Resumo: Um cadáver macho, adulto de irara (Eira barbara) foi cedido pelo Centro de Triagem de Animais Silvestres (Cetas), localizado em Salvador/Bahia, ao Setor de Anatomia Veterinária da Escola de Medicina Veterinária e Zootecnia da Universidade Federal da Bahia. Trata-se de um mamífero carnívoro que pertence à Família Mustelidae e Subfamília Mustelinae que contém o Gênero Eira, representado apenas pela Espécie Eira barbara. Objetivamos a investigação da topografia vertebromedular do espécime e assim verificar a relação da medula espinal com o canal vertebral; a identificação, origem, emergência e quantificação dos nervos espinhais relacionados com a medula espinal. Pesquisa número 43245-1 autorizada pelo Sistema de Autorização e Informação em Biodiversidade (Sisbio-ICMBio/IBAMA). O exemplar foi fixado em solução de formaldeído a 10% e posteriormente dissecado e radiografado. Foram identificados oito pares de nervos espinhais cervicais, quatorze torácicos e na porção lombossacral da medula espinhal seis nervos espinais lombares, três sacrais e mais de três nervos espinhais caudais. O término da medula espinal ocorreu no nível quinta vértebra lombar. Os oito segmentos medulares cervicais localizaram-se entre a primeira e sétima vértebras cervicais. Os quatorze nervos espinais torácicos originaram-se na porção cranial das vértebras respectivas. Os segmentos medulares lombares, sacrais e caudais restringiram-se à região lombar da coluna vertebral. O deslocamento cranial dos segmentos medulares foi observado no oitavo cervical, terceiro, quarto e quinto lombares e todos os segmentos sacrais e caudais. As informações obtidas poderão ser utilizadas para análises comparativas com as demais espécies e com a adoção de medidas que visem proporcionar o bem-estar animal e a preservação da espécie.

This study aimed to validate the contents of an Instrument for Nursing Consultation in the Home Visit of people with Spinal Cord Injury (INCEVDOP-LM), based on the Self-Care Deficit Theory. The methodological development study was conducted with spinal cord injured (SCI) people ascribed in the Family Health Units the city of Natal/RN/Brazil, and with the nurses of these institutions. The study was conducted from Januray 2012 to January 2013 in two phases: the first aimed to identify the need for self-care of persons with SCI, and the second to develop and validate the INCEVDOP-LM. The first phase consisted of a census study of people with SCI living in Natal/RN. In the second phase, a non-probabilistic convencience sample of subjects was selected to form two groups: First stage - Group 1 of the first stage was comprised by 73 adults with SCI diagnosed with paraplegia or tetraplegia, with cognitive function preserved and that were registered to some family health unit; Group 2 of the Second phase was composed of six experts that were nurses with doctoral formation, scientific experience in the area of technology development or assistance to persons with SCI, and with publications in periodicals Qualis A2. Data collection of the first phase was conducted through home visits of people with SCI that responded three instruments: Questionnaire I (comprised of demographic and socioeconomic variables), The Competency Rating Scale for Self-care (ASA) and the Barthel Index (an instrument for evaluation of functional capacity). The research for the second phase was conducted in two stages: I-construction of the INCEVDOP LM; II-validation of the INCEVDOP-LM. The instrument and an evaluation form were forwarded to the experts for the validation. The correlations between the responses were analyzed by the Kappa test, with accepting values of>0.75. The evaluation criteria were: organization, clarity, simplicity, readability, appropriateness of vocabulary, objectivity, accuracy, reliability and suitability and the positive responses with frequency values of≥90% were considered excellent. The chi-square test was used to investigate the differences between proportions. The study attended to the principles of Human Rights CNS Resolution 196/96. Results were reported by means of four articles derived from the study. The findings indicate that the items that showed disagreement among experts (k=0.02) were diagnoses, interventions and evaluation of the nursing features pertaining to the domains of Nutrition, Hygiene, Elimination, Physical, Social and Psychological, and of the Ability to perform work activities feature. Agreement among the experts were reported for the other items, with kappa ranging from 0.72 to 1. After removing items with disagreement, all criteria achieved excellent rates and no significant differences were observed between the proportions of responses of evaluation of experts (p>0.05). We conclude that the instrument shows validity to serve as a guide for nurses to conduct a systematic consultation during the home visit to people with spinal cord injury, with emphasis on self-care. The instrument must go through other levels of validation when applied in the clinical setting

Las neuronas de la raíz coclear son las primeras neuronas del sistema nervioso central a recibir la información auditiva proveniente de la cóclea y se conectan con centros de integración sensoriomotora del tronco encefálico, especialmente con el núcleo reticular caudal del puente. Funcionalmente las neuronas de la raíz coclear están envueltos en el circuito elemental del reflejo auditivo de sobresalto juntamente con las células ganglionares del órgano de Corti, el núcleo reticular caudal del puente y las motoneuronas de la médula espinal. El reflejo auditivo de sobresalto presenta una serie de modulaciones como la habituación, la sensibilización, la inhibición por estímulo previo y la potenciación por un estímulo adverso. Las alteraciones en la reacción refleja de sobresalto y sus diferentes modulaciones poseen valor diagnóstico en la clínica médica de enfermedades neurodegenerativas y psiquiátricas como Parkinson y Esquizofrenia. Las modulaciones del reflejo auditivo de sobresalto ocurren mediante la influencia de diversos núcleos sobre los componentes del circuito elemental de este reflejo. El núcleo menos estudiado del circuito neural del reflejo auditivo de sobresalto es el núcleo de la raíz coclear. El patrón de conectividad eferente de las neuronas de la raíz coclear es bien conocido, sin embargo poco se conoce sobre los orígenes de sus aferencias y la identidad neuroquímica de las mismas. Estudios previos demostraron que el soma y en las dendritas de las neuronas de la raíz coclear están cubiertos por botones sinápticos de cuatro tipos, sugiriendo que existan diversos orígenes para estos terminales axonales, con características neuroquímicas propias. El conocimiento de las aferencias a las neuronas de la raíz coclear es relevante para comprender cual el papel del núcleo de la raíz coclear en las modulaciones del reflejo auditivo de sobresalto. The primary trigger to periodic limb movement (PLM) during sleep is still unknown. Its association with the restless legs syndrome (RLS) is established in humans and was reported in spinal cord injury (SCI) patients classified by the American Spinal Injury Association (ASIA) as A. Its pathogenesis has not been completely unraveled, though recent advances might enhance our knowledge about those malfunctions. PLM association with central pattern generator (CPG) is one of the possible pathologic mechanisms involved. This article reviewed the advances in PLM and RLS genetics, the evolution of CPG functioning, and the neurotransmitters involved in CPG, PLM and RLS. We have proposed that SCI might be a trigger to develop PLM.

OBJETIVO: Avaliar a eficácia do tratamento cirúrgico da mielorradiculopatia espondilótica cervical na produção de melhora neurológica pós-operatória, aferida em pontos pela escala da JOA e taxa de recuperação e as complicações do tratamento. MÉTODOS: Análise dos prontuários e os exames de imagem de 200 indivíduos submetidos a tratamento cirúrgico da mielorradiculopatia cervical no HC-FMUSP, no período de janeiro de 1993 a janeiro de 2007. A avaliação clínica foi quantificada pela escala da JOA, com média de segmento de 06 anos e 08 meses. RESULTADOS: Evidenciou-se melhora neurológica pós-operatória nas vias anterior e posterior, exceto nas laminectomias sem fusão, onde houve piora neurológica tardia. A via anterior mostrou um significante maior índice de complicações, relacionados a déficit de fusão intervertebral, deslocamento de enxerto, síndrome de disco adjacente, disfonia, disfagia, o mau posicionamento de enxerto e placas, lesão de raiz nervosa e significativo maior índice de re-intervenção cirúrgica. Na via posterior maior ocorrência de instabilidade em cifose pós-operatória na laminectomia, não sendo observada na laminoplastia, esta última com índices semelhantes aos encontrados na via anterior. Não houve melhora da dor axial nas laminoplastias e houve piora nas laminectomias, enquanto que nas discectomias e corpectomias houve significativa melhora do sintoma. CONCLUSÃO: As vias anterior e posterior foram eficazes em produzir melhora neurológica, exceto as laminectomias sem fusão. A via anterior produziu mais complicações, mas trata melhor a dor.

OBJETIVO: Identificar os fatores clínicos dos indivíduos, fatores sociais, ambientais e dos exames de imagem que se correlacionam ao resultado final de melhora neurológica em pacientes submetidos ao tratamento cirúrgico da mielopatia espondilótica cervical. MÉTODOS: A avaliação clínica foi quantificada pela escala deficitária da JOA. Analisamos 200 casos de mielorradiculopatia cervical, operados no HC-FMUSP, no período de janeiro de 1993 a janeiro de 2007. A média de segmento foi de 06 anos e 08 meses. A análise radiológica foi baseada nos critérios de instabilidade de White e scala de Kellgren. RESULTADOS: Em 80% houve melhora, 14% estabilização e em 6% piora do quadro neurológico. A piora neurológica não foi associada com nenhum fator clínico, ambiental ou de imagem. A melhora neurológica foi diretamente proporcional a menor idade na cirurgia, ausência de co-morbidade, sinal de Hoffman, atrofia muscular, hipersinal medular na RNM, menor período de evolução pré-operatório, melhor status neurológico pré-operatório e inversamente proporcional ao diâmetro AP do canal medular e multiplicidade de compressões. Identificou-se associação com o tabagismo. Mais de 70 anos, evolução superior a 24 meses, atrofia muscular, pontuação JOA igual ou inferior a sete pontos e diâmetro AP do canal inferior ou igual a seis mm não foram associado à melhora.

Hemangioblastomas of the central nervous system (CNS) are low-grade highly vascularized tumors that may be sporadic or associated with Von Hippel-Lindau disease. Extradural hemangioblastomas are uncommon and those located extra and intradurally are even rarer. This study uses an illustrative case and literature review to discuss the difficulties to consider the correct diagnosis and to select the best surgical approach. A 57 years-old white male patient presented with myelopathy and right C5 radiculopathy. The images showed a lobulated, hourglass shaped, highly enhanced extra/intradural lesion that occupied the spinal canal and widened the C4-C5 right intervertebral foramen. Total resection of the intradural lesion was achieved through a posterior approach, but the extradural part could only be partially removed. Complete improvement was observed after four months of follow-up and the residual tumor has been followed up clinically and radiologically. Even though the preoperative impression was of a spinal schwannoma, the histopathological examination revealed grade I hemangioblastoma as per WHO. Despite their rarity, current complementary exams allow considering the diagnosis of hemangioblastoma preoperatively. That is essential to a better surgical planning in view of the particular surgical features of this lesion.

Spinal cord injury (SCI) is a devastating neurological disorder that affects thousands of people each year. Although in recent decades significant progress has been made in relation to understanding the molecular and cellular events underlying the nervous damage, spinal cord injury is still a highly disabling condition for which there is no curative therapy. People affected by spinal cord injuries manifested dysfunction or loss, temporary or permanent, of motor, sensory and / or autonomic functions depending on the spinal lesion damaged. Currently, the incidence rate of this type of injury is approximately 15-40 cases per million people worldwide. At the origin of these lesions are: road accidents, falls, interpersonal violence and the practice of sports. In this work we placed the hypothesis that HA is one of the component of the scar tissue formed after a compressive SCI, that it is likely synthetised by the perilesional glial cells and that it might support the permeation of the glial scar during the late phase of SCI. Nowadays, much focus is drawn on the recovery of CNS function, made impossible after SCI due to the high content of sulfated proteoglycans in the extracellular matrix. Counterbalancing the ratio between these proteoglycans and hyaluronic acid could be one of the experimental therapy to re-permeate the glial scar tissue formed after SCI, making possible axonal regrowth and functional recovery. Therefore, we established a model of spinal cord compression in mice and studied the glial scar tissue, particularly through the characterization of the expression of enzymes related to the metabolism of HA and the subsequent concentration thereof at different distances of the lesion epicenter. Our results show that the lesion induced in mice shows results similar to those produced in human lesions, in terms of histologic similarities and behavioral results. but these animals demonstrate an impressive spontaneous reorganization mechanism of the spinal cord tissue that occurs after injury and allows for partial recovery of the functions of the CNS. As regards the study of the glial scar, changes were recorded at the level of mRNA expression of enzymes metabolizing HA i.e., after injury there was a decreased expression of HA synthases 1-2 (HAS 1-2) and an increase of the expression HAS3 synthase mRNA, as well as the enzymes responsible for the HA catabolism, HYAL 1-2. But the amount of HA measured through the ELISA test was found unchanged after injury, it is not possible to explain this fact only with the change of expression of enzymes. At two weeks and in response to SCI, we found synthesized HA by reactive astrocytes and probably by others like microglial cells as it was advanced by the HA/GFAP+ and HA/IBA1+ cells co-location.
