Quasi-experimental study, prospective with quantitative approach, performed at the Hospital do Coração in Natal, aimed at verified the existence of difference between the care given by health professionals to the patients under mechanical ventilation (MV) in the Intensive Care Unit, before and after an educative intervention. The population was of 31 professionals, with data collected between november 05 of 2007 to march 27 of 2008. The results show a yong population, female gender, middle level of education, nursing technique, working between 05 and 09 years on nursing profession, and 01 to 04 years on Intensive Care Unit; almost all, never had an kind of training over prevent pneumonia associated to mechanical ventilation; from those that had training, occur on the work place with duration from 12 to 24 hours. About endotracheal intubation, the cuff was tested with a sterilized syringe had a positive change after a educative intervention, increased from 75,0% to 100,0%; the sterile guide was used on 75,0% before and 100,0% after an educative intervention. Regarding endotracheal suction procedure, was not informed to the patient on 72,7% before, however was informed on 56,7% after; the hands was not previously washed 68,5% before, however was 63,3% after the procedure; mask was used on 74,2 % opportunities before and 76,7% after; the aspiration catheter had adequated size on 98,9% observation before and 100,0% after; the gaze was sterilized on 95,7% before and 100,0% after; the ventilator was connected to the patient during the aspiration intervals on 94,4% observation before and 100,0% after; the ambu bag was clean and protected on 76,1% before and 85,7% after; the aspiration catheter was discarded after be used on 98,9% before and 100,0% after; FIO2 was turned to the begging value on 32,9% observation before and 12,0% after; before the procedure 71,9% professions washed their hands and 73,3% after; before, notes of aspiration results were performed on 70,8% observation and 86,7% after. Regarding devices used on respiratory tract, aspirator flasks were not swapped on 84,6% observations before and 71,0% after; daily látex extention change was not performed on 93,6% observation before and 87,1% after; the ambu bag change was not performed on 50,0% observation before even if was duty or unprotected and on 75,8% opportunities was changed, after; nebulization was not prepared with sterile fluids or manipulated aseptically on 65,2% observation before, perhaps was on 71,7% after; before nebulizers were not changed on 65,2% observations, perhaps were on 60,9% after. Concerning ventilator breathing circuits, condense fluids cumulated on circuits were removed on 55,0% opportunities before, and 64,0% after; moisturizer was not filled with sterile water when already had small amount of liquid inside on 78,4% observations before, and 90,2% after; MV circuits were changed on 97,0% observations on presence of visible duty or when presents some kind of failure, before and 98,4% after. About body position, on 51,3% observations the decubitus position change were done before and 78,2% after; fowler position was maitened on 95,5% observations before and 98,2% after; Regarding respiratory physiotherapy, enteral diet was not interrupted before respiratory physiotherapy on 94,9% before and 90,0% after; respiratory physiotherapy devices were not disinfected or sterile on 69,6% observations before but they re on 60,0% after; before the cateter was not tested before introduction enteral diet or medications on 100,0% but after was done on 15,2%. About enteral feeding, intestine motility and measure of stomach contents were not done on 100,0% observations before, but was 15,2% after. We conclude that 05 of 07 valuated procedures in relation to MV, had a significant improvement on quality of care given after educative intervention, when compared before intervention

Objective: the purpose of this study was to determine the effects of prone positioning on cardiorespiratory stability and weaning outcome of preterm infants during weaning from mechanical ventilation. Methods: from January to December 1999, a sample of 42 preterm infants, with birthweight < 2,000 g, mechanically ventilated in the first week of life, were randomly divided, in the beginning of the weaning process, in two groups according to the position: supine position (n = 21) or prone position (n = 21). Heart rate, respiratory rate, transcutaneous oxygen saturation and ventilatory parameters were recorded every one hour. Length of the weaning process and complications were also assessed. Results: in both groups the mean gestational age was 29 weeks, most of the patients presented very low birthweight and respiratory distress syndrome. The mean length of the weaning process was 2 days. There were no differences between the groups regarding respiratory rate, heart rate and transcutaneous oxygen saturation, however, oxygen desaturation episodes were more frequent in supine position (p = 0.009). Ventilatory parameters decreased faster and reintubation was less frequent in the prone group (4% versus 33%). No adverse effects of prone positioning were observed. Conclusion: these results suggest that prone position is a safe and beneficial procedure during the weaning from mechanical ventilation and may contribute to weaning success in preterm infants.

Este trabalho apresenta quatro estudos experimentais que avaliaram a transferência de funções de faces expressando emoções para figuras abstratas equivalentes a elas. Os objetivos específicos de cada estudo podem ser articulados em torno de uma proposta de investigação de propriedades quantitativas de relações de equivalência. O Capítulo 1 descreve um experimento conduzido para investigar se o grau de transferência de funções pode ser afetado pela apresentação atrasada dos estímulos de comparação em tarefas de emparelhamento ao modelo (matching atrasado). Foi constatado que sim: parece haver maior transferência quando os estímulos de comparação são apresentados com atraso. O estudo descrito no Capítulo 2 manipulou, além do emparelhamento atrasado, o número de estímulos mediadores das relações estabelecidas entre as faces e as figuras. O aumento no número de estímulos mediadores dificultou a formação de classes de equivalência e diminuiu a transferência de funções, mas esse efeito foi menos pronunciado quando as relações foram treinadas com emparelhamento atrasado ao modelo. O Capítulo 3 apresenta um experimento conduzido para verificar se figuras abstratas adquirem propriedades de expressões faciais apresentadas sempre muito rapidamente. Nesse delineamento, a transferência de funções ocorreu em grau menor do que quando as faces são apresentadas por períodos mais longos. O Capítulo 4 apresenta um estudo que avaliou o tempo que os participantes gastavam para encontrar figuras abstratas equivalentes a certos padrões faciais expressivos descritos como mais rapidamente localizáveis. Os resultados replicaram apenas parcialmente a transferência de funções demonstrada em um estudo anterior. Os estudos que compõem este trabalho fornecem evidências de que o grau de transferência de funções pode variar sistematicamente de acordo com os parâmetros experimentais adotados. Variações na transferência de funções indicam variações no grau de relacionamento de estímulos equivalentes.

A pneumonia associada a ventilação mecânica é a infecção hospitalar mais comum em pacientes de unidade de terapia intensiva. Estratégias de prevenção podem ser mais bem elaboradas com o conhecimento dos fatores de risco para esta infecção. Com o objetivo principal de identificar fatores associados com maior risco de desenvolvimento de pneumonia em pacientes que recebem ventilação mecânica, foi realizado estudo retrospectivo, caso controle, não pareado, em uma unidade de terapia intensiva, clínico-cirúrgica, de um hospital universitário na cidade de Belém do Pará, Brasil. O período de estudo foi de 19 meses (janeiro de 2003 a julho de 2004). Os critérios de definição foram adaptados a partir dos critérios dos Centers for Diseases Control and Prevention. Foram avaliadas características demográficas, procedimentos invasivos, morbidades associadas e variáveis dependentes de tempo (ventilação mecânica, nutrição, exposição a drogas), entre outros fatores, em 27 casos e 27 controles. A pneumonia associada à ventilação mecânica teve uma taxa bruta e incidência por 1000 ventiladores/dia de 10,6% e 12,3 episódios, respectivamente. O tempo médio de permanência na unidade de terapia intensiva dos pacientes foi de 34,2 ± 27,7 dias, enquanto dos controles foi de 15,4 ± 13,6 dias (p=0,003). O tempo médio para início da pneumonia foi de 14,29 ± 9,16 dias. A taxa global de mortalidade foi similar nos dois grupos (OR=1,60; p=0,576). A análise univariada demonstrou que medicamentos administrados em aerossóis (OR=4,75; p=0,01) e uso de curares (OR=8,61; p=0,003) estiveram associados a um maior risco de desenvolvimento da infecção. Conclui-se que a pneumonia associada a ventilação mecânica esteve associada ao uso de curares e aerossóis, sendo que medidas preventivas poderiam ser direcionadas a estes fatores por serem eles, potencialmente modificáveis.

INTRODUCTION: During mechanical ventilation (MV), the airways may accumulate secretions. Patients are submitted to Respiratory Therapy (RT) and tracheal aspiration when in MV, alone or associated, to eliminate these secretions. OBJECTIVE: The objective was to compare the effects of different protocols of bronchial hygiene in blood pressure, heart rate, oxygen saturation and respiratory rate of patients undergoing MV. MATERIALS AND METHODS: We conducted a prospective, randomized, controlled crossover, with intentional non-probabilistic sample in the Medical School Hospital of Marília. We included patients in invasive MV who were submitted to three different bronchial hygiene protocols: PP - physiotherapy protocol (manual chest compression and manual hyperinflation); AP - aspiration protocol; and PP + AP. Respiratory rate, systolic blood pressure (SBP), diastolic blood pressure (DBP), oxygen saturation and heart rate were evaluated in three moments: before (M1), immediately after (M2) and 30 minutes after (M3) for each protocol. The differences among protocols and times were assessed using ANOVA and post hoc Student Newman-Keus (p < 0.05). RESULTS: We studied eighteen 71.2 ± 13.9 year-old patients with 15.1 ± 17.7 days of MV. There were no differences among protocols. There was a significant decreasing in SBP (p = 0.0261) and DBP (p = 0.0119) from M2 to M3 in the aspiration protocol. CONCLUSION: There was a decrease of blood pressure on MV patients after 30 minutes of aspiration and no change in the other variables, and there was no difference among protocols.

Patients that are mechanically ventilated in ICUs are constantly exposed to different pathogens, which present multiantibiotic resistance. Among these microorganisms, is MRSA (Meticillin-Resistant Staphylococcus aureus) considered to be a therapeutic challenge due to its resistance to β-lactam antibiotics. Therefore, this study proposed to identify species of Staphylococcus spp. isolated from mechanically ventilated patients in ICU, the gene mecA detection and the genes of the enterotoxins A (sea), B (seb), C (sec-1) and D (sed) in samples of S. aureus, as well as the phenotypic resistance determination to oxacillin using the disc-diffusion method with discs of oxacillin and cefoxitin. The samples collection occurred during in a period of 19 months, obtaining samples from 232 patients. A percentage of 39% (70) of Gram-positive cocci were found; which 82,8% (58) were identified as Staphylococcus spp,. among these, 75,8% (44) corresponded to S. aureus species and 47,7% were identified as MRSA. It was found resistance to both drugs in 31,8% of the S. aureus samples, 16 (36,3%) had the gene sea and 11 (25%) had the sec-1 gene. Among the coagulase-negative staphylococci obtained, the species most found was S. epidermidis, corresponding to 43% (6). The results revealed that one of the most important etiologic agents of VAP amid the Gram-positive cocci is the species S. aureus, with special attention to MRSA. The presence of enterotoxins genes in S. aureus did not showed determinant role in VAP, but the presence of these superantigens can contribute worsening the patient’s prognosis, since they are associated with intense inflammatory response
