Considerada como uma das mais importantes doenças crónicas na infância, a diabetes mellitus tipo 1 foi a base para a realização deste estudo de caso. A fim de conhecer e compreender as variações das concentrações glicémicas numa criança desportista com diabetes tipo 1, ao longo de 6 meses, foram efetuados registos diários das suas concentrações de glicémia em 5 diferentes momentos do dia (antes das refeições e durante o sono); registo dos momentos de prática de exercício físico; entrevista à enfermeira responsável pelo acompanhamento do referido indivíduo; conversas com os progenitores e uma recolha bibliográfica que esclarecesse toda a dinâmica e rotinas de crianças com diabetes mellitus tipo 1, bem como o seu efeito na prática desportiva, tentando perceber os benefícios que poderá induzir perante esta condição de saúde. Através da associação entre os resultados obtidos e a revisão literária, foi possível perceber a causa de ocorrência de híper e hipoglicémias. Ademais, foi possível inferir que a prática desportiva em crianças com diabetes mellitus tipo 1 é, na grande maioria dos casos, benéfica. Concluindo, o desporto proporciona, para além dos efeitos já conhecidos, uma diminuição na insulina necessária para um adequando equilíbrio glicémico e adaptações fisiológicas que melhoram a condição de saúde.

This paper presents a study of families of children with type 1 diabetes mellitus, emphasizing the identification of social supports and networks to strengthen interventions aimed at health promotion. The approach selected was a qualitative research, using a case study design. Four families of children with diabetes type 1 were studied, totalling seven participants. Data were collected between April and June 2007, through in-depth interviews and the construction of a genogram and an ecomap. The results presented the families` characterization and testimonies grouped in the following categories: social support, social networks and family roles. To promote care in practice, there is a need to identify the characteristics of each family and resources available that provide better living conditions. We concluded that identifying supports and social networks allows for more personalized care delivery to each family with a view to health promotion.

Objective: The present study evaluated the relationship between periodontal disease and its clinical variables in Brazilian non-diabetic pregnant women (C), gestational diabetes mellitus (GDM), or type 1 diabetes mellitus (T1DM). Subjects and methods: A periodontal exam was performed in one hundred and sixty-one pregnant women (GDM:80; T1DM:31; C:50) by a single-blinded calibrated examiner who recorded plaque index (PI), gingival index (GI), bleeding index (BI), gingival margin location (GM), probing depth (PD), clinical attachment level (CAL), bleeding on probing (BOP), and tooth mobility index (MI). The medical variables were age, pregestational body mass index (pre-BMI), fasting plasma glucose (FPG), and glycated hemoglobin (HbA(1c)). Results: The GI, GM, PD, CAL, BOP, and MI were significantly higher (P < 0.01) among GDM and T1DM than for C. The PI was higher in GDM and similar between C and T1DM. The Adjusted Final Model for medical variables to evaluate the effects of groups on periodontal parameters confirmed these results. Conclusions: The presence of periodontal disease was significantly higher in Brazilian diabetic pregnancies (GDM and T1DM) when compared to non-diabetic pregnant women (C). The degree of periodontal disease was similar between the GDM and T1DM groups. Age, pregestational BMI, and HbA(1c) were factors related to CAL development in these two types of diabetes mellitus.

To establish the incidence of type 1 diabetes among children (infants to 14 years of age) in the city of Passo Fundo, Rio Grande do Sul, Brazil (population under 15 years = 50,098), during the period of January to December 1996, a retrospective and prospective population-based registry was established, using physician reports of newly diagnosed patients under 15 years of age with type 1 diabetes as the primary source of case identification. Primary and nursery schools and a general call through the media (newspapers, radio and television) was the secondary source. Data were calculated according to the methods recommended by the WHO (1990). Six new cases were identified. Case ascertainment was estimated at 100%. The incidence of type 1 diabetes in the year 1996 was 12/100,000 inhabitants. These data indicate that the incidence of childhood type 1 diabetes in a subtropical region in the Southern part of Brazil was similar to that observed in developed countries throughout the world. The inability to demonstrate the North-South gradient is probably due to the European origin of inhabitants of the city.

Syfte: Syftet med denna studie var att undersöka hur patienter upplevde patient-undervisningen samt hur sjuksköterskans roll såg ut i patientundervisningen. Metod: Uppsatsen har genomförts som en systematisk litteraturstudie. Artiklar har sökts i databaserna ELIN samt Cinahl. Artiklar publicerade mellan 1999-2009 och som var tillgängliga i fulltext inkluderades i resultatdelen som omfattade 14 kvalitativa studier samt en litteraturstudie. Resultat: I resultatet framgick att patienterna upplevde att gruppundervisning var ett bra alternativ till enskild undervisning då de kunde ta del av andras erfarenheter och på det sättet få mer kunskap om sjukdomen. Dessutom upplevde de att det var givande att träffa personer med samma sjukdom. I studien framgick att enskild undervisning var önskvärt från både patienter och sjuksköterskor då det kunde upplevas lättare att ta upp de frågor de hade där. Sjuksköterskan kunde vara ett stort stöd för patienten. Förutom att förmedla sina kunskaper och undervisa patienten var lyssna, uppmuntra och stötta något som sjuksköterskorna ansåg vara viktigt i mötet med patienten. Något som togs upp av patienter och sjuksköterskorna var att de ansåg att det vore bra om patienterna under besöken träffade samma personer då det skulle underlätta uppföljningarna.

Syfte: att kartlägga hemtjänstpersonalens kunskaper om diabetes och diabetesrelaterade läkemedel, agerandet i vissa specifikt beskrivna sjukdomssituationer samt kunskaper om för verksamheten gällande delegeringsrutiner.Metod: Studien genomfördes med en strukturerad enkät som innehöll frågor om diabetes, diabetesrelaterade läkemedel, hemtjänstpersonalens handlande vid vissa specifikt beskrivna sjukdomsfall samt frågor om för verksamheten gällande delegeringsrutiner. Totalt delades 120 enkäter ut till hemtjänstpersonal i tre olika kommuner. En svarsfrekvens på 70% erhölls. Resultat: De största bristerna i hemtjänstpersonalens kunskaper handlade om läkemedel och insulinpreparat. Personalen hade inte heller helt klart för sig skillnader mellan typ 1 och typ 2 diabetes. De flesta visste hur ett normalt blodsockervärde bör ligga men många visste inte i vilka situationer som ett blodsockervärde var angeläget att kontrollera. De flesta kunde identifiera tecken på lågt blodsocker och visste delvis hur de skulle agera i den aktuella situationen. Det var svårare för personalen att känna igen högt blodsocker där endast en tredjedel kände igen symtomen. Positivt var ändå att fler än de som kände igen symtomen (51 %) handlade korrekt i situationen och kontaktade sjukvård. Delegeringsförfarandet visade sig vara olika. De flesta svarade att de fick teoretisk genomgång inför varje beviljad delegering. Ungefär 46 % ansåg sig ha tillräckliga kunskaper för att ge insulin till vårdtagare.

Denna litteraturstudies syfte var att identifiera och evidensgradera de omvårdnadsåtgärder med vilka sjuksköterskan kan motivera patienter med diabetes typ-2 till egenvård. De 27 utvalda artiklarna, som svarade mot uppsatsens syfte, söktes från databaserna Cinahl, Elin@dalarna, Medline och PubMed. För att kunna bedöma utsedda kvantitativa och kvalitativa artiklars vetenskapliga kvalitet granskades artiklarna utifrån modifierade granskningsmallar efter förlagor av Forsberg och Wengström (2008). Kvalitetsbedömningen gav vid handen att av 27 artiklar, var 3 av hög vetenskaplig kvalitet och 24 artiklar var av medelgod kvalitet men ingen artikel bedömdes vara av låg kvalitet. På basen av artikelkvaliteten evidensgranskades de funna omvårdnadsåtgärderna i enlighet med Bahtsevanis (2008) rekommendationer. I litteraturstudien identifierades fyra huvudområden i omvårdnadsåtgärder inom diabetesvården: Livsstilsförändringar, Utbildning, Egenvård samt Personalens betydelse. Omvårdnadsåtgärderna Livsstilsförändringar, Utbildning och Egenvård vilade på måttligt vetenskapligt underlag och omvårdnadsåtgärden Personalens betydelse vilade på stark vetenskaplig grund. Resultaten diskuterades utgående från omvårdnadsteoretiker Orems humanistiska modell, som inbegriper egenvårdsbrist, som är en beskrivning av och förklaring till varför en person behöver omvårdnad, vilken sker när egenvårdskapaciteten är mindre än personens egenvårdskrav. Modellen bygger på antaganden om och generella kännetecken på till exempel omvårdnadssituationer och inkluderar begreppen omvårdnadskapacitet, hjälpmetoder och olika omvårdnadssystem.

Syfte: Syftet med litteraturstudien var att söka evidens för metoder som kan stödja patienter med typ-2 diabetes till livsstilsförändringar. Metod: En litteraturstudie, som innehåller 16 kvantitativa och kvalitativa vetenskapliga artiklar, som var publicerade mellan 2000-2010. Databasen som användes var Cinahl och sökmotorn Elin@Dalarna. Sökorden som använts var: diabetes type 2, type 2 diabetes, lifestyle changes, patient education, empowerment, health behaviour och self-management. Resultat: I resultaten kom uppsatsförfattarna fram till olika kategorier som visar olika åtgärder till livsstilsförändringar. Studierna baserades på olika åtgärder såsom; datorintervention, utbildningar, telefonsamtal, motiverande samtal, transteoretiska modellen och sjuksköterskespecialister. Empowerment var enligt uppsatsförfattarna det övergripande temat som de flesta studier baserade åtgärderna på.

Worldwide, the diabetes mellitus is considered a serious problem of public health; it also involves high costs for its treatment and its complications. Even though the onset of diabetes mellitus type 1 is on 5% to 10% of the diabetic population, it is the most aggressive type of anifestation; furthermore, it is the commonest chronic disease in childhood and adolescence. Such data show the importance of understanding the development of this disease from the moment the individual perceives it, according to their experience, observing the complexity of the phenomenon thoroughly. This study aimed to comprehend the experience of the adolescent with onset of diabetes mellitus type 1 in order to learn how these young individuals perceive and reason their place in the world when they experience this illness. The methodological strategy was based on qualitative research of phenomenological inspiration. We applied the main ideas of heideggerian ontology and some concepts of existential phenomenological psychology to guide our reflexive path. The participants were 10 young individuals (06 female and 04 male), ages between 15 and 18, under treatment in the city of Recife, Pernambuco. The methodological resource was the narrative, which allowed us access to the adolescents experience. The comprehension of the reports was based on the heideggerian hermeneutics, whose priority is the interpretation of the meaning expressed by the participants speech. The results showed that factors such as social, family, economic and cultural context greatly influence the perception and the way the individuals deal with, perceive and reason their experience. We noticed that the diabetes type 1 may often represent a limitation, but it does not make it impossible to be part of the world with others. The data show a significant affective ambivalence regarding the content more directly related to the disease. This demonstrates the unique way which each one gives meaning and reasons their condition of existence. This research points out the need to understand the complexity of this phenomenon in a more complete manner, considering the adolescent in their timely and historical context. We believe we can encourage thinking that may become actions which are more fit to the reality researched regarding all the parties involved with the subject, such as health professionals, adolescents, family, support programs and public health policies

A 19-year-old female with type 1 diabetes for four years, and a 73-year-old female with type 2 diabetes for twenty years developed sudden-onset nephrotic syndrome. Examination by light microscopy, immunofluorescence, and electron microscopy (in one case) identified minimal change disease (MCD) in both cases. There was a potential causative drug (meloxicam) for the 73-year-old patient. Both patients were treated with prednisone and responded with complete remission. The patient with type 1 diabetes showed complete remission without relapse, and the patient with type 2 diabetes had two relapses; complete remission was sustained after associated treatment with cyclophosphamide and prednisone. Both patients had two years of follow-up evaluation after remission. We discuss the outcomes of both patients and emphasize the role of kidney biopsy in diabetic patients with an atypical proteinuric clinical course, because patients with MCD clearly respond to corticotherapy alone or in conjunction with other immunosuppressive agents. Der Free Fatty Acid Receptor 1 (FFAR1) ist ein G-Protein gekoppelter Rezeptor, welcher neben einer hohen Expression im Gehirn auch eine verstärkte Expressionsrate auf den β-Zellen des Pankreas aufweist. Diese Expressionsmuster machen ihn zu einem idealen Target für die Visualisierung der sogenannten β-Zell-Masse mittels molekularer bildgebender Verfahren wie der PET. Eine Entwicklung geeigneter Radiotracer für die β-Zell-Bildgebung würde sowohl für die Diagnostik als auch für die Therapie von Typ-1- und Typ-2-Diabetes ein wertvolles Hilfsmittel darstellen.rnAufbauend auf einem von Sasaki et al. publiziertem Agonisten mit einem vielversprechendem EC50-Wert von 5,7 nM wurden dieser Agonist und zwei weitere darauf basierende 19F-substituierte Moleküle als Referenzverbindungen synthetisiert (DZ 1-3). Für die 18F-Markierung der Moleküle DZ 2 und DZ 3 wurden die entsprechenden Markierungsvorläufer (MV 1-3) synthetisiert und anschließend die Reaktionsparameter hinsichtlich Temperatur, Lösungsmittel, Basensystem und Reaktionszeit für die nukleophile n.c.a. 18F-Fluorierung optimiert. Die abschließende Entschützung zum fertigen Radiotracer wurde mit NaOH-Lösung durchgeführt und die Tracer injektionsfertig in isotonischer NaCl-Lösung mit radiochemischen Ausbeuten von 26,9 % ([18F]DZ 2) und 39 % ([18F]DZ 3) erhalten.rnZusätzlich wurde ein Chelator zur 68Ga-Markierung an den Liganden gekoppelt (Verb. 46) und die Markierungsparameter optimiert. Nach erfolgter Markierung mit 95 % radiochemischer Ausbeute, wurde der Tracer abgetrennt und in vitro Stabilitätsstudien durchgeführt. Diese zeigten eine Stabilität von mehr als 90 % über 120 min in sowohl humanem Serum (37 °C) als auch isotonischer NaCl-Lösung.rnMit einem ebenfalls synthetisierten fluoreszenzmarkierten Derivat des Liganden (Verb. 43) wurden erste LSM-Bilder an sowohl Langerhansschen Inseln als auch FFAR1-tragenden RIN-M Zellen durchgeführt, welche einen vielversprechenden Uptake des neuen Liganden in die Zellen zeigen. Weitere Untersuchungen und biologische Evaluierungen stehen noch aus. Mit den Referenzsubstanzen wurden zusätzlich Vitalitätsstudien an Langerhansschen Inseln durchgeführt, um einen negativen toxischen Einfluss auszuschließen.rn

La diabetes comprende un conjunto de enfermedades metabólicas que se caracterizan por concentraciones de glucosa en sangre anormalmente altas. En el caso de la diabetes tipo 1 (T1D, por sus siglas en inglés), esta situación es debida a una ausencia total de secreción endógena de insulina, lo que impide a la mayoría de tejidos usar la glucosa. En tales circunstancias, se hace necesario el suministro exógeno de insulina para preservar la vida del paciente; no obstante, siempre con la precaución de evitar caídas agudas de la glucemia por debajo de los niveles recomendados de seguridad. Además de la administración de insulina, las ingestas y la actividad física son factores fundamentales que influyen en la homeostasis de la glucosa. En consecuencia, una gestión apropiada de la T1D debería incorporar estos dos fenómenos fisiológicos, en base a una identificación y un modelado apropiado de los mismos y de sus sorrespondientes efectos en el balance glucosa-insulina. En particular, los sistemas de páncreas artificial –ideados para llevar a cabo un control automático de los niveles de glucemia del paciente– podrían beneficiarse de la integración de esta clase de información. La primera parte de esta tesis doctoral cubre la caracterización del efecto agudo de la actividad física en los perfiles de glucosa. Con este objetivo se ha llevado a cabo una revisión sistemática de la literatura y meta-análisis que determinen las respuestas ante varias modalidades de ejercicio para pacientes con T1D, abordando esta caracterización mediante unas magnitudes que cuantifican las tasas de cambio en la glucemia a lo largo del tiempo. Por otro lado, una identificación fiable de los periodos con actividad física es un requisito imprescindible para poder proveer de esa información a los sistemas de páncreas artificial en condiciones libres y ambulatorias. Por esta razón, la segunda parte de esta tesis está enfocada a la propuesta y evaluación de un sistema automático diseñado para reconocer periodos de actividad física, clasificando su nivel de intensidad (ligera, moderada o vigorosa); así como, en el caso de periodos vigorosos, identificando también la modalidad de ejercicio (aeróbica, mixta o de fuerza). En este sentido, ambos aspectos tienen una influencia específica en el mecanismo metabólico que suministra la energía para llevar a cabo el ejercicio y, por tanto, en las respuestas glucémicas en T1D. En este trabajo se aplican varias combinaciones de técnicas de aprendizaje máquina y reconocimiento de patrones sobre la fusión multimodal de señales de acelerometría y ritmo cardíaco, las cuales describen tanto aspectos mecánicos del movimiento como la respuesta fisiológica del sistema cardiovascular ante el ejercicio. Después del reconocimiento de patrones se incorpora también un módulo de filtrado temporal para sacar partido a la considerable coherencia temporal presente en los datos, una redundancia que se origina en el hecho de que en la práctica, las tendencias en cuanto a actividad física suelen mantenerse estables a lo largo de cierto tiempo, sin fluctuaciones rápidas y repetitivas. El tercer bloque de esta tesis doctoral aborda el tema de las ingestas en el ámbito de la T1D. En concreto, se propone una serie de modelos compartimentales y se evalúan éstos en función de su capacidad para describir matemáticamente el efecto remoto de las concetraciones plasmáticas de insulina exógena sobre las tasas de eleiminación de la glucosa atribuible a la ingesta; un aspecto hasta ahora no incorporado en los principales modelos de paciente para T1D existentes en la literatura. Los datos aquí utilizados se obtuvieron gracias a un experimento realizado por el Institute of Metabolic Science (Universidad de Cambridge, Reino Unido) con 16 pacientes jóvenes. En el experimento, de tipo ‘clamp’ con objetivo variable, se replicaron los perfiles individuales de glucosa, según lo observado durante una visita preliminar tras la ingesta de una cena con o bien alta carga glucémica, o bien baja. Los seis modelos mecanísticos evaluados constaban de: a) submodelos de doble compartimento para las masas de trazadores de glucosa, b) un submodelo de único compartimento para reflejar el efecto remoto de la insulina, c) dos tipos de activación de este mismo efecto remoto (bien lineal, bien con un punto de corte), y d) diversas condiciones iniciales. ABSTRACT Diabetes encompasses a series of metabolic diseases characterized by abnormally high blood glucose concentrations. In the case of type 1 diabetes (T1D), this situation is caused by a total absence of endogenous insulin secretion, which impedes the use of glucose by most tissues. In these circumstances, exogenous insulin supplies are necessary to maintain patient’s life; although caution is always needed to avoid acute decays in glycaemia below safe levels. In addition to insulin administrations, meal intakes and physical activity are fundamental factors influencing glucose homoeostasis. Consequently, a successful management of T1D should incorporate these two physiological phenomena, based on an appropriate identification and modelling of these events and their corresponding effect on the glucose-insulin balance. In particular, artificial pancreas systems –designed to perform an automated control of patient’s glycaemia levels– may benefit from the integration of this type of information. The first part of this PhD thesis covers the characterization of the acute effect of physical activity on glucose profiles. With this aim, a systematic review of literature and metaanalyses are conduced to determine responses to various exercise modalities in patients with T1D, assessed via rates-of-change magnitudes to quantify temporal variations in glycaemia. On the other hand, a reliable identification of physical activity periods is an essential prerequisite to feed artificial pancreas systems with information concerning exercise in ambulatory, free-living conditions. For this reason, the second part of this thesis focuses on the proposal and evaluation of an automatic system devised to recognize physical activity, classifying its intensity level (light, moderate or vigorous) and for vigorous periods, identifying also its exercise modality (aerobic, mixed or resistance); since both aspects have a distinctive influence on the predominant metabolic pathway involved in fuelling exercise, and therefore, in the glycaemic responses in T1D. Various combinations of machine learning and pattern recognition techniques are applied on the fusion of multi-modal signal sources, namely: accelerometry and heart rate measurements, which describe both mechanical aspects of movement and the physiological response of the cardiovascular system to exercise. An additional temporal filtering module is incorporated after recognition in order to exploit the considerable temporal coherence (i.e. redundancy) present in data, which stems from the fact that in practice, physical activity trends are often maintained stable along time, instead of fluctuating rapid and repeatedly. The third block of this PhD thesis addresses meal intakes in the context of T1D. In particular, a number of compartmental models are proposed and compared in terms of their ability to describe mathematically the remote effect of exogenous plasma insulin concentrations on the disposal rates of meal-attributable glucose, an aspect which had not yet been incorporated to the prevailing T1D patient models in literature. Data were acquired in an experiment conduced at the Institute of Metabolic Science (University of Cambridge, UK) on 16 young patients. A variable-target glucose clamp replicated their individual glucose profiles, observed during a preliminary visit after ingesting either a high glycaemic-load or a low glycaemic-load evening meal. The six mechanistic models under evaluation here comprised: a) two-compartmental submodels for glucose tracer masses, b) a single-compartmental submodel for insulin’s remote effect, c) two types of activations for this remote effect (either linear or with a ‘cut-off’ point), and d) diverse forms of initial conditions.
