how these two variables relate to increases in body dissatisfaction over time. In the present study, I investigated how high initial levels of DT and SBC correlate with changes in body dissatisfaction and ideal body by surveying 110 first-year women at the beginning and end of their first semester. There was no significant relationship between high initial DT and SBC and changes in either body dissatisfaction or ideal body. However, high initial SBC was almost significantly correlated with change in ideal body due to women with low SBC choosing larger bodies at follow-up. In addition, women with high initial DT and SBC had higher body dissatisfaction than women with low initial levels of both variables. Women with high initial SBC chose thinner ideal bodies than women with low initial SBC. Lastly, change in body dissatisfaction was negatively correlated with change in ideal body. If replicated, I would hope these findings could contribute to a better understanding of how women’s perception of their bodies changes over the course of their first semester in college and inform interventions to address this potential risk factor for disordered eating.

The aim of this study was to assess the way volleyball teams score with regard to: whether or not they won the game, whether they were the home or away team, the level of the opposing teams, and the type of confrontation. The sample was composed of 118,083 plays from 794 men’s volleyball matches and 125,751 plays from 719 women’s matches of Spain’s first division clubs (from the 2002-2003 season to the 2006-2007 season). The variables studied were: the way points were obtained in each play, being the home or away team, the level of the teams, the result of the match, and the type of confrontation between the teams with regard to their level. The results demonstrate that for both men’s and women’s teams, the majority of the points were obtained in attack and by opponent errors. Differences were found with regard to the way points were obtained when winning or losing the match was taken into account as well as when considering the level of the teams. This paper discusses the differences found with regard to whether the team is home or visiting and the type of confrontation.
