The tibial plateau leveling osteotomy (TPLO) is a relatively new and innovative surgical treatment for the cranial cruciate ligament rupture in the canine species. The real intent of the procedure is to provide functional stability to the stifle joint by eliminating or neutralizing the cranial tibial thrust during weight bearing instead to restore the cranial cruciate ligament function. The proposal of this study is to report a review of the TPLO procedure, emphasizing procedure, surgical technique, post operative care and complications. The TPLO procedure consists in a radial osteotomy in the tibial plato and rotation of the caudal plateau in order to obtain a desired angle, After the leveling of the tibial plateau, a bone plate and screws are used to stabilize the osteotomy until bone is healed up. The complications that have been associated with the procedure include tibial tuberosity fracture and patellar tendon tendinosis. This procedure has become increasingly more popular for surgical treatment of cranial cruciate ligament injuries in large breed dog. The long term clinical results have not been completely elucidated yet. It has been showed that this technique doesn`t halt the degenerative joint disease.

SOX18 is a transcription factor that is transiently expressed in nascent endothelial cells during embryonic development and adult neovascularization. This protein belongs to the SOX family of transcription factors, ih,which are proving to be some of the key regulators of cell-type specification in the vertebrate embryo. Natural mutations in the Sox18 gene have been shown to result to cardiovascular dysfunction, in some cases leading to death. Available evidence thus implicates Sox18 as an important regulator of vascular development, most likely playing a key role in endothelial cell specification. However; the genetic knockout of Sox18 in mice has produced a confounding result that complicates our understanding of the molecular mode of action of the SOX18 protein. We speculate that Sox18 inky act in a redundant fashion with closely related genes such as Sox7 and/or Sox17. (C) 2001, Elsevier Science Inc.

Background and Purpose Previous research regarding the symmetry of trans-tibial amputees has examined weight distribution and various gait parameters between prosthetic and sound limbs. However, to date, no known research has determined if asymmetry is present in the strength of the hip abductor muscles or if correlations exist between these categories of symmetry. The purpose of the present study was, therefore, to document asymmetry present in stance, strength and gait measures, and to determine the relationship between these variables. Method Twenty-three elderly, unilateral trans-tibial amputees stood on two adjacent forceplates whilst the weight distribution and standard deviation (SD) of the anterior-posterior and the medio-lateral centre of pressure excursion (COPE) under each limb was recorded during four 40 s trials: quiet stance (QS), with eyes open and eyes closed; and even stance (ES), with eyes open and eyes closed. Gait measures (velocity, cadence, step and stride lengths, stance:swing ratio and period of double support) over 10 m of fast, yet safe walking and measures of the strength of hip abductor muscles were also obtained by use of a stride analyser and a dynamometer, respectively. Results No significant differences were found between QS and ES measures. However, significantly more weight was taken on the sound limb than on the amputated limb. Notably, more anterior-posterior movement occurred under the sound limb than the amputated limb, with this becoming more apparent with the eyes closed. Movement in the medio-lateral direction was found to be the same between sides. No differences in muscle strength or gait measures between limbs were demonstrated. However. strong hip abductor muscles were correlated with increased weight-bearing on the amputated limb, improved gait parameters and reduced medio-lateral COPE under the amputated limb. Conclusions This research confirms the asymmetrical nature of amputee stance and demonstrates symmetry of strength and gait measures between limbs. The correlations between hip abductor muscle strength, weight distribution and gait measures illustrates the importance of pre- and postoperative training of these muscles. Nodal shoot cultures of 'Clone 003', a selected Australian papaya cultivar, were cultured on modified De Fossard medium supplemented with chemicals that either promote ethylene evolution or inhibit action while in culture. Nodal shoot cultures grown in the presence of 1-aminocyclopropane carboxylic acid (ACC, 1.0 mM) resulted in a significant reduction in percent fresh and dry weights, shoot length, leaf area, petiole length and chlorophyll content, but leaf development was significantly increased. In contrast, nodal cultures grown in the presence of silver thiosulphate (STS, 0.5 mM) significantly produced the highest percentage of fresh and dry weights, shoot length, leaf production, leaf area expansion, petiole length and leaf chlorophyll content. Nodal cultures and rooted whole plantlets placed in medium-sized (125 mL) culture vessels had significantly better growth than those cultures placed in small (70 mL) or in large (250 mL) vessels. Cultures grown in medium-sized vessels had higher fresh and dry weights, longer shoots, more leaves and larger leaf area than those cultures placed in smaller or larger vessels. Similarly, values for said growth parameters and for chlorophyll content of the nodal and rooted whole plantlets were higher when they were incubated under high light intensity of 120 mumol m(-2)s(-1) at a prevailing temperature of either 20+/-1 C or 25+/-1 C.

Introdução: A iniciação da marcha, enquanto tarefa motora complexa que consiste na transição de uma postura mantida pelo apoio simultâneo dos dois membros inferiores para um equilíbrio dinâmico, permitindo a progressão anterior do corpo, constitui um exemplo que implica uma correta sequência de ativação muscular. Objetivos: Verificar a modificação da fase de iniciação da marcha face à aplicação de um programa de recuperação funcional, analisando a sequência de ativação dos músculos tibial anterior e solear. Registar as repercussões funcionais na participação nas diferentes atividades da vida diária, em contexto padronizado e social. Metodologia: Nos dois participantes em estudo foi realizada uma avaliação antes e após um programa de intervenção, segundo a abordagem do Conceito de Bobath, através da Classificação Internacional de Funcionalidade, Incapacidade e Saúde, da Fugl Meyer Assessment of Sensorymotor Recovery After Stroke, electromiografia e plataforma de forças. Resultados: Após a aplicação do programa de recuperação funcional, verificaram-se alterações na sequência de ativação do tibial anterior e solear. O músculo tibial anterior passou a ser o primeiro a ser recrutado nesta sequência. Conclusão: Foi possível verificar modificações durante a fase de iniciação da marcha, face a um programa de recuperação funcional, em que a sequência de ativação do tibial anterior e solear, tendencialmente, se assemelharam ao comportamento em indivíduos saudáveis, repercutindo-se numa melhoria funcional na participação nas atividades da vida diária.

O presente estudo tem por objectivo principal relatar o caso duma atleta de esqui na montanha com 18 anos de idade, com uma lesão do Ligamento Cruzado Anterior (LCA), lesão do menisco medial e fractura do prato tibial medial que foi submetida a tratamento cirúrgico, realizando tratamento de fisioterapia no pré e pós-operatório. O programa de reabilitação tinha como objectivo reduzir o edema e conquistar a amplitude de movimento livre de dor assim como reestabelecer os níveis de controlo neuromuscular prévios à lesão. A atleta passou por cinco reavaliações durante seu o período de reabilitação e como parâmetros utilizaram-se as escalas de funcionalidade de Cinccinatti e Lisholm, a perimetria da coxa, os graus de amplitude de movimento e a sua força muscular pelo método de 1RM. Os resultados foram os esperados para cada fase da sua reabilitação. No pré-operatório a atleta apresentava um déficit na força muscular do membro inferior lesionado, principalmente no quadricípite ( 3+ inicialmente e 4 na reavaliação antes da cirurgia), discrepância na perimetria muscular da coxa (-2,5 centímetros em relação ao membro contralateral medidos a 5 centímetros acima do bordo superior da patela, e -1,5 centímetros após o programa de reabilitação pré-operatória). No pós-operatório, a força muscular do quadricípite era de 3 inicialmente e de 5 no final do programa de reabilitação, a diferença de 2,5 centímetros de perimetria em relação ao membro contralateral inicialmente encontrada no pós-operatório foi reduzida a uma mínima diferença na sua última avaliação. Quanto à funcionalidade, a atleta inicialmente apresentava valores de 120 e 61 nas escalas de Cinccinatti e Lysholm e no final do programa de reabilitação de 320 e 95, respectivamente. Por fim, conclui-se que a atleta no final de quatro meses, apresentou melhorias sensíveis no seu quadro e muito provavelmente retornará às competições de forma integral.

Introdução: As dificuldades encontradas em individuos após Acidente Vascular Encefálico, ao nível da marcha, influenciam significativamente o retorno ao trabalho, a participação na comunidade ou o desempenho nas actividades da vida diária. Objectivo: Neste trabalho, procurou-se verificar qual o efeito de um programa de intervenção em fisioterapia nos ajustes posturais antecipatórios que ocorrem previamente ao movimento voluntário e assim pré-determinar a sua contribuição para a estabilidade postural e o equilíbrio. Material e Métodos: Para testar os pressupostos inerentes, relatam-se dois casos clínicos de sujeitos do sexo masculino com diagnóstico de Acidente Vascular Encefálico. Estes foram submetidos a um programa de intervenção em fisioterapia, durante 10 semanas consecutivas, baseado no Conceito de Bobath e que teve em conta o principal problema de cada indivíduo. Foi monitorizada a actividade electromiográfica do ventre medial do Tibial Anterior e Solear na fase de pré-activação da marcha, em dois momentos distintos, no início e no fim da intervenção. Resultados: Dos resultados obtidos evidenciam-se as diferenças entre os tempos médios de pré-activação, bem como as diferenças entre a percentagem da contracção isométrica voluntária máxima atingida pelos músculos Tibial Anterior e Solear, direito e esquerdo, entre os dois momentos de avaliação, em ambos os sujeitos. Verificou-se, no entanto, que a actividade electromiográfica dos referidos músculos possui grande variabilidade. Conclusão: Os resultados sugerem que o programa de intervenção em fisioterapia parece ter tido influência no recrutamento da actividade muscular do TA e do SO, uma vez que, em termos médios absolutos, ocorreram diferenças após a implementação das estratégias e procedimentos da intervenção.

Introdução: os APA´s ocorrem imediatamente antes do movimento e preparam-no tornando-o mais harmonioso e eficiente. Os pacientes com lesão do SNC apresentam frequentemente alterações no sistema de controlo postural interferindo significativamente nas suas AVD´s, como no início da marcha. Objetivo: descrever as alterações no tempo de ativação e sequência de ativação muscular do TA e do SOL no início da marcha em pacientes com AVE, face a uma intervenção em fisioterapia. Metodologia: A avaliação realizou-se antes e após um programa de intervenção, segundo a abordagem do Conceito de Bobath, através da electromiografia, plataforma de forças e máquina fotográfica para a avaliação do tempo de ativação muscular do tibial anterior e do solear no início da marcha. Recorreu-se também à Classificação Internacional de Funcionalidade e à Fulg-Mayer Assessment of Motor Recovery after Stroke. Resultados: Obteve-se uma diminuição dos valores registados pela EMG nos tempos de ativação muscular do TA e do SOL bilateralmente, e alterações na sequência de ativação. Verificaram-se modificações nos resultados da Classificação Internacional de Funcionalidade e da Fulg-Mayer Assessment of Motor Recovery after Stroke. Conclusão: O programa de intervenção segundo o Conceito de Bobath, induziu mudanças nos tempos de ativação muscular e na sequência de ativação dos músculos TA e SOL no início da marcha em pacientes com AVE.

The aim of the present study was to investigate variations in oxidized LDL (oxLDL) at the onset of acute myocardial infarction (AMI) and over the recovery period, exploring their relationship with coronary disease severity. A follow-up of 50 AMI patients was evaluated against 25 healthy volunteers (reference group). The AMI patients were evaluated at three time points: at admission before the administration of IIb/IIIa inhibitors and angioplasty, and two and 40 days after intervention. Plasma oxLDL concentrations were measured by ELISA. oxLDL was found to be significantly higher in AMI patients in the acute phase relative to reference levels, decreasing progressively over the recovery period. The results also demonstrated that oxLDL levels were decreased in patients with the left circumflex artery (LCX) as culprit vessel compared to the left anterior descending coronary (LAD) or right coronary artery (RCA). The results highlight a significant increase in oxLDL concentration related to coronary artery disease severity, as conditions such as LCX lesions are usually associated with a favorable prognosis, contrasting with LAD-associated conditions that can compromise large areas of myocardium. The results thus suggest that oxLDL may constitute a promising marker in assessment of AMI evolution.

We reviewed 19 patients (24 knees) with patellofemoral instability treated surgically with antero-medialisation of the tibial tubercle and lateral retinacular release. Twenty-two knees had recurrent patellar dislocation and two patellar subluxation. Lateral retinacular release was performed arthroscopically in 15 knees. Average follow-up was 52 (16-86) months. There was one postoperative haemarthrosis and one failed fixation, which needed surgical revision. The average Lysholm score improved from 63.3 to 98 and only one knee had persistent patello-femoral pain postoperatively. The patellar tilt angle improved from 9.4 degrees to 5.5 degrees . There were no redislocations. We find that the surgical technique produces a consistent correction of patellar instability, but long-term studies are needed to confirm whether it can prevent arthritic degeneration.

O consenso atual para o tratamento do pé equino-varo congénito é o tratamento conservador pelo método de Ponseti. A transposição do tendão tibial anterior para a terceira cunha é uma cirurgia que pode ser usada no tratamento da recidiva dinâmica do pé equino-varo congénito que se caracteriza pelo reaparecimento do varosupinação. Foram revistos 37 casos de pé equino-varo, idiopático (33) e não idiopático (4) de 29 crianças de ambos os sexos com idades compreendidas entre os 2 e os 8 anos que apresentavam recidiva de pé equino-varo com reaparecimento do varo-supinação de 2005 a 2008. Os doentes foram submetidos a correção cirúrgica com transposição do tendão tibial anterior para a terceira cunha. A avaliação clínica dos resultados foi feita com um follow-up mínimo de 6 meses. A avaliação pós operatória revelou bons resultados em 31 pés intervencionados. Quanto aos outros casos, 3 mantiveram uma supinação ligeira do pé sem dor ou limitação funcional e em 3 casos o resultado final foi mau. A transferência do tendão tibial anterior para a terceira cunha apresenta bons resultados na recidiva do pé equino-varo congénito.

OBJECTIVE:Endograft mural thrombus has been associated with stent graft or limb thrombosis after endovascular aneurysm repair (EVAR). This study aimed to identify clinical and morphologic determinants of endograft mural thrombus accumulation and its influence on thromboembolic events after EVAR. METHODS: A prospectively maintained database of patients treated by EVAR at a tertiary institution from 2000 to 2012 was analyzed. Patients treated for degenerative infrarenal abdominal aortic aneurysms and with available imaging for thrombus analysis were considered. All measurements were performed on three-dimensional center-lumen line computed tomography angiography (CTA) reconstructions. Patients with thrombus accumulation within the endograft's main body with a thickness >2 mm and an extension >25% of the main body's circumference were included in the study group and compared with a control group that included all remaining patients. Clinical and morphologic variables were assessed for association with significant thrombus accumulation within the endograft's main body by multivariate regression analysis. Estimates for freedom from thromboembolic events were obtained by Kaplan-Meier plots. RESULTS: Sixty-eight patients (16.4%) presented with endograft mural thrombus. Median follow-up time was 3.54 years (interquartile range, 1.99-5.47 years). In-graft mural thrombus was identified on 30-day CTA in 22 patients (32.4% of the study group), on 6-month CTA in 8 patients (11.8%), and on 1-year CTA in 17 patients (25%). Intraprosthetic thrombus progressively accumulated during the study period in 40 patients of the study group (55.8%). Overall, 17 patients (4.1%) presented with endograft or limb occlusions, 3 (4.4%) in the thrombus group and 14 (4.1%) in the control group (P = .89). Thirty-one patients (7.5%) received an aortouni-iliac (AUI) endograft. Two endograft occlusions were identified among AUI devices (6.5%; overall, 0.5%). None of these patients showed thrombotic deposits in the main body, nor were any outflow abnormalities identified on the immediately preceding CTA. Estimated freedom from thromboembolic events at 5 years was 95% in both groups (P = .97). Endograft thrombus accumulation was associated with >25% proximal aneurysm neck thrombus coverage at baseline (odds ratio [OR], 1.9; 95% confidence interval [CI], 1.1-3.3), neck length ≤ 15 mm (OR, 2.4; 95% CI, 1.3-4.2), proximal neck diameter ≥ 30 mm (OR, 2.4; 95% CI, 1.3-4.6), AUI (OR, 2.2; 95% CI, 1.8-5.5), or polyester-covered stent grafts (OR, 4.0; 95% CI, 2.2-7.3) and with main component "barrel-like" configuration (OR, 6.9; 95% CI, 1.7-28.3). CONCLUSIONS: Mural thrombus formation within the main body of the endograft is related to different endograft configurations, main body geometry, and device fabric but appears to have no association with the occurrence of thromboembolic events over time.

In order to compare the development of strata in the early stages of secondary forest succession with vessel parameters of the tree species, a forest inventory was carried out in 4-year (Q1: 48 m2), 11-year (Q2: 400 m2) and 20-year (Q3: 400 m2) forests and vessel parameters were investigated from stem cross sections of 18 species obtained in Q2. Thirty three species (21 families), 77 species (35 families), 39 species (20 families) were found in Ql, Q2, Q3, respectively. The percentage of dead individuals, dead stems and the percentage of individuals with multiple stems increased with time after clear cutting. Also, the total D2H of Q3 was 26.1 times that of Q1, and the development of strata started in Q2 and Q3. The image analysis of vessel size, area and number of vessels revealed that species which reach the forest canopy had a large D2H value, vessel diameter and area, while species which remain near the forest floor had smaller ones. Poecilanthe effusa (Huber) Ducke is an example of the latter case, with a large number of individuals and abundant sprouting of new stems from stumps, but with high mortality.

OBJECTIVE: To test the hypothesis that left ventricular hypertrophy (LVH) reduces the electrocardiographic and functional effects of right coronary artery occlusion. METHODS: We analysed 215 patients (166 males and 49 women,age of 58.9±10.6 years), with occlusion of the right coronary artery without other associated lesions. There was no significant difference (p>0.05) in age and gender distribution between the 78 patients with LVH (left ventricular mass >100g/m²) (Group A) when compared with the 137 patients without LVH (left ventricular mass <100g/m²) (Group B). RESULTS: The electrocardiographic finding of transmural necrosis was more often found in group B patients than in group A patients (56.9% and 30.8%, respectively; p<0.05). The left ventricular function parameters of group A were better than those of group B: the ratio end-diastolic pressure/systolic pressure (EDP/SP) (A: 0.108±0.036; B: 0.121±0.050; p<0.05); the end-diastolic volume index (A: 75.9±31.3ml/m²; B: 88.0±31.0ml/m²; p<0.01); the end-systolic volume index (A: 16.0±10.0ml/m²; B: 27.0 ±20.0ml/m²; p<0.001); the ejection fraction (A 78.6±10.8%; B 67.7±17.9%; p<0.001); the anteroinferior shortening (A: 43.9±10.3%; B: 35.1±12.8%; p<0.001). A higher degree of coronary tortuosity was observed in group A than in group B (78.2% and 24.1%; p<0.001) and also a more frequent absent or minimal diaphragmatic hypokinetic area (A: 80.8%; B: 54.0%; p<0.05). CONCLUSION: LVH reduces the effects of myocardial sequela and protects LV function when right coronary occlusion develops.
