Descrevem-se os quadros clínico-patológicos e laboratoriais de equinos inoculados experimentalmente com a peçonha de Caudisona durissa terrificus (Crotalus durissus terrificus na antiga nomenclatura), com a finalidade de fornecer subsídios que favoreçam a compreensão desse tipo de acidente ofídico em equinos. O veneno liofilizado foi diluído em 1ml de solução salina a 0,9% e inoculado por via subcutânea em cinco equinos, nas doses de 0,12mg/kg (um animal), 0,066mg/kg (dois animais) e 0,03mg/kg (dois animais). O veneno causou a morte do equino que recebeu a dose de 0,12mg/kg e de um dos dois que receberam a dose de 0,066mg/kg, com evolução de 27h27min e 52h29min, respectivamente. O segundo animal que recebeu a dose de 0,066mg/kg também adoeceu, mas recuperou-se após 12 dias da inoculação. A dose de 0,03mg/kg determinou quadros não fatais do envenenamento, com período de evolução que variou entre 6 e 10 dias. O quadro clínico caracterizou-se por considerável aumento de volume no local de inoculação (escápula) que se estendeu por todo o membro, apatia e cabeça baixa, alterações locomotoras evidenciadas pelo arrastar das pinças no solo, decúbito e dificuldade para levantar, redução dos reflexos auricular, palatal, do lábio superior e de ameaça, e aumento das frequências cardíaca e respiratória. Os exames laboratoriais revelaram leucocitose por neutrofilia e linfocitose em apenas dois animais. Houve aumento das enzimas creatina quinase (CK), dehidrogenase láctica (DHL) e da ureia, e também redução nos níveis séricos de cálcio, fósforo e magnésio. O tempo de tromboplastina parcial ativada (TTPA) aumentou nos equinos que morreram. Os achados de necropsia foram edema do tecido subcutâneo em todo o membro em que foi aplicado o veneno, sufusões no epicárdio dos ventrículos cardíacos esquerdo e direito, e bexiga com áreas hemorrágicas em grande parte da mucosa. Ao exame histopatológico observaram-se fígado com moderada vacuolização difusa, afetando mais a zona intermediária do lóbulo hepático, leve dilatação dos sinusoides hepáticos em algumas áreas e rim com leve dilatação dos túbulos uriníferos, principalmente no córtex.

Foram analisados 14 equídeos (13 equinos e 1 muar) com diagnóstico clínico e histológico de raiva provenientes de quatro regiões do Brasil. O curso clínico médio foi de quatro dias de evolução, incluindo incoordenação motora, paralisia dos membros pélvicos, paresia dos membros torácicos e decúbito. Os achados histopatológicos caracterizaram-se por meningoencefalite e meningomielite não supurativa com infiltrado perivascular linfoplasmocitário. Corpúsculos de Negri foram observados em 64,28% (9/14) dos casos, principalmente na medula espinhal cervical e nos neurônios de Purkinje do cerebelo. Em 55,55% (5/9) dos equídeos analisados tiveram resultado positivo pela técnica de imunofluorescência direta para raiva. Todos os casos foram positivos na imuno-histoquímica para raiva, cujas reações foram mais evidentes no córtex temporal, córtex occipital e medula espinhal cervical. A técnica de imuno-histoquímica foi essencial para confirmar todos os casos de raiva nos equídeos.

O objetivo do trabalho foi estabelecer a relação entre a ecobiometria renal com medidas de conformação corporal como a distância atlanto-coccígea (DAC) e a altura (H) de cães adultos saudáveis, obtendo-se parâmetros de normalidade para avaliar o tamanho e volume renal, bem como estabelecer valores de referência para avaliar a perfusão sanguínea dos rins por meio do índice de resistividade (IR) e do índice de pulsatilidade (IP) do ramo principal da artéria renal. No estudo foram utilizados 22 cães adultos sem raça definida, sendo 11 machos e 11 fêmeas. Os animais foram previamente aferidos quanto a DAC e a H. Os exames ultra-sonográficos foram realizados com um aparelho HDI 4000 PHILIPS munido de um transdutor microconvexo multifreqüêncial (5-8 MHz), dispositivos Doppler Colorido e Doppler de Fluxo. Os animais foram posicionados em decúbito lateral direito ou esquerdo, de acordo com o rim a ser avaliado. Os diâmetros longitudinal (DL) e dorsoventral (DDV) dos rins foram mensurados na secção longitudinal e, o diâmetro transversal (DT) foi aferido no plano transversal. O volume (V) foi calculado automaticamente pelo software do ultra-som. Com o uso do Triplex Doppler, o IR e o IP das artérias renais direita e esquerda foram obtidos. Todos os dados foram apresentados em média ± EPM. Análises de regressão linear foram realizadas tendo o DL, DDV, DT e V como variáveis dependentes e a DAC e H como variáveis independentes. Os IR e IP dos rins direito e esquerdo foram comparados pelo teste t de Student. A DAC variou de 54-78cm para machos e 37-71cm para fêmeas e a altura variou entre 34-64 cm para os machos e 24-57cm para as fêmeas. As médias obtidas para DL, DDV, DT e V dos rins esquerdo e direito foram: 5,24±0,27cm, 3,07±0,15cm, 3,07±0,9cm, 28,01±3,4mL e 4,50±0,19cm, 2,88±0,14cm, 2,71±0,15cm, 21,27±2,6mL, respectivamente. As análises de regressão linear entre as medidas lineares e volume renal com a DAC e a H foram significativas para os interceptos e coeficientes de regressão (P<0,01). Houve diferenças estatísticas quando comparado os IR e IP entre os rins direito e esquerdo (P=0,001), sendo que as médias para IR e IP dos rins esquerdo e direito foram 0,62±0,08; 1,34±0,18 e 0,70±0,06; 1,62±0,13; respectivamente. Os dados obtidos no presente trabalho podem auxiliar na avaliação do tamanho, volume e perfusão dos rins de cães adultos.

The diameters and areas of portal vein, caudal vena cava and abdominal aorta are useful measurements in dogs. These values can be easily measured by ultrasonographic exam, and variations of normality can be an important indicator of hepatic or extra-hepatic alterations. This study aimed to measure the diameter and areas of portal vein, caudal vena cava and abdominal aorta inhealthy dogs, with normal corporal score, divided in groups according to the body weight, and assess whether the data are influenced by animal weight. Thirty dogs were examined and divided into three groups (Group A: ≤ 10 kg Group B: from 10.1 to 20.0 kg; Group C: ≥ 20.1 kg). To measure thediameters and areas of portal vein, caudal vena cava and abdominal aorta, the animal was kept in left lateral decubitus position and the transducer was placed on the right lateral abdominal wall, at approximately the 10th or 11th intercostal space, in the porta hepatis region. The diameters and areas of the portal vein, caudal vena cava and abdominal aorta were significantly lower for dogs in Group A with respect to other groups and the dogs from Groups B and C had similar results with each other. The diameters and areas of the portal vein, caudal vena cava and abdominal aorta may vary with the animal size, and reference values must be specific for small, medium and large dogs. Purpose: The aims of the study were to measure endoscopically the retrolingual pharynx during wakefulness and sleep before and after maxillomandibular advancement surgery and to quantify the changes observed. Materials and Methods: Eighteen patients with mild to severe grade obstructive sleep apnea hypopnea were evaluated during wakefulness while sitting and lying down and during induced sleep in dorsal decubitus while breathing naturally. Images of the retrolingual region of the pharynx were captured with a nasofibroscope and recorded on a DVD using the Sony Vegas 8.0 software (Sony Creative Software, Madison, WI). The images captured in greater and smaller aperture were measured with the Image J software (produced by Wayne Rasband, United States National Institutes of Health, Bethesda, MD) in linear anteroposterior and linear laterolateral areas. A correction factor was then applied to equalize the size of the images and thus compare them to one another. Results: The postoperative dimensions of the pharynx always increased significantly in all measurements compared with the preoperative ones. During induced sleep in dorsal decubitus, there was a greater gain in the area of smaller aperture (201.33%). Conclusions: The proposed method showed that the dimensions of the pharynx always increased significantly after surgery for maxillomandibular advancement, although the gain was not homogeneous in all dimensions and also varied according to state of consciousness. The greatest gain was observed in the area of smaller aperture with the patient in induced sleep, thus reducing the collapse of the pharynx. (C) 2012 Elsevier Inc. All rights reserved.

Background: Percutaneous nephrolithotomy (PCNL) can be performed in the prone or in the supine position. Comparisons between the two techniques in obese patients are rare in the current literature. Methods: The records of obese patients (body mass index >30) who underwent PCNL in the prone or complete supine positions were reviewed. All patients had a noncontrast CT before and after the procedure. Stones were graded according to the Guy stone score and complications according to the Clavien grading. The stone-free rates, operative time, surgical complications, and hospital stay were analyzed. Results: A total of 56 PCNL were performed in 42 patients. Twenty-four PCNL were performed in the prone and 32 in the total supine position. Stone-free rate on the first postoperative day was 50% in the prone and 46.9% in the supine position (P = 1.0). Final stone-free rates were 83.3% and 78.1%, respectively (P = 0.74). Mean operative time was 164.6 minutes in the prone and 120.3 minutes in the supine position (P = 0.0017), and hospital stay was 4.38 and 2.68 days (P = 0.014), respectively. The transfusion rate was 20.8% in the prone and zero in the supine position patients (P = 0.01). Excluding Guy IV stones, transfusion rate was 8.3% in the prone position (P = 0.1). Significant surgical complications rate was 12.5% in the prone and 3.1% in the supine position (P = 0.302). Conclusion: PCNL performed in the prone or in the complete supine position in obese patients presents similar outcomes. The supine decubitus position has the advantages of a significantly shorter operative time and hospital stay.

FUNDAMENTO: pacientes com valvopatia mitral podem evoluir com congestão pulmonar, que aumenta o trabalho dos músculos respiratórios; essa sobrecarga pode alterar o padrão respiratório com predomínio do deslocamento torácico ou presença de movimentos paradoxais. OBJETIVO: a) estudar o padrão respiratório e movimento toracoabdominal (MTA) em pacientes com doença mitral b) estudar o efeito do posicionamento nos parâmetros respiratórios c) correlacionar hipertensão pulmonar com presença de incoordenação do MTA. MÉTODOS: o padrão respiratório e o MTA de pacientes com doença mitral foram avaliados por pletismografia respiratória por indutância, nas posições dorsal e sentada, durante dois minutos de respiração tranquila. Analisou-se volume corrente (Vc) e tempos respiratórios e as variáveis do MTA. RESULTADOS: de 65 pacientes incluídos, 10 foram retirados, 29 participaram do grupo estenose mitral e 26 do grupo insuficiência mitral. O Vc, a ventilação pulmonar e o fluxo inspiratório médio aumentaram significantemente na posição sentada, sem diferenças entre os grupos. O MTA manteve-se coordenado entre os grupos e as posições; no entanto, cinco pacientes na posição dorsal apresentaram incoordenação (três no grupo estenose mitral; dois no grupo insuficiência mitral) com correlação significante com valores de pressão de artéria pulmonar (r = 0,992, p = 0,007). CONCLUSÃO: o padrão respiratório e o MTA não apresentam diferenças entre pacientes com estenose ou insuficiência mitral. A posição sentada aumenta o Vc sem alterar os tempos respiratórios. A presença de incoordenação toracoabdominal na posição dorsal esteve associação à hipertensão pulmonar.

OBJECTIVE: Anatomic reduction and stable fixation by means of tissue- preserving surgical approaches. INDICATIONS Displaced acetabular fractures. Surgical hip dislocation approach with larger displacement of the posterior column in comparison to the anterior column, transtectal fractures, additional intraarticular fragments, marginal impaction. Stoppa approach with larger displacement of the anterior column in comparison to the posterior column. A combined approach might be necessary with difficult reduction. CONTRAINDICATIONS Fractures > 15 days (then ilioinguinal or extended iliofemoral approaches). Suprapubic catheters and abdominal problems (e.g., previous laparotomy due to visceral injuries) with Stoppa approach (then switch to classic ilioinguinal approach). SURGICAL TECHNIQUE: Surgical hip dislocation: lateral decubitus position. Straight lateral incision centered over the greater trochanter. Entering of the Gibson interval. Digastric trochanteric osteotomy with protection of the medial circumflex femoral artery. Opening of the interval between the piriformis and the gluteus minimus muscle. Z-shaped capsulotomy. Dislocation of the femoral head. Reduction and fixation of the posterior column with plate and screws. Fixation of the anterior column with a lag screw in direction of the superior pubic ramus. Stoppa approach: supine position. Incision according to Pfannenstiel. Longitudinal splitting of the anterior portion of the rectus sheet and the rectus abdominis muscle. Blunt dissection of the space of Retzius. Ligation of the corona mortis, if present. Blunt dissection of the quadrilateral plate and the anterior column. Reduction of the anterior column and fixation with a reconstruction plate. Fixation of the posterior column with lag screws. If necessary, the first window of the ilioinguinal approach can be used for reduction and fixation of the posterior column. POSTOPERATIVE MANAGEMENT: During hospital stay, intensive mobilization of the hip joint using a continuous passive motion machine with a maximum flexion of 90 degrees . No active abduction and passive adduction over the body's midline, if a surgical dislocation was performed. Maximum weight bearing 10-15 kg for 8 weeks. Then, first clinical and radiographic follow-up. Deep venous thrombosis prophylaxis for 8 weeks postoperatively. RESULTS: 17 patients with a mean follow-up of 3.2 years. Ten patients were operated via surgical hip dislocation, two patients with a Stoppa approach, and five using a combined or alternative approach. Anatomic reduction was achieved in ten of the twelve patients (83%) without primary total hip arthroplasty. Mean operation time 3.3 h for surgical hip dislocation and 4.2 h for the Stoppa approach. Complications comprised one delayed trochanteric union, one heterotopic ossification, and one loss of reduction. There were no cases of avascular necrosis. In two patients, a total hip arthroplasty was performed due to the development of secondary hip osteoarthritis.

OBJECTIVE The aim of the therapy is mechanical and functional stabilization of high dislocated hips with dysplasia coxarthrosis using total hip arthroplasty (THA). INDICATIONS Developmental dysplasia of the hip (DDH) in adults, symptomatic dysplasia coxarthrosis, high hip dislocation according to Crowe type III/IV, and symptomatic leg length inequality. CONTRAINDICATIONS Cerebrospinal dysfunction, muscular dystrophy, apparent disturbance of bone metabolism, acute or chronic infections, and immunocompromised patients. SURGICAL TECHNIQUE With the patient in a lateral decubitus position an incision is made between the anterior border of the gluteus maximus muscle and the posterior border of the gluteus medius muscle (Gibson interval). Identification of the sciatic nerve to protect the nerve from traction disorders by visual control. After performing trochanter flip osteotomy, preparation of the true actetabulum if possible. Implantation of the reinforcement ring, preparation of the femur and if necessary for mobilization, resection until the trochanter minor. Test repositioning under control of the sciatic nerve. Finally, refixation of the trochanteric crest. POSTOPERATIVE MANAGEMENT During hospital stay, intensive mobilization of the hip joint using a continuous passive motion machine with maximum flexion of 70°. No active abduction and passive adduction over the body midline. Maximum weight bearing 10-15 kg for 8 weeks, subsequently, first clinical and radiographic follow-up and deep venous thrombosis prophylaxis until full weight bearing. RESULTS From 1995 to 2012, 28 THAs of a Crow type IV high hip-dislocation were performed in our institute. Until now 14 patients have been analyzed during a follow-up of 8 years in 2012. Mid-term results showed an improvement of the postoperative clinical score (Merle d'Aubigné score) in 86 % of patients. Good to excellent results were obtained in 79 % of cases. Long-term results are not yet available. In one case an iatrogenic neuropraxia of the sciatic nerve was observed and after trauma a redislocation of the arthroplasty appeared in another case. In 2 cases an infection of the THA appeared 8 and 15 months after index surgery. No pseudoarthrosis of the trochanter or aseptic loosening was noticed.

Background. Decubitus ulcers can become complicated by pelvic osteomyelitis. Little is known about the epidemiology of pressure ulcer-related pelvic osteomyelitis. Methods. We performed a retrospective cohort study of adult patients with pressure ulcer and pelvic osteomyelitis admitted to an academic center from 2006 to 2011. Data on clinical presentation, diagnostic evaluation, and treatment during the index admission were collected. Outcome measures included length of hospital stay and number of readmissions in the subsequent year. Results. Two hundred twenty patients were included: 163 (74%) were para/quadriplegic and 148 (67%) were male (148; 67%). Mean age was 50 (±18) years. Pelvic osteomyelitis was the primary admission diagnosis for 117 (53%). Fifty-six (26%) patients had concurrent febrile urinary tract infection. Wound cultures collected for 113 patients (51%) were notable for methicillin-resistant Staphylococcus aureus (37; 33%), Streptococci (19; 17%), and Pseudomonas spp (20; 18%). Plain films were obtained in 89 (40%) patients, computed tomography scans were obtained for 81 (37%) patients, and magnetic resonance images were obtained for 40 (18%) patients. Most patients received osteomyelitis-directed antibiotics (153; 70%), 134 of 153 (88%) of which were scheduled to receive ≥6 weeks of treatment. Fifty-five (25%) patients underwent surgery during the index admission; 48 (22%) patients received a combined medical-surgical approach. One third of patients had ≥2 readmissions during the subsequent year. Patients treated with a combined approach were less likely to be readmitted than those who received antibiotics alone (0 [range, 0-4] vs 1 [0-7] readmissions; P = .04). Conclusions. This is one of the largest cohort studies of pressure ulcer-related pelvic osteomyelitis to date. Significant variations existed in diagnostic approach. Most patients received antibiotics; those treated with a combined medical-surgical approach had fewer hospital readmissions.

Bakgrund: Kroniska sår innefattas av bensår, fotsår, trycksår, diabetessår, sårskada, tumörer, reumatiska sår och vårdskador vid komplikationer efter kirurgiska ingrepp, som inte har läkt inom 6 veckor. Forskning visar att trots att förebyggande strategier används, utvecklas det sår som kräver behandling. I mötet med vården är en god omvårdnad och en god vårdrelation viktig. Bristen på dessa leder ofta till ett lidande som individen måste kämpa emot med copingstrategier. Syfte: Syftet med denna litteraturstudie var att belysa personers erfarenhet av att leva med kroniska sår. Metod: En litteraturöversikt gjordes i denna studie med 15 vetenskapliga artiklar, som bestod av både kvantitativ och kvalitativ metod. Resultat: 3 huvudkategorier identifierades: begränsningar, lidande samt coping. Deltagarna ansåg att en brist på information samt begränsningar i vardagen påverkade individens sociala umgänge och fysiska aktivitet. Smärta, skam och rädsla ledde till depression. För att hantera detta använde individerna sig av familjen och stöd av vårdpersonalen Slutsats: Det framkom att individernas dagliga liv påverkades av det kroniska såret. Den konstanta smärtan orsakade sömnsvårigheter hos individen, vilket ytterligare försvårade hanteringen av smärtan och vardagen. Kontinuitet hos vårdpersonal och i behandlingen gav personerna en trygghet och en ökad förståelse för sitt tillstånd. Det belystes att det var viktigt att vårdpersonalen ser individen bakom såret, och inte bara lägger sitt fokus på sårläkningen och behandlingen.

Background: Labor pain is a legacy given by bipedalism, sendentarism and greater fetos robustness. It is expected that nurses mostly apply non-pharmacological methods of pain relief, putting the account of the mother to your preferences. Aim: To describe the women preferences about pain relief no-pharmacological measures. Method: Convenience sample of 180 parturients, with mean age of 30.81 years (SD = 5.31), ranging from 17 to 43 years. Results: all positions are valued as a pain relief way. The most valued, classified as "strongly relief" is the lateral decubitus in bed (45.7%), followed by walking (40.4%) and sitting swing (38.9%). Conclusion: Mothers show preference for traditional measure as lateral position on the bed. A greater familiarity with other relief measures could facilitate labor experience.
